    Abstract: Postoperative complications increase the workload of nursing staff as well as the financial and mental distress suffered by patients. The objective of this study is to identify clinical factors associated with postoperative complications after liver cancer resection surgery. Methods Data from liver cancer resections occurring between January 1st, 2019 to December 31st, 2019 was collected from the Department of Liver Surgery in West China Hospital of Sichuan University. The Kruskal–Wallis test and logistic regression were used to perform single-factor analysis. Stepwise logistic regression was used for multivariate analysis. Models were established using R 4.0.2 software. Results Based on data collected from 593 cases, the single-factor analysis determined that there were statistically significant differences in BMI, incision type, incision length, duration, incision range, and bleeding between cases that experienced complications within 30 days after surgery and those did not. Stepwise logistic regression models based on Kruskal–Wallis test and single-factor logistic regression determined that BMI, incision length, and duration were the primary factors causing complications after liver resection. The adjust OR of overweight patients and patients with obesity (stage 1) compared to low weight patients were 0.12 (95% CI:0.02–0.72) with p = 0.043 and 0.18 (95% CI:0.03–1.00) with p = 0.04, respectively. An increase of 1 cm in incision length increased the relative risk by 13%, while an increase of 10 min in surgical duration increased the relative risk by 15%. Conclusions The risk of postoperative complications after liver resection can be significantly reduced by controlling factors such as bleeding, incision length, and duration of the surgery.

    Abstract: Vulnerability is a major concern for power networks. Malicious attacks have the potential to trigger cascading failures and large blackouts. The robustness of power networks against line failure has been of interest in the past several years. However, this scenario cannot cover weighted situations in the real world. This paper investigates the vulnerability of weighted power networks. Firstly, we propose a more practical capacity model to investigate the cascading failure of weighted power networks under different attack strategies. Results show that the smaller threshold of the capacity parameter can enhance the vulnerability of weighted power networks. Furthermore, a weighted electrical cyber-physical interdependent network is developed to study the vulnerability and failure dynamics of the entire power network. We perform simulations in the IEEE 118 Bus case to evaluate the vulnerability under various coupling schemes and different attack strategies. Simulation results show that heavier loads increase the likelihood of blackouts and that different coupling strategies play a crucial role in the cascading failure performance.

    Abstract: Background: Post-operative atrial fibrillation (POAF) is one of the most common complications of cardiac surgery. However, the underlying mechanism is not well understood. Alterations in the gut microbiota are associated with the development of atrial fibrillation (AF). The aim of this study was to explore the relationship between gut microbiota and POAF. Methods: Fecal samples were collected before surgery from 45 patients who underwent coronary artery bypass grafting with POAF and 90 matched patients without POAF (1:2). 16S rRNA sequencing was used to detect the microbiome profiles of 45 POAF patients and 89 matched patients (one sample in the no-POAF group was deleted owing to low quality after sequencing). Plasma 25-hydroxy vitamin D level was measured by ELISA. Results: Compared to the patients without POAF, gut microbiota composition was remarkably changed in the patients with POAF, with an increase in Lachnospira, Acinetobacter, Veillonella and Aeromonas, and a decrease in Escherichia–Shigella, Klebsiella, Streptococcus, Brevundimonas and Citrobacter. Furthermore, plasma 25-hydroxy vitamin D levels were decreased in POAF patients and negatively correlated with an abundance of Lachnospira. Conclusions: The gut microbiota composition between patients with and without POAF is significantly different, implying that gut microbiota may play a role in the pathogenesis of POAF. Further studies are needed to fully clarify the role of gut microbiota in the initiation of AF.

    Abstract: A high-sensitivity and large-capacity free space optical (FSO) communication scheme based on the soliton microcomb (SMC) is proposed. Using ultra-large bandwidth stabilized SMC with a frequency interval of 48.97 GHz as the laser source, 60 optical wavelengths modulated by 2.5 Gbit/s 16-Pulse position modulation (PPM) are transmitted in parallel. A corresponding outfield high-sensitivity 150 Gbit/s FSO communication experiment based on the SMC was carried out with 1 km space distance. Our experimental results show that the best sensitivity of the single comb wavelength which has higher OSNR can reach −52.62 dBm, and the difference is only 1.38 dB from the theoretical limit under the BER of 1 × 10 −3 without forward error correction (FEC). In addition, at BER of 1 × 10 −3 , 16-PPM has a higher received sensitivity of 6.73dB and 3.72dB compared to on-off keying (OOK) and differential phase shift keying (DPSK) respectively. Meanwhile, taking the advantage of multi-channel SMC, 60 × 2.5 Gbit/s can achieve 150 Gbit/s large-capacity free-space transmission. For comparison, commercially available single-wavelength laser based FSO communication system have also been performed in the outfield. The outfield experimental results demonstrated the feasibility of high-sensitivity, large-capacity PPM FSO communication based on SMCs and provided a new perspective for the future development of large-capacity, long-haul FSO communication.

