    Abstract: Background: The liver biopsy is considered by many experts to be the most specific diagnostic tool used to assess the nature and severity of liver diseases such as hepatitis C. Liver biopsies are important for many reasons, such as accurate diagnosis or ruling out any coexisting liver diseases, staging and grading the severity of liver disease, treatment decisions, patient and provider reassurance, and as a benchmark for gauging future progression. Methods: In this observational study, 50 patients were recruited from different Medicine units of Dhaka Medical College Hospital, Dhaka, from May, 2008 to June, 2009. After preliminary selection of patients, a specific protocol was followed which include patients particulars, clinical features and clinical diagnosis, biochemical parameters, radiological and other investigative procedures and finally percutaneous needle biopsy of liver was done. The objective of the study was to establish the correlation of clinical presentation of chronic parenchymal liver disease with histopathological findings and establish liver biopsy as a tool for diagnosis. SPSS version 16.0 was used to analyze the data. Quantitative data were presented in the form of tables and figures. Chi-square test and student t test was done to find out the statistical significance. Results: Histopathologically chronic liver disease (CLD) was found to be the commonest lesion (pd0.05), which was 24 (48%) followed by hepatocellular carcinoma 13(26%) and secondary deposit 02(4%). Eleven cases consist of other findings including normal. CLD commonly presented with loss of appetite (82%), Jaundice (74%), Weight loss (68%), hepatic facies (54%).Hepatocellular carcinoma commonly presented with hepatomegaly (100%), jaundice (61.84%), weight loss (76.92%), and ascites (53.85%). Hepatomegaly was constant feature (100%) of all secondary metastasis. Among two cases of secondary carcinoma one (50%) was adenocarcinoma and another one (50%) was anaplastic type. Out of 29 cases of clinically diagnosed chronic liver disease 24 cases were confirmed histopathologically which was statistically significant (pd0.05). Conclusion: For the establishment of diagnosis and treatment it is mandatory to have a good correlation between clinical features and histopathological finding. Histopathology could detect diseases which were not considered clinically and specific management could only be done depending on histopathology. Therefore, if there is no contraindication, for confirmation of diagnosis liver biopsy still remains the corner stone modality.     Abstract: Peutz-Jeghers syndrome is characterized mainly by the presence of hamartomatous polyposis of gastrointestinal tract and melanin spots on the lips and buccal mucosa. The polyps are mostly found in the small bowel and less frequently seen in the stomach and colon. Intestinal polyps in short term may cause chronic bleeding, intestinal occlusion and intussusceptions. There is small but significant risk of small bowel adenocarcinoma, cancer of pancreas, breast, ovary and endometrium. A case of peutz-jeghers syndrome was presented with a complaint of episodes of colicky abdominal pain and vomiting; small bowel intussusception was diagnosed and an operation was performed in Colorectal Surgery unit of Bangabandhu Sheikh Mujib Medical University (BSMMU) Hospital, Dhaka, jejuno-ileo-caecal polyps were removed by enterotomy. On discharge she was instructed to attend our unit four weeks later for colonoscopic removal of sigmoid polyps and also advised to attend breast and cervical screening program.     Abstract: Objectives: Evaluation of the international prostate symptom score(IPSS) and quality of life score before and following TURP in BPH patients and correlation with objective parameters(Qmax and PVR). Methods: This purposive clinical trial was carried out in the Department of Urology, Sir Salimullah Medical College and Mitford Hospital, Dhaka, from January 2008 to December 2008. Total fifty patients between 50-72 years attending for the treatment of BPH with LUTS were included for the study according to inclusion & exclusion Patients with carcinoma prostate riteria. Selected patients underwent transurethral resection of prostate. All of them had baseline IPSS, QoL score, Qmax, voided volume, voiding time and PVR. They were followed after 1 month and 3 months with same parameter and compared with the baseline values. Correlation of IPSS and QoL score was done with Qmax and PVR. Results: The baseline IPSS in this study was 25.18±1.45. At one month and three months follow up visits after TURP, the IPSS was decreased to 15.0±1.07 and 8.14±0.76 respectively. The mean QoL score was 5.30± 0.46 before TURP. After one month of TURP it was 3.20± 0.45 and after 3 months of TURP it was 1.86 ± 0.57. There was significant negative correlation between the QoL score and Qmax in the preoperative period (r = -0.606, p=0.001), at the 1st follow up visit after one month of TURP (r = -0.171, p=0.235) and at the 2nd follow visit after three months of TURP (r = -0.680, p =0.001). There was positive correlation between the QoL score and PVR in the preoperative period (r = 0.394, p=0.005), at the 1st follow up visit after one month of TURP (r = 0.047, p =0.748) and at the 2nd follow up visit after three months of TURP (r = 0.471, p=0.235). Conclusion: There was significant improvement of quality of after TURP and There was significant negative correlation between the QoL score and Qmax and positive correlation between the QoL score and PVR.     