    Abstract: In recent years, dental malpractice claims have increased dramatically worldwide. The purpose of the present study is to analyze claims related to orthodontic treatment involving periodontal problems that resulted in legal decisions in Israel. This study analyzed legal claims registered by Medical Consultants International (MCI) between 2005 and 2018. Only closed cases of orthodontic claims involving periodontal problems in which a decision was made were included. The parameters studied included patients’ demographic data, the main reasons of the claim, and complications. Statistical significance was found for aesthetic damage, which was more common in claims of females (p = 0.035) and in older claims (p = 0.004); tooth damage was more common in claims of older patients (p = 0.032); violation of autonomy was higher in private practice (p = 0.047) and in more recent claims (p = 0.001). As orthodontic treatment is becoming more popular in older patients, and as lawsuit claims become more common in recent years, the orthodontists should always analyze and document the periodontal status of their patients before and during treatment in order to maintain professional practice and avoid future claims.

    Abstract: To study the trigeminal nerve neuropathy prevalence after mandibular fractures and follow nerve recovery and to examine associations with clinical features. Subjects: The files of 304 patients treated for 424 mandibular fractures in Hadassah Medical Center between 2001 and 2008 were analyzed. Twenty-five patients with paresthesia were examined by electro-diagnostic conduction test (quantitative sensory testing). Results: The most common fracture sites were the symphysis (28.5%) and condyle (21.4%). A strong correlation between fracture severity and treatment modality was found. Nerve damage occurred in 13.4% of nondisplaced fractures and in 65.3% of 〉 5 mm displaced fractures. Patients who were treated by open reduction internal fixation demonstrated the biggest difference in quantitative sensory testing values between the affected and nonaffected sides. C nerve fibers were the most damaged and least regenerated. There was correlation between damage perception and objective test results. Conclusions: There is a correlation between fracture type and displacement severity on neurologic deficit and prognosis. The objective neurosensory damage was manifested by higher nerve excitation threshold on the injured side. Patients should be informed on the prognosis of neurologic deficit according to fracture type.

    Abstract: Purpose: The aim of this study was to evaluate the survival rate of immediately provisionalized implants with up to 5 years follow‐up. Materials and Methods: The study consisted of 226 patients, 113 consecutive patients with immediately provisionalized dental implants (cases) and 113 randomly selected, age‐, gender‐, and implant position‐matched controls with conventional late implant loading. Survival rate and incidence of complications were recorded. Results: Follow‐up ranged from 6 to 60 months. Smoking was reported by 20.8% of patients. Maxillary incisors and mandibular lateral incisors were the most common areas for implant placement. Conventionally loaded implants were narrower ( p  = .03) and shorter ( p  = .001). Immediate implantation into a fresh extraction socket was performed in 69% of the cases and in 36.3% of the controls ( p  = .001). Implant survival rate was 96.5%. Of the eight failed implants, six were immediately provisionalized and two were conventionally loaded. No statistically significant difference was found in survival rates between groups ( p   〉  0.05). Five of the failed implants (case group) were immediately loaded implants placed in fresh extraction sockets. Conclusion: Immediate implant provisionalization achieved similar high success rates compared with the conventional, delayed approach. As immediate implant provisionalization is mainly desired in the anterior region, the high success rates are encouraging.

    Abstract: The research objectives were to determine the prevalence of impacted teeth in orthodontic patients in Israel, examining the epidemiological and genetic linkage. Methods: Data was collected from the cohort of patients treated in Tel-Aviv University between 2010–2017. Forty-five patients treated for impacted teeth were randomly selected, compared to 45 control patients, paired by age and sex. Collected data included: week of birth, type of birth, medical history, dental anomalies, treatment referee, chief complaint, treatment modalities and treatment duration. Results: The periodic prevalence of impacted teeth was 11.9%. There was a statistical tendency in the week of birth and type of birth (earlier birth week and more caesarean sections in the study group). Statistical significance was found for systemic disease, chief complaint, referee for treatment and treatment duration. Dental anomalies for the patient or first-degree relatives found odds ratio for the presence of an impacted tooth up to 6 times higher. Conclusions: There is epidemiological data related to impacted teeth, such as preterm births and caesarean sections, with the need for further research.

    Abstract: Background: The aim of this study was to examine whether the thickness of the preoperative soft tissue affects the degree of soft tissue movement relative to the hard tissue movement after orthognathic surgery. Methods: This is a retrospective examination of lateral cephalometric X-rays of patients who underwent orthognathic surgery in single medical center, during a 5-year period. Demographic and operative data were collected. Soft tissue thickness was measured in preoperative cephalometric X-ray, followed by superimposition of postoperative ( 〉 6 months) cephalometric X-rays by overlapping fixed bony points. Results: In the upper jaw, there was a decrease in the relative movement of soft tissue with an increase in the initial thickness in surgeries performing advancements up to 5 mm (r = −0.288). In mandibular advancements, there was a clear decrease in soft tissue movement ratio with an increase in the initial soft tissue thickness (r = −0.418), while there was no correlation in mandibular setback cases (r = 0.062). The same correlation between the decrease in relative soft tissue movement and the increase in initial soft tissue thickness was found in advancement genioplasty (r = −0.411). Conclusion: This research shows a general pattern of decrease in relative movement of soft tissue in orthognathic surgery correlated to an increase in its initial thickness.

    Abstract: Background: The aim of this study was to evaluate the survival rate of immediately provisionalized dental implants immediately placed into fresh extraction sockets. Methods: The study consisted of 87 consecutive patients, ranging in age from 21 to 76 years (average: 53.30 ± 13.34 years), who received 210 immediately provisionalized, immediately placed dental implants between the years 2000 and 2005. Data were recorded regarding the survival rate of these implants and the incidence of complications. Results: Follow‐up ranged from 6 to 52 months (mean: 15.60 ± 12.60 months). Smoking, past and present, was reported by 28.7% and 20.7% of patients, respectively. The maxillary incisors were replaced most frequently, followed by the mandibular lateral incisors. Most of the implants were 〉 13 mm in length and ≥3.75 mm in diameter. There were 47 (22.4%) single restored implants and 163 (77.6%) splinted implants. Overall, the implant survival rate was 97.6% (five implants failed). Complications, such as swelling, inflammation, and pain, were observed in 24 (11.4%) of the implants. No relation was found between complications and failure. The present study failed to reveal a relationship between implant survival rate and smoking, implant dimensions, and area of implantation. Conclusion: Immediately provisionalized immediate implants can serve as a predictable procedure with high survival rates.

    Abstract: Background: The aim of this study was to assess the long‐term success and survival rates of implants replacing a single molar between two natural teeth and to evaluate the influence of implant characteristics on implant success. Methods: The study was based on a consecutive cohort of 81 patients who received implants to replace a single molar between the years 1994 and 2004. Inclusion criteria for patients were having an implant replacing a molar between two natural teeth and follow‐up data of at least 6 months. Data were recorded regarding the incidence of complications and success and survival rates of these implants. Results: The range of follow‐up was from 6 to 125 months (mean: 36 months). Smoking was reported by 18.5% of patients. The replacement of a mandibular molar was more frequent (87.7%), with 25.9% of the implants placed immediately after tooth extraction. Two implants were used to replace a single molar in seven patients (8.6%). The failure rate was 7.4% (six implants failed: three had broken necks, and three failed because of infection or bone loss). Complications included suppuration in 11.1% of implants and a pocket around the implant in two patients (2.5%). No relation was found among failure, complications, timing of implant placement, and smoking habits. Conclusion: A single implant can serve as a good long‐term and predictable treatment modality to replace a single molar with low complication and failure rates.