    Abstract: DNA hydrogels have attracted increasing attention owing to their excellent permeability and high mechanical strength, together with thixotropy, versatile programmability and good biocompatibility. However, the moderate biostability and immune stimulation of DNA have arisen as big concerns for future potential clinical applications. Herein, we report the self‐assembly of a novel l ‐DNA hydrogel, which inherited the extraordinary physical properties of a d ‐DNA hydrogel. With the mirror‐isomer deoxyribose, this hydrogel exhibited improved biostability, withstanding fetal bovine serum (FBS) for at least 1 month without evident decay of its mechanical properties. The low inflammatory response of the l ‐DNA hydrogel has been verified both in vitro and in vivo. Hence, this l ‐DNA hydrogel with outstanding biostability and biocompatibility can be anticipated to serve as an ideal 3D cell‐culture matrix and implanted bio‐scaffold for long‐term biomedical applications.

    Abstract: Objectives. To investigate the potential role of extracellular cold-inducible RNA-binding protein (CIRP) in total aortic arch replacement under hypothermic circulatory arrest. Methods. The serum extracellular CIRP levels at five time points in 96 patients with Stanford A aortic dissection were detected. Overall change trend of CIRP levels at five time points was described, and the risk factors for 30-day mortality after surgery were analyzed. Results. The serum extracellular CIRP levels increased gradually after surgery, starting to rise significantly at approximately 12 h postoperatively, reaching or approaching a peak at approximately 24 h postoperatively, and ceasing to rise significantly after approximately 48 h postoperatively. Age, troponin-I, urodilatin, cooling time, cardiopulmonary bypass time, cross-clamp time, duration of surgery, and CIRP level at the end of surgery in the death group were significantly higher than those in the survival group. Multivariable analysis indicated that CIRP level at the end of surgery, age, urodilatin, and cross-clamp time were independent risk factors for postoperative 30-day mortality. Conclusion. Extracellular CIRP is closely related to the perioperative condition and prognosis of hypothermic circulatory arrest.

    Abstract: Reducing energy consumption of data centers is an important way for cloud providers to improve their investment yield, but they must also ensure that the services delivered meet the various requirements of consumers. In this paper, we propose a resource management strategy to reduce both energy consumption and Service Level Agreement (SLA) violations in cloud data centers. It contains three improved methods for subproblems in dynamic virtual machine (VM) consolidation. For making hosts detection more effective and improving the VM selection results, first, the overloaded hosts detecting method sets a dynamic independent saturation threshold for each host, respectively, which takes the CPU utilization trend into consideration; second, the underutilized hosts detecting method uses multiple factors besides CPU utilization and the Naive Bayesian classifier to calculate the combined weights of hosts in prioritization step; and third, the VM selection method considers both current CPU usage and future growth space of CPU demand of VMs. To evaluate the performance of the proposed strategy, it is simulated in CloudSim and compared with five existing energy–saving strategies using real-world workload traces. The experimental results show that our strategy outperforms others with minimum energy consumption and SLA violation.

    Abstract: Overweight and moderate obesity confer a survival benefit in chronic diseases such as coronary artery disease and chronic kidney disease, which has been termed the “obesity paradox”. However, whether this phenomenon exists in trauma patients remains controversial. We performed a retrospective cohort study in abdominal trauma patients admitted to a Level I trauma center in Nanjing, China between 2010 and 2020. In addition to the traditional body mass index (BMI) based measures, we further examined the association between body composition-based indices with clinical severity in trauma populations. Body composition indices including skeletal muscle index (SMI), fat tissue index (FTI), and total fat-to-muscle ratio (FTI/SMI) were measured using computed tomography. Our study found that overweight was associated with a four-fold risk of mortality (OR, 4.47 [95% CI, 1.40–14.97], p = 0.012) and obesity was associated with a seven-fold risk of mortality (OR, 6.56 [95% CI, 1.07–36.57] , p = 0.032) compared to normal weight. Patients with high FTI/SMI had a three-fold risk of mortality (OR, 3.06 [95% CI, 1.08–10.16], p = 0.046) and double the risk of an intensive care unit length of stay ≥ 5 d (OR, 1.75 [95% CI, 1.06–2.91] , p = 0.031) compared to patients with low FTI/SMI. The obesity paradox was not observed in abdominal trauma patients, and high FTI/SMI ratio was independently associated with increased clinical severity.
