    Abstract: 〈 p 〉 〈 strong 〉 Background: 〈 /strong 〉 Tibia shaft fractures are most common lower limb long bone fractures. In approximately 80% of patients, tibia fracture occurs in diaphysis region. The procedure of intramedullary interlocking nailing is done by positioning the injured extremity at minimum of 90 degree of flexion in the knee joint to achieve optimal exposure of correct entry point. The tension of quadriceps tendon causes a typical apex anterior angulation of proximal fragment. Supra patellar nailing is done with knee in semi extended position, reducing the anterior angulation. The aim of this study is to study outcomes of supra patellar access of intramedullary nailing for tibia fractures. 〈 /p 〉 〈 p 〉 〈 strong 〉 Methods: 〈 /strong 〉 It is a descriptive longitudinal study which was carried out from October 2019 to September 2021 in Dr. Balasaheb Vikhe Patil rural medical college, Loni, Ahmednagar. In this study 60 case of closed shaft tibia fractures were treated by supra-patellar nailing technique. 〈 /p 〉 〈 p 〉 〈 strong 〉 Results: 〈 /strong 〉 In our study we selected, 60 patients with tibia fracture admitting in our institute. All patients underwent operative procedure in the form of intramedullary interlocking tibia nail which was done with suprapatellar approach. Out of all the 60 patients treated with suprapatellar approach, excellent results with good range of movements with excellent lower extremity functional scale score. 〈 /p 〉 〈 p 〉 〈 strong 〉 Conclusions: 〈 /strong 〉 Supra-patellar approach was found to provide excellent outcome for proximal 1/3 〈 sup 〉 rd 〈 /sup 〉 of tibia fracture. 〈 /p 〉

    Abstract: Background: Tibia is more often involved in open fractures because of its subcutaneous location. Severe open fractures of tibia are associated with high complication rate and poor long term outcomes. 70% of pediatric tibial fractures are isolated injuries and approximately 30% of pediatric shaft tibia fractures are associated with fibula fracture. AIM: The aim is to study clinical and radiological outcomes of patient with tibial and femoral fractures, who are treated with closed reduction and internal fixation with nailing versus treated conservatively by casting. Material and method: 40 cases with femur or tibia fractures treated with nailing and conservatively between 1st January 2022 to 30thNovember 2022 at rural medical college, loni a rural tertiary care hospital was analyzed retrospectively out of which 16 patients treated conservatively and 4 patients underwent operative management for tibia fractures while 16 patients were treated operatively and 4 patients treated conservatively for femur fractures. Results: Total 40 patients of shaft femur and shaft tibia fractures were treated conservatively with cast application and Closed reduction and internal fixation by elastic nailing. In comparision for simple transverse fracture, we can conclude that in most patients fracture can be treated conservatively to minimize soft tissue damage but only in unacceptable criteria for conservative management we can opt for minimally invasive procedure like TENS nailing. Conclusion: Immobilization time is shorter in surgical group but we have started mobilizing with cast application. It is the only advantage of surgical management which has been given in conservatively managed patients. Outcomes after 3 months.

    Abstract: Background: Tibial plateau fractures are quite challenging for orthopaedic surgeons,and earlier, most of tibial plateau fractures were treated conservatively which resulted in joint line incongruity, early osteoarthritis, and knee stiffness. Now, treatment of these fractures has changed. We did this study to compare the outcome of different modalities of treatment in tibial plateau fractureswith advantages and disadvantages. Materials and Methods: During 2 years from April 2015 to May 2017, 30 patients were treated for closed tibial plateau fractures, of which 4 patients were treated by conservative methods, and 26 patients were treated by surgical methods. Results: Result showed the higher involvement of young, middle-aged males than older females with maximum Type I fractures. Operative results were better in complicated fractures which treated with buttress plate where there was no much difference in the outcome of simple TypesI and II fractures, and hence, we preferred conservative modality for them. Conclusion: The correct method of management of tibial condylar fractures depends on good clinical judgment. If rational treatment is to be instituted, the surgeon must have sound knowledge of the pattern of the injury and a clear understanding of the knee examination and imaging studies and must be familiar with a variety of techniques available at present for treating tibial condyle fractures.     Abstract: Background: Meniscal tears are the most common injury of the knee, with an incidence of meniscal injury resulting in meniscectomy of 66 per 100,000 populations per year. Menisci are essential for the normal function of the knee joint. In the modern diagnosis of meniscal injury and planning of the appropriate intervention, a good patient history and clinical examination have been supplemented with precise non-invasive imaging. Conservative approach has moved, with the correct indication, towards surgical treatment. Interestingly, the very first treatment for meniscal lesions was repair and not resection, suggesting that keeping the meniscus in place was the right approach all along. Aim and Objective: To study the Arthroscopic Management of Meniscal Injury of Knee Joint and its Functional Outcome Material and Methods: A prospective study was carried out among patients with meniscal injuries of knee joint and getting admitted under orthopaedic department at rural care center. All patients having meniscal injuries having age between 20 to 60 years, getting confirmed of meniscal injury on MRI and willing to participate were considered for the study. Thus such 50 cases were studied and evaluated. Patients with infection in and around the knee joint were excluded from the study. Data analysis was done using Software Open Epi version 2.3.1. Results: Mean age in years was 42.8+4.6. Majority 60% were females. Common mode of injury was road traffic accident (75%). Medial meniscus was commonly involved (80%). 48% cases showed good functional outcome, 32% showed excellent and 10% showed fair outcome. Complications were seen among 13% cases. Conclusion: The need to accurately evaluate the knee injuries is very crucial for the proper management and outcome; otherwise it will lead to chronic debility to the patient.     Abstract: 〈 p class="abstract" 〉 〈 strong 〉 Background: 〈 /strong 〉 The knee is a complex synovial joint allowing flexion, extension, anteroposterior gliding and internal-external rotation. The major articular surfaces are the medial and lateral condyles of the femur and patellar surface. The aim of the study was to compare the findings of clinical examination versus magnetic resonance imaging (MRI) for the diagnosis of ligamentous injuries of knee in rural area. 〈 /p 〉 〈 p class="abstract" 〉 〈 strong 〉 Methods: 〈 /strong 〉 Present study was an observational study carried out at rural medical college under orthopaedic department. All patients coming with ligamentous injury to orthopaedic department were studied. Thus such 50 cases were studied. All cases were referred to radiology department for MRI of knee. MRI reports and clinical reports were compared and analysis was done. Cases contraindicated for MRI examination were excluded from study. General demoghraphic history, clinical presentation and MRI reports were recorded. All data was compiled in Microsoft excel and analysed using OpenEPi 2.3.1. 〈 strong 〉 〈 /strong 〉 〈 /p 〉 〈 p class="abstract" 〉 〈 strong 〉 Results: 〈 /strong 〉 Mean age was 46.4±11.2 years. Majority were males 70% and 30% were females. Swelling 62% was most commonly seen followed by knee pain. Majority 58% cases had grade 3 tear on MRI, followed by 36% had grade II and only 6% had grade I.   〈 /p 〉 〈 p class="abstract" 〉 〈 strong 〉 Conclusions: 〈 /strong 〉 MRI is an excellent, non-invasive, radiation free imaging modality with multilane capabilities and excellent soft tissue delineation. It can accurately detect, localize and characterize various internal derangements of the knee joint and help in arriving at a correct anatomical diagnosis thereby guiding further management of the patient. 〈 /p 〉

