    Abstract: Objective: An increased global dependence on digital screen use has resulted in a tremendous increase in screen-related ocular complaints such as dryness, ocular strain, burning sensation, grittiness, and transient blurring. This study aims to focus on the correlation between the duration of screen exposure and dry eyes among young medical students with the help of the combination of a questionnaire based on subjective symptoms of dry eyes and screen time and further tests to confirm dry eyes in those having symptoms. Numerous mechanisms have been mentioned in screen-associated evaporative dry eyes, including reduced blink rate, hazardous effects of blue light emitted from digital screens, and inflammatory changes. Methods: A cross-sectional study was conducted among 253 medical students at MIMS, Uttar Pradesh, from January 2021 to December 2022. Upon obtaining informed consent, all participants were given Schirmer 1 and tear film breakup time (TBUT) test. Patients with Schirmer 1 and TBUT reading 〈 10 mm were identified as dry-eye patients. After that, a pre-designed questionnaire was applied to all dry-eye participants that asked about different symptoms of dry eyes and screen time and the correlation between subjective symptoms of dry eyes, dry-eye tests, and screen time was evaluated. Results: About 20.8% of medical students had dry eyes, and those who used their phones for 2–5 h had statistically higher rates of the condition. The length of breaks was also found to be strongly correlated with dry eye. Conclusion: Prolonged duration of digital screen exposure in any form (laptop and mobile phones, etc.) is directly related to the risk of dry-eye disease in the long term. Majority of the students (75%) were willing to reduce their screen time as a preventive measure toward dry eye.

    Abstract: Objective: Children with cancer often develop febrile illnesses after cytotoxic chemotherapy. It is of paramount importance to determine which children have serious bacterial infections in this vulnerable period. We studied the ability of interleukin 6 (IL-6), high sensitivity C-Reactive Protein (hsCRP), tumor necrosis factor alpha (TNFα) and procalcitonin (PCT) to act as a sensitive diagnostic marker for bacterial sepsis in febrile neutropenic children and their utility as a predictive marker for serious infections. Design: Prospective cohort study. Setting: Sir Ganga Ram Hospital. Patients: Sixty-two episodes in children (aged 5 months to 17 yrs) with a known malignancy/aplastic anemia who presented with fever and neutropenia. Interventions: Serial blood samples were obtained (admission, and 48 hrs), and concentrations of PCT, IL-6, TNF α and hsCRP were determined. Demographic and laboratory data from the patients were collected from the medical records. Measurements and Main Results: Out of 62 episodes of febrile neutropenia, fifteen (24%) of the children had proven sepsis (13 positive blood cultures, 1 invasive aspergillosis and 1 local abscess with fever). Plasma levels of IL-6, hsCRP, TNFα and PCT were increased at all time points in children with sepsis compared with those without sepsis. In culture positive patients the value of TNF-alpha was higher but statistically not significant. No major differences were found in hsCRP concentrations. IL-6 at 0 and 48 hrs were reliable markers for sepsis (area under the curve 0.73 and 0.77, respectively). Using cutoff values of IL-6 & gt; 137.5 pg/mL at 0 hrs and & gt;69 pg/mL at 48 hrs of admission produced a screening test for sepsis with 67% sensitivity, 75% specificity at 0 hrs and 75% sensitivity and 78% specificity at 48 hrs respectively. Also using the cutoff values of 1.73ng/ml for procalcitonin at 0 hrs was a good screening test with 73%sensitivity and 70% specificity. When IL-6 cutoff was brought down to 20pg/ml at 0 hr then sensitivity increased to 100%. 42/62 children with febrile neutropenia had IL6 & gt;20 which included all 15 proven sepsis cases and all 6 deaths. Rest of 20 patients all had negative cultures and no deaths. PCT value of & gt;1 ng/ml was seen in 39/62 and included 12 of 15 cases of proven sepsis. PCT was & lt; 1ng/ml in a case of invasive aspergillosis and another case of candida line infection and a case of local abscess with fever. Conclusions: Our data shows the utility of a rapid and sensitive assay for interleukin-6 along with procalcitonin as a sensitive diagnostic marker of bacterial sepsis in febrile neutropenic children. The use of these markers as a clinical tool may allow for early discharge on oral antibiotics or no antibiotics in patients with IL6 less than 20 pg/ml and PCT less than 1 ng/ml.

    Abstract: Anomalous muscles donot usually cause symptoms but are of academic interest. Axillary arch is a variable muscular slip encountered in axilla. Purpose of the present study was to report a rare case of axillary arch muscle. Knowledge of this muscle variation and the possibility of finding it during axillary procedures are crucial for lymph node staging, lymphaedenectomy and for differential diagnosis in compressive pathologies of axillary vessels and brachial plexus. During routine dissection of left axilla in a 60-year old male cadaver, an anomalous muscular slip was encountered. The slip was extending between latissimus dorsi muscle to coracoid process, deltoid muscle, fascia covering biceps and coracobrachialis and was confirmed as axillary arch muscle. The embryological basis, genetics and clinical implications have been discussed. This rare variation will be of interest not only to anatomists but also for clinicians and surgeons dealing with this area.

    Abstract: The term “neurodegenerative disorders” refers to a group of illnesses in which deterioration of nerve structure and function is a prominent feature. Cognitive capacities such as memory and decision-making deteriorate as a result of neuronal damage. The primary difficulty that remains is safeguarding neurons since they do not proliferate or regenerate spontaneously and are therefore not substituted by the body after they have been damaged. Millions of individuals throughout the world suffer from neurodegenerative diseases. Various pathways lead to neurodegeneration, including endoplasmic reticulum stress, calcium ion overload, mitochondrial dysfunction, reactive oxygen species generation, and apoptosis. Although different treatments and therapies are available for neuroprotection after a brain injury or damage, the obstacles are inextricably connected. Several studies have revealed the pathogenic effects of hypothermia, different breathed gases, stem cell treatments, mitochondrial transplantation, multi-pharmacological therapy, and other therapies that have improved neurological recovery and survival outcomes after brain damage. The present review highlights the use of therapeutic approaches that can be targeted to develop and understand significant therapies for treating neurodegenerative diseases.

    Abstract: Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries.
