Kurzfassung: Background: Blended learning offers opportunities for the complexity of learning in clinical education. Student peer assessment is widely used as a form of formative assessment in early clinical exposure programs, especially clinical communication skills training. This study aimed to describe clinical communication skills competencies of second-year students and to identify the relationships between peer and faculty assessment of communication skills in a blended learning program format. Methods: A total of 474 second-year general medical students and dental students participated in the study. Peer and lecturer assessment forms with a 5-point Likert scale according to the Calgary-Cambridge guide format were used to evaluate students’ performance of basic communication skills, relationship building, and history taking. Pearson’s correlation coefficients and paired t-test were applied. A p-value 〈 0.05 was considered statistically significant. Results: Most of students were rated at distinction level (score at 7-8.4) in communication skills. Mean of the overall score by peer and faculty assessment were 7.46 ± 1.03 and 7.17 ± 0.68, respectively. Peers rarely provided negative ratings on subcategories of communication skills. Skills of understanding the patient’s perspectives and gathering information were the most reported skills needed to improve among students. Significant positive correlations were found between peer and faculty evaluations for building relationship, establishing initial rapport, and gathering information domains (p 〈 0.01). Students tended to grade their colleagues higher for building relationship (3.88 ± 0.62) and establishing initial rapport domains (3.72 ± 0.61) than other domains, meanwhile, teachers tended to grade building relationship (3.80 ± 0.55) and gathering information domains (3.64 ± 0.38) higher than other domains. Conclusion: The findings suggest that student peer evaluation can be valuable for clinical education. As part of a formative assessment, it can be also used for faculty to evaluate students’ clinical communication skills performance in innovative medical education programs. Key words: peer assessment, clinical communication skills, practice of medicine, early clinical practice, blended learning.

Kurzfassung: Patient satisfaction has been considered an important metric to measure and improve the quality of health care services. Aims: This study aimed to capture patient satisfaction of the quality of services and explore the disparities in patients’ satisfaction by different type of health facilities. Methods: A cross-sectional investigation was conducted in 886 participants following the guidelines of Ministry of Health on assessment of patients’ satisfaction of out-patient services with 5 main indices: the average patient satisfaction score (PSI), the average patient satisfaction percentage, comprehensive satisfaction index (CSI), percentage of continuing to utilize health services, and percentage of perceiving health care expectaion. Results: The PSI of outpatient services among all partcipants reached 3.75 out of 5 points, corresponding to a satisfaction percentage of 64.8%. Of which, the PSI on different attributes of healthcare settings varied from 3.69/5 (lowest for district-level health facilities) to 3.92/5 (highest for private health settings). CSI was at 13.5%, the percentage of continuing to utilize health services reached 96.2% and people perceived 81.5% of their expectation. Primary care facilities have lower levels of satisfaction than the provincial, central and private facilities (p 〈 0.05). Conclusion: Robust efforts should be taken in investing the infrastructure and performance of primary care settings, and improving the administrative procedures of public health settings. Furthermore, local authority should pay attention to promote the awareness of community to availability of health services at primary care settings.

Kurzfassung: Patient-centered care is widely acknowledged as an important goal in healthcare delivery. Research has demonstrated that patient perceptions of patient-centred care can be linked to clinical and long-term outcomes in addition to being a useful metric for quality improvement efforts. Objectives: 1) To identify patients’ perspectives and experiences of patient-centerd care in some communes in Thua Thien Hue province; 2) To determine the association between the patients’ perspectives, experiences on patient-centered care and utilization of the health services. Methods: A cross sectional descriptive study was conducted in 313 patients living in Thua Thien Hue province. A structured questionnaire was use to investigate 4 main domains of patient-centered care: Information and Education, Relationships with doctors and other health professionals, making decisions, motivate patients about self-management. We used test ANOVA to analyze the association between patient-centered care and health care utilization among participants (p 〈 0.05). Results: The percentage of good patient-centered care (≥ 3.75 points) was 43.1% (Mean = 3.58 (0.57)). Patients using health services at the commune health centers had a higher experience in patient-centered care compared to district, province and central hospitals (p 〈 0.005). Domains “Encourage patients to take care of themselves” and “Relationship factor with doctors” were evaluated low. There were statistically significant associations between patient-centered care and primary health care facilities, quality of life utilization of periodic health checkup, and patient activation in health care (p 〈 0.05). Conclusion: Out study illustrates that patients’ perspectives and experiences of patient-centered care are still low. There is a need to improve communication and counseling skills of health care providers and increase patient motivation for self-care Key words: patient-centered care, primary care, health care utilization

