    Abstract: Background: B-cell chronic lymphocytic leukemia (CLL) is a common blood cancer characterized by high prevalence of malignant B cells in peripheral blood. Small lymphocytic lymphoma (SLL) is considered to be a different presentation of the same disease, with the malignant B cells primarily localized in lymph nodes. Natural killer (NK) cells are innate immune effectors that can spontaneously identify and kill malignant cells, especially hematopoietic cancers. In peripheral blood of CLL patients, NK cells are chronically exposed to significant tumor burden, which is predicted to influence their phenotype and function. Effective NK cell function may be particularly beneficial in CLL patients, since commonly-used monoclonal antibody therapies (e.g. rituximab, alemtuzumab) rely at least partially on ADCC-mediated by NK cells. Methods: We performed a prospective analysis of biomarkers on fresh peripheral blood lymphocytes from 25 untreated CLL patients, 10 untreated SLL and 17 age-matched healthy controls by 10-color flow cytometry. All subjects signed IRB approved informed consent forms. Our study analyzed 180 distinct biomarker parameters, with a particular focus on NK and T cells. Differences in biomarker expression between patients with SLL, CLL, and healthy controls were compared by Wilcoxon rank-sum test. Results: Absolute numbers of NK and T cells per µl of blood were significantly higher in CLL patients, and this correlated with increased B cell numbers. As indicators of immune suppression, the frequency of regulatory T cells was significantly increased in CLL samples, as were levels of PD-1 expression on T cells and CD56dim NK cells. NK cells in CLL expressed higher levels of CD27, which is characteristic of a less mature phenotype, and CD56dim cells expressed lower levels of NKG2D. Compared to healthy controls, CLL samples displayed a marked reduction in degranulation by CD56dim NK cells in response to transformed 721.221 B cells, either with or without rituximab. CD56dim NK cells from CLL patients were also less viable under resting conditions or when challenged with target cells, especially in ADCC responses. We further observed a striking reduction in the frequency and viability of KIR3DL1+ NK cells, which progressed over time in most CLL patients. Surprisingly, CLL patients with the highest levels of PD-1 expression on NK cells possessed genes for both KIR3DL1 and its ligand, HLA-Bw4. Our findings were also clearly evident in a CLL patient compared to her healthy monozygotic twin, thereby providing compelling support for the results in the full patient cohort. The altered expression levels of nearly all of the NK cell biomarkers and degranulation were less pronounced in blood samples from SLL patients, presumably due to low tumor burden in peripheral blood. Conclusions: CLL patients have increased numbers of NK cells in peripheral blood, but these NK cells are less mature, are significantly depleted of the KIR3DL1+ subset, and have deficits in degranulation response, reduced expression of NKG2D activating receptor, increased expression of inhibitory PD-1, and enhanced susceptibility to activation-induced death when challenged with tumor targets and rituxumab. Our findings support the hypothesis that immune dysfunction in CLL may be due in part to a selective loss of mature KIR3DL1+ NK cells, possibly upon encountering overwhelming tumor burden in peripheral blood, and CLL patients may benefit from therapeutic strategies that augment NK cell function.     Abstract: BACKGROUND: Interaction between the immune system and follicular lymphoma (FL) is well recognized from immunohistochemical as well as microarray studies, but is not yet fully elucidated. A recent report suggested that a higher absolute lymphocyte count (ALC) predicted a longer time to progression and improved response to rituximab in FL (Behl et al, Br J Haem2007). The GELA group reported a correlation between peripheral blood natural killer cell count and event free survival in diffuse large B cell lymphoma (Plonquet et al Ann Oncol2007). The influence of lymphocyte subsets in the peripheral blood (PB) on FL outcome has not been reported. PATIENTS AND METHODS: We retrospectively analyzed the immunophenotype from peripheral blood flow cytometry (PBFC) at Fox Chase Cancer Center in FL patients (pts). We identified 127 pts (82 newly diagnosed and 45 previously treated). We recorded the presence or absence of circulating monoclonal B cells (CLC) and survival in all as well as a complete PBFC panel in & gt; 90% of pts. FLIPI and IPI scores were calculated from data at the time of diagnosis. Overall survival and time to treatment from date of original diagnosis and from date of PBFC were analyzed in relation to ALC, CLC, the lymphocyte subsets CD3+CD4+, CD3+CD8+, CD3−CD56+/CD16+ (NK cells) and FLIPI and IPI scores. A low CD4 count was defined as & lt;500 cells/mm3, a low NK cell count as & lt;150 cells/mm3 and a low ALC as & lt;1500 cells/mm3. RESULTS: There were 63 females and 64 males; median age at presentation was 56 yrs (range 31–88). Median follow up intervals from original diagnosis and from PBFC were 46 months (range 3.5 – 207) and 24 months (range 0.7–138), respectively. This abstract deals principally with the 82 newly diagnosed pts. Median follow up in the newly diagnosed subset was 27.8 months (range 0.7 to – 138); thirty-five pts were subsequently treated, and 47 pts remained under observation. FLIPI scores at diagnosis were: low - 48 pts, intermediate - 25, high - 7 and unknown - 2. IPI scores were: low - 52 pts, low-intermediate - 24, high-intermediate/high - 4 and unknown - 2. CLC were present in 21% of pts. At last follow up, 74 pts were still alive. By univariate analysis, the most significant predictors for inferior overall survival (OS) from time of PBFC were low CD4 count (P = 0.03), low NK cell count (P = 0.02), and presence of CLC (P = 0.03). There was insufficient power for significance in multivariate analysis. At last follow up, survival for the low vs. high CD4 and NK cell groups was 85% vs. 97% and 85% vs.98%, respectively. FLIPI and IPI scores, low ALC, and low CD8 did not correlate with OS from time of diagnosis or time of PBFC. With respect to time to initial treatment, only a low ALC (P & lt;0.001) and FLIPI (P & lt;0.001) were significant. In multivariate analysis for OS of all 127 pts, low NK cell count remained significant (P = 0.03), while low CD4 count was marginally significant (P = 0.08). CONCLUSION: Analysis of PB lymphocytes by PBFC has prognostic value in previously untreated FL pts. Low CD4 and NK cell counts as well as the presence of CLC are the most significant predictors for OS in newly diagnosed FL. Our findings suggest the importance of an intact immune system. They merit further prospective studies to ascertain the impact of treatment on subsets of pts with specific immunophenotypes and may prove useful in FL management.

