    Abstract: Introduction. Central auditory processing involve normal sound perception, speech recognition, ability of cognition and memory. Accordingly speech recognition difficulties may manifest due to changes at any segment of auditory processing. Cortical auditory evoked potentials (CAEPs) and behavioral measures provide insight into the neural mechanisms underlying speech recognition. These disorders are observed both in young and elderly population. Aim of the study. To evaluate central auditory processing for subjects of different age and hearing level through the presentation of noise using CAEPs and behavioral measures of speech discrimination. However due to complicated connectivity in auditory pathway it is difficult to identify the central auditory processing damage. Behavioral tests in conjunction with electrophysiological examination will reveal more complicated information for differenciation between peripheral and central auditory processing disorders. Materials and methods. Three groups of subjects participated: young normal hearing, young hearing-impaired and elderly hearing-impaired subjects. To minimaze subject variables, the CAEPs (wave peaks P1, N1, P2, N2, P3, N3 in miliseconds) was investigated using passive listening paradigme. The CAEPs were elicited by 1,1s change in frequency 1000 Hz and 2000 Hz in pure tones presented at 65, 70 and 75 dB SPL. Sentence recognition tests in quite and noise, Digit Pairs (DP) and Word Pairs (WP) were developed in Latvian language prior to investigation. They were presented to all subjects. Results. The most prominent finding was the increased latency of P3, N3 in elderly and also in younger hearing impaired adults groups and highly differed within groups. More prolonged latencies were find of N1, P2 in elderly hearing impaired group than in younger hearing impaired group adults. Conclusions. During this investigation the CAEPs are performed for the first time in our clinic therefore the main standarts are determined for our laboratory. The speech recognition tests (Sentence recognition test, DD, DW) are developed in Latvian. The present findings suggest patterns of CAEP are variable within individuals however shows that auditory perception and cognitive function is not only a result of aging and must be associated with a general slowing of neuronal processing or decreased neuronal synchrony within the central auditory nervous system. Determination of central auditory processing capacity level is of crucial significance to prognose and evaluate the hearing result after hearing prosthetics and to appraise indications for hearing prosthetics, cochlear and middle ear implantation including.

    Abstract: Objectives: Analyze the literature data on the incidence and manifestation of carpal tunnel syndrome (CTS), as well as the pathogenesis and treatment options. Perform a specially designed, validated test - Pain Detection. Evaluate the objective state of patients with CTS - sensory impairment, compression test. Assess patients’ neurologic and neurophysiologic data before and after the blockade and evaluate its effectiveness. Perform a specially designed, validated test - Patients` Global Impression of Change scale (PGIC) one month after corticosteroid injection (CSI).Methods and Materials: The study includes an analysis of 55 arms of patients of different age with mild and moderate CTS who came for a neurological examination at the Neurology Outpatient Department of the Pauls Stradiņš Clinical University Hospital during the period of 01.08.2018 – 01.01.2019. All patients were analyzed clinically and neurophysiologically before CSI and one month after CSI. A Pain Detect scale, PGIC scale, compression tests and sensory tests were used for the evaluation of clinical symptom. A median nerve sensory and motor nerve conduction study was performed.Results: According to the Pain Detect scale, 60% of patients showed neuropathic pain before CSI, and 78% of patients presented clinical effectiveness after CSI. 98% of patients present clinical effectiveness after CSI in the PGIC scale. 85% of patients had improvement in neurophysiological studies – motor distal latency decreased after CSI. Before CSI, the average motor distal latency was 5.7ms (range 4.5-12.9ms SD±1.5), which was on average 130% from the maximal norm (range 102- 293 SD±36). After the CSI, the average motor distal latency was 5.2ms (range 3.8-10.7ms SD±1.3), which was on average 120% from the maximal norm (range 88-243 SD±30). We didn’t find any significant correlation between the improvement of the patient's clinical condition and the improvement of electrophysiological outcomes.Conclusions: The study concludes that the Pain Detect sensitivity for neuropathic pain evaluation of patients with CTS is 60%. Patients show clinical and neurophysiological improvement after CSI, but there is no correlation between neurophysiological and clinical improvement. The study concludes that the PGIC scale can be used to quickly assess the effectiveness of therapy.

