    Abstract: Persistent sciatic artery (PSA) is a very rare congenital vascular anomaly, occurs in 0.025% to 0.04% of the population, and occasionally associated with thromboembolic events or aneurismal diseases. We herein share experience of diagnosis and successful treatment of a Vietnamese patient with PSA complicated with distal thromboembolism. A 70-year-old woman was admitted to the hospital for acute lower limb ischemia. A computed tomography angiography (CTA) revealed complete occlusion of the left common iliac artery. Surgical embolectomy was carried out and abnormal distal left femoral artery was noted intraoperatively. Postoperative CTA showed a persistent sciatic artery (PSA) on the left and adequate perfusion of the left lower limb. During 2 years follow-up, the patient remained symptom-free with good perfusion to the distal left popliteal artery, anterior and posterior tibial arteries.

    Abstract: The global water crisis is being exacerbated by climate change, even in the United States. Recycled water is a feasible alternative to alleviate the water shortage, but it is constrained by humans’ perceptions. The current study examines how residents’ water scarcity awareness and climate change belief influence their willingness to use recycled water directly and indirectly. Bayesian Mindsponge Framework (BMF) analytics was employed on a dataset of 1831 residents in Albuquerque, New Mexico, an arid inland region in the US. We discovered that residents’ willingness to use direct recycled potable water is positively affected by their awareness of water scarcity, but the effect is conditional on their belief in the impacts of climate change on the water cycle. Meanwhile, the willingness to use indirect recycled potable water is influenced by water scarcity awareness, and the belief in climate change further enhances this effect. These findings implicate that fighting climate change denialism and informing the public of the water scarcity situation in the region can contribute to the effectiveness and sustainability of long-term water conservation and climate change alleviation efforts.

    Abstract: Mục đích: Tồn tại động mạch (ĐM) ngồi là tình trạng hiếm gặp, chỉ chiếm 0,025% - 0,05% dân số. Bệnh nhân thường nhập viện vì các biến chứng như tắc mạch, thiếu máu mạn tính hoặc phình ĐM ngồi. Phương pháp: Chúng tôi xin chia sẻ kinh nghiệm chẩn đoán và điều trị một trường hợp, bệnh nhân nữ, 70 tuổi, vào viện sau 3 ngày bị tắc cấp tính do huyết khối từ động mạch chậu gốc trái, được phẫu thuật lấy huyết khối. Chụp cắt lớp vi tính mạch máu (CTA) và siêu âm sau mổ phát hiện tồn tại ĐM ngồi hai bên và huyết khối gây tắc ĐM ngồi bên trái. Kết quả: Điều trị nội khoa đạt kết quả khả quan sau 3 tháng phẫu thuật. Bệnh nhân hết tình trạng đau cách hồi. Siêu âm cho kết quả lưu thông động mạch chày trước, chày sau bình thường. Kết luận:  Tồn tại động mạch ngồi là tình trạng rất hiếm gặp. Chẩn đoán chủ yếu dựa vào CTA và siêu âm. Lựa chọn phương pháp điều trị phụ thuộc vào đặc điểm bất thường giải phẫu.

    Abstract: Plasmodium falciparum has acquired resistance to artemisinin in Southeast Asia, with mutations in the P. falciparum Kelch-13 ( Pfk13 ) gene associated with the resistance phenotype. The widespread use of Artemisinin-based combination therapy (ACT)s in Southeast Asia has led to the selection and spread of parasites carrying mutations in Pfk13 . We characterised the allele diversity of Pfk13 and pfg377 , an artemisinin-resistance neutral polymorphic gene, in parasite DNA extracted human blood from in southern Vietnam in 2003, 2012, 2015 and 2018. Method This study was conducted in Bu Gia Map commune, Binh Phuoc province, Vietnam, from May 2018 to January 2019. Twenty-four samples from 2018 to 2019, 30 from 2003, 24 from 2012 and 32 from 2015 were analysed. Malaria-infected human blood was collected by finger-prick and used for molecular analysis. A nested-PCR targeting the small subunit ribosomal RNA gene was used for Plasmodium species identification, followed by amplification and nucleotide sequencing of Pfk13 and region 3 of pfg377 . Archived blood samples collected in the same region in 2012 and 2015 were also analysed as above for comparison. Results The genetic diversity of Pfk13 and pfg377 was lower in 2018–2019 compared to 2012 and 2015. The number of distinct Pfk13 mutants decreased from three in 2012 and 2015, P553L, V568G and C580Y, to one, C580Y in 2018–2019. In 2018–2019, the frequency of C580Y mutant strains was 71% (17/24 isolates). All samples were wild type in 2003. In 2012 and 2015, there were single-strain infections as well as co-infections with two mutant strains or with mutant and wild strains, whereas there were no co-infections in 2018. pfg377 allele diversity decreased from five alleles in 2012 to two alleles in 2018–2019. Conclusion The genetic diversity of P. falciparum was reduced at the two genetic loci surveyed in this study, Pfk13 and pfg377 . In the case of the former gene, we observed an increase in the prevalence of parasites carrying the C580Y gene, known to confer reduced susceptibility to ACTs. The reduction in the diversity of pfg377 may be linked to the clonal expansion of parasite strains carrying the C580Y mutation, leading to an overall reduction in parasite genetic diversity across the population.

