    Abstract: Ambulatory ECG monitoring is suitable for patients with symptoms which may be caused by arrhythmia such as palpitation, light-headedness, syncope or chest discomfort. Patients with symptoms occurring daily or almost daily or those who have syncope without warning may be evaluated with a 24 hour Holter monitoring. Objective of this study is to determine incidences of various arrhythmias on 24-hour electrocardiographic Holter monitoring. This retrospective cross-sectional descriptive study was carried out in the cardiology department of NICVD by reviewing the Holter monitoring report to study different types of cardiac arrhythmias. Adult patients of either gender, referred for Holter monitoring with symptoms of palpitation, dizziness and syncope were evaluated for arrhythmia. The data analysis of the 100 patients who were studied, showed that there were 75 (75%) males and 25 (25%) females. 25% of patients were below 40 years whereas 75% were above 40 years of age. 67(67%) patients developed arrhythmia and others were normal. 38% report showed ventricular ectopic, 11% sick sinus syndrome, 5% ventricular tachycardia, 3% complete heart block. Supraventricular ectopic, atrial flutter, atrial fibrillation and Wolf Parkinson White syndrome each developed in only 1% of patients. Twenty four hour Holter monitoring is an important investigation for evaluation of patients with palpitation, dizziness and syncope. Arrhythmias were detected frequently in both symptomatic and asymptomatic patients. One must be careful to avoid attributing a symptom to an arrhythmia until a close temporal relationship is demonstrated.Medicine Today 2014 Vol.26(2): 71-74

    Abstract: Objective: Metabolic syndrome is the cluster of vascular risk factors including insulin resistance, elevated bloodpressure, hyperlipidemia, and obesity. Metabolic syndrome is strongly associated with cardiovascular andcerebrovascular disease. The aim of this study is to find out association between stroke and metabolic syndrome inour population.Methods: In this observational study, a total number of fifty randomly selected patients were studied from July2009 to December 2009 at Medicine ward of Dhaka Medical College Hospital. Definition of metabolic syndromewas taken from the guidelines by the National Cholesterol Education Program Adult Treatment Panel III.Results: Among the study population 46% had metabolic syndrome. This study estimated that Ischaemic stroke ismore common among metabolic syndrome patients (65.2%), but patient without metabolic syndrome had morehaemorrhagic stroke (55.6%).Only 12% patient was found obese. With or without metabolic syndrome most of thepatient had elevated TG (64%) and raised blood pressure (64%).Conclusion: Without obesity, metabolic syndrome is an important risk factor for cerebrovascular disease.     Abstract: The facility location plays an important role in an industry's supply chain management to facilitate logistics management requirements, making the sourcing and distribution process smooth and fast. The purpose of this study is to determine the ideal place to determine the location of a garment industry. This research uses the Center of Gravity method to calculate the optimal location that will be considered. This method employs an (X-Y) coordinate system to cover the geographical map of the study areas and identifies the coordinates for the new facility's location. It endeavored to identify the optimal location for the garment industry in Bangladesh. It was pinpointed where the industry can obtain raw materials from various regions of the country and a market, labor force, and energy source. Information was gathered regarding the volume of business activity at existing facilities. From the results of this study, it was found that the selected facility's location at the coordinates (23.68497454, 90.5928119), the arable land of the village of Sreepatir Char, Sonargaon, Narayanganj, which is the centralized position according to the supplier's distance, market distance, and availability of labor. Finally, we have shown that the identified location is appropriate for the facility's placement.

