Abstract: Currently, whether and when intraocular pressure (IOP)-lowering medication should be used in glaucoma suspects with high myopia (GSHM) remains unknown. Glaucoma suspects are visual field (VF) defects that cannot be explained by myopic macular changes or other retinal and neurologic conditions. Glaucoma progression is defined by VF deterioration. Here we describe the rationale, design, and methodology of a randomized controlled trial (RCT) designed to evaluate the effects of medically lowering IOP in GSHM (GSHM study). Methods The GSHM study is an open-label, single-center, RCT for GSHM. Overall, 264 newly diagnosed participants, aged 35 to 65 years, will be recruited at the Zhongshan Ophthalmic Center, Sun Yat-sen University, between 2020 and 2021. Participants will be randomly divided into two arms at a 1:1 ratio. Participants in the intervention arm will receive IOP-lowering medication, while participants in the control arm will be followed up without treatment for 36 months or until they reach the end point. Only one eye per participant will be eligible for the study. If both eyes are eligible, the eye with the worse VF will be recruited. The primary outcome is the incidence of glaucoma suspect progression by VF testing over 36 months. The secondary outcomes include the incidence of changes in the optic nerve head morphology including the retinal nerve fiber layer, and retinal ganglion cell-inner plexiform layer loss, progression of myopic maculopathy, visual function loss, and change in the quality of life. Statistical analyses will include baseline characteristics comparison between the intervention and control groups using a two-sample t -test and Wilcoxon rank sum test; generalized linear models with Poisson regression for the primary outcome; Kaplan-Meier curve and log-rank test for the incidence of the secondary outcome; and longitudinal analyses to assess trends in outcomes across time. Discussion To the best of our knowledge, the GSHM study is the first RCT to investigate the impact of medically lowering IOP in GSHM. The results will have implications for the clinical management of GSHM. Trial registration ClinicalTrials.gov NCT04296916. Registered on 4 March 2020

Abstract: Additional simethicone (SIM) can improve adequate bowel preparation and adenoma detection rate (ADR). However, there is no consensus on the optimal dose of SIM. In this study, we compared the adequate bowel preparation rate with supplementation of split‐dose 2 L polyethylene glycol (PEG) with low‐dose SIM (200 mg) versus high‐dose SIM (1200 mg). Methods This was a prospective, randomized, observer‐blinded trial involving consecutive subjects undergoing colonoscopy. The primary outcome was adequate bowel preparation as assessed by Boston Bowel Preparation Scale (BBPS) score. Results Four hundred subjects were randomly allocated to low‐dose SIM or high‐dose SIM group. Baseline characteristics were comparable in the two groups ( P   〉  0.05). No significant between‐group differences were observed with respect to total bubble scale (BS) (8.49 ± 1.00 vs 8.39 ± 1.10, P  = 0.07), total BBPS score (8.70 ± 0.81 vs 8.29 ± 1.18, P  = 0.98), ADR (33.68% vs 31.79%, P  = 0.69) or withdrawal time (13 [range, 10–16] min vs 13 [10–15] min, P  = 0.96). The intubation time in low‐dose SIM group was significantly shorter than that in high‐dose SIM group (8 (4–16) min vs 10 [6–17] min, P  = 0.04). In addition, BS scores as well as diminutive ADR in right colon were superior in the low‐dose SIM group (2.68 ± 0.59 vs 2.52 ± 0.73, P  = 0.03 and 54.29% vs 30.30%, P  = 0.046, respectively). Conclusion Addition of low‐dose SIM to split‐dose 2 L PEG was as effective as addition of high‐dose SIM with respect to adequate bowel preparation, ADR and patient tolerance. However, low‐dose SIM was superior with respect to intubation time, right colon BS scores, right colon diminutive ADR and cost savings.

