    Abstract: Carbon monoxide poisoning is one of the common causes of poisoning in patients. However, few studies have focused on the pediatric group. We performed this study in order to clarify the clinical characteristics of pediatric patients with Carbon monoxidepoisoning. Methods: We retrospectively reviewed the medical records of pediatric patients (age 〈 14 years)  diagnosed with CO intoxication and  admited in the Hyperbaric Oxygen chamber of the military hospital MOHAMED V of RABAT between January 2018 and March 2020, epidemiologic and clinical data were collected, analyzed  and compared with  literature. Results: A total of 112 children with CO poisoning were identified. Their ages ranged from one to 14 years, symptoms were nausea and vomiting (31%), headache (23%), altred consciousness (19%), dizziness (15%) and seizure (8%).  Most events (67.8%) occurred during winter, (100%) of cases were recorded as accidental poisoning. Only one child developed delayed neurologic sequelae, with no death enregistred in our series. The most common cause of intoxication was improperly vented exhaust produced by gas, hot water heaters (98%).(And) (20%) of patients needed two or more sessions of Hyperbaric Oxygen therapy. Conclusion: The treatment of Carbon monoxide poisoning is a race against time. Patients requiringHyperbaric Oxygen therapy must be carefully selected within the first few hours, and kept under close observation. Children with Carbon monoxide poisoning had good outcomes in this series.

    Abstract: Background: Hepatic tuberculosis (TB) is a rare entity, poorly described in the literature. It presents with nonspecific clinical manifestation and imaging findings mimicking other liver disease making diagnosis challenge. We report a case of isolated hepatic tuberculosis in asymptomatic cabin crew member diagnosed accidentally by investigating a liver chemistry test disorder. Case Report: A 28 year-old Moroccan cabin crew attendant came to the aeromedical expertise center to perform her periodical medical assessment. She had no medical history. Physical examination was unremarkable. Liver chemistry test values were elevated. Imaging studies of the liver suggested malignant disease. Histological examination showed isolated hepatic tuberculosis. Discussion: Isolated hepatic TB is very uncommon, lacks typical clinical manifestation and radiological features, but remains an important differential diagnosis of hepatic nodules especially in endemic areas. This observation highlights the benefit of liver chemistry test evaluation realized systematically for periodical aeromedical fitness to diagnose some pathology in asymptomatic stage avoiding complications. Hepatic TB is a cause of temporarily unfitness because it’s not compatible with flight safety conditions. Aircrew with hepatic TB will need to be assessed for both complete disease control and full resolution of any iatrogenic side effect of anti-tubercular drugs before returning to fly under some restrictions.

    Abstract: INTRODUCTION: Coronary artery disease (CAD) is a cause of death in 75% of patients with diabetes. Its often asymptomatic nature delays diagnosis. In aeronautics, it can cause in-flight incapacitation, beyond which it represents a major fear for the medical expert. Screening for CAD is still a topical subject with the advent of new cardiovascular (CV) risk biomarkers and more effective screening tests. We report the experience of the Aeromedical Expertise Center of Rabat in this screening of diabetic pilots, with a recommendations review. METHODS: A prospective study over 1 yr included diabetic pilots who benefited from systematic screening for CAD after a CV risk stratification. Coronary angiography is performed if a screening test is positive. Subsequent follow-up is carried out in consultation with the attending physician with regular evaluation in our center. RESULTS: There were 38 pilots included in our study. The average age was 55 ± 4.19 yr and about 73% had a high CV risk. CAD was detected in 4 cases (10.52%) who had abnormal resting electrocardiograms and required revascularization with the placement of active stents. Approximately 75% of pilots with CAD returned to fly through a waiver with restrictions. DISCUSSION: Screening for coronary disease in diabetics is controversial, and current recommendations are not unanimous. In our study, the screening did not identify coronary diabetic pilots who could benefit from bypass surgery. Nevertheless, coronary disease was diagnosed, justifying grounding to preserve flight safety, which is an absolute priority in aviation medicine.
