    Abstract: In nonurban areas with limited access to thrombectomy-capable centers, optimal prehospital transport strategies in patients with suspected large-vessel occlusion stroke are unknown. Objective To determine whether, in nonurban areas, direct transport to a thrombectomy-capable center is beneficial compared with transport to the closest local stroke center. Design, Setting, and Participants Multicenter, population-based, cluster-randomized trial including 1401 patients with suspected acute large-vessel occlusion stroke attended by emergency medical services in areas where the closest local stroke center was not capable of performing thrombectomy in Catalonia, Spain, between March 2017 and June 2020. The date of final follow-up was September 2020. Interventions Transportation to a thrombectomy-capable center (n = 688) or the closest local stroke center (n = 713). Main Outcomes and Measures The primary outcome was disability at 90 days based on the modified Rankin Scale (mRS; scores range from 0 [no symptoms] to 6 [death] ) in the target population of patients with ischemic stroke. There were 11 secondary outcomes, including rate of intravenous tissue plasminogen activator administration and thrombectomy in the target population and 90-day mortality in the safety population of all randomized patients. Results Enrollment was halted for futility following a second interim analysis. The 1401 enrolled patients were included in the safety analysis, of whom 1369 (98%) consented to participate and were included in the as-randomized analysis (56% men; median age, 75 [IQR, 65-83] years; median National Institutes of Health Stroke Scale score, 17 [IQR, 11-21] ); 949 (69%) comprised the target ischemic stroke population included in the primary analysis. For the primary outcome in the target population, median mRS score was 3 (IQR, 2-5) vs 3 (IQR, 2-5) (adjusted common odds ratio [OR], 1.03; 95% CI, 0.82-1.29). Of 11 reported secondary outcomes, 8 showed no significant difference. Compared with patients first transported to local stroke centers, patients directly transported to thrombectomy-capable centers had significantly lower odds of receiving intravenous tissue plasminogen activator (in the target population, 229/482 [47.5%] vs 282/467 [60.4%]; OR, 0.59; 95% CI, 0.45-0.76) and significantly higher odds of receiving thrombectomy (in the target population, 235/482 [48.8%] vs 184/467 [39.4%]; OR, 1.46; 95% CI, 1.13-1.89). Mortality at 90 days in the safety population was not significantly different between groups (188/688 [27.3%] vs 194/713 [27.2%]; adjusted hazard ratio, 0.97; 95% CI, 0.79-1.18). Conclusions and Relevance In nonurban areas in Catalonia, Spain, there was no significant difference in 90-day neurological outcomes between transportation to a local stroke center vs a thrombectomy-capable referral center in patients with suspected large-vessel occlusion stroke.     Abstract: The long-term impact of intentional weight loss on cardiovascular events remains unknown. We describe 12-month changes in body weight and cardiovascular risk factors in PREvención con DIeta MEDiterránea (PREDIMED)-Plus, a trial designed to evaluate the long-term effectiveness of an intensive weight loss lifestyle intervention on primary cardiovascular prevention. RESEARCH DESIGN AND METHODS Overweight/obese adults with metabolic syndrome aged 55–75 years (n = 626) were randomized to an intensive weight loss lifestyle intervention based on an energy-restricted Mediterranean diet, physical activity promotion, and behavioral support (IG) or a control group (CG). The primary and secondary outcomes were changes in weight and cardiovascular risk markers, respectively. RESULTS Diet and physical activity changes were in the expected direction, with significant improvements in IG versus CG. After 12 months, IG participants lost an average of 3.2 kg vs. 0.7 kg in the CG (P & lt; 0.001), a mean difference of −2.5 kg (95% CI −3.1 to −1.9). Weight loss ≥5% occurred in 33.7% of IG participants compared with 11.9% in the CG (P & lt; 0.001). Compared with the CG, cardiovascular risk factors, including waist circumference, fasting glucose, triglycerides, and HDL cholesterol, significantly improved in IG participants (P & lt; 0.002). Reductions in insulin resistance, HbA1c, and circulating levels of leptin, interleukin-18, and MCP-1 were greater in IG than CG participants (P & lt; 0.05). IG participants with prediabetes/diabetes significantly improved glycemic control and insulin sensitivity, along with triglycerides and HDL cholesterol levels compared with their CG counterparts. CONCLUSIONS PREDIMED-Plus intensive lifestyle intervention for 12 months was effective in decreasing adiposity and improving cardiovascular risk factors in overweight/obese older adults with metabolic syndrome, as well as in individuals with or at risk for diabetes.

    Abstract: The mesozooplankton community was analyzed over a 6-year period (2013-2018) during the post-eruptive stage of the submarine volcano Tagoro, located south of the island of El Hierro (Canary Archipelago, Spain). Nine cruises from March 2013 to March 2018 were carried out in two different seasons, spring (March-April) and autumn (October). A high-resolution study was carried out across the main cones of Tagoro volcano, as well as a large number of reference stations surrounding El Hierro (unaffected by the volcano). The zooplankton community at the reference stations showed a high similarity with more than 85% of the variation in abundance and composition attributable to seasonal differences. Moreover, our data showed an increase in zooplankton abundance in waters affected by the volcano with a higher presence of non-calanoid copepods and a decline in the diversity of the copepod community, indicating that volcanic inputs have a significant effect on these organisms. Fourteen different zooplankton groups were found but copepods were dominant (79%) with 59 genera and 170 species identified. Despite the high species number, less than 30 presented a larger abundance than 1%. Oncaea and Clausocalanus were the most abundant genera followed by Oithona and Paracalanus (60%). Nine species dominated ( & gt;2%): O. media, O. plumifera , and O. setigera among the non-calanoids and M. clausi, P. nanus , P. parvus, C. furcatus, C. arcuicornis , and N. minor among the calanoids. After the initial low abundance of the copepods as a consequence of the eruption, an increase was observed in the last years of the study, where besides the small Paracalanus and Clausocalanus, the Cyclopoids seem to have a good adaptive strategy to the new water conditions. The increase in zooplankton abundance and the decline in the copepod diversity in the area affected by the volcano indicate that important changes in the composition of the zooplankton community have occurred. The effect of the volcanic emissions on the different copepods was more evident in spring when the water was cooler and the mixing layer was deeper. Further and longer research is recommended to monitor the zooplankton community in the natural laboratory of the Tagoro submarine volcano.

