    Abstract: Abstract: 15 kids (1-3 years old) with recurrent infections received therapy with Broncho-munal (1 capsule/day) from the day 1 till the day 30 after measles vaccination. The control group of 29 kids of the same age did not receive any immunomodulatory therapy after the vaccination. 12 out of 15 (80%) kids in the Broncho-munal group developed no complications after the vaccination compared with 20 out of 29 kids (68,9%) in the control group. The frequency of intercurrent infections was 3,5 times lower in the Broncho-munal group. The kids in the Broncho-munal group developed neither lymphopenia nor leukopenia. They had increased counts of CD25+ and CD20+ cells by the day 30 after the vaccination, prolonged production of the specific antibodies as well as higher geometric mean titers of the anti-measles antibodies on the day 45 after the vaccination. Taken together these observations suggest that Broncho-munal protected from the transitory immunosuppressive effects of the vaccination.

    Abstract: Relevance. Cancer therapy forms a temporary immunosuppressive state, which determines an increase in the frequency and severity of infectious diseases. Vaccination is a highly effective and safe way to protect against infection, but people with immunodeficiency have risks of inefficiency and complications. To substantiate the need for immunoprophylaxis in cancer patients after therapy, it is important to understand the preservation of their specific response after previous vaccinations. The aim of the study was to assess the safety of antibodies to vaccine–controlled infections in children with oncological diseases after therapy Materials and methods. The safety of antibodies to vaccine-controlled infections was studied in 3 groups: 1 -in patients with oncological (n=62); 2-in the group (n=43) withoutoncological diseases, but who received immunosuppressive (IST) and/or polychemotherapy (PCT) and/or hematopoietic stem cell transplantation (HSCT), and 3– in healthy children (n=31 – comparison group). The concentration of antibodies was determined by the ELISA method. The minimum protective level was considered to be the amount for measles ³0.18 IU/ml, rubella - ³25 IU/ml; hepatitis B - ³10 IU/ml; diphtheria – 0.03 IU/ml and higher. The coefficient of positivity, estimated as protective against mumps, was ³1.0. Results. It was found that from 41.7% to 93.7% of children with cancer lose post-vaccination immunity to the studied vaccine antigens. The number of children who retained the protective level of antibodies in groups 1 and 2 was significantly less than in the comparison group. There were no significant differences in the level of those protected from diphtheria and rubella. The maximum effect on the loss of antibodies is provided by the performed HSCT. For diphtheria and rubella antibodies, the differences are not pronounced. The possible connection of genetic breakdowns in 35 examined children with oncological diseases and the safety of antibodies was analyzed. It turned out that in the presence of chromosomal deletions, antibodies to measles were lost in 100% of cases and to diphtheria in 75%, which was different from other chromosomal abnormalities. Conclusion. The safety of antibodies in patients with a history of cancer is influenced by the presence of HSCT in therapy, the type of genetic breakdown, as well as the peculiarity of the vaccine antigen. Children with oncological diseases, as well as with non-oncological ones, but who have received HSCT therapy, should be vaccinated again against vaccine-controlled infections, despite the indication of the presence of vaccinations before therapy.

    Abstract: The formation of biobanks in the structure of scientific and treatment and diagnostic institutions with prospects for interregional integration is a fundamental link in monitoring and predicting diseases of various origins, creating and testing highly effective diagnostics, and developing novel therapeutic agents. Aim . To describe standard operating procedures and principles for the formation of bioresource collections (BRC) in medical institutions with biobanking. Material and methods . The data of scientific and practical biomedical projects using BRC obtained from patients with genetic, multifactorial and infectious diseases in St. Petersburg and Surgut are presented. As of September 2022, the BRC collected on the basis of the Pediatric Research and Clinical Center for Infectious Diseases of the Federal Medical and Biological Agency includes biosamples from 1619 patients, and the BRC collected in the Medical Institute of Surgut State University includes biosamples from 450 patients and healthy individuals of different sex and age. The selection of biosamples from apparently healthy individuals and patients with various diseases can serve as a strategically important resource for future research in terms of etiology, epidemiology, the development of regulatory environment and scales, innovations in the development of diagnostic approaches and treatment of the Russian population.

    Abstract: The study was carried out to evaluate timeliness of detection of cervix malignant neoplasms against the background of supply of necessary manpower in the region. The indices of active detection of cervix neoplasm and its stages, degree of neglect of tumor process and lethality during the first year of diagnosed disease are considered. The enumerated indices are correlated with quality of medical services шт part of availability and characteristics of training of available personnel. The comparison of data of the Omsk region with average national data was implemented. The corresponding indices of Omsk and municipal districts of oblast in 2004-2014 were also compared. The percentage of active detection ofcervix cancer decreased to the end ofanalyzed period. However, in municipal districts of oblast it continues to be quite high. The detection of disease at early stages (I-II) in the region is on the whole significantly lower as compared with average national indices. The indices of degree of neglect of tumor process in Omsk, in spite of better conditions of diagnostic of this pathology, are significantly lower as compared with corresponding indices in municipal districts. During analyzed period, annual lethality in the Omsk region decreased up to 1.6 times (decreasing rate - 35.6%). The process proceeded especially actively in rural districts of the oblast (decreasing rate - 70.5%). The lethality at first year of diagnosed disease was absent in 25 municipal districts of region. The staffing of oncologists in "The clinical oncologic dispensary" makes up to 100% and 36.8% out of them have highest category. In the central district hospitals, the staffing of specialists made up to 81.3%. Factually in all institutions this position is combined by specialists of another profile i.e. therapists, obstetrician-gynecologists, surgeons, etc.

