    Abstract: Survivors of polytrauma experience long-term and short-term burden that influences their lives. The patients’ view of relevant short-term and long-term outcomes should be captured in instruments that measure quality of life and other patient-reported outcomes (PROs) after a polytrauma. The aim of this systematic review is to (1) collect instruments that assess PROs (quality of life, social participation and activities of daily living) during follow-up after polytrauma, (2) describe the instruments’ application (eg, duration of period of follow-up) and (3) investigate other relevant PROs that are also assessed in the included studies (pain, depression, anxiety and cognitive function). Methods and analysis The systematic review protocol is developed in line with the Preferred Reporting Items for Systematic Review and Meta-Analysis Protocols statement. MEDLINE, EMBASE, Cumulative Index to Nursing and Allied Health Literature, PsycINFO, Cochrane Central Register of Controlled Trials and the trials registers ClinicalTrials.gov and WHO International Clinical Trials Registry Platform will be searched. Keywords, for example, ‘polytrauma’, ‘multiple trauma’, ‘quality of life’, ‘activities of daily living’ or ‘pain’ will be used. Publications published between January 2005 and the most recent date (currently: August 2016) will be included. In order to present the latest possible results, an update of the search is conducted before publication. The data extraction and a content analysis will be carried out systematically. A critical appraisal will be performed. Ethics and dissemination Formal ethical approval is not required as primary data will not be collected.     Abstract: We (1) collected instruments that assess health-related quality of life (HRQoL), activities of daily living (ADL) and social participation during follow-up after polytrauma, (2) described their use and (3) investigated other relevant patient-reported outcomes (PROs) assessed in the studies. Design Systematic Review using the Preferred Reporting Items for Systematic Review and Meta-Analysis guideline. Data sources MEDLINE, Embase, CINAHL, PsycINFO, CENTRAL, as well as the trials registers ClinicalTrials.gov and WHO ICTRP were searched from January 2005 to April 2018. Eligibility criteria All original empirical research published in English or German including PROs of patients aged 18–75 years with an Injury Severity Score≥16 and/or an Abbreviated Injury Scale≥3. Studies with defined injuries or diseases (e.g. low-energy injuries) and some text types (e.g. grey literature and books) were excluded. Systematic reviews and meta-analyses were excluded, but references screened for appropriate studies. Data extraction and synthesis Data extraction, narrative content analysis and a critical appraisal (e.g. UK National Institute for Health and Care Excellence) were performed by two reviewers independently. Results The search yielded 3496 hits; 54 publications were included. Predominantly, HRQoL was assessed, with Short Form-36 Health Survey applied most frequently. ADL and (social) participation were rarely assessed. The methods most used were postal surveys and single assessments of PROs, with a follow-up period of one to one and a half years. Other relevant PRO areas reported were function, mental disorders and pain. Conclusions There is a large variation in the assessment of PROs after polytrauma, impairing comparability of outcomes. First efforts to standardise the collection of PROs have been initiated, but require further harmonisation between central players. Additional knowledge on rarely reported PRO areas (e.g. (social) participation, social networks) may lead to their consideration in health services provision.     Abstract: Zusammenfassung. Hintergrund: Vor dem Hintergrund des steigenden Interesses an freiwilligen Berichts- und Lernsystemen in der stationären Langzeitpflege ist es notwendig, systematisch die Faktoren einer erfolgreichen Implementierung näher zu betrachten. Ziel des Beitrags ist es daher, den Nutzen dieser Meldesysteme darzustellen sowie konkrete Empfehlungen zu deren Implementierung anhand des Consolidated Framework for Implementation Research zu diskutieren. Einflussfaktoren einer erfolgreichen Implementierung: Eine erfolgreiche Implementierung hängt von einer systematischen Vorgehensweise ab. Folgende Einflussfaktoren sind nach der Analyse mit dem Consolidated Framework for Implementation Research wesentlich: (i) eine standardisierte und einfach handhabbare Gestaltung des Meldesystems; (ii) die Berücksichtigung externer Anreizsysteme; (iii) die Schaffung einer positiven Organisations- und Sicherheitskultur, die über das Implementierungsklima und Ressourcen die Fähigkeit zur Implementierung herstellt; (iv) die Wahrnehmung von Bedürfnissen, Wissensständen und individuellen Veränderungsprozessen; sowie (v) die frequente Evaluation des Implementierungsprozesses. Ausblick: Sichtbare Interventionen und eine Weiterentwicklung der Sicherheitskultur sind wichtige Schritte zur Förderung einer nachhaltigen Implementierung von Berichts- und Lernsystemen. Derzeit erscheint es notwendig, Barrieren von Implementierungsprozessen in den Blick zu nehmen. Es ist zu empfehlen, Messinstrumente zur Evaluation der Implementierungsprozesse passgenau zu entwickeln und zu erproben sowie Merkmale nachhaltiger Implementierung weiter zu untersuchen.

