    Abstract: Background: Post tubal ligation syndrome has been a topic of debate and the association of Dysfunctional uterine bleeding (DUB) has been identified as a long-term complication. In refractory cases, hysterectomy can be considered a treatment option for managing the post-ligation syndrome due to prolonged menstrual complaints. But very few studies summarize the fact in our country's context. Considering of epidemiological data, the study was designed to assess the incidence of hysterectomy for DUB with post-tubal ligation. Methods: The hospital-based cross-sectional type of descriptive study was conducted in the inpatient Department of Gynae & Obst in Rajshahi Medical College Hospital (RMCH), From January 2019 to July 2019. Women suffering from DUB and having a history of post tubal ligation were included in the study. After selecting the patients, informed written consent was taken from the subjects. All patients were subjected to a detailed history, clinical examination, and relevant investigation. In necessary cases, hysterectomy was done as a treatment option. Data were collected using a semi-structured questionnaire designed for the study by the researcher. Data analysis and presentation were made by statistical software SPSS 23. Results: Total 100 patients with DUB were included in this study. The mean age was 40.9±6.28 SD (years), range: 25-54 years. The majority of patients (52%) belonged to the age group (41 – 50 years). About 59% came from rural areas. The mean parity was 3.43±1.34. The incidence of hysterectomy among DUB patients with a previous history of tubal ligation was 25%. The mean duration of tubal ligation was 13.4±6.3 years. The usual presentation per vaginal bleeding pattern at presentation was menorrhagia (47%), followed in second and third by metrorrhagia (20%) and postmenopausal bleeding (17%). Besides, 78% of patients also complained of tiredness, and 12% complained of pelvic pain. And Mean duration of symptoms was 13.44 months. Conclusion: About one-fourth of the tubal ligation patients underwent a hysterectomy in their subsequent life, and usually presented with menorrhagia. However, further studies are needed to finalize the incidence rate.

