    Abstract: Whether imaging parameters would independently predict stroke recurrence in low-risk minor ischemic stroke (MIS) or transient ischemic attack (TIA) according to traditional score system (such as ABCD 2 score, which was termed on the basis of the initials of the five factors: age, blood pressure, clinical features, duration, diabetes) remains unclear. We sought to evaluate the association between imaging parameters and 1-year stroke recurrence in patients with TIA or MIS in different risk stratum stratified by ABCD 2 score. Methods: We included patients with TIA and MIS (National Institutes of Health Stroke Scale score ≤3) with complete baseline vessel and brain imaging data from the Third China National Stroke Registry III. Patients were categorized into different risk groups based on ABCD 2 score (low risk, 0–3; moderate risk, 4–5; and high risk, 6–7). The primary outcome was stroke recurrence within 1 year. Multivariable Cox proportional-hazards regression models were used to assess whether imaging parameters (large artery stenosis, infarction number) were independently associated with stroke recurrence. Results: Of the 7140 patients included, 584 patients experienced stroke recurrence within 1 year. According to the ABCD 2 score, large artery stenosis was associated with higher stroke recurrence in both low-risk (adjusted hazard ratio, 1.746 [95% CI, 1.200–2.540]) and moderate-risk group (adjusted hazard ratio, 1.326 [95% CI, 1.042–1.687] ) but not in the high-risk group ( P 〉 0.05). Patients with multiple acute infarctions or single acute infarction had a higher risk of recurrent stroke than those with no infarction in both low- and moderate-risk groups, but not in the high-risk group. Conclusions: Large artery stenosis and infarction number were independent predictors of 1-year stroke recurrence in low-moderate risk but not in high-risk patients with TIA or MIS stratified by ABCD 2 score. This finding emphasizes the importance of early brain and vascular imaging evaluation for risk stratification in patients with TIA or MIS.

    Abstract: The extraordinary mode (X-mode) lower cut-off frequency is proposed for use in the reflectometry diagnostic on ITER for the electron density profile measurement, which is a trade-off between extreme plasma parameters and the accessible probing frequency. In contemporary experiments, the lower cut-off frequency can be identified at the probing frequency below the electron cyclotron frequency ( f ce ) under certain plasma conditions. We provide here, for the first time, the experimental validation of the use of the lower cut-off frequency for the density profiles via the reflectometry measurement on EAST. The corresponding group delay of the lower cut-off frequency evolves continuously with the upper one, revealing a reasonable radial coverage extension of reflectometry measurement toward the plasma core. It is concluded that the lower cut-off frequency can be used as a supplement to the upper one in the density profile inversion process, which is of particular interest in the high magnetic field and/or density discharge to extend the radial coverage of reflectometry measurement.

    Abstract: Traditional qPCR instrument is combined with CMOS and a personal computer, and a photoelectric feedback automatic fluorescence detection system is designed to realize quantitative real-time PCR. The key to reaction efficiency lies in how to ensure that the temperature of the detection reagent completely matches the set temperature. However, for most traditional real-time fluorescent PCR systems, the temperature cycling is controlled by detecting the temperature of the heating well plate. It cannot directly measure the temperature in the reaction reagent PCR tube, which will cause the deviation in the actual temperature of the reagent to be as expected. Therefore, in this paper, we raise a method of directly detecting the temperature in the reaction tube of the reagent during the temperature cycling is adopted. According to the deviation from the expected value, the set temperature of the PCR instrument is adjusted to make the actual temperature of the reagent closer to the expected value. Through this method, we also realized the temperature calibration and optimization of the TEC circulation system we built. Experiments show that this low-cost, portable real-time quantitative PCR system can detect and analyze pathogens in situ.

