    Abstract: Background: Rising of multidrug resistance among uropathogenic bacteria is leading our future generation to a crisis. Nevertheless, the era of antibiotic failure due to bacterial resistance has brought interest to other medical science like Traditional Medicine, alternative medicine. Objectives: This study was undertaken to determine the current antibiotic resistance situation among common bacterial uropathogens and suggesting prevention measure. Materials and Methods: A total of 212 patients male 132,female 80 selected. Mid-urine samples were collected. Isolated organisms were identified by conventional methods. Then different antibiotics representing different families of antibiotics were tested on isolated organisms. Results: The most frequently isolated gram negative bacteria was E.Coli (92%) followed by Staphylococcus(7.07%),Klebsiella(7%), Pseudomonas(4.25%). Resistance to Amoxicilline, Ciprofloxacin, Cefixim, Ceftriaxone, Cefuroxim, Cefradin, Cefotaxim was more than 70% of all isolates of E.Coli stains. There was relatively low resistance rate to Nitrofurantoin, Gentamycin,Imipenem, Meropenem, Amikacin, Ceftazidim . However, there was emerging resistance to Ciprofloxacin, specially for common bacteria. Conclusion: It is urgent need to make a policy for antibiotics use . We suggest all health care professional of traditional medicine and modern medicine to combat against antibiotics resistance. KYAMC Journal.2021;12(1): 32-35

    Abstract: Penile fracture is an uncommon urological emergency, especially in Bangladesh. The other name is traumatic rupture of the tunica albuginea and corpora cavernosa in the erect penis. It occurs when an erect penis face to buckle under the pressure of a blunt sexual trauma. Patient gives the typical history of immediate detumescence, severe pain, swelling and eggplant deformity of the penile shaft due to penile injury. Immediate surgical exploration and repair of corpora Cavernosa with tunica albugenia is the most effective treatment modality. In normal cases diagnosis is made from history, physical examination alone. In some special cases ultrasonogram, radiological images, including retrograde urethrography or cavernosography are mandatory for proper diagnosis.     Abstract: Background: The penile size has always been a symbol of manliness and pride. Many adult think that there penile length are not adequate and goes to different medical disciplines. There is no study in Bangladesh regarding penile size and somatometric parameter. The results will help in counseling patients afraid of their penile size , also in clinical and research purpose. Objective: To assess the penile size in different dimensions such as flaccid, stretch, and girth (circumferential) penile length and its relationship with age, height, weight, and BMI. Materials and Methods: The study was conducted on 300 adult males with no penile abnormalities. Length of penises in flaccid, stretch, and circumference were measured. Age ,height, weight were measured and BMI (body mass index) was calculated. Correlation of penile dimensions with somatometric parameters were analyzed. Results: The average flaccid penile length(FPL) was 7.81 cm, the mean stretch length(SPL) was 11.21 cm, and the mean circumferential length(CPL) was 8.65 cm. The penile length in the three dimensions was significantly different from each other (p 〈 0.05). A significant correlation was found among the three penile dimensions (p 〈 0.05). Conclusion: The penile size in different dimensions can be varied with age,height,weight and BMI. KYAMC Journal. 2021;12(3): 122-126

    Abstract: COVID-19 has caused over 528 million infected cases and over 6.25 million deaths since its outbreak in 2019. The uncontrolled transmission of the SARS-CoV-2 virus has caused human suffering and the death of uncountable people. Despite the continuous effort by the researchers and laboratories, it has been difficult to develop reliable efficient and stable vaccines to fight against the rapidly evolving virus strains. Therefore, effectively preventing the transmission in the community and globally has remained an urgent task since its outbreak. To avoid the rapid spread of infection, we first need to identify the infected individuals and isolate them. Therefore, screening computed tomography (CT scan) and X-ray can better separate the COVID-19 infected patients from others. However, one of the main challenges is to accurately identify infection from a medical image. Even experienced radiologists often have failed to do it accurately. On the other hand, deep learning algorithms can tackle this task much easier, faster, and more accurately. In this research, we adopt the transfer learning method to identify the COVID-19 patients from normal individuals when there is an inadequacy of medical image data to save time by generating reliable results promptly. Furthermore, our model can perform both X-rays and CT scan. The experimental results found that the introduced model can achieve 99.59% accuracy for X-rays and 99.95% for CT scan images. In summary, the proposed method can effectively identify COVID-19 infected patients, could be a great way which will help to classify COVID-19 patients quickly and prevent the viral transmission in the community.