    Abstract: The aim of the study was to clarify correlations between body mass index (BMI), blood pressure (BP), and serum levels of cytokines in female migraine patients. A total of 14 migraineurs with aura, and 12 without aura during their interictal period were compared with 25 controls. Interleukin-8 (IL-8), soluble intercellular adhesion molecule-1 (sICAM-1), soluble vascular cell adhesion molecule-1 (sVCAM-1), matrix metalloproteinase-9 (MMP-9), interferon gamma (IFN-γ), monocyte chemoattractant protein-1 (MCP-1), transforming growth factor alpha (TGF-α), and plasminogen activator inhibitor-1 (PAI-1) were measured. Migraineurs have elevated levels of IL-8, but decreased serum levels of PAI-1 and sICAM-1 during the interictal period, regardless of aura. BMI correlates with BP, and also with IFN-γ and MMP-9 only in patients with aura. There are three correlations in migraine patients with aura that are absent in patients without aura: between IL-8 and PAI-1; MMP-9 and IL-8; and IL-8 and sICAM-1. Migraineurs without aura, on the other hand, have correlations that patients with aura do not have (between PAI-1 and MCP-1, sICAM-1; between MMP-9 and sICAM-1, MCP-1; between TGF-α and PAI-1, MMP-9, sICAM-1; between sICAM-1 and MMP-9, PAI-1, MCP-1; as well as between sVCAM-1 and MCP-1). PAI-1, TGF, and MMP-9 could be used as biomarkers to distinguish migraineurs from healthy individuals.

    Abstract: Loss of an arm is a devastating condition that can cross all socioeconomic groups. A major step forward in rehabilitation of amputees has been the development of myoelectric prostheses. Current robotic arms allow voluntary movements by using residual muscle contraction. However, a significant issue is lack of movement control and sensory feedback. These factors play an important role in integration and embodiment of a robotic arm. Without feedback, users rely on visual cues and experience overwhelming cognitive demand that results in poorer use of a prosthesis. The complexity of the afferent system presents a great challenge of creating a closed-loop hand prosthesis. Several groups have shown progress providing sensory feedback for upper limb amputees using robotic arms. Feedback, although still limited, is achieved through direct implantation of intraneural electrodes as well as through non-invasive methods. Moreover, evidence shows that over time some amputees develop a phantom sensation of the missing limb on their stump. This phenomenon can occur spontaneously as well as after non-invasive nerve stimulation, suggesting the possibility of recreating a sensory homunculus of the hand on the stump. Furthermore, virtual reality simulation in combination with mechanical stimulation of skin could augment the sensation phenomenon, leading to better interface between human and robotic arms.

    Abstract: Fibromyalgia (FM) is a chronic widespread pain disorder that impacts 0.5%-7% of the general population worldwide. The aetiology and pathogenesis of the disease are still unknown. Human herpesvirus-6 and -7 belong to the family Herpesviridae, subfamily Betaherpesvirinae, and genus Roseolovirus and are immunomodulating viruses potentially pathogenic to the nervous system. Presence of anti-HHV-6 and -HHV-7 antibodies and viral genomic sequences, viral loads, HHV-6 variant-specificity, and TNF-α level were studied in 41 FM patients and 50 healthy individuals using polymerase chain reactions, restriction endonuclease analysis and ELISA. There was no difference in the presence of anti-HHV-6 and anti-HHV-7 IgG class antibodies between FM patients and control group individuals. Viral sequences were found in 80.5% of FM patients and in 62.0% of controls. Significantly higher rate of concurrent HHV-6 and HHV-7 infection and higher viral loads in peripheral blood were detected in FM patients compared to the control group individuals. Plasma viremia was detected only in FM patients. Significantly higher TNF-α levels were detected in virus positive FM patients. From all positive cases only in two FM patients HHV-6A was revealed. Significantly higher detection frequency of concurrent HHV-6 and HHV-7 infection, simultaneous HHV-6 and HHV-7 activation, higher viral loads and TNF-α expression levels in primary FM patients than in control group individuals indicate the potential involvement of Roseoloviruses in development of this disorder.