    Abstract: A coherent mode (CM) induced by the supersonic molecular beam injection (SMBI) was observed in Ohmic plasmas of EAST tokamak. The CM existed in the spectra of the electron temperature fluctuation and density fluctuation, but not in the spectrum of Mirnov fluctuation. The radial location of the CM was about 10 cm inside the separatrix (normalized minor radius ρ ∼ 0.75 ). In most cases, the CM has multiple discrete frequencies between 5 ∼ 20 kHz with an interval of ∼1 kHz. Occasionally, there is only one dominant frequency. The poloidal scale of the CM (poloidal wavenumber k θ ∼ 0.4 c m − 1 , poloidal mode number m ∼ 20 ) is between that of typical ion turbulence and macroscopic magnetohydrodynamic activities. Statistical analysis results suggest that the onset of the CM requires a high temperature gradient and a relatively low density gradient. Furthermore, the transport analysis shows that the SMBI-caused perturbation propagates inward faster in plasmas with stronger CM, which implies that the CM might be correlated to a high local transport level. However, the impact of the CM on macroscopic plasma parameters including stored energy, energy confinement time, and SMBI-caused mean density fluctuation is too weak to be found in comparison with the random fluctuation and the measurement errors, which suggests that the CM does not have a clear influence on the global confinement.

    Abstract: Fluorescent quantitative PCR (qPCR) and digital PCR (dPCR) are two mainstream nucleic acid quantification technologies. However, commercial dPCR and qPCR instruments have a low integration, a high price, and a large footprint. To solve these shortcomings, we introduce a compound PCR system with both qPCR and dPCR functions. All the hardware used in this compound PCR system is commercially available and low-cost, and free software was used to realize the absolute quantification of nucleic acids. The compound PCR provides two working modes. In the qPCR mode, thermal cycling is realized by controlling the reciprocating motion of the x axis. The heating rate is 1.25 °C s −1 and the cooling rate is 1.75 °C s −1 . We performed amplification experiments of the PGEM-3zf (+)1 gene. The performance level was similar to commercial qPCR instruments. In the dPCR mode, the heating rate is 0.5 °C s −1 and the cooling rate is 0.6 °C s −1 . We performed the UPE-Q gene amplification and used the sequential actions of the two-dimensional mechanical sliders to scan the reaction products and used the method of regional statistics and back-inference threshold to get test results. The result we got was 1208 copies per μL −1 , which was similar to expectations.

    Abstract: Compared to carotid endarterectomy, carotid artery stenting (CAS) is reportedly associated with higher perioperative risks in elderly patients. To verify the long-term safety and efficacy of CAS with embolic protection in elderly patients, we retrospectively reviewed the medical records of patients with carotid stenosis treated between January 2003 and March 2010 at the Department of Neurology of a large university hospital in China. We included patients with symptomatic, moderate, or severe carotid stenosis of atherosclerotic etiology (other etiologies were excluded), with a disability score ≤ 3 on the modified Rankin Scale, and who received CAS instead of carotid endarterectomy. The clinical endpoints studied were stroke recurrence and all-cause death. The 84 patients included in this study (median follow-up, 8.08 years) were stratified according to age at surgery ( 〈 70 vs. ≥70 years), and no significant between-group difference was found regarding baseline characteristics. Of the 14 patients (16.67%) who experienced a defined clinical endpoint, 4 (7.14%) were aged 〈 70 years and 10 (35.71%) were aged ≥70 years ( P = 0.002 ). Overall mortality was 14.29% (12/84), with 3 (5.36%) and 9 (32.14%) deaths among patients aged 〈 70 and ≥ 70 years, respectively ( P = 0.002 ). Heart disease and cancer accounted for most deaths. The two groups did not differ regarding stroke recurrence, disability score, or rate of in-stent restenosis (blockage ≥ 50%), but patients aged ≥70 years had a higher risk of mortality (odds ratio, 8.3684; 95% confidence interval, 2.048–34.202; P = 0.003 ), and age was an independent risk factor for death (odds ratio, 20.054; 95% confidence interval, 3.094–129.987, P = 0.002 ). Among elderly patients in Southwest China, CAS can effectively prevent stroke recurrence without increasing the risk of stroke-related death, but the risk of all-cause death is higher, with age as an independent risk factor. Careful patient selection is of key importance in the treatment of symptomatic carotid artery stenosis.

