    Abstract: Osteoporosis is associated with decreases in bone mineral density (BMD) and is diagnosed using dual-energy X-ray absorptiometry (DXA). Computed tomography (CT), performed in routine practice, can also be used to evaluate bone quality without additional cost. Purpose To determine whether Hounsfield units (HU), a standardized CT attenuation coefficient, measured from the femoral head correlated with DXA-measured BMD. Material and Methods We evaluated 82 patients (14 men, 68 women; mean age, 67 years) undergoing femoral DXA and CT (non-enhanced abdominopelvic and hip scans) with 130 kV to determine whether HU correlated with T-scores. HU were measured by two radiologists using the largest spherical region of interest including the medullary bone of the femoral head from the junction point of the most caudal section of the femoral head with the femoral neck in 5-mm axial sections. The correlations of both sides’ HU values with their ages and DXA femur T-score were evaluated. Results HU values obtained from both femoral heads showed significant variation between the osteoporotic and non-osteoporotic groups (both P = 0.000) and strongly correlated with each other and DXA femur T-scores (left r = 0.75, right r = 0.73, respectively). In ROC curve analysis, predictive power of left HU values in identifying patients with osteoporotic femur DXA T-score was 0.905, and for right HU values it was 0.924. Osteoporosis cutoff values were 198 HU and 204 HU for the left and right hips, respectively. Conclusions HU obtained from CT performed in routine practice correlated with the DXA scores, thus providing an alternative method to determine regional bone quality without additional cost. This may be useful when choosing a fixation method, especially in trauma cases with already-performed abdominopelvic or pelvic CT in emergency services.

    Abstract:  Sakrum kırıklarının teşhisi ve tedavisi günümüzde halen zorlayıcı olabilmektedir. Literatürde sakral kırıkların tedavisi için çeşitli cerrahi yöntemler bildirilmiştir. Çalışmamızın amacı kliniğimizde posterior perkütan transiliak plak ile tedavi edilen hastaların fonksiyonel sonuçlarını incelemektir. Haziran 2013 ile Eylül 2020 tarihleri arasında posterior pelvik halka yaralanması nedeniyle cerrahi tedavi uygulanan ve en az 6 aylık takip süresi olan 56 hasta retrospektif olarak incelendi. Postoperatif 6. ayda demografik veriler, radyolojik görüntüler, Pelvik Sonuç Skorları (POS) ve Kısa Form-36 (SF-36) sonuçları analiz edildi. Hastaların 6. ay muayenesinde saptanan POS verileri incelendiğinde ortalama pelvik sonuç skoru 32 (26-37) ve SF-36 sonuçları 80 (48-90) olarak bulundu. Ortalama cerrahi işlem süresi 32,2 dakika ve ameliyat sırasında alınan ortanca floroskopi çekim sayısı 2 (1-6) idi. Posterior perkütan transiliak plak tespiti yapılan hastaların 6. ay muayenelerinde fonksiyonel sonuçları tatmin ediciydi. Bu teknik ile ameliyat süresinin kısa ve floroskopi maruziyetlerinin düşük olduğu görüldü. Kısa bir öğrenme eğrisine sahip olan bu tekniğin sakrum cerrahisinde güvenle kullanılabileceğine inanıyoruz. Bu nedenle pelvik cerrahi konusunda tecrübesi olmayan cerrahlar için bu yöntem daha kolay ve daha uygun bir yöntem gibi görünmektedir.

    Abstract: The management of comminuted quadrilateral fractures remains challenging, and treatment options are constantly evolving. The purpose of the present study was to examine the outcomes of 2 different fixation techniques in the management of comminuted quadrilateral fractures. Twenty-two patients with comminuted quadrilateral acetabular fractures were surgically treated with interfragmentary lag screw (group 1) and square bracket-shaped tubular (SBST) plate technique (group 2), in addition to suprapectineal and infrapectineal pelvic reconstruction plate fixation between January 2016 and July 2019 at our clinic. 2 years follow-up control data of each group were compared in terms of radiological and functional results, and complications. According to the functional score comparison, the mean Merle d’Aubigne Postel scoring system (MAP) score was 15.2/15.6 ( P = .632), and the mean Harris hip scoring (HHS) system score was 74.65/77.3 ( P = .664) in groups 1 and 2, respectively. Radiological comparison was performed according to matta radiological criteria (MRC), and 2 excellent, 6 good, 2 poor, 4 excellent, 4 good, and 4 poor radiological results were observed in groups 1 and 2, respectively. intraarticular screw penetration was detected in 3 patients in group 1, while there was no articular implant penetration in group 2 ( P = .001). We believe that satisfactory results can be obtained with the SBST plate technique, offering functional and clinical outcomes that are similar to those of the interfragmentary screw technique. The SBST plate technique is superior in terms of avoiding intraarticular screw penetration and related revision surgery.

