    Abstract: Langerhans cell histiocytosis (LCH) merupakan suatu penyakit neoplasia inflamatif sel dendritik mieloid langka dengan manifestasi klinis yang bervariasi, ditandai dengan adanya sel mononuklear positif terhadap CD1a/S100/CD207 yang menginfiltrasi sistem organ termasuk kulit. Kejadian LCH di RSUP Dr Sardjito dari tahun 2014–2019 sebanyak 11 kasus. Makalah ini melaporkan satu kasus LCH pada anak berusia 1,5 tahun dengan keluhan utama bintik-bintik merah di seluruh tubuh dan kulit kepala berkerak sejak 6 bulan yang lalu. Pemeriksaan status dermatovenereologis menunjukkan pada kepala, wajah, dada, punggung, kedua tangan dan kaki, serta punggung kaki nampak papul purpurik multipel tersebar, kepala tertutup krusta kekuningan, kuku tangan dan kaki tampak hiperkeratosis. Pada abdomen didapatkan hepatosplenomegali. Pada pasien ini dilakukan pemeriksaan histopatologis dan imunohistokimia S-100 didapatkan positif pada sitoplasma sel tumor dan CD1a positif pada sitoplasma dan membran sel tumor. Diagnosis LCH ditegakkan berdasarkan kondisi klinis dan histopatologis. Pasien mendapatkan terapi pelembab dan regimen kemoterapi.Kata kunci : Langerhans cell histiocytosis, diagnosis, imunohistokimia

    Abstract: Psoriasis pustulosa generalisata (PPG) adalah penyakit multisistem yang cukup sulit diterapi. Sejauh ini masih sedikit penilaian keberhasilan terapi PPG menggunakan alat uji yang objektif. Penelitian ini menilai evaluasi terapi PPG menggunakan pustular symptom score (PSS). Rancangan penelitian ini adalah deskriptif retrospektif. Subjek penelitian yaitu semua penderita PPG derajat berat yang berobat di poliklinik dermatologi dan venereologi RSUP dr. Sardjito tahun 2017-2019. Pustular symptom score dievaluasi selama 4 bulan berupa penurunan skor ke derajat ringan, remisi, PSS 50, PSS 75, dan PSS 100. Terdapat 37 subjek PPG derajat berat selama 2017-2019. Perbaikan derajat keparahan menjadi ringan pada subjek yang mendapat metotreksat, metilprednisolon (MP), kombinasi metotreksat dan MP, dan kombinasi narrow-band ultraviolet B (NBUVB) dan MP adalah sebesar 100%; sedangkan perbaikan pada kombinasi siklosporin dan MP sebesar 90%. Metotreksat dan kombinasi metotreksat dan MP memberikan remisi dan pencapaian PSS 75 tertinggi di antara terapi lainnya. Rerata skor PSS terendah di akhir pengamatan juga didapatkan pada kedua kelompok tersebut. Pemberian metotreksat, MP, kombinasi metotreksat dan MP, kombinasi siklosporin dan MP, serta kombinasi NBUVB dan MP mampu memperbaiki derajat keparahan menjadi ringan. Metotreksat, baik tunggal maupun dikombinasi dengan MP, memberikan hasil lebih baik pada penurunan skor PSS dibandingkan modalitas terapi lain.Kata kunci : Psoriasis, PPG, PSS, metotreksat, siklosporin

    Abstract: Erupsi obat (EO) saat ini masih menjadi masalah kesehatan karena kasusnya cukup tinggi. Penderita infeksi Human Immunodeficiency Virus (HIV) merupakan kelompok yang rentan mengalami EO. Salah satu pilihan terapi lini pertama HIV adalah nevirapin (NVP) yang diketahui paling sering menyebabkan EO. Manifestasi EO akibat nevirapin terbanyak, yaitu erupsi makulopapular, merupakan reaksi hipersensitivitas tipe IV. Cara menentukan obat penyebab EO seringkali hanya berdasarkan dugaan dari anamnesis. Uji tempel obat (UTO) merupakan uji kulit in vivo yang relative aman dan murah untuk mengidentifikasi penyebab EO yang didasari reaksi hipersensitivitas tipe IV.Tujuan penelitian ini untuk mengetahui persentase nilai positif UTO pada penderita HIV yang diduga mengalami EO karena nevirapin, serta menguji konsentrasi terbaik dan keamanan  nevirapin yang dapat digunakan sebagai alergen UTO.Penelitian ini merupakan suatu penelitian deskriptif non-analitik. Uji tempel obat nevirapin dilakukan pada 25 subjek HIV dengan EO yang diduga karena nevirapin. Alergen nevirapin menggunakan pelarut vaselin album dengan konsentrasi 10%, 20%, dan 30%. Proporsi nilai positif UTO nevirapin tertinggi pada konsentrasi 30%, didapatkan pada 32% subjek, diikuti konsentrasi 20% pada 24% subjek dan konsentrasi terendah, yaitu 10% pada 20% subjek. Tidak ditemukan efek samping pada UTO nevirapin. Uji tempel obat nevirapin dengan konsentrasi 30% pelarut vaselin album berguna dan aman untuk mengidentifikasi alergi obat yang diduga karena nevirapin pada penderita HIV. Kata kunci: erupsi obat,  uji tempel obat, nevirapin, HIV

    Abstract: Background  Most patients who require skin prick testing cannot deal with their pruritus without taking antihistamines (AH). Orally administered AH has a quick onset of action, but it will suppress skin test responses (STR) from several days to weeks. In this study, we aimed to determine the onset and duration of action of single topical AH application by observing histamine‐STR suppression over time. Methods  A two‐step, randomized, intraindividual parallel‐comparative, double‐blind, placebo‐controlled trial was conducted on the volar side of the forearm. Step 1 was aimed to determine the onset, while step 2 determined the duration of action. The topical AH tested was a single application of 5% doxepin hydrochloride cream, while 10 mg/ml histamine dihydrochloride was used to test the skin responses. Results  Our 10 subjects’ mean age was 35.8 ± 3.179 years. Histamine wheal response was suppressed starting on minute 90 and the wheal width were back to ≥ 7 mm 2 on minute 270. Significant histamine reactivity difference between genders ( P  = 0.201) and atopic status ( P  = 1.000), which could be a source of bias in histamine STR, was not found among our subjects. Conclusion  Single application of topical AH has an onset of action in 90 min and duration of action 〈  180 min. Because of its short duration of action, topical AH can be used in a patient who needs AH but is scheduled to undergo skin prick testing after a few hours, without influencing the patient's STR.

