    Abstract: Fever, chest pain, cough in a young lady usually does not ring any alarm bell. We present here a case of a young lady who had typical symptoms of community acquired pneumonia who initially responded to regular antibiotics but later was investigated to have large cell carcinoma. Presentation of lung cancer as non-resolving pneumonia is not a very common phenomenon and diagnosis can present a diagnostic challenge to the clinician.     Abstract: Acute confusional state account for a substantial proportion of admissions to emergency wards. As the etiology of acute confusional state are numerous and presentation also variable, so many time create confusion regarding the actual diagnosis and thus delay the prompt management which may result in fatal outcome. The objectives of this study was to explore the etiology and age & sex distribution of acute confusional state among hospitalized patients. In this prospective hospital based study, 345 adult patients presenting with acute confusional state were collected from medicine unit Dhaka Medical College Hospital during the period of July’08 to October’08. Patients known to have previous history of stroke, chronic dementing neurological disease were excluded from the study. This study revealed that, patients with acute confusional state constitute about 21% of total hospital admission in medicine wards. Peak incidence of acute confusional state was found to be in two age groups: one in 2nd to 3rd decades and another in 5th to 6th decades. The first peak was due to acute poisoning, CNS infection and hepatic encephalopathy; and the second peak was due to stroke, electrolyte imbalance, systemic infection and disturbance of glucose metabolism. This study also showed male preponderance over female and metabolic causes higher than the structural causes. Stroke comprised the highest percentage (25%) of cases in this series.     Abstract: Background: Diabetes mellitus (DM) continues to be a major public health problem. Multiple factors have a role in the skin manifestations of DM. Cutaneous manifestations of DM are very important to the clinician. Methods: Current study was carried out in the Department of Dermatology and Venereology, Shaheed Suhrawrdy Medical College Hospital, Dhaka, spanning from 1st January 2017 till 30th June 2017 over a period of six months. Adult patients already diagnosed to be suffering from type 2 DM presenting with cutaneous manifestations were included in the study. Results: Majority (68.0%) patients had diabetes 〉 5 years, 16.7% had 〈 1 year and 15.3% had 1-5 years. Family history of DM was found in 70.7% in this study. In this study bacterial infection and fungal infection were more common in female patients (60.0% vs 62.0% respectively). Regarding types of dermatoses, fungal infection was more common in this study 50(33.3%). Others were bacterial infection 20(13.3%), viral infection 7(4.7%) and parasitic infection 7(4.7%). Papulo squamous disease was found 31(20.7%) patients, other diseases were 32(21.3%).Viral infection was more in male patients (71.4%). Parasitic infection was high in female patients 6(85.7%). Papulo squamous diseases was found 21(67.7%) in female patients. Conclusion: In this study fungal infection, bacterial infection, viral infection and parasitic infections were found to be the more common cutaneous dermatoses among adult diabetic patients. Bacterial infection and fungal infection were more common in female patients. TAJ 2018; 31(1): 21-28

    Abstract: Chronic Myeloid Leukaemia is very uncommon in children. It accounts for 2 to 5% of all the childhood leukaemias. The incidence is 〈 1 case per 1,00,000 population in younger than 20 years of age per year. Two distinct forms have been described- namely the Juvenile and Adult type. Absence of Philadelphia chromosome is the hallmark of Juvenile type CML ( JCML). Adult type CML is extremely rare in childhood. Here, we report one such a case of adult type of CML in an 11 year old girl who presented to us with pallor, fever and hepato-splenomegaly. At presentation, it seems to be a case of Visceral Leishmaniasis. But, peripheral blood film and bone marrow features were consistent with CML. Philadelphia chromosome was positive. Though biological behavior and prognosis are identical to that of adult CML, we are reporting this case because of its extremely uncommon incidence.Bangladesh J Medicine Jul 2014; 25 (2) : 72-75

    Abstract: Tuberculosis is very common in our country. It can involve many organs and have broad spectrum of image presentation. It may resemble malignant diseases, especially in the elderly and thus confuse the judgement of the clinician. We report a case of a 58 year old gentleman presenting with chronic cough, low grade fever, weight loss for 16 months; features of progressive marrow failure for 8 months and severe low back pain without any neurological features for 2 months. The CT scan of spine revealed lytic lesions of vertebrae sparing the intervertebral discs, thus resembling metastatic lesions of bone. We approached this case as malignant disease initially, but found it to be a disseminated tuberculosis infection involving lungs, bone marrow and spine after a series of investigations. Though disseminated TB is not uncommon in our country, the purpose of presenting this case is to give emphasis that, clinicians should consider Potts disease (spinal tuberculosis) in the differential diagnosis of patients with back pain and destructive vertebral lesions. Proper diagnosis and anti-tuberculous treatment with or without surgery will result in cure of the patientBangladesh J Medicine Jul 2016; 27(2) : 81-85