    Abstract: Takayasu arteritis, formerly known as pulseless disease, is a chronic idiopathic vasculitis which affects the large vessels in the body. First described in the 1800s, this rare condition is more commonly found in Asian women in their 40s. Herein, we report the case of a young woman whose exertional angina and claudication were the initial presentation of active Takayasu arteritis. The importance of modern technology of imaging such as CT, MRI and angiography, can often have paramount importance for confirming a diagnosis and the extent of the pathology. Newer modalities of investigation helps in evaluation of vascular involvement and its haemodynamic effects on cardiovascular system. Previously majority of patient had to be diagnosed clinically. Now a days by CT peripheral Angiogram this disease can be diagnosed. Invasive procedure of peripheral Angiogram is troublesome but single IV dye injection can revealed the actual scenario.     Abstract: Coarctation of the aorta is a congenital heart disease where there is constriction of Aorta. Coarctation accounts for 15-20 % of congenital cardiac defects and is more common in males than in females. It occurs in 1 of 10,000 live births. Coarctation of the aorta is a condition for which surgical outcomes are good and children will lead normal and healthy lives without restriction of their activity. They will typically catch up in growth and have normal development following the recovery period of the surgery. Coarctation of the aorta is typically treated by surgery to remove the narrowed segment of the aorta. Typically the surgery is best done when the child is between 18 and 24 months of age. Sometimes coarctation can be corrected by a balloon angioplasty procedure done during cardiac catheterization. A plastic tube will be inserted in the narrowed segment and a balloon in the tube will be inflated to stretch the narrowed segment. Sometimes a mess tube is then inserted (called a stent) to hold the vessel open. Some infants will be very sick, requiring care in the intensive care unit (ICU) prior to the procedure and could possibly even need emergency repair of the coarctation. Others, exhibiting few symptoms, will have the repair scheduled on a less urgent basis. A small device, called a stent, may also be placed in the narrowed area after the balloon dilation to keep the aorta open.     Abstract: Background: In our country, there are many studies on stroke, its associated conditions and their effect on stroke patients outcome, but a few studies on electrolytes status in stroke patients have been done in our country, even outside. Method: A total number of 100 randomly selected, clinically and CT proven acute stroke patients were studied at medicine units of Dhaka Medical College Hospital. Electrolytes status in different type of acute stroke patients and their association with some common clinical presentation were identified and correlated. Result: Out of 100 patients, 29% were in between 51-60 years age group & 72% were male and 28% were female patients. Majority 53% patients had Ischaemic stroke, 45% Intracerebral haemorrhage (ICH) and only 2% had Subarachnoid haemorrhage (SAH). 53% of total acute stroke patient had dyselectrolytaemia. Among 100 acute stroke patients, 66.66% haemorrhagic stroke and 28.30% ischaemic stroke patients had headache (p 〈 0.05). 70.66% haemorrhagic and 30.23% ischaemic stroke patients had vomiting (p 〈 0.05). 53% of total acute stroke patient had dyselectrolytaemia. Among 100 acute stroke patient 49 (49%) presented with vomiting from which 73.46% had dyselectrolytaemia (p 〈 0.01). 54 (54%) presented with headache from which 74.07% had dyselectrolytaemia (p 〈 0.001). 62.22% of haemorrhagic stroke (p 〈 0.05) & 43.39% of ischaemic stroke (p 〉 0.05) patients had dyselectrolytaemia. Total 36% of all stroke patients had serum sodium imbalance & 31% had serum potassium imbalance. In haemorrhagic stroke & ischaemic stroke patients, hyponatraemia (17% & 13%), hypernatraemia (1% & 3%), hypokalaemia (19% & 11%), hyperkalaemia (0% & 1%), hypochloraemia (9% & 6%) respectively. Conclusion: In haemorrhagic stroke, the incidence of electrolytes imbalance was more than ischaemic stroke and which were mostly hyponatraemia and hypokalaemia.     Abstract: A 65 year lady with chest and back pain was referred to our hospital with suspected case of acute coronary syndrome. A computed tomography (CT) aortic angiogram showed dissection of abdominal and thoracic aorta. Patient was initially treated by drugs for controlling heart rate and blood pressure with analgesics but due to progression of disease and involvement of left subclavian artery ostium referred for surgical intervention. She underwent left carotid-subclavian bypass and TEVAR (Thoracic endovascular aortic repair) with good outcome. The present case is a good demonstration revealing that in absence of clinical suspicion and diagnostic imaging, acute aortic dissection represents a diagnostic dilemma which requires immediate action.Anwer Khan Modern Medical College Journal Vol. 6, No. 1: January 2015, Pages 46-49

    Abstract: EECP is used to stabilize the coronary circulation in patients with severe coronary artery disease when maximal medical therapy and/or invasive procedures have proven inadequate. This prospective observational study was done for a period of 12 months. 40 patients with angina and angiographically proven coronary artery disease, who met the inclusion and exclusion criteria was enrolled in this study. Among 40 enrolled patients 30 (70%) patients completed their treatment. All the patients were in between 42 to 80 years age group. All patients required sublingual nitrate before started EECP treatment. But after completion of treatment, 75% patient required no nitrates. Who completed full course, 100% were improved their angina symptoms from class II, III, IV to class I. Overall 80% patients improved their quality of life. Those completed full course of treatment 100% patients improved their quality of life. EECP Heart therapy is now world-wide accepted alternative treatment option for those patients, who are not fit for surgery or any kind of interventional procedure and those developed heart failure.     Abstract: Aims: Radial approach is gaining the momentum as a default technique for coronary procedures. Limited trails are available for post coronary artery bypass graft (CABG) patients to compare the merits of femoral & radial access. Methods: It is a single-center study conducted in between January, 2013 to December, 2015. During this study period, post CABG patients were blindly assigned to its five high volume operators. Coronary angiography & intervention procedures were performed by left radial or femoral approach as per assigned operator's choice. Contrast volume was the primary endpoint whereas the procedure & fluoroscopy time, procedural success, access site major bleeding, pre discharge major adverse cardiac event (MACE) were the secondary endpoint both for coronary angiogram (CAG) & percutaneous coronary intervention (PCI). Results: Total 380 post CABG patients were included in this study period. Radial access (n=155) was lower than femoral access (n=225). Compared with femoral access, diagnostic CAG required relatively lower contrast volume though statistically not significant via radial access (70±34 vs. 72±40 ml, p=0.267). Procedure time (25.2±10.7 vs. 26.9±6.8 min, p=0.735), fluoroscopy time (10.7±5.5 vs. 9.5±4.7 min, p=0.424) were almost similar in both access for CAG. Other secondary clinical endpoints were similar among both groups. Interestingly, adhoc PCI was more frequent in radial group (n=54 out of 155, 34.8%) than in femoral group (n=44 out of 225, 19.6%) with p=0.01. Contrast volume in between two groups was pretty similar with p=0.226. The incidence of other secondary endpoints was also not statistically significant. Conclusion: Coronary angiography for post CABG patients through left radial approach seems to be effective, non-inferior in terms of contrast volume, procedure & fluoroscopy time & other clinical end points comparing to femoral access.
