    Abstract: Introduction: Hypertension is a cardiovascular disorder rapidly emerging as a major health problem in developing countries. Uncontrolled or poorly managed hypertension leads to several complications such as coronary heart disease, peripheral vascular disease and kidney disease. These complications account for approx 9.4 million deaths worldwide every year. Consequently, it is an urgent need for authorities to act upon this issue. This study was done to determine the prevalence of Hypertension in Deurali village of Nuwakot. Methods: A descriptive cross-sectional study was done in the Deurali Village of Nuwakot district from May 2019 to July 2019. Ethical approval was taken from the Ethical Review Board of Nepal Health Research Council. A simple random sampling method was applied. Blood pressure was measured on three occasions along with the use of a self-administered questionnaire. Descriptive statistical analysis was done. Results: The study showed the prevalence of hypertension among the adult population to be 20 (8.5%) [8.5%±1.83% at 95% CI]. Prevalence was found to be higher among female than male. Age group more than 60 were major sufferers of the condition. The study population consisted of the majority of  Tamang community. Almost 213 (91%) participants were reported to have taken alcohol at some point in their life whereas only 104 (45%) were smokers. Conclusions: The prevalence of hypertension was found to be lower than the previous study done in similar settings.

    Abstract: Glucose-6-phosphate dehydrogenase (G6PD) deficiency is currently a threat to malaria elimination due to risk of primaquine-induced haemolysis in G6PD deficient individuals. The World Health Organization (WHO) recommends G6PD screening before providing primaquine as a radical treatment against vivax malaria. However, evidence regarding the prevalence and causing mutations of G6PD deficiency in Nepal is scarce. Methods A cross-sectional, population-based, prevalence study was carried out from May to October 2016 in 12 malaria-endemic districts of Nepal. The screening survey included 4067 participants whose G6PD status was determined by G6PD Care Start™ rapid diagnostic test and genotyping. Results The prevalence of G6PD deficiency at the national level was 3.5% (4.1% among males and 2.1% among females). When analysed according to ethnic groups, G6PD deficiency was highest among the Janajati (6.2% overall, 17.6% in Mahatto, 7.7% in Chaudhary and 7.5% in Tharu) and low among Brahman and Chhetri (1.3%). District-wise, prevalence was highest in Banke (7.6%) and Chitwan (6.6%). Coimbra mutation (592 C 〉 T) was found among 75.5% of the G6PD-deficient samples analysed and Mahidol (487 G 〉 A) and Mediterranean (563 C 〉 T) mutations were found in equal proportions in the remaining 24.5%. There was no specific geographic or ethnic distribution for the three mutations. Conclusions This study has identified populations with moderate to high prevalence of G6PD deficiency which provides strong evidence supporting the WHO recommendations to screen G6PD deficiency at health facility level before the use of primaquine-based radical curative regimen for Plasmodium vivax .

    Abstract: Background and Aims: Coronary collateral circulation can develop early after STEMI and patients having these coronary collaterals have favorable outcomes. The aim of this study was to evaluate the prognostic impact of coronary collateral circulation in STEMI. Methods: This is an observational prospective study of 106 consecutive STEMI patients undergoing Primary PCI admitted and treated at a tertiary cardiac centre from May 2019 to April 2020. Clinical profile, complications at index admission and atone month follow up were analyzed. Results: Out of 106 consecutive patients, 50(47%) had early coronary collateral supplying the infarct related artery. The baseline characteristics of the patients in the two groups, with collateral and without collateral, were similar. Among patients with collateral circulation, only 5% had very well developed (Rentrop3) collaterals. The presence of collateral was strongly associated with presence of preexisting angina (p=0.007) and delayed presentation to hospital (p= 0.04). Coronary collateral was more common in non-diabetics, non-anterior wall STEMI and those with mutivessel disease. Compared with the patients without collateral supply, those who had collateral had fewer incidence of in-hospital heart failure (p=0.03) and post MI pericarditis (p=0.04). Conclusion: In STEMI, development or recruitment of early collateral supply to the infarct related artery was associated with lower rates of heart failure, post MI pericarditis, cardiogenic shock, hospital stay and in-hospital deaths. At 1 month, patients with collateral supply had fewer angina recurrence, reinfarcton and stent thrombosis.

    Abstract: Background and Aims: Coronary artery bypass graft is the treatment of choice for left main disease. However the results from several large multicenter trials have shown that the angioplasty of left main disease is an alternative treatment. The aim of this study was to assess the outcome of patients undergoing angioplasty of left main disease. Methods: This prospective study was conducted at Manmohan cardiothoracic vascular and transplant center, Maharajgunj, Kathmandu, Nepal. Twenty-Five patients with a diagnosis of left main coronary artery disease who underwent coronary angioplasty were enrolled in the study. The cases were followed up and outcomes on follow up were assessed. Results: Patients with left main disease enrolled in this study belonged to the age group of 51 to 77 years. The mean age of patients was 64.7±8.12 with 68% (17) male and 32% (8) female. Among 25 patients 3 patients had single vessel disease, double vessel in 10 patients and triple vessel in 12 patients. Three patients (12%) had previous history of percutaneous coronary intervention and 1 (4%) had previous history of coronary artery bypass graft. All patients underwent successful procedure with unprotected left main stenting in 14 cases (56%). There was few major and minor complications and no procedure related mortality and in hospital death. Conclusion: Left main coronary angioplasty is a safe and alternative procedure to CABG with low incidence of major clinical events.