Kurzfassung: Background: Communication Skills (CS) in Medicine has been considered one of the most fundamental competencies to be required of medical professionals. However, there is not guarantee that students fully grasp the usefulness of CS for clinical practice. The objectives of our study were therefore to (1) describe Year 2 to Year 5 medical students’ attitudes towards CS training and its associations, and to (2) investigate Year 2 to Year 5 medical students’ self-assessment of CS training and effective medical CS training. Methods: This is a non-randomized control trial study, using the Communication Skills Attitudes Scale (CSAS) designed by Rees from Nottingham University and Gap-Kalamazoo Communication Skills Assessment Form (G-KCSF) designed by Rider from Harvard University. Results: In general, students showed positive attitudes towards CS training (positive attitude scale (PAS): 3.55 (0.5); negative attitude scale (NAS): 2.77 (0.43). Females showed higher scores on the PAS than males (p 〈 0.001) and lower scores on the NAS (p 〈 0.001). The second- or third-year medical students showed higher scores on the PAS (p 〈 0.001) than their fourth- and fifth-year counterparts. The scores of all essential elements of CS were highest among the second-year students, decreasing among the third- and fourth-year students before increasing again among the fifth-year students. The students who had medical CS training scored higher on the PAS, lower on the NAS, and on all essential elements of CS. Conclusions: Medical students at Hue University of Medicine and Pharmacy expressed positive attitudes towards CS training. Training medical students in CS was able to improve their attitudes and CS.

Kurzfassung: Background: Assessing students’ learning readiness has become more important to establish on-target learning and teaching environments, particularly since blended learning and online learning are widely implemented in medical education. This study aims (1) to examine the reliability and validity of Online Learning Readiness Scale (OLRS) and (2) to identify the levels of online learning readiness for blended learning methods using virtual reality and its associations. Methods: A cross-sectional study with 102 fifthyear medical students was conducted. The data were analyzed through exploratory factor analysis and confirmatory factor analysis. Pearson correlation analysis was utilized to investigate the relationships among dimensions of OLRS. Results: The VN-version of the OLRS scale consists of 16 items in four dimensions, namely Self-directed learning - SDL, Motivation for learning - MFL, Computer & Internet self-efficacy - CIS, Online communication self-efficacy - OCS. Confirmatory factor analysis indicates the model fit of the OLRS scale in a blended learning course context. The composite reliability and construct validity of the scale was found to be within the acceptable range. Students showed high readiness for blended learning course, of which SDL was the dimension having the highest score of readiness, followed by MFL and CIS dimensions. There were positive correlations across all dimensions of OLRS (p 〈 0.001). Statistically significant associations were found between gender, health professional orientation, computer self-efficacy and their experience of medical education environment and dimensions of OLRS. Conclusion: The results indicate that the OLRS is a reliable and valid measurement tool in the assessment of online learning readiness levels of blended learning course using virtual reality. It is recommended to develop a reliable scale to assess the perceptions and readiness of instructors undertaking the move to blended teaching. Key words: confirmatory factor analysis, blended learning, Online Learning Readiness Scale, virtual reality, medical education

Kurzfassung: Background: Medical students play important frontline roles in the prevention, early detection, and treatment of hepatitis C. This study investigated knowledge and attitudes toward hepatitis C among 5th- and 6th-year medical students and possible associated factors. Methods: A cross-sectional survey was conducted among 2000 students from eight medical universities using a self-administered structured questionnaire. Results: The mean knowledge and attitude scores for hepatitis C were 20.1 ± 4.0 (out of 26) and 10.6 ± 2.9 (out of 20), respectively. Approximately, three-quarters (74.4%) of the participants had a good knowledge score, but only a small proportion (3.1%) obtained a good attitude score. Although the participants had fairly high knowledge about the causes, consequences, and transmission routes of hepatitis C, there were important gaps in their knowledge about hepatitis C screening and treatment. In multivariate analysis, female students, 5th-year students, and students from the central provinces had significantly higher knowledge and attitude scores. There was a low positive correlation between knowledge and attitude scores. Conclusion: This study points out the need to update the medical training curriculum to improve the knowledge and attitude of students about hepatitis C infection.