    Abstract: Objective: To review the current understanding of testicular pain, possible etiology and the available treatment options. Materials and Methods: A literature search was made for abstract, original and review articles in the Medline, hand-searched relevant reference lists and conference proceedings and medical text book using the word “testicular pain” and orchialgia to find out possible etiology, understanding of testicular pain and treatment options. Results: After literature search, it has found that management of testicular pain sometimes a challenging work for an Urologist. Management differs for the original etiology and for testicular and extra testicular causes. Sometimes symptomatic treatment improve the quality of life but in few cases surgery is mandatory for relieve of pain. Conclusions: Management of chronic testicular pain depends on careful assessment of the causes. Relieve of symptoms is not always possible with analgesic only, in that case blockade of the spermatic cord will be helpful. Decision for surgery should not be undertaken quickly for there is no guarantee that there will always be resolution of pain For that reason patient should be counseled accordingly.     Abstract: Background: Urolithiasis is one of the most prevalent urological disorders and the prevalence of urinary stones has increased world wide1. The management of urinary calculi was revolutionized by the introduction of extracorporeal shockwave lithotripsy (ESWL) in 1980 and the first successful ESWL treatment was accomplished in Germany by Dr. Christian Chaussy using a Dornier HM1 lithotripter. ESWL is a safe, effective and non-invasive method2.Purpose: To observe the outcome of pushback stenting and ESWL versus in situ ESWL for upper ureteric stone.Materials and methods: It was a quasi-experimental study. The study was under went in the department of urology, Dhaka Medical College and Hospital, between July'2012 to June'2014. Total 60 patients of single upper ureteric stone who satisfy inclusion and exclusion criteria were enrolled in this study. Selected patients were dived into two groups, group A and group B. Group-A for pushback stenting and ESWL and group-B for in situ ESWL. Results were compared in terms of clearance rates, number of shock waves, sessions, incidence of complications and failure rate.Results: Failure of ESWL was significantly higher in Group B (23.33%) than Group A (10%). These results were statistically significant.Conclusion: Pushback stenting and ESWL is better than in situ ESWL for upper ureteric stone.KYAMC Journal Vol. 8, No.-1, Jul 2017, Page 4-9

    Abstract: Purpose: We reviewed the literatures to see history, regimen of BCG and how BCG become a standard therapy for bladder carcinoma.Materials and Methods: We reviewed the previous literature describing the bacillus Calmette-Guerin (BCG) vaccine as an anticancer agent and its success as the most effective immunotherapy used against human bladder cancer and its adverse effects.Results: The association between tuberculosis and carcinoma is well established, It demonstrate that Bacillus Calmette-Guerin have immunological reactivity, inhibiting the tumor growth and progression in experimental animal models, led to clinical trials showing that intravesical Bacillus Calmette-Guerin eradicate and prevent recurrence and progression of superficial bladder carcinoma The exact mechanism of action of intravesical BCG instillation is still under investigation. However, it appears that BCG is mediated by the local immune response, mainly through T-helper cell response. It reduces the recurrence rate by an average of 40% and progression by more than 20% in papillary tumors, Carcinoma-in-situ over the patients without BCG therapy.Conclusions: For the last 45 yrs Bacillus Calmette- Guerin therapy has remained first line intravesical immunotherapy for superficial bladder cancer, an outstanding example of successful immunotherapy.KYAMC Journal Vol. 8, No.-1, Jul 2017, Page 56-60

    Abstract: Adenomatoid tumor is a very rare benign tumor of mesothelial origin. It is very difficult to distinguish carcinoma from an adenomatoid tumor. This is a report of a case of an adenomatoid tumor of the right epididymis of a man of 38-year-old who came to Urology OPD with pain and palpable swelling on the right testis. He also complaints of severe burning during ejaculation. We diagnose the case by ultrasonogram (USG) and Fine Needle Aspiration Cytology (FNAC). Right epididymectomy has done and histopathology revealed adenomatoid tumor.