    Abstract: Background: Endometriosis is a common gynecologic disease associated with a significant burden on women's health and healthcare systems. Currently approved hormonal treatments for Endometriosis can effectively control symptoms but may have clinically relevant side effects that limit their use. Objective: This study aimed to compare the Efficacy and safety of commonly used drugs dienogest and danazol in patients with Endometriosis. Methods and Material: This randomized clinical trial was conducted at the department of Obst and Gynae in Rajshahi Medical College Hospital from June 2020 to May 2021. A total of 100 diagnosed cases of Endometriosis were included in this study according to inclusion and exclusion criteria. The study population was divided into two groups randomly by flipping a coin; group A is the dienogest group (experimental group), and group B is the danazol group (control group). Informed written consent was obtained from all cases. Patients were followed up after 3 months of treatment following a standard management plan. After data collection, data analysis was done using SPSS version 23. Result: Mean age of the study patients was 28.72±4.71 (SD) years in group A and 29.52±5.23 (SD) years in group B patients with a majority in the age group 26-30 years. Age and other socio-demographic variables were statistically similar in both groups. Symptomatic improvement was observed with statistical significance in both groups regarding visual analog scale (VAS) pain score and severity of dyspareunia and bleeding pattern according to pictorial blood loss assessment chart (PBAC) score (p 〈 .05). But post-treatment VAS pain score and severity of dyspareunia and bleeding pattern was similar in both groups (p 〉 .05). Pre-treatment size of ovarian endometrioma was 4.02±0.227 cm (SD) in group A patients and 3.98±0.212 cm (SD) in group B patients.     Abstract: Background: Nowadays, Infertility is increasing worldwide. Many regimens are used in the treatment of infertility. Among them, a combination of oral aromatase inhibitor drugs and gonadotropin is recommended as a very effective regimen for inducing ovulation. But there is a scarcity of related data in the Bangladesh context. The present study was designed to compare the effect of letrozole and the combination of letrozole with low-dose intramuscular injection of HMG on ovulation and pregnancy. Methods: This randomized controlled trial was conducted at the department of Obstetrics and Gynaecology in Rajshahi Medical College Hospital from June 2020 to May 2021. A total of 95 patients with a history of infertility were selected as the study population according to inclusion and exclusion criteria. Written informed consent was obtained from each patient. All of them were randomly distributed into two groups, group-A, and group B, by the flipping coin method, where group-A (Experimental group) patients received letrozole with low dose intramuscular injection of HMG (45 patients) and group B (Control group) patients received Letrozole only (50 patients). Relevant investigations were done, along with scheduled follow-ups. Results: Mean age of the study patients was 26.86±3.71 (SD) years, whereas the majority were ≤30 years (74.7%). Age was statistically similar among the patients of both groups. The endometrial thickness, follicular diameter, and a number of mature follicles were statistically higher among patients who received letrozole with low dose intramuscular injection of HMG than the patients who received only letrozole. Maximum follicular diameter ≥18mm (91.1%) in group-A among them (60%) achieved in 1st Cycle. Maximum endometrial thickness was achieved in the first Cycle in both groups (60% vs. 32%) among group-A patients.     Abstract: Background: Premature rupture of membrane (PROM) is linked to significant fetal and maternal morbidity and mortalities. The maternal and fetal outcomes in PROM are very important to decrease maternal and child mortality and for better management and prevention of complications. Objective: To find out maternal & fetal outcomes of preterm premature rupture of the membranes. Methods and Material: This is a cross-sectional observational study carried out in the Department of Obstetrics & Gynaecology, 250 Beded District Hospital, Chapainawabganj, Rajshahi, Bangladesh from January 2016 to July 2016. 50 pregnant women with preterm premature rupture of the membranes. Result: The majority of patients with preterm PROM (30%) belong to the age group 31-35 years. The mean age of the patients and SD were 26.4±6.58. 60% of respondents were multipara and 40% primipara. Most of the patients had irregular antenatal checkups (56%), 24% had regular antenatal checkups, and 20% had no antenatal checkups. Twenty percent of patients developed labor pain within 15 hours of rupture of membrane, 24% within 16-30 hours, 56% within 31-45 hours, 10% within 46-60 hours, and 12% without the onset of labour pain. 37 (74%) patients with preterm PROM had a normal vaginal delivery, and 13(26%) delivered by caesarian Section. Forty-three percent of patients delivered within 24 hours of the ruptured membrane, followed by 24% of patients within 24-48 hours, and 28% of patients within more than 48 hours. Among 50 patients, 12 (24.0%) had a previous history of abortion, 24 (48%) had H/O preterm delivery due to PROM, 2 (4.0%) had H/O MR, and 6 (12.0%) had H/O D & C. Among 50 respondents with PPORM 4 (8.0%) had diabetes mellitus, (12.0%) had HTN, 15 (2.0%) had urinary tract infection and 10 (20.0%) had lower genital tract infection. In this study, the total number of the alive baby was 42, and the stillborn baby was 2. Out of 42 alive babies, 18(42.86%) were affected by the consequence of preterm PROM and the birth process. Among 48 alive babies, 6(12.50%) developed birth asphyxia, 5(10.42%) developed jaundice, 3(6.25%) developed neonatal sepsis, and 4 (8.33%) developed respiratory distress syndrome. Maternal morbidity were chorioamnionitis 4(8%) puerperal sepsis 5(10%), postpartum endometritis 2(4%), abruptio placenta 1(2%) and wound infection 2(4%). Conclusion: The incidence of preterm PROM was more in multipara. The majority of the patients were from lower- and middle-class families. This study found some factors, e.g., Coitus, the recent history of abortion, M.R., D & C, and previous history of PROM. The study was done with a small population, but it introduces us to certain risk factors that can be prevented. Proper health education, patient motivation, improved health hygiene, and adequate maternity and childcare services are needed to reduce morbidity and mortality.

    Abstract: The present study reports the prevalence of hepatitis C virus (HCV) infection among 70,200 voluntary blood donors. Out of them, 364 (0.52%) donors were anti-HCV positive of which 308(0.44%) were male and 56(0.08%) were female. As the donors were categorized by profession, highest number of voluntary donors were students (58%) followed by Business man (21%), Service holder (19 %) and house wife (2%). Most of the donors (43%) were in the age group of 18-25yrs. Among the professional groups of voluntary blood donors, percentage of anti HCV positive cases was highest in businessmen group (1.09%) followed by housewife (0.98%), service holders (0.73%) and students (0.22%).Bangladesh J Med Microbiol 2010; 03 (02): 1-3

    Abstract: Androgen Insensitivity Syndrome (AIS) is an inherited X-linked recessive disease with a mutation in the Androgen Receptor (AR) gene resulting in - functioning of Y sex chromosome and abnormality on X sex chromosome, where affected individuals have male chromosomes and male gonads with complete or partial feminization of the external genitals. Recently a case of complete Androgen Insensitivity Syndrome (CAIS) is reported at Shaheed Suhrawardy Medical College, Dhaka. A 15 years old school girl was admitted with primary amenorrhea with bilateral inguinal hernia. Diagnosis of complete AIS is confirmed by discovering an adult male testosterone level, 46, XY karyotype, absence of axillary and pubic hair and a shallow vagina, with no cervix or uterus.