    Abstract: Background and Aims:Transcatheter closure of the secundum atrial septal defect (ASD) has become an accepted alternative to surgical repair. We aimed to analyze and compare the changes in cardiac hemodynamics with transthoracic echocardiography (TTE) before, within 48 hours, and after 3 months of ASD closure. Methods: This was a prospective, single-centered study of 43 patients who underwent ASD device closure in the Manmohan Cardiothoracic Vascular and Transplant Center during June 2020 to June 2021 with Amplatzer Septal Occluder under transesophageal and fluoroscopic guidance. The patients were evaluated with TTE before, at 48 hours, and 3 months after the procedure. Results: At 48 hours and 3 months of device closure, the right atrial major dimension, the maximum blood flow velocity at the pulmonary valve orifice, mean flow velocity, velocity time integral, and E peak and A peak blood flow velocity at the tricuspid valve orifice were significantly reduced (P 〈 0.001). At 3 months, the dimensions and ejection fraction of the left ventricle showed significant increment (P 〈 0.001).   Likewise, the right atrial minor dimension and area, right ventricular basal, mid, and longitudinal dimensions, tricuspid annular plane systolic excursion, right ventricular Tei Index, and fractional area change were significantly reduced (P 〈 0.001). The main pulmonary artery diameter, pulmonary artery systolic and mean pressure, and the pulmonary vascular resistance and index were significantly reduced (p 〈 0.001). The procedural success rate was 97.6%. Conclusion: Echocardiographic evaluation demonstrated that cardiac hemodynamics and loading conditions improved significantly at 3 months after percutaneous closure of ASD. The transcatheter closure of ASD was safe with good short-term outcomes.

    Abstract: Background: With increase in treatment options for rehabilitation of partially edentulous patients, the use of cast partial denture in clinical practice as well as teaching curriculum has been decreased. The objective of this study was to evaluate the perspective of dental professionals towards use of cast partial denture in Nepal. Methods: The study consisted of nine close-ended questions regarding the use of cast partial denture by dental professionals in clinical practice throughout Nepal. The weblink of the questionnaire were shared via E-mail to dentists practicing in Nepal, to obtain a response of 350. Statistical analysis was done using SPSS version 26. Results: The study showed that the most preferred choice of rehabilitation was fixed partial denture by all dental professionals around 52.6% of , 60% of prosthodontists and 48.9% by other specialist dentists.The least preferred choice was Removable partial denture about 16.6% by general dental practitioners, 18.5% by prosthodontists and 13.3% by other specialist dentists.Among the different types of removable partial dentures, acrylic partial denture was the most preferred one and the primary reason for not recommending cast partial dentures by the dental professionals was too complicated procedure to be carried out. Nevertheless, majority of the dentists were in favor of giving more importance for teaching cast partial dentures in graduation curriculum. Conclusions: The study concluded that the use of cast partial denture is declining in clinical practice in Nepal due to the complicated procedure of fabrication and esthetic concern. Therefore, there is a need to emphasize the significance of teaching of cast partial dentures in graduation curriculum.

    Abstract: Introduction: Protein energy malnutrition (PEM) is associated with adverse clinical outcomes for various cardiac diseases, but its effect on outcomes of hypertensive emergencies (HTNE) has not been studied. We examined the impact of PEM on in-hospital outcomes of HTNE using the largest inpatient database in the United States. Methods: We reviewed the National Inpatient Database from 2017 to 2018 to identify admissions for HTNE and stratified them to those with PEM (PEM+) and without PEM (PEM-). Multivariable logistic regression was used to compare categorical and continuous variables after adjusting for confounders. Primary outcome included in-hospital mortality. Secondary outcomes included mean length of stay (LOS), total hospital charge (THC), and acute kidney injury (AKI). Results: Of 749,115 admissions with HTNE, 34,555(4.6 %) had PEM. A total of 27,855 (3.72%) deaths were observed inpatient for HTNE. Among PEM + cohort, 2,310 of 34,555 (6.68 %) died compared with 25,545 of 714,560 (3.57 %) among PEM- cohort [AOR 1.29, 95% CI 1.17-1.45, p 〈 0.001]. Both LOS (7 days,95% CI [6.63] -7.38, p 〈 0.001) and THC ($86,928, 95% CI [61,632]-74,831, p 〈 0.001) were significantly higher in PEM + cohort. PEM + cohort also had higher odds of AKI [5360 of 35,555(44.5%) vs 22,950 of 714,560 (31.12%);(AOR 1.39, 95% CI 1.31-1.46, p 〈 0.001)]. Conclusion: Concomitant diagnosis of PEM in HTNE is associated with higher odds of in-hospital mortality and AKI . It also increases burden on healthcare.Higher vigilance and early risk stratification may be important for better outcomes in HTNE admissions with PEM. Further prospective studies are required to evaluate the measures to mitigate negative outcomes.

    Abstract: The diagnosis of extra-pulmonary tuberculosis by Mycobacterium tuberculosis infection in retro­pharyngeal and thyroid abscess is challenging due to a lack of rapid, sensitive and specific diag­nostic assay. Hence, we evaluated the performance of highly sensitive polymerase chain reaction (PCR) to detect the M. tuberculosis in the retropharyngeal and thyroid abscess. The microscopy and culture were negative test result however; PCR result had demonstrated the presence of M. tuberculosis in the same specimens with retropharyngeal and thyroid abscess. Therefore, it is rec­ommended to perform PCR to detect undiagnosed cases, which were not detected by convention­al approaches for the diagnosis, of extra-pulmonary tuberculosis.