Abstract: An elevated mean platelet volume is associated with increased platelet activation and thus may predict thrombotic events including major cardiac adverse events after CABG surgery. The association of mean platelet volume with in-hospital outcome of patients after coronary artery bypasses grafting surgery was investigated in this study. Preoperative mean platelet volume was obtained prospectively in 8^ consecutive patients undergoing conventional coronary artery bypass surgery. Postoperative mortality and major adverse events were recorded in the early postoperative Period. Combined adverse events including post operative Myocardial Infarction (Ml), arrhythmia, bleeding and death occurred in 27 patients (33.3%) during the early follow-up. The preoperative mean platelet volume levels were found to be associated with postoperative adverse events (p 〈 0.05). In multivariate logistic regression models, the preoperative mean platelet voluue levels was found strong independent predictor of combined adverse events after surgery (OR 1.968, p=0.008).University Heart Journal Vol. 14, No. 1, Jan 2018; 24-27

    Abstract: Clinical features of Systemic Lupus Erythematosus (SLE) worsens when it involves kidneys. Even then in our socioeconomic context the patients who seek admission are usually at later stages. Moreover histological classes may not be previewed by clinical and laboratory features. The aim of the study is to show variation of clinical features and laboratory findings in relation to pathological classes of Lupus nephritis. It was a cross sectional descriptive study enrolling 30 female patients admitted in medicine and nephrology department of Chittagong Medical College. Majority of the patients are of age range 21-40 years (73%); belonging to middle class family (90%) and educated upto level of secondary school certificate (43%). Oedema (93%), normotension (73%) and anaemia (93%) are common clinical features. Though all patients had macroscopic proteinuria only 20 % patients had massive proteinuria ( 〉 3gm%). Serologically Nineteen patients are ANA positive (63%) and all patients had Anti-dsDNA positivity. Class III and Class IV comprises 70% of patients. Estimated Glomerular filtration rate (eGFR) is more in class V ( mean 77.77) and class III (mean 76.86) than class II ( mean 52.67) and class IV ( mean 59.95) signifying eGFR cannot estimate severity. In conclusion without renal biopsy Lupus nephritis class cannot be ascertained and so class specific management cannot be given. JCMCTA 2012 ; 23 (2): 34-37

    Abstract: Background: To find out the renal morphological changes after Extracorporeal Shock Wave Lithotripsy (ESWL) both in short and long term. Materials and methods: This prospective study was carried out in the in the Department of Urology, Chittagong Medical College Hospital, Chittagong from January 2009 to January 2010. Patients with symptom, radio logically proved renal stone(s) measuring less than 2 cm in diameter and who are normotensive were enrolled in this study. Results: All 50 patients were divided into four age groups. The age ranged from 38 to 68 years and the maximum number was found in the age group of 51-60 years. The mean age was 50.9 years with Standard Deviation (SD)±11.2 years. Out of which 58.0% were male and rest 42.0% were female patients. All patients had normal morphological kidney ESWL but after ESWL 4(8.0%) had peri renal hematoma hydronephrosis / increased size of the kidney. The difference was not statistically significant (p 〉 0.05) before and after ESWL. Sub capsular hematoma or peri renal hematoma, hydronephrosis of kidney and increased size of the kidney diagnosed by USG of KUB region and CT scan. All patients had normal kidney before ESWL but after ESWL 6(12.0%) had paren chymal scarring / decreased size of the kidney. The difference was statistically significant (p 〈 0.05) before and after ESWL. Morphological changes of the kidney like parenchymal scarring and decreased size of the kidney diagnosed by USG of KUB region. All patients had normal kidney before ESWL but after ESWL 3(6.0%) had hydronephrosis, obstruction in peliviuretuic junction and ureter, stein strasse formation. The difference was not statistically significant (p 〉 0.05) before and after ESWL. Conclusion: ESWL is a good option for the management of renal stone less than 2cm. ESWL is considered a safe and well-tolerated procedure. Serious complications are rare, but need to be considered in certain number of patients with complications at short and long term after ESWL. Transient renal morphological changes occurs within short time and at variable period after ESWL.Post ESWL complication’s like renal morphological changes are rare, which become normal with in variable period.