    Abstract: Multiple sclerosis (MS) is a neurological disease of unknown aetiology. Several research groups reported an increased level of human endogenous retroviruses HERV-W and HERV-H RNAs in cerebrospinal fluid, plasma and supernatants of cell cultures from MS individuals. To quantify the abundance of extracellular virion-associated HERV, RNAs in blood, plasma samples from Latvian MS patients, patients with other neurological diseases (OND), as well as blood donors (BD), were retrospectively studied by using both our previously published and newly developed quantitative Real-time reverse transcription PCR assays (QPCRs) targeting different polymerase (pol) gene regions of HERV-W and HERV-H. Unspecific signals due to incomplete removal of DNA were monitored by running the assays with and without reverse transcription (RT±) in parallel. According to our score, a few MS, OND and healthy controls gave borderline signals simultaneously with both newly developed HERV-H and HERV-W QPCRs, but the rest were negative. All borderline positive samples also had small amounts of non-retroviral cellular mRNA with possible origin from cell-free circulating RNA fragments, apoptotic bodies or exosomes, which can mimic the previously described virus-like particles. The results do not confirm the previous reports on prevalence of HERV-H or -W virion-associated RNA in plasma of MS patients.

    Abstract: Fibromialģija (FM) ir hroniska slimība, kas izpaužas ar difūzām muskuloskeletālām sāpēm, nogurumu, miega un emocionāliem traucējumiem. Slimība, iespējams, ir saistīta ar A delta un C nervu šķiedru disfunkciju. Termāla kvantitatīva sensora testēšana (QST) analizē mazmielinizētas A delta nervu šķiedras un nemielinizētas C nervu šķiedras, kas atbild par nociceptīvo sensoru sistēmu un spinotalāmisko ceļu. Pieņemot, ka FM sāpēm ir neiropātisks raksturs, pieaug arī QST vērtība kā papildus diagnostiskam testam. Pētījumā tika iekļauts 51 pacients. Slimības simptomi tika objektivizēti, izmantojot Noguruma smaguma skalu (Fatigue Severity Scale), Fibromialģijas ietekmes anketu (Fibromialgia Impact Questionnaire) un 2010. gada American College of Rheumatology (ACR) FM diagnostiskos kritērijus. QST tika veikta ar termālo stimulu pēdu un plaukstu dorsālajās virsmās. QST rezultāti tika salīdzināti ar 20 atbilstoša vecuma un dzimuma kontroles grupas pacientiem. FM pacientiem tika konstatēta ievērojama aukstuma un siltuma percepcijas un sāpju sliekšņu atšķirība, salīdzinot ar kontroles grupas pacientiem, un tas norāda uz neiropātisko sāpju raksturu FM pacientiem. Izmaiņas ir vairāk izteiktas siltuma percepcijas un karstuma sāpju sliekšņos. Iespējams, tas liecina, ka FM pacientiem C šķiedras ir vairāk skartas un siltuma percepcijas un karstuma sāpju sliekšņi ir jūtīgākas modalitātes, ko var izmantot FM diagnostikā. Tika arī atklātas statistiski ticamas negatīvas korelācijas starp siltuma un aukstuma percepcijas sliekšņiem un starp karstuma un aukstuma sāpju sliekšņiem, kas var norādīt uz centrālo sensibilizāciju vai defektīvo sāpju inhibējošu sistēmu FM pacientiem.