    Abstract: A carbon nanotube (CNT)/gold nanoparticle (NP) nanocomposite was synthesized by simultaneously reducing the Au ions and depositing Au NPs on the surface of a CNT. The functional groups were investigated with Fourier transform infrared spectra. From the Raman spectra, the D‐band and G‐band of the CNT were identified. The deposition of nanometer‐sized Au NPs on the CNT sites was observed by transmission electron microscopy. The photodegradation of methylene blue (MB) in aqueous solutions was studied using various photocatalysts, including TiO 2 , TiO 2 ‐SiO 2 , CNT/TiO 2 , CNT/TiO 2 ‐SiO 2 , Au/TiO 2 , CNT‐Au/TiO 2 , and CNT‐Au/TiO 2 ‐SiO 2 composites. CNT addition leads to a synergic effect, improving the photoactivity of the catalysts. A possible physically based mechanism was proposed involving the reduction of electron‐hole recombination and fast electron‐transfer possibility.

    Abstract: Diagnostic test evaluation for African swine fever (ASF) in field settings like Vietnam is critical to understanding test application in intended populations for surveillance and control strategies. Bayesian latent class analysis (BLCA) uses the results of multiple imperfect tests applied to an individual of unknown disease status to estimate the diagnostic sensitivity and specificity of each test, forgoing the need for a reference test. Methods Here, we estimated and compared the diagnostic sensitivity and specificity of a novel indirect ELISA (iELISA) for ASF virus p30 antibody (Innoceleris LLC.) and the VetAlert™ ASF virus DNA Test Kit (qPCR, Tetracore Inc.) in field samples from Vietnam by assuming that disease status 1) is known and 2) is unknown using a BLCA model. In this cross-sectional study, 398 paired, individual swine serum/oral fluid (OF) samples were collected from 30 acutely ASF-affected farms, 37 chronically ASF-affected farms, and 20 ASF-unaffected farms in Vietnam. Samples were tested using both diagnostic assays. Diagnostic sensitivity was calculated assuming samples from ASF-affected farms were true positives and diagnostic sensitivity by assuming samples from unaffected farms were true negatives. ROC curves were plotted and AUC calculated for each test/sample combination. For comparison, a conditionally dependent, four test/sample combination, three population BLCA model was fit. Results When considering all assumed ASF-affected samples, qPCR sensitivity was higher for serum (65.2%, 95% Confidence Interval [CI] 58.1–71.8) and OF (52%, 95%CI 44.8–59.2) compared to the iELISA (serum: 42.9%, 95%CI 35.9–50.1; OF: 33.3%, 95%CI 26.8–40.4). qPCR-serum had the highest AUC (0.895, 95%CI 0.863–0.928). BLCA estimates were nearly identical to those obtained when assuming disease status and were robust to changes in priors. qPCR sensitivity was considerably higher than ELISA in the acutely-affected population, while ELISA sensitivity was higher in the chronically-affected population. Specificity was nearly perfect for all test/sample types. Discussion The effect of disease chronicity on sensitivity and specificity could not be well characterized here due to limited data, but future studies should aim to elucidate these trends to understand the best use of virus and antibody detection methods for ASF. Results presented here will help the design of surveillance and control strategies in Vietnam and other countries affected by ASF.

