Abstract:
La fracture de fatigue reste une pathologie fréquente en milieu sportif et militaire. Si son incidence semble diminuer ces
dernières années au sein des armées, probablement du fait d’une meilleure progressivité de la formation initiale, elle augmente en pratique civile. Au travers de trois observations cliniques, nous mettons en évidence une disparité des pratiques médicales
avec un retard manifeste au diagnostic et à la mise en place du traitement. Le but de ce travail est donc d’harmoniser cette prise en charge en proposant des stratégies diagnostique et thérapeutique. L’échographie pourrait être proposée en première
intention pour les os superficiels. La radiographie est associée à un retard diagnostique, notamment quand elle est réalisée précocement. En dehors de certaines localisations pour lesquelles la scintigraphie osseuse reste préférée, l’imagerie par
résonnance magnétique est devenue l’examen paraclinique de choix. Ces deux examens permettent en outre d’établir le grade de sévérité de ces fractures, lequel est corrélé au temps de repos nécessaire avant la reprise des activités sportives. La
topographie des lésions permet, quant à elle, de préciser le risque des complications dont va dépendre le traitement. Celui-ci est le plus souvent fonctionnel : repos voire décharge, antalgiques et kinésithérapie dont les indications et les techniques
restent à préciser. Le traitement chirurgical d’emblée ou secondaire est exceptionnel.