Abstract: The underground powerhouse of the Shuangjiangkou hydropower station is one of the largest caverns under construction in China, and its stability during construction is crucial for safe construction. To study the stability of the surrounding rock during excavation, the displacement and stress of the surrounding rock were monitored by multi-point displacement meters and bolt stress meters. Based on the monitoring data, the elastic modulus, Poisson’s ratio, friction angle, and cohesion of surrounding rock were inversely analyzed by the PSO-BP algorithm. Then, the back-analyzed parameters were used to simulate the subsequent excavations and predict the stability of surrounding rock during the following construction. The analysis results show that the surrounding rocks were generally stable during the initial four stages of excavation, and the main factors affecting their stability were blasts and unfavorable geological structures, including the lamprophyre vein and the F1 fault. These unfavorable geological structures also significantly decrease the mechanical parameters of surrounding rock as demonstrated by back analysis, and the stability prediction results show that the omnibus bar cave and the tailrace tunnel were at the greatest risk of instability during the subsequent excavations. This study provides a practical analysis for engineering excavation of the underground caverns.

Abstract: Background Underwater endoscopic mucosal resection (UEMR) is a promising strategy for nonpedunculated colorectal polyp removal. However, the efficacy and safety of the technique for the treatment of ≥ 10-mm colorectal polyps remain unclear. We aimed to comprehensively assess the efficacy and safety of UEMR for polyps sized 10–19 mm and ≥ 20 mm. Methods PubMed, EMBASE, and the Cochrane Library databases were searched for relevant articles from January 2012 to November 2019. Primary outcomes were the rates of adverse events and residual polyps. Secondary outcomes were the complete resection, en bloc resection, and R0 resection rates. Results 18 articles including 1142 polyps from 1093 patients met our inclusion criteria. The overall adverse event and residual polyp rates were slightly lower for UEMR when removing colorectal polyps of 10–19 mm vs. ≥ 20 mm (3.5 % vs. 4.3 % and 1.2 % vs. 2.6 %, respectively). The UEMR-related complete resection rate was slightly higher for colorectal polyps of 10–19 mm vs. ≥ 20 mm (97.9 % vs. 92.0 %). However, the en bloc and R0 resection rates were dramatically higher for UEMR removal of polyps of 10–19 mm vs. ≥ 20 mm (83.4 % vs. 36.1 % and 73.0 % vs. 40.0 %, respectively). In addition, univariate meta-regression revealed that polyp size was an independent predictor for complete resection rate (P = 0.03) and en bloc resection (P = 0.01). Conclusions UEMR was an effective and safe technique for the removal of ≥ 10-mm nonpedunculated colorectal polyps. However, UEMR exhibited low en bloc and R0 resection rates for the treatment of ≥ 20-mm polyps.

Abstract: Two conserved 14‐3‐3 proteins orthologous to S accharomyces cerevisiae   Bmh 1/2 are poorly understood in filamentous fungi. Here we show that Bmh 1 and Bmh 2 contribute equally to the fundamental biology and physiology of B eauveria bassiana by targeting many sets of proteins/enzymes. Single Bmh deletion caused similar upregulation of another. Excellent knockdown (∼91%) expressions of Bmh 1 in Δ Bmh 2 and Bmh 2 in Δ Bmh 1 resulted in equally more severe multiphenotypic defects than the single deletions, including G 2 / M transition, blastospore size, carbon/nitrogen utilization, conidiation, germination and conidial tolerances to high osmolarity, oxidation, cell wall stress, high temperature and UV ‐ B irradiation. All the deletion and deletion/knockdown mutants showed similar defects in blastospore yield and density, hyphal septation and cell size, hyphal responses to most chemical stresses and virulence. All the defects were evident with altered transcripts of phenotype‐related genes and well restored by each Bmh complementation. Our Bmh 1 ‐ and Bmh 2 ‐specific transcriptomes generated under osmotic and oxidative stresses revealed up to 6% genes differentially expressed by at least twofold in the fungal genome. Many of those were greatly depressed or co‐depressed in Δ Bmh 1 and Δ Bmh 2 . Our findings provide a thorough insight into the functions and complementary effects of the two 14‐3‐3 proteins in the filamentous entomopathogen.