    Abstract: Batik, a resist-dyeing technique to decorate a special cotton fabric, has been practiced for centuries in Indonesia. Unfortunately, as an informal enterprise, batik industry activities lack work safety and health regulations. This study aimed to identify potential health hazards, including inventorying the chemicals to which the workers are exposed, the PPE profile, and investigating the prevalence of occupational skin diseases (OSD) in the batik industry. A cross-sectional study and an inventory of exposure to the chemicals were done in traditional batik workplaces in 5 districts in Yogyakarta province, Indonesia. The chemicals were classified as potential sensitizers/irritants, and the workers were examined and interviewed using the Nordic Occupational Skin Questionnaire-2002/LONG. Of 222 traditional batik workers, OSD were diagnosed in 61 (27.5%) workers, with occupational contact dermatitis was the most common OSD encountered (n = 23/61; 37.7%) (allergic contact dermatitis n = 7/23; irritant contact dermatitis n = 16/23). A smaller portion of other OSD was also encountered including callus, miliaria, and nail disorder (9%, 6.3%, and 5.9%, respectively). During each step of the traditional batik manufacturing process, the workers are exposed to substances that act as irritants and/or as potential contact allergens. However, only one-fourth of the workers used PPE regularly, particularly during the coloring process and wax removal (wet processes). Traditional batik manufacturing process exposes the worker to various physical and chemical hazards, resulting in a high prevalence of occupational skin diseases, especially contact dermatitis among the employees.

    Abstract: Flagellate dermatitis is a relatively rare reaction to toxicity. It appears as skin lesions with erythematous patches or papules of linear, multiple, flagellate structures. Flagellate dermatitis can be triggered by several causes, which are most commonly associated with bleomycin. This paper reports two cases of flagellate dermatitis, one in a patient with germ cell carcinoma and another in a patient with osteosarcoma who were both diagnosed with flagellate dermatitis after the administration of bleomycin.

    Abstract: Batik is one of the Indonesian traditional arts made by decorating fabric using the resist dyeing technique. Currently, batik manufacturing serves as an important source of livelihood in Indonesia. However, the production process of traditional batik involves a range of chemicals and some repetitive physical movements, making batik production inseparable from physical and chemical hazards which increase the risk of developing occupational skin diseases (OSD). This study aimed to identify the risk factors related to OSD, including occupational contact dermatitis (OCD), among traditional batik manufacturing workers in Yogyakarta, Indonesia. Results The study demonstrated that working duration (≥ 8 h daily) and type of work (wet process only) were statistically significant as risk factors for developing OSD. While for OCD, our results found that gender (male), history of atopy, and type of work (all and wet phase only) were risk factors for OCD development among traditional batik manufacturing workers.

    Abstract: The combination of chemotherapy with lapatinib and capecitabine in human epidermal growth factor receptor 2 (HER2) positive breast cancer is quite effective. The combination of these two agents increases the risk of dermatological toxicities. A woman, 38 years old, HER2 positive breast cancer with a combination of chemotherapy agents between lapatinib and capecitabine gives an overview of skin toxicities such as acneiform eruptions, palmar-plantar erythrodysesthesia (PPE), and paronychia. Therapy, in this case, aims at clinical improvement. The combination of lapatinib and capecitabine has a side effect profile like each drug. Combined use of the two agents increases the incidence of skin side effects, including acneiform eruptions, PPE, and paronychia, compared to monotherapy. Early awareness of the side effects of chemotherapeutic agents is needed for early treatment to prevent the worsening of the condition and discontinuation of chemotherapeutic agents due to drug side effects.

    Abstract: Skin damage among healthcare workers has been reported by many centers around the world. Occupational hand dermatitis is one of the most commonly known occupational skin diseases and a socially significant health issue. The use of gloves is one of the risk factors for the occurrence and/or aggravation of hand dermatitis. This cross-sectional study involved healthcare workers in 14 referral hospitals for COVID-19 throughout Indonesia. Questionnaires were distributed to the participants, which consisted of the subject’s characteristics, glove-related skin problems, history of glove use, and clinical history. This study involved a total of 845 healthcare workers. Approximately 156 healthcare workers (18.46%) had glove-induced hand dermatitis during the pandemic. Itchy skin was the most common symptom (44.23%), and the palm was the most frequently complained area (48.72%). There was a significant association between glove use and glove-induced hand dermatitis among healthcare workers. In particular, equal to or more than 2 hours per day of glove use was significantly associated with hand dermatitis. Glove-induced hand dermatitis also had a significant association with the subject’s history of atopic dermatitis and previous history of hand dermatitis. The use of gloves by healthcare workers should be considered carefully, especially in individuals at increased risk, including those who use gloves for 2 hours or more per day and those who have a history of atopic or hand dermatitis, in order to prevent the incidence of glove-induced hand dermatitis among healthcare workers, as well as to provide a safe working environment.