    Abstract: The diversity of protists was researched in the Alboran Sea (SW Mediterranean Sea) by means of high-throughput sequencing technologies based on the amplification of the V9 region of 18S rRNA. Samples were collected at different depths in seven stations following an environmental gradient from a coastal upwelling zone to the core of an oligotrophic anticyclonic gyre (AG). Sampling was performed during summer, when the water column was stratified. The superphyla Alveolata, Stramenopila and Rhizaria accounted for 84% of the total operational taxonomic units (OTUs). The most diverse groups were Dinophyceae (21% of OTUs), Marine Alveolates-II (MALV-II; 20%), Ciliophora (9%) and MALV-I (6%). In terms of read abundance, the predominant groups were Dinophyceae (29%), Bacillariophyta (14%), MALV-II (11%) and Ciliophora (11%). Samples were clustered into three groups according to the sampling depth and position. The shallow community in coastal stations presented distinguishable patterns of diatoms and ciliates compared with AG stations. These results indicate that there was a strong horizontal coupling between phytoplankton and ciliate communities. Abundance of Radiolaria and Syndiniales increased with depth. Our analyses demonstrate that the stratification disruption produced by the AG caused shifts in the trophic ecology of the plankton assemblages inducing a transition from bottom-up to top-down control.

    Abstract: In the Bay of Algeciras (BA), intensive urban and industrial activityis underway, which is potentially responsible for the release of significant quantities of nutrients. However, the assessment of the impact of these discharges is complex. Nutrient concentration in the surface layer is per se strongly variable due to the variability associated with the upwelling of nutrient-enriched deep Mediterranean water (MW), which in turn is regulated by atmospheric forcing. The aim of this study is to determine the effects of changes in the upwelling intensity on the load of nitrate and phosphate in the BA and to appraise their impact on chlorophyll a variability. Based on this analysis, the possible influence of the nutrients released from land-based sources is indirectly inferred. Data and samples collected during nine research cruises carried out in different seasonal cycle periods between 2010 and 2015 in the BA were analysed. The vertical variation of temperature and salinity indicates that the MW upwelling was favoured in spring, as occurred in other coastal areas of the northern Alboran Sea. However, principal component analysis conducted on physical and chemical data reveals that shifts in nutrients and chlorophyll a in the euphotic layer are poorly explained by changes in the upwelling intensity. Furthermore, during some of these research surveys (particularly in summer), chlorophyll a concentrations were higher in the BA as compared to a nearby coastal area also affected by MW upwelling. Scarce information about land-based pollution sources precludes quantitative analysis of the impact of nutrient loads on water quality; however, the available data suggest that the main source of allochthanous inorganic nitrogen over the period 2010–2015 in the BA was nitrate. Therefore, it is reasonable to hypothesize that the high concentrations of nitrate and chlorophyll a in BA in summer are a consequence of those discharges. Our study highlights the need of more exhaustive inventories of sewage and river discharges to adequately rate their impact in the BA.

    Abstract: The waters of the Canary Islands are considered a hotspot for marine biodiversity, especially regarding cetacean species. Based on this fact, this study pays attention to the spatial distribution pattern of cetacean species and the conservation role of the Natura 2000 Network, a set of Special Areas of Conservation (SACs), which were defined mainly based on data compiled in 1996, under the framework of the European Habitats Directive. In recent years, the declaration of conservation areas for cetaceans between the Tenerife—La Gomera Islands by two global conservation programs, Mission Blue (“Hope Spots”) and Whale Heritage Site (“Whale Sanctuary”) sent clear signals of scientific and social interest to promote better protection of the cetacean species in the Canary Islands. The main aim of the designated SACs is the conservation of its biological and ecological diversity, ensuring the long-term survival of the target species in the waters around islands. In this case, the enactment of the SACs was based only on the sparse data available for the common bottlenose dolphin, Tursiops truncatus . This study shows that the spatial distribution of cetaceans in the Canary archipelago generated from a large database of cetacean sightings, from 2007 to 2018. The results obtained show the main marine areas where the different cetacean species are distributed around the different islands of the archipelago. The spatial distribution maps of the cetacean species, when compared with the existing SACs of the Natura 2000, show the need to extend these SACs into the open sea to include more cetacean species and a larger number of individuals for better conservation of the endangered marine mammals. As a consequence, some suggestions were proposed to improve and update the role of SACs in European Northeast Atlantic waters as a key environmental tool for cetacean conservation. The data supporting the recent declarations of these two new milestones the “Hope Spot” and the “Whale Sanctuary” enhance more keystone information to promote a large marine protected area in the Eastern Atlantic Ocean, such as the “Macaronesian Biodiversity and Ecological Migration Corridor for Cetaceans,” a conservation figure that has been already proposed in the scientific literature as a deserving candidate of governmental regulations and policies by Portugal and Spain; it would also require joint cross-border cooperation efforts for marine spatial planning.
