    Abstract: The circadian rhythm is an endogenous process that has a periodicity of approximately 24 hours and stimulates the anticipation of repeated and regular events that occur daily, enabling the regulation of most major physiological systems in humans and animals. Thus, studies have shown that disruption of circadian rhythm and sleep is a frequent cause of a series of pathologies for the general population, including pregnant women and their fetus. The purpose of this article is to review the literature on the relationship between circadian rhythm and sleep disorders in pregnant women and the influence on the development of the fetus, as well as the future developing adult. Pregnancy is a critical period in which the systems of organs of a new human body are developing, and any disturbance in the environment in which the fetus grows can lead to developmental disorders that alter the metabolic and physiological functions of the fetus. Thus, maintaining an undisturbed circadian rhythm, including during pregnancy, protects the health of both the mother and the child, reducing the susceptibility to the development of certain postnatal diseases later in life.

    Abstract: Stroke in younger adults is a less common event in comparison to older ones, but there have been registered a negative trend and an earlier onset of stroke in the past decades. Stroke in younger adults represents an emerging public health issue that needs strengthening, prevention, early diagnosis, and management. The aim of the present research is to investigate the clinical features, risk factors and other diagnostic findings of the young population that developed stroke.

    Abstract: Major cognitive impairment is a significant public health problem despite advances in patient management. The purpose of the research was to analyze the knowledge, attitudes, and practices of mental health specialists regarding the management of major cognitive disorders in the Republic of Moldova. Material and method: The research were descriptive online survey type. A structured questionnaire with self-completion questions was used. The questionnaires were pre-tested and validated prior to conducting the research. Results: Barriers identified: 50.0% lack of social support services, 34.4% lack of a national guide/program, 46.9% lack of interdisciplinary collaboration, 43.8% problems in communicating with the patient, 25, 0% difficulties in establishing the diagnosis and developing the treatment plan, 31.3% in communication with relatives/caregiver, and 40.6% in the diagnosis and treatment of concomitant diseases. During consultations, 75.0% of those interviewed quantify the degree of impairment of cognitive function, 71.9% know national and international guidelines/protocols, 90.6% consider training in social and cultural activities useful, 75.0% – performing exercises physical therapy, 65.6% physical therapy, 56.3% cognitive-behavioral therapy, 53.1% speech therapy sessions, 46.9% psychotherapy, and 28.1% state that all options are useful. Positive attitudes 96.9% believe that the patient with cognitive disorders must benefit from access to medical services according to individual needs and that the patient has the right to live with dignity, without suffering, 81.3% are of the opinion that patients must be included in social and cultural activities, 62.5% support that people with cognitive disorders should benefit from institutionalization for palliative care. Negative attitudes 50.0% believe that patients consume time, emotions, and resources, and 25.0% mention that they are a burden on the healthcare system. Conclusion: The most frequent difficulties in the approach and management of patients with cognitive disorders are cited as the lack of social support services for patients or families, interdisciplinary collaboration/difficulty referring to other specialists and a national guide/program for cognitive disorders.

    Abstract: Transverse myelitis (TM) is a clinical syndrome in which an inflammatory process causes neuronal damage to the spinal cord, leading to varying degrees of muscle weakness, sensory changes, and autonomic dysfunction. MT can be a form of presentation of a multi-focal CNS disease, multi-systemic, infectious, paraneoplastic, demyelinating, autoimmune or idiopathic disease.The purpose of the research was to analyze the cases of transverse myelitis codes in patients treated and hospitalized in the “Diomid Gherman” Institute of Neurology and Neurosurgery.Material and method: 22905 medical records of patients treated in the Institute of Neurology and Neurosurgery between February 2018 and April 2022 were analyzed, 53 patients diagnosed with transverse myelitis were selected and 44 records were eligible, analyzed and included in the study. Results: The average age of the subjects included in the research was 50.9±11.7 years, they were on treatment for an average of 11.2±8.04 days. Neurological manifestations were presented in 43 people. The clinical presentation included: sensitivity disorders (75%), sphincteric disorders (61.4%), limb paresthesia (40.9%), motility disorders (95.5%), fever (4.5%), depression (6.8%), headache (15.9%), back pain (15.9%). The most frequent investigations performed on patients in the study group was CT/MRI thoracic, cervical, lumbo-sacral, cerebral, EMG. Lumbar puncture was performed in 32 patients (72.7%) with CSF examination. Laboratory analyzes showed signs of inflammation - 54.5%, elevated blood sugar - 40.8%, dyslipidemia - 38.6% and anemia - 25%. Corticosteroid treatment was performed in 25 subjects (56.8%) with improvement in most cases (80%). Frequent complications were urinary infection (52.3%), pneumonia (9.1%), 2 deaths (4.5%). Conclusion: The analysis of the institutional cohort of patients with transverse myelitis confirmed that the diagnosis of MT continues to present a challenge and requires a complex approach, correct both clinical and paraclinical.