    Abstract: Metastasis to the gastrointestinal tract from lung cancer is very uncommon and is often asymptomatic. Although small bowel metastasis may commonly occur, metastases to the stomach and colon are uncommon. Case report In this paper, we present a previously healthy 57-year-old male patient, a 60-packet per year smoker, who was taken to the emergency room with complaints of increasing abdominal pain, rectal bleeding, weight loss, and dyspnea for the last three months. Endoscopic examination revealed polypoid lesions in the duodenum and the descending colon. We diagnosed neuroendocrine small-cell lung cancer based on histopathological and immunohistochemical staining. Management and outcome: A cisplatin (d1, 60 mg/m 2 /day)–etoposide (d1 to d3, 120 mg/m 2 /day) regimen was given every three weeks as palliative chemotherapy. After the three course of chemotherapy, the lung radiograph showed a decline in hilar expansion and there was no pleural effusion. Then, he died of acute respiratory failure two weeks after radiotherapy of brain. Discussion Gastrointestinal tract metastasis of lung cancer is recognized synchronously with or rarely before diagnosis. It is generally recognized after the diagnosis of lung cancer. These patients often have other concurrent body metastases. Prognosis is poor, and survival expectation is short. The most common metastases to the gastrointestinal tract are squamous and large cell lung cancer metastases. Our aim is to emphasize the importance of immunohistochemical examination for masses in the gastrointestinal tract and to present this rare case of synchronous duodenal and colonic metastases of small-cell lung cancer.

    Abstract: Non-Radyografik aksiyel spondiloartrit (nr-axSpA), sistemik lupus eritematozus (SLE), sekonder antifosfolipid antikor sendromu (APS) ve Sjögren Sendromu (SjS) birlikteliğine sahip 55 yaşında bir kadın hastayı sunuyoruz. Nr-AxSpA tanısı enflamatuar bel ağrısı, insan lökosit antijeni B27 pozitifliği ve manyetik rezonans görüntülemede sakroileit varlığına dayanılarak yapıldı. SLE tanısı yanaklarında kelebek şeklinde döküntü, enflamatuar artrit, fotosensitivite, el parmaklarının dorsal eklemler arası alanı tutan eritem, antinükleer antikor (ANA) ve anti-dsDNA pozitifliği, alopesi, düşük serum kompleman seviyeleri, lökopeni ve trombositopeni varlığı ile konuldu. Pozitif labiyal tükürük bezi biyopsisi ile desteklenen sicca semptomları, düşük Schirmer testi, Anti SSA/Ro ve anti-SSB/La pozitifliği ile SjS tanısı konuldu. Ayrıca bu hastada 16. haftada düşük ve 33 yaşında serebrovasküler olay öyküsü vardı. IgG ve IgM antikardiolipin antikorlarının iki kez pozitif olduğu bulundu. Bu nedenle, kendisine sekonder APS tanısı konuldu. Hasta Ax-SpA, SLE, SjS ve APS için sınıflandırma kriterleri doldurmaktadır. Bilindiği üzere farklı genetik, etiyopatogenetik ve klinik özellikleri olan bu dört hastalığın bir arada bulunduğu ilk vaka sunumudur.

    Abstract: The aim of this study was to evaluate the normal dimensions and shape of the sella turcica (ST) in the Turkish population using multidetector computed tomography. Methods: This single-center, retrospective study included 188 patients who met the study criteria and had paranasal multidetector computed tomography taken between January 2019 and December 2019. The patients included in the study comprised 77 females and 111 males. The whole patient group was separated into 3 age groups of 18 to 25 years (group 1), 25 to 40 years (group 2), and 40 years and over (group 3). They were also separated according to gender. Results: The mean length of the ST was determined as 8.52 ± 1.42 mm (min-max 4.61–12.73 mm), mean height as 7.00 ± 1.31 mm (min-max 3.00–10.51 mm), mean aperture as 6.50 ± 2.00 mm (min-max 2.24–12.51 mm), and mean width as 11.01 ± 1.50 mm (min-max 7.78–14.94 mm). No statistically significant difference was determined between the length, height, width, and aperture size values of the ST according to gender and age groups. Conclusion: The results of this study demonstrated no significant difference in ST dimensions according to gender or age groups. It can be considered that the shape and dimensions of the ST can be more accurately evaluated with computed tomography and classification can be more robustly applied.