    Abstract: Purpose : To characterize clinical and laboratory features of infectious mononucleosis (IM) of EBV and HHV-6 etiology among children. Material and methods . A single-center prospective study was performed at Pediatric Research and Clinical Center for Infectious Diseases in 2021 — 2022, which included 84 patients with IM EBV (n = 40), HHV-6 (n = 11), EBV + HHV-6 (n = 33). Methods of laboratory diagnostics were clinical, biochemical blood tests, qualitative and quantitative PCR of blood (whole blood, plasma) to herpesvirus 4—6 types. Results. Patients with EBV IM were significantly older than children with HHV-6 IM and EBV + + HHV-6 IM (р ≤ 0,004). The main complaint of patients was fever (median 39,0°С, Q1—Q3 38,5—39,3°С). Nasal stuffiness (93%), tonsillitis (86%), peripheral cervical lymphadenopathy (100%) dominated among clinical manifestations in all groups. Splenomegaly was noted more rarely than hepatomegaly in children. There were no significant differences in the level of leukocytes among patients (р 〉 0,05). Increased values of lymphocytes, atypical mononuclear cells, hepatic cytolysis syndrome were characteristic of EBV-monoinfection (р ≤ 0,007). On the contrary, level of neutrophils was significantly higher in the HHV-6 IM group. Genotype HHV-6B was detected in 100% of cases. Assay of EBV viral load showed following: this indicator in plasma (р = 0,0008) and whole blood (р = 0,028) in EBV patients was significantly higher than among EBV + HHV-6 IM. A direct moderate correlation was found between viral loads in different human fluids (r = 0,413, р 〈 0,05). Conclusions. The results emphasize the importance of clinical and etiological diagnosis, including HHV-6 determination, for the well-timed registration of children with IM and their coverage with regular medical check-up.

    Abstract: Currently, the most effective way to diagnose hereditary defects of the immune system is molecular genetic research, the results of which are evaluated in conjunction with the data of clinical and laboratory studies. Aims of the sudy: to evaluate the frequency and spectrum of rare genetic variants associated with the development of primary immunodeficiency (PID) in children with recurrent infections. Materials and methods: DNA samples from 113 children with recurrent infections were analyzed by targeted multigene sequencing of 338 PID-associated genes. Results: Pathogenic variants appropriate to the potential diagnosis of PID were identified in 8% of patients. Interestingly, 47.8% of children had variants associated with auto-inflammatory disorders.

    Abstract: In the article there are presented results of the analysis of the questionnaire survey of 236 respondents, the executed survey according to specially developed questionnaire has been directed to the study of medico-social features of patients of the gynecologic unit of the Clinical Oncologic Dispensary. The typical patient was established to be typical urban resident aged of 52.6 ± 0.6 years, who has a college education (the highest or specialized secondary), satisfactory living conditions, married and equal smooth relationships in a family. On the one hand, the most part of women (76.3%) were quite active physically during the day (are involved in the work on a personal plot, the private house or played sports), from the other side - 33,8% of respondents had excess weight (25 〈 IMT 〈 30). 82.2% of women indicated that they had financial difficulties. The presence of such a harmful habit as smoking was demonstrated by 16.9% of respondents, however, the majority of women (56.7%) do not drink alcohol at all. The obstetric and gynecologic history included menses in the period from 12 to 14 years (79.3%), the most part of the interviewed cases did not show disorders in menses of various character. Average age of the beginning of sex life accounted for 19.5 ± 0.12 years, the average number of pregnancies per 1 woman amounted to 3.4 ± 0.12. Among the leading somatic diseases there were indicated diseases of the circulatory system (27.5%), digestive tract (19,4%), the musculoskeletal system (14%), urinary tract (9.9%) and pathology of respiratory organs (7.5%). The hereditary burdened malignant process took place in 42.6% of respondents, at the same time in most of respondents (51.3%) the disease was registered for the first time.

    Abstract: The aim: to characterize the clinical and etiological features of facial palsy (FP) in children of different ages at the present stage and their correlation with the outcome and duration of the disease. Materials and methods: the data of 68 children with FP were obtained. The neurological examination with an assessment of the level of facial nerve damage and the severity of FP using the House-Brackmann scale (HB), the concomitant symptoms and the etiological verification of a possible infectious agent were performed to all children. Also, their correlation with the outcomes and duration of the disease were assessed. Results: in the etiological structure infectious FP are more common in children under 12 years of age, with a predominance of herpesviruses and enteroviruses, whereas, more than half of the cases in children over the age of 12 years are idiopathic (Bell’s palsy), less often herpesviruses. A longer course and the probability of an unfavorable are significantly more often observed in the group of children under 12 years of age. There was also a significant correlation between the severity of FP according to HB with the outcomes and duration of the disease. Conclusion: The results confirm the importance and necessity of laboratory confirmation of a possible etiological agent associated with the development of FP. The severity of FP doesn’t depend on the age of the child, but correlates with the outcome and duration of the disease, more less with the synkinesia. A promising direction for further research is to clarify the climatic factors affecting the incidence of FP.