    Abstract: Migraine is a disorder of multisensory integration of somatosensory, visual, auditory, olfactory stimuli, which subsequently can lead to various combinations of symptoms: headache, photophobia, phonophobia, nausea/vomiting, osmophobia.The purpose of the study: To isolate the characteristics of pain phenomenon (frequency, duration, and severity of migraine symptoms) in relation to associated symptoms in migraine patients. Materials and Methods: 94 patients with frequent episodic and chronic migraine were included in a cross-sectional study. Research methods used: clinical, descriptive, statistical. Migraine patient questionnaires were used as a research instrument. Primary data were entered into Excel programs and processed in MedCalc. Results and discussion: A statistically significant positive correlation between phonophobia and throbbing headache was observed in the conducted study. Statistically significant positive correlations were found between the duration of a migraine attack (in hours) and some migraine-related symptoms, such as: anxiety, shortness of breath. Conclusions: Thus, intensity rather than duration of headache may be the trigger for most, but not all, of the associated symptoms. This study demonstrated the clinical significance of the pain phenomenon and symptoms associated with migraine. In order to isolate the specificity of migraine attacks and elucidate their features, the study of autonomic symptoms accompanying the pain phenomenon is a particularly important part of establishing a correct diagnosis.

    Abstract: Despite the increasing number of reports on the spectrum of neurological manifestations of COVID‐19 (neuro‐COVID), few studies have assessed short‐ and long‐term outcome of the disease. Methods This is a cohort study enrolling adult patients with neuro‐COVID seen in neurological consultation. Data were collected prospectively or retrospectively in the European Academy of Neurology NEuro‐covid ReGistrY ((ENERGY). The outcome at discharge was measured using the modified Rankin Scale and defined as ‘stable/improved’ if the modified Rankin Scale score was equal to or lower than the pre‐morbid score, ‘worse’ if the score was higher than the pre‐morbid score. Status at 6 months was also recorded. Demographic and clinical variables were assessed as predictors of outcome at discharge and 6 months. Results From July 2020 to March 2021, 971 patients from 19 countries were included. 810 (83.4%) were hospitalized. 432 (53.3%) were discharged with worse functional status. Older age, stupor/coma, stroke and intensive care unit (ICU) admission were predictors of worse outcome at discharge. 132 (16.3%) died in hospital. Older age, cancer, cardiovascular complications, refractory shock, stupor/coma and ICU admission were associated with death. 262 were followed for 6 months. Acute stroke or ataxia, ICU admission and degree of functional impairment at discharge were predictors of worse outcome. 65/221 hospitalized patients (29.4%) and 10/32 non‐hospitalized patients (24.4%) experienced persisting neurological symptoms/signs. 10/262 patients (3.8%) developed new neurological complaints during the 6 months of follow‐up. Conclusions Neuro‐COVID is a severe disease associated with worse functional status at discharge, particularly in older subjects and those with comorbidities and acute complications of infection.

    Abstract: The aim of this study was to assess the neurological complications of SARS‐CoV‐2 infection and compare phenotypes and outcomes in infected patients with and without selected neurological manifestations. Methods The data source was a registry established by the European Academy of Neurology during the first wave of the COVID‐19 pandemic. Neurologists collected data on patients with COVID‐19 seen as in‐ and outpatients and in emergency rooms in 23 European and seven non‐European countries. Prospective and retrospective data included patient demographics, lifestyle habits, comorbidities, main COVID‐19 complications, hospital and intensive care unit admissions, diagnostic tests, and outcome. Acute/subacute selected neurological manifestations in patients with COVID‐19 were analysed, comparing individuals with and without each condition for several risk factors. Results By July 31, 2021, 1523 patients (758 men, 756 women, and nine intersex/unknown, aged 16–101 years) were registered. Neurological manifestations were diagnosed in 1213 infected patients (79.6%). At study entry, 978 patients (64.2%) had one or more chronic general or neurological comorbidities. Predominant acute/subacute neurological manifestations were cognitive dysfunction ( N  = 449, 29.5%), stroke ( N  = 392, 25.7%), sleep–wake disturbances ( N  = 250, 16.4%), dysautonomia ( N  = 224, 14.7%), peripheral neuropathy ( N  = 145, 9.5%), movement disorders ( N  = 142, 9.3%), ataxia ( N  = 134, 8.8%), and seizures ( N  = 126, 8.3%). These manifestations tended to differ with regard to age, general and neurological comorbidities, infection severity and non‐neurological manifestations, extent of association with other acute/subacute neurological manifestations, and outcome. Conclusions Patients with COVID‐19 and neurological manifestations present with distinct phenotypes. Differences in age, general and neurological comorbidities, and infection severity characterize the various neurological manifestations of COVID‐19.

    Abstract: Objectives. To confirm the efficiency of using local anesthesia with lidocaine and epinephrine in hand surgery. Materials and methods. 75 patients in whom we used local anesthesia with 1% lidocaine and epinephrine in solution 1: 100 000. We evaluated the amount of used anesthetic, the time to onset of anesthesia, intraoperative bleeding, the duration of intervention and hospitalization, immediate postoperative complications. Results. It has been no case of digital necrosis or other vascular complications and no intraoperative bleeding. The amount of the used anesthetic varied. In no case it was necessary to use of phentolamine as an antidote to the effects of adrenaline. Conclusions. Wide awake local anesthesia no tourniquet technique is safe, having many advantages: it is not necessary to use the tourniquet with or without intravenous sedation, the surgeon and patient comfort is maximum, there is no risk of digital necrosis, hospitalization time is short and the costs are minimal.