    Abstract: A linear DNA molecule may be labelled at a fixed locus by a minute complementary radioactive molecule. A collection of identical molecules is to be so labelled and each one independently cut at a random number, N , of random places, X ( N Poisson, X uniform). Fragments containing the label are to be collected and assayed by length. It is shown that the recovery pattern (fragment length distribution) contains a jump discontinuity at the fixed locus and may be used to determine the distance between the attachment site and the nearest end of the molecule. The recovery pattern under the hypothesis that the collection of molecules are circularly permuted, i.e. the labelled locus is uniformly distributed over the length of the molecule, does not contain such discontinuities. The case where the labelling molecule has a non-negligible extent is also treated.

    Abstract: Administration of either lamivudine (2′-deoxy-3′-thiacytidine) or l -FMAU (2′-fluoro-5-methyl-β- l -arabinofuranosyluracil) to woodchucks chronically infected with woodchuck hepatitis virus (WHV) induces a transient decline in virus titers. However, within 6 to 12 months, virus titers begin to increase towards pretreatment levels. This is associated with the emergence of virus strains with mutations of the B and C regions of the viral DNA polymerase (T. Zhou et al., Antimicrob. Agents Chemother. 43:1947–1954, 1999; Y. Zhu et al., J. Virol. 75:311–322, 2001). The present study was carried out to determine which of the mutants that we have identified conferred resistance to lamivudine and/or to l -FMAU. When inserted into a laboratory strain of WHV, each of the mutations, or combinations of mutations, of regions B and C produced a DNA replication-competent virus and typically conferred resistance to both nucleoside analogs in cell culture. Sequencing of the polymerase active site also occasionally revealed other mutations, but these did not appear to contribute to drug resistance. Moreover, in transfected cells, most of the mutants synthesized viral DNA nearly as efficiently as wild-type WHV. Computational models suggested that persistence of several of the WHV mutants as prevalent species in the serum and, by inference, liver for up to 6 months following drug withdrawal required a replication efficiency of at least 10 to 30% of that of the wild type. However, their delayed emergence during therapy suggested replication efficiency in the presence of the drug that was still well below that of wild-type WHV in the absence of the drug.

    Abstract: During a hepadnavirus infection, viral DNA integrates at a low rate into random sites in the host DNA, producing unique virus-cell junctions detectable by inverse nested PCR (invPCR). These junctions serve as genetic markers of individual hepatocytes, providing a means to detect their subsequent proliferation into clones of two or more hepatocytes. A previous study suggested that the livers of 2.4-year-old woodchucks ( Marmota monax ) chronically infected with woodchuck hepatitis virus contained at least 100,000 clones of 〉 1,000 hepatocytes (W. S. Mason, A. R. Jilbert, and J. Summers, Proc. Natl. Acad. Sci. USA 102: 1139-1144, 2005). However, possible correlations between sites of viral-DNA integration and clonal expansion could not be explored because the woodchuck genome has not yet been sequenced. In order to further investigate this issue, we looked for similar clonal expansion of hepatocytes in the livers of chimpanzees chronically infected with hepatitis B virus (HBV). Liver samples for invPCR were collected from eight chimpanzees chronically infected with HBV for at least 20 years. Fifty clones ranging in size from ∼35 to 10,000 hepatocytes were detected using invPCR in 32 liver biopsy fragments (∼1 mg) containing, in total, ∼3 × 10 7 liver cells. Based on searching the analogous human genome, integration sites were found on all chromosomes except Y, ∼30% in known or predicted genes. However, no obvious association between the extent of clonal expansion and the integration site was apparent. This suggests that the integration site per se is not responsible for the outgrowth of large clones of hepatocytes.
