    Abstract: Das intensivmedizinische Setting ist seit jeher eng mit respiratorischer Insuffizienz und Beatmung verbunden. Ist eine Intubation zwingend erforderlich, gibt es eine Vielzahl an Beatmungsverfahren und -konzepten. Als allgemeine Weisheit gilt hier: Mit der Intubation fängt das Weaning an. Dabei stellt sich die Frage, welches Weaningverfahren zur positiven Beeinflussung relevanter Patienten-Outcomes am effektivsten ist. In dieser Literaturübersichtsarbeit soll diese Frage auf Basis aktueller Evidenz beantwortet werden. – Der Beitrag belegte den 1. Platz beim 25. intensiv-Pflegepreis.

    Abstract: Assessing nursing-related competences becomes increasingly relevant. Therefore, psychometrically tested and contentual appropriate instruments are needed. The Nurse Professional Competence (NPC) Scale seems convenient to assess nurses' competences in German-speaking countries. This article describes the translation and cultural adaption of the NPC Scale English-language version for the German-speaking linguistic area of Austria (AUT), following the respective principles defined by the International Society for Pharmaoeconomics and Outcome Research (ISPOR). The aim was to provide a German-language version of the NPC Scale usable for the Austrian specific linguistic and cultural area. Due to polydimensionality of the scale and the extensive number of items being stepwise revised by researchers, several innovative methodological approaches were required to ensure transparent and comprehensible decision-making, data-revision and consensus-gainig throughout the overall process. Useful methods are presented to cope with challenges accompanying the coverage of decentralized data-revision and consensus-finding within the translation and cultural adaption of a polydimensional scale with a high number of items. The Nurse Professional Competence Scale, 88 items – German Austrian language version is conceptually, semantically and idiomatically equivalent compared to the NPC Scale original version and is recommendable for the usage in the target country's nursing context from a linguistic point of view.

    Abstract: Fragility fractures of the hip are one of the most important triggers of poor health outcomes in older adults. They often result in limitations in the ability of patients to care for themselves and mortality remains high. Orthogeriatric co-management may improve outcomes for this high-risk population. However, the impact on long-term results has not yet been definitively clarified. The purpose of this study was to present the influence of orthogeriatric co-management on mortality and self-care ability, as measured by the Barthel score, one year after hip fracture in people ≥ 60 years. A systematic literature search was performed in accordance with the process steps of identification, selection, and evaluation, with a systematic search of the MEDLINE, CINAHL Complete and Cochrane Library databases in the period from February to March 2022. Articles in English and German published between 2012 and 2022 were included. Twelve studies were finally used. Six studies demonstrated a statistically significant reduction in the one-year mortality rate. Only one of four studies evaluating self-care ability showed a significant improvement when the patient was treated on a specialised ward. Orthogeriatric co-management seems to be beneficial in positively influencing one-year mortality and self-care ability. In view of the heterogeneous results, the implementation of this care model can only be recommended to a limited extent.

    Abstract: To assess health literacy (HL) of patients with total knee arthroplasty (TKA) and evaluate its impact on patient reported outcome measures (PROMs) as well as investigating outcome differences in inpatient and outpatient rehabilitation. Methods In this study, HL and PROMs of older patients were assessed preoperatively (T0) and after 3 (T1), 6 (T2), and 12 (T3) months. HL was assessed with the short form of the European HL Questionnaire; pain scores, functional restrictions, and activity levels with standardized PROMs. Results Limited HL was observed in 70.6% of patients at T0. HL improved from baseline to follow-ups (p  〈  0.001). There was no impact of HL on PROMs. Pain scores were higher in patients undergoing outpatient rehabilitation (p = 0.022). No differences were found between the rehabilitation settings for the other outcomes. Conclusion The study shows that limited HL is prevalent in patients with scheduled TKA. Rehabilitation seems to have a positive effect on increasing HL.

    Abstract: The assessment of nursing-related competences by suitable instruments has become more relevant. Internationally, applicable instruments have been developed. The German-language version of the Nurse Professional Competence (NPC) Scale seems to be appropriate to measure competences of registered nurses in Austria. The psychometric properties of the scale have not been tested so far. The aim of this study was to examine the content validity of the German version of the NPC Scale. A mixed methods design was applied. Qualitative data were summarized by interpretative-reductive technique; the content validity index (CVI) was used to analyze the quantitative data. Data interpretation was performed by merging the results of the quantitative and qualitative analysis. As a result of the content analysis, five categories were determined to summarize the comments and critique. These categories referred to insufficient precision of terms and items, lacking profile-specific scale content to the theoretical construct of nursing-related competences, missing adequacy of the scale for the use in all nursing-related settings, and annotations for the revision of single items. Quantitative analysis showed 85 of 88 items as content valid by computing each single item. The dimension-specific CVI/Averages ranged between 0.90 and 0.97, the CVI/Average for the whole scale was 0.93. After merging the results of both qualitative and quantitative data analysis, the NPC Scale can actually not be evaluated as a content valid instrument for assessing nursing-related competences in an Austrian context. Substantial item-specific and dimension-specific deficiencies imply that competences cannot be thoroughly assessed. A substantial contentual revision of the current German version of the NPC Scale is recommended.

    Abstract: Breast cancer is one of the most prevalent malignancies in women worldwide and affects all dimensions of health. The aim of this meta-aggregative review is to describe the experience of diagnosis and the subsequent period of therapy of women with breast cancer. Following a systematic literature research, ten publications were included in the meta-aggregative review and the study results were synthesized by categorization. Emotional burden, challenges, psychosocial support, and individual coping strategies were described by affected women, and disease-related information sharing and fulfilment of expectations in health professionals were experienced as unsatisfactory. There is a need for improvement in the context of information and counseling measures by health professionals.
