Abstract: Mục tiêu: Phân tích mối liên quan giữa các bất thường xuất hiện trong quá trình phân chia của phôi giai đoạn sớm và kết quả lâm sàng.

Abstract: Đặt vấn đề và mục tiêu: Mặc dù phương pháp tiêm tinh trùng vào bào tương noãn (ICSI) đã cải thiện tỷ lệ thụ tinh ống nghiệm trong những năm gần đây, tuy nhiên, vẫn có tỷ lệ khoảng 1-5% số ca thất bại thụ tinh hoàn toàn sau ICSI.

Abstract: Named entity recognition (NER) is a fundamental task which affects the performance of its dependent task, e.g. machine translation. Lifelong machine learning (LML) is a continuous learning process, in which the knowledge base accumulated from previous tasks will be used to improve future learning tasks having few samples. Since there are a few studies on LML based on deep neural networks for NER, especially in Vietnamese, we propose a lifelong learning model based on deep learning with a CRFs layer, named DeepLML–NER, for NER in Vietnamese texts. DeepLML–NER includes an algorithm to extract the knowledge of “prefix-features” of named entities in previous domains. Then the model uses the knowledge in the knowledge base to solve the current NER task. Preprocessing and model parameter tuning are also investigated to improve the performance. The effect of the model was demonstrated by in-domain and cross-domain experiments, achieving promising results.

Abstract: Of the estimated 10 million people affected by (TB) each year, one-third are never diagnosed. Delayed case detection within the private healthcare sector has been identified as a particular problem in some settings, leading to considerable morbidity, mortality and community transmission. Using unannounced standardised patient (SP) visits to the pharmacies, we aimed to evaluate the performance of private pharmacies in the detection and treatment of TB. Methods A cross-sectional study was undertaken at randomly selected private pharmacies within 40 districts of Vietnam. Trained actors implemented two standardised clinical scenarios of presumptive TB and presumptive multidrug-resistant TB (MDR-TB). Outcomes were the proportion of SPs referred for medical assessment and the proportion inappropriately receiving broad-spectrum antibiotics. Logistic regression evaluated predictors of SPs’ referral. Results In total, 638 SP encounters were conducted, of which only 155 (24.3%) were referred for medical assessment; 511 (80·1%) were inappropriately offered antibiotics. A higher proportion of SPs were referred without having been given antibiotics if they had presumptive MDR-TB (68/320, 21.3%) versus presumptive TB (17/318, 5.3%; adjusted OR=4.8, 95% CI 2.9 to 7.8). Pharmacies offered antibiotics without a prescription to 89.9% of SPs with presumptive TB and 70.3% with presumptive MDR-TB, with no clear follow-up plan. Conclusions Few SPs with presumptive TB were appropriately referred for medical assessment by private pharmacies. Interventions to improve appropriate TB referral within the private pharmacy sector are urgently required to reduce the number of undiagnosed TB cases in Vietnam and similar high-prevalence settings.

Abstract: This study describe the current situation and some influencing factors in attracting and retaining human resources in subclinical departments of Thu Duc District Hospital during the 2017-2019 period. This study use cross-sectional descriptive with qualitative and quantitative research methods combined. The study was conducted on 150 sub-clinical staffs of Thu Duc District Hospital from May 2020 to September 2020, in addition to verifying the evaluation reports. In the period of 2017 – 2019, subclinical departments had attracted 49 new employees and maintained 99 employees who remained in the workforce. Financial independence had contributed positively to increasing income and creating open policies to attract and retain human resources. In attracting and maintaining human resources in subclinical departments, positive factors were training and development; negative factors included: policies are not specific and leaders had not attached importance to clarify subjects, standard conditions and administration. The hospital need to improve infrastructure and working conditions. Heads of subclinical departments ought to pay more attention to policies to attract and retain human resources. Keywords: Attracting, retaining, human resources, subclinical, district hospital.

Abstract: This study examines the relationship between foreign direct investment (FDI), institutional quality and human development) in host countries from 2002 to 2019, using the Human Development Index [HDI] as the measure of human development. This study utilized a panel dataset of 143 countries, including both developed and developing economies, over a 17-year period. Additionally, the study employed a GMM (generalized method of moments) estimator to address unobservable heterogeneity and simultaneity. This study reveals a significant positive relationship between FDI and human development, with a stronger effect observed in developing countries compared to in developed countries. Notably, the impact of FDI-HDI nexus is larger in countries with moderately high-quality institutions, irrespective of their income level. Furthermore, good governance plays a crucial role in enhancing human development, as developing economies with high governance quality experience a greater impact of FDI on HDI compared to other countries. The findings of this study suggest that attracting FDI can be beneficial for enhancing the HDI, especially in developing countries. Additionally, the study highlights governance as a moderating factor in the relationship between FDI and HDI. Improving governance quality can enhance the positive impact of FDI on human development in host countries, especially in developing countries.

Abstract: The AFFINITY trial (Assessment of Fluoxetine in Stroke Recovery) reported that oral fluoxetine 20 mg daily for 6 months after acute stroke did not improve functional outcome and increased the risk of falls, bone fractures, and seizures. After trial medication was ceased at 6 months, survivors were followed to 12 months post-randomization. This preplanned secondary analysis aimed to determine any sustained or delayed effects of fluoxetine at 12 months post-randomization. Methods: AFFINITY was a randomized, parallel-group, double-blind, placebo-controlled trial in adults (n=1280) with a clinical diagnosis of stroke in the previous 2 to 15 days and persisting neurological deficit who were recruited at 43 hospital stroke units in Australia (n=29), New Zealand (4), and Vietnam (10) between 2013 and 2019. Participants were randomized to oral fluoxetine 20 mg once daily (n=642) or matching placebo (n=638) for 6 months and followed until 12 months after randomization. The primary outcome was function, measured by the modified Rankin Scale, at 6 months. Secondary outcomes for these analyses included measures of the modified Rankin Scale, mood, cognition, overall health status, fatigue, health-related quality of life, and safety at 12 months. Results: Adherence to trial medication was for a mean 167 (SD 48) days and similar between randomized groups. At 12 months, the distribution of modified Rankin Scale categories was similar in the fluoxetine and placebo groups (adjusted common odds ratio, 0.93 [95% CI, 0.76–1.14]; P =0.46). Compared with placebo, patients allocated fluoxetine had fewer recurrent ischemic strokes (14 [2.18%] versus 29 [4.55%] ; P =0.02), and no longer had significantly more falls (27 [4.21%] versus 15 [2.35%] ; P =0.08), bone fractures (23 [3.58%] versus 11 [1.72%] ; P =0.05), or seizures (11 [1.71%] versus 8 [1.25%] ; P =0.64) at 12 months. Conclusions: Fluoxetine 20 mg daily for 6 months after acute stroke had no delayed or sustained effect on functional outcome, falls, bone fractures, or seizures at 12 months poststroke. The lower rate of recurrent ischemic stroke in the fluoxetine group is most likely a chance finding. Registration: URL: http://www.anzctr.org.au/ ; Unique identifier: ACTRN12611000774921.