    Abstract: Dengue fever is a mosquito borne viral infection found in tropic & sub-tropic regions. The symptomatic dengue infection follows an uncomplicated course; however, unusual manifestations of this disease are now being increasingly reported as expanded dengue syndrome which incorporates wide spectrum of uncommon presentation of this common disease which does not fall into either dengue shock syndrome or dengue hemorrhagic fever. We report a case of a 30 year old lady who presented with high grade fever, rash, diffuse abdominal pain & vomiting, who ultimately developed shock and features of multi organ failure. We diagnosed her as a case of expanded dengue syndrome with multi organ failure ( hepatitis with very high transaminase levels, pancreatitis, acalculous cholecystitis, coagulation failure & acute kidney injury) with septicaemia. With prompt diagnosis and aggressive management, this potentially fatal patient was cured.Bangladesh J Medicine Jan 2018; 29(1) : 45-47

    Abstract: DRESS syndrome (Drug reaction with Eosinophilia and Systemic Symptoms) is a rare but potentially life-threatening drug hypersensitivity reaction characterized by fever, exfoliative dermatitis and maculopapular rash, lymphadenopathy, eosinophilia, leucocytosis, and involvement of internal organs as liver, lung, heart, and kidney. The disorder starts within 2–8 weeks after taking an offending drug. Long latency between drug treatment and onset of symptoms along with variability of presentation make the diagnosis quite difficult. Here, we are presenting a case report of 58 year old male presented with short history of high grade fever with extensive skin rash. Investigations revealed leucocytosis with gross eosinophilia. He gave history of taking carbamazepine for more than last 3 months prescribed by his neurologist. According to the RegiSCAR scoring system, our case could be classified as definite DRESS syndrome. After withdrawal of the offending drug carbamazepine and starting high dose of systemic corticosteroid, he improved dramatically. As this syndrome is quite uncommon, so we are reporting this case.Bangladesh J Medicine Jul 2018; 29(2) : 91-93

    Abstract: During COVID-19 pandemic, the increasing incidence of dengue in Monsoon season has become a further threat especially in the dengue endemic countries of Southeast Asia like Bangladesh. Co-infection of dengue with COVID-19 has created a number of practical challenges to combat the diseases simultaneously, especially at a time when healthcare resources are already exhausted tackling the COVID pandemic. We are presenting the case report of a 14 year old girl who suffered from both RT-PCR positive COVID -19 infection & NS1 positive Dengue. She was managed in a Private Hospital of Dhaka as Dengue fever Group C (due to severe plasma leakage) with severe COVID-19 infection (as required oxygen therapy). She had a critical course of illness, having continued fever, cough, breathlessness, desaturation along with thrombocytopenia & plasma leakage.     Abstract:   Klebsiella pneumoniae and Mycobacterium tuberculosis coinfection is one of the most lethal combinations with high mortality specially if there is delay in diagnosis and proper management. Patients with uncontrolled diabetes mellitus are prone to develop this type of serious infection.There is a considerable overlap in the clinical presentation of these critical pulmonary infections hence there is need for a high index of clinical suspicion, appropriate judicious investigations, and promt management to improve survivality. Here, we have presented Mycobacterium tuberculosis and Klebsiella pneumonia co-infection presenting as cough, chest pain, and shortness of breath in an elderly male with multiple co-morbidities, like uncontrolled diabetes mellitus, hypertension, prosthetic aortic valve in situ (for aortic stenosis) & previous history of ishchaemic stroke. This combination is rare and to the best of our knowledge not reported in the literature from Bangladesh. Bangladesh J Medicine 2022; 33: 52-57

    Abstract: The second most common cause of death is malignancy. In clinical practice, genitourinary tumours are commonly encountered. Urological malignancies are raising due to increasing tobacco consumption, air and water pollution (arsenic, nitrate, chlorinated hydrocarbon) by unbalanced development, unhealthy diet, chronic infection, uncontrolled pesticides & fungicides used, food adulteration, using Chinese herbal products containing aristolochic acid, obesity and longer life expectancy. Over the past few years, advancement of diagnostic imaging, surgical technique, radiotherapy, chemotherapeutics and targeted agents have helped improve treatment outcomes. The incidence of urological malignancies is increasing like other malignancies in Bangladesh, which is poorly addressed. There is generally poor documentation of urological malignancies in Bangladesh.