    Abstract: U  tim  nguyên  phát  là  một bệnh  rất  hiếm  gặp,  chiếm  khoảng  0,0017  – 0,28%. Trong đó, u xơ đàn hồi thể nhú kết hợp với bệnh lý mạch vành cực kỳ hiếm gặp. Chúng tôi xin báo cáo một trường hợp theo dõi sau phẫu thuật 4 tháng u xơ đàn hồi thể nhú ở thất trái kết hợp với bệnh lý mạch vành. Bệnh nhân nữ 67 tuổi, vào viện vì đau tức ngực trái, siêu âm tim qua thành ngực và thực quản phát hiện u kích thước khoảng 14x18mm, bám xung quanh ½ trên chân trụ cơ trước bên, mật độ âm bằng cơ tim, di động nhiều, hở van 2 lá và van 3 lá nhẹ, không tăng áp phổi, chức năng tâm thu thất trái trong giới hạn bình thường. Chụp động mạch vành: hẹp 40% thân chung động mạch vành, 80% động mạch liên thất trước, 90% lỗ vào động mạch mũ và 80% động mạch vành phải. Bệnh nhân được phẫu thuật tim hở bằng đường mở ngực dọc giữa xương ức, cắt u thất trái qua đường mở nhĩ trái qua van 2 lá, bắc 3 cầu động mạch chủ - động mạch vành. Mô bệnh học sau mổ là u xơ đàn hồi thể nhú. Sau phẫu thuật bệnh nhân ổn định, ra viện sau mổ ngày. Hiện tại 4 tháng sau mổ bệnh nhân hoàn toàn khỏe mạnh, siêu âm tim không phát hiện tái phát.

    Abstract: African swine fever (ASF) is a notifiable viral disease of pigs and wild boars that could lead to serious economic losses for the swine industry. Objectives The aim of this study was to identify risk factors in the early phase of ASF outbreaks in Vietnamese swine herds during the first epidemic year. Methods The period of interest for this case–control study was February to July 2019. A questionnaire was administered in northern Vietnam where all early cases of ASF were reported. Producers of herds with reported cases were asked to provide information starting from the day of onset of clinical signs as well as 30 days prior to that day. The period of interest for controls was within the 6 months of the first outbreak in Vietnam (February 2019). Questionnaires included 55 questions; responses were received from 67 cases and 115 controls. Logistic regression analysis was used to identify factors associated with ASF status. Results Thirty‐seven variables of interest (among a total of 55 variables) were associated with ASF status in univariate analysis ( p   〈  0.05). These 37 variables were assessed for inclusion in the multivariate analysis by backward stepwise selection. Six variables remained significant as ASF risk factors in the final model: distance to farm within 500 m, distance of irrigation systems within 200 m, total number of pigs (≤500), absence of dressing rooms for workers/visitors before entering the farm, poor hygienic practices for people within the farm, and poor hygienic practices at pig loading/unloading locations. Conclusions These results may help in understanding the epidemiology of ASF in Vietnam and provide a scientific basis for optimization of current interventions and development of new tools and strategies to reduce transmission of ASF.

    Abstract: Tuần hoàn ngoài cơ thể ngày càng phát triển đáp ứng theo yêu cầu của phẫu thuật tim hở. Những tiến bộ về công nghệ máy và hệ thống phổi nhân tạo đã đem lại sự chính xác và an toàn của kỹ thuật. Tuy nhiên do kết cấu phức tạp của hệ thống nên vẫn tiềm tàng xảy ra lỗi kỹ thuật ở một số phổi nhân tạo. Do tỷ lệ gặp rất thấp nên việc đào tạo liên tục và luyện tập tình huống giả định thường xuyên là yếu tố quyết định giúp người thực hiện kỹ thuật phát hiện và xử trí kịp thời những biến cố nguy hiểm này. Chúng tôi xin thông báo một trường hợp mất chức năng oxy hóa máu của phổi nhân tạo được phát hiện trong giai đoạn bắt đầu chạy tuần hoàn ngoài cơ thể. Việc đưa ra quyết định và thực hiện thay phổi trong thời gian ngắn giúp giảm thiểu thấp nhất các bất lợi cho người bệnh.