    Abstract: Ginkgo diterpene lactone meglumine (GDLM) has attracted much attention because of its potential neuroprotective properties in ischemic stroke. The efficacy of GDLM in patients with acute ischemic stroke (AIS) needs to be verified by well-designed randomized clinical trials. Objective To assess the efficacy and safety of GDLM in patients with AIS. Design, Setting, and Participants This multicenter, randomized, double-blind, placebo-controlled, parallel-group trial involved 3448 patients who had AIS, were aged 18 to 80 years, had a clinically diagnosed AIS symptom within 48 hours of onset, had a modified Rankin Scale (mRS) score of 0 or 1 prior to onset, and had a National Institutes of Health Stroke Scale score ranging from 4 to 24. The trial took place at 100 centers in China from February 1, 2016, to May 1, 2018. The mRS is a global stroke disability scale with scores ranging from 0 (no symptoms or completely recovered) to 6 (death). The National Institutes of Health Stroke Scale is a tool used by clinicians to quantify impairment caused by stroke (range, 0-42, with higher scores indicating greater severity). Data were analyzed from January 2019 to December 2022. Interventions Patients were randomized to receive GDLM or placebo once daily via intravenous infusion in a 1:1 ratio. The treatment was dispensed within 48 hours after symptoms and continued for 14 days. Interventions of thrombolysis and thrombectomy were not permitted during the treatment. Main Outcomes and Measures The primary outcome was the proportion of patients with an mRS of 0 or 1 on day 90 after randomization. Safety outcomes included adverse events and serious adverse events. Results A total of 3448 patients were randomized, with 1725 patients assigned to the GDLM group and 1723 patients assigned to the placebo group. The median (IQR) age of the patients was 63 (55-71) years, and 1232 (35.7%) were women. The primary outcome on day 90 occurred in 877 patients (50.8%) in the GDLM group, and 759 patients (44.1%) in the placebo group (risk difference, 6.79%; 95% CI, 3.46%-10.10%; odds ratio, 1.31; 95% CI, 1.15-1.50; relative risk, 1.15; 95% CI, 1.08-1.24; P   & amp;lt; .001). Adverse events occurred relatively equally between the 2 groups (303 [17.6%] vs 298 [17.3%] ; risk difference, 0.27%; 95% CI, −2.26% to 2.80%; odds ratio, 1.02; 95% CI, 0.85-1.21; relative risk, 1.02; 95% CI, 0.88-1.17; P  = .83). Conclusions and Relevance Among patients with AIS in this randomized clinical trial, GDLM improved the proportion of patients achieving favorable clinical outcomes at 90 days compared with placebo.     Abstract: A new long-pulse high electron temperature ( T e ) regime has been achieved on experimental advanced superconducting tokamak by pure radio frequency heating. In this new scenario, there are mainly two confinement states involving two magneto-hydrodynamic (MHD) modes, one of which is identified as m / n  = 1/1 kink mode (where m and n are the poloidal and toroidal mode numbers, respectively). The frequency evolution of the kink mode is investigated through the three-dimensional, toroidal, and nonlinear Hall-MHD code CLT. We firstly find that the frequency of the m / n  = 1/1 kink mode significantly increases during each sawtooth crash and then confirmed it through the experimental data. The simulation results indicate that the increase of the mode frequency is mainly due to the significant increase of the electron diamagnetic frequency nearby the reconnection region. We have also observed the internal transport barrier (ITB) during the m / n  = 1/1 kink mode. To further investigate this m / n  = 1/1 kink mode in this new regime, the multi-scale interactions between the m / n  = 1/1 kink mode and turbulence are discussed.