    Abstract: We aimed to investigate the perception of the implementation success of reporting and learning systems in German hospitals, the perceived relevance of the implementation outcomes and whether and how these implementation outcomes are monitored. An reporting and learning system is a tool used worldwide for patient safety that identifies and analyses critical events, errors, risks and near misses in healthcare. Methods A pretested exploratory cross-sectional online survey was conducted with reporting and learning system experts from 51 acute care hospitals. For communicative validation, the results were discussed in person in an expert panel discussion (N=23). Results Fifty-three per cent (n=27) of the participants (N=51) of the online survey perceived that their reporting and learning system was being comprehensively and successfully implemented. However, no service or patient outcomes were reported to ultimately capture the concept of implementation success. Most of the participants reported a (high) relevance of the implementation outcomes’ acceptability and sustainability. In total, 44 measures were provided to monitor implementation outcomes. However, most of the quantitative measures were based on the (relative) number of entered reports. Qualitative measures were reported in relation to the ‘quality of the report’. In general, the measures were poorly specified. Conclusion There is an underestimated need to develop validated ‘implementation patient safety indicator(s) (sets)’ to monitor implementation outcomes of reporting and learning systems. We also identified a potential need to facilitate awareness of the concept of implementation success and its relevance for patient safety. Drafts of indicators that could be used as a starting point for the further development of ‘implementation patient safety indicators’ were provided.

    Abstract: The aim of this systematic scoping review was to identify and analyze indicators that address implementation quality or success in health care services and to deduce recommendations for further indicator development. This review was conducted according to the Joanna Briggs Manual and the PRISMA Statement. CINAHL, EMBASE, MEDLINE, and PsycINFO were searched. Studies or reviews published between August 2008 and 2018 that reported monitoring of the quality or the implementation success in health care services by using indicators based on continuous variables and proportion-based, ratio-based, standardized ratio–based, or rate-based variables or indices were included. The records were screened by title and abstract, and the full-text articles were also independently double-screened by 3 reviewers for eligibility. In total, 4376 records were identified that resulted in 10 eligible studies, including 67 implementation indicators. There was heterogeneity regarding the theoretical backgrounds, designs, objectives, settings, and implementation indicators among the publications. None of the indicators addressed the implementation outcomes of appropriateness or sustainability. Service implementation efficiency was identified as an additional outcome. Achieving consensus in framing implementation outcomes and indicators will be a new challenge in health services research. Considering the new debates regarding health care complexity, the further development of indicators based on complementary qualitative and quantitative approaches is needed.

    Abstract: Das Deutsche Netzwerk Versorgungsforschung e.V. (DNVF) fördert seit Jahren die methodische Qualität von Versorgungsforschungsstudien auf der Basis von Memoranden und anderen Initiativen. Die Qualitäts- und Patientensicherheitsforschung (QPSF) gilt als Kerngebiet der Gesundheitsversorgungsforschung. Das vorliegende Memorandum erläutert wesentliche etablierte Fragestellungen und Methoden der QPSF. Vor dem Hintergrund der besonderen gesundheitspolitischen Bedeutung des Themas werden Methoden der Messgrößenentwicklung und -prüfung, die Risikoadjustierung, Methoden zur Erhebung von Patientensicherheitsdaten, Instrumente zur Analyse sicherheitsrelevanter Ereignisse und Methoden zur Evaluation der meist multiplen und komplexen QPSF-Interventionen behandelt. Zudem werden vordringliche Forschungsthemen benannt.

    Abstract: Ziel der vorliegenden Arbeit war es, Empfehlungen für ein Kerncurriculum für Masterstudiengänge der Versorgungsforschung zu erarbeiten. Hierzu wurde im ersten Schritt eine standardisierte Online-Befragung von Hochschullehrenden durchgeführt. Im zweiten Schritt wurden die Modulhandbücher der existierenden Studiengänge in Bezug auf ihre Lehr-Lerninhalte analysiert. Die Inhalte wurden im dritten Schritt ergänzt durch eine Literaturrecherche. Im letzten Schritt wurden die Empfehlungen innerhalb ein er Expertenrunde final diskutiert. Die resultierenden Empfehlungen umfassen 13 Themen zu fünf Leitfragen mit 26 Unterthemen. Die Schwerpunktthemen stammen aus den Bereichen der Grundlagenwissenschaften im Kontext der Versorgungsforschung, des Gesundheitssystems und der Gesundheitspolitik, dem (empirischen) Versorgungsforschungsprozess und dem Wissenstransfer. Die vorliegenden Empfehlungen dürfen als Diskussionsgrundlage und Ausgangspunkt für Weiterentwicklungen verstanden werden. Die Erarbeitung von Empfehlungen ist ein fortlaufender Prozess und eine fortwährende Aufgabe der wissenschaftlichen Gemeinschaft, da sich die Kernkompetenzen von Versorgungsforschenden kontinuierlich neuen Forschungsgegenständen, neuen Forschungsmethoden und sich verändernden Rahmenbedingungen anpassen müssen.