    Abstract: 〈 p 〉 〈 strong 〉 Background: 〈 /strong 〉 Osteoarthritis (OA) is a degenerative joint disease due to degradation of articular cartilage, proliferative reformation of subchondral bone and low degree of synovitis resulting in reduced quality of life (QOL), being the major cause of pain and disability in the elderly population. Platelet-rich plasma (PRP) is an autologous mixture of concentrated platelets and growth factors produced by centrifugation of whole blood, used to treat bone, tendon and ligament injuries. The growth factors released by PRP have been shown to promote cell recruitment, proliferation and angiogenesis resulting in a decrease in the expression of inflammatory enzymes. The aim was to study the effectiveness of intra-articular PRP injections in early-stage OA knee patients and to evaluate the clinical outcome and QOL at 6 months. 〈 /p 〉 〈 p 〉 〈 strong 〉 Methods: 〈 /strong 〉 We conducted an observational study of 100 patients with early OA knee changes with age group & gt;50 years and injected their knees with PRP injections and serial follow up assessment was done. 〈 /p 〉 〈 p 〉 〈 strong 〉 Results: 〈 /strong 〉 We saw significant improvement in quality of lives evaluated as per world health organization quality of life (WHOQOL) questionnaire after PRP injections on their follow-up. Significant reduction in pain, reduction in joint stiffness and improvement in physical activities shown by significant reduction in Western Ontario and McMaster universities osteoarthritis index (WOMAC) scores. 〈 /p 〉 〈 p 〉 〈 strong 〉 Conclusions: 〈 /strong 〉 OA of knee is one of the commonest rheumatological problems after 50 years of age. It was commonly observed in females more than 60 years age with sedentary lifestyle. PRP therapy has given excellent results in pain relief, reduction of joint stiffness and improvement in physical activities and improvement in QOL. 〈 /p 〉

    Abstract: Background: Degenerative osteoarthritis of the knee joint, is typically the result of wear and tear and progressive loss of articular cartilage. Common clinical symptoms include knee pain that is gradual in onset and worse with activity, knee stiffness and swelling, crepitus. Patients with knee osteoarthritis have decreased levels of physical activity in comparison to their healthy counterparts. Aim: To study the role of arthroscopy in management of degenerative osteoarthritis of knee. Material and methods: A prospective study was carried out among patients having symptoms and signs of degenerative OA knee coming to Orthopedic OPD at Pravara Rural Medical College. Patients more than 50 years of age, medically fit for surgery, failed to improve on conservative treatment and willing to participate were considered for the study. Thus such 30 cases were studied. All cases were treated by arthroscopic interventions. Statistical analysis was done using open epi version 2.3.1. Results: Mean age was 58.7+12.4 years, majority being females 60%. 14% cases had meniscal tear. On Kellgreen and Lawrence’s arthroscopic grading majority had grade II and III. On final outcome on follow up showed excellent results. 48% excellent, 42% good, and 10% fair. Conclusion: Arthroscopic intervention is an effective method for treatment of degenerative osteoarthritis of knee in patients with negligible complications due to its minimally invasive nature and early mobilization. Patients with grade I and grade II degenerative osteoarthritis have good results.
