Abstract: We aimed in this survey to assess fertility and reproductive health awareness among medical students. Methods: A total of 354 female and male final year medical students (193 females and 157 males) were recruited for this survey at the Kasr Al-Ainy Faculty of Medicine, who attended the lectures of the andrology and sexually transmitted disease department. Results: Our study demonstrated that the mean ages of the female and male medical students when they want to have their first child were 26.95 ± 2.924 and 28.7 ± 3.94, respectively, and and p = 0, while the mean ages of the female and male medical students when they want to have their last child were 32.5 ± 3.2 and 34.7 ± 5.1, respectively, and p = 0.001. Our study demonstrates that a majority of the female and male medical students moderately estimated the efficacy of the assisted reproductive technique to take a baby home, and the means of their estimation were 42.620% ± 12% and 43.5% ± 13.1% and p = 0.482. Moreover, our study showed that 75 (39.1%) females were convinced that vaginal delivery has a potential negative effect on the sexual functions. However, 99 (51.6%) females did not see any potential negative effect of vaginal delivery. In addition, 18 (9.4%) females responded in an indefinite way to this question. Finally, our study demonstrated that 145 (92.4%) males rejected the proposal of vasectomy. Conclusions: Interestingly, our study revealed increased estimation by the participants of the efficacy of the assisted reproductive technique to take a baby home, despite the fact that they were medical students who had already studied the subject of gynecology in their fifth year and should have estimated it properly.

Abstract: We aimed in the current study to identify the predictive factors of ED occurrence in healthy individuals following penile fracture surgical repair as well as the effect of penile rehabilitation in the form of daily tadalafil 5 mg intake for 1 month for patients who suffered from ED after penile fracture incident. Method The current study was a prospective case–control study. Twenty-five patients were enrolled into the study starting from January (2022) to February (2023). Furthermore, time of presentation was determined, and length of tear intra-operative was measured, and then, a follow-up 1 week postoperatively in the outpatient clinic was conducted. All patients were instructed to start intercourse at least 2 weeks after the first visit provided that the wound epithelialized. Potent patients returned back home. A rehabilitation course of daily tadalafil 5 mg for 1 month was prescribed for patients who started complaining of ED that was confirmed by evaluation with the Arabic validated version of the international index of erectile function (ArIIEF-5). The rehabilitation therapy was terminated by resumption of normal erectile function. Thus, re-evaluation with the ArIIEF-5 was determined according to their response to therapy. Also, the patients were evaluated by hospital anxiety and depression scale (HADS) before and after penile fracture repair. Results The current study had demonstrated that a 1% increase in age determines an increase in odds ratio for post-penile fracture ED with 73.6% and 1 cm increase in the length of tear determines an increase in odds ratio for post-penile fracture ED with 20.04 times. Conclusion The current study enhances the proper counseling of these patients prior to repairing the defect about the probability of ED occurrence as well as initiating early penile rehabilitation to help these patients resuming their normal sexual activity as soon as possible.

Abstract: Bipolar disorder is an episodic mood disorder that causes unusual shifts in mood and affects the ability to carry out day-to-day tasks. Aim Evaluation of the sexual function in men and women with bipolar disorder during remission. Methods 60 men and women with bipolar disorder and 60 age-matched control subjects were included. A Structured Clinical Interview of Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition was performed to confirm the diagnosis, and Hamilton Depression Rating Scale (HAM-D) to assess the depressive symptoms and Young Mania Rating Scale (YMRS) to assess the manic symptoms. We assessed sexual function in both sexes using international indices of erectile function for men and sexual function of women. Main Outcome Measures Scores of HAM-D, YMRS, Arabic versions of International Index of Erectile Function (IIEF) and female sexual function (FSFI). Results Depressive symptoms showed insignificant adverse correlation with the IIEF in men, whereas they revealed adverse significant correlations with the items of female sexual function index (FSFI) except pain (correlation coefficients [r] = 0.349 [P = .059] , r = 0.680 [P & lt; .001], r = 0.574 [P = .001] , r = 0.517 [P = .003], r = 0.569 [P = .001] , r = 0.532 [P = .002], r = 0.609 [P & lt; .001], r = 0.342 [P = .065] , respectively). Interestingly, the manic symptoms were associated with sexual hyperfunctioning in the men and were insignificantly correlated with the different items of FSFI in the women (r = 0.414 [P = .023], r = 0.043 [P = .821] , r = 0.100 [P = .601], r = 0.108 [P = .571] , r = 0.183 [P = .332), r = 0.086 [P = .651], r = 0.022 (P = .907), r = 0.109 [P = .565] , respectively). There was a positive correlation between the affected women’s satisfaction and their partners’ score (r = 0.374, P =.042). Finally, the mean score of pain was statistically higher in the female cases than the normal ones (mean ± SD = 3.6 ± 0.4, 3.4, ± 0.6; P = .036). Clinical implications A different approach may be mandatory for manic female patients. Strengths & Limitations This study is one of the few studies that evaluated the sexual function in patients with bipolar disorder especially during remission. Some limitations of this study must be mentioned. We failed to evaluate the female partners of the diseased males. Quality of life of the recruited patients was not assessed. Finally, the study included patients who were not drug free. Conclusion Manic symptoms in female bipolar I patients are associated with sexual hypofunctioning.

Abstract: The present study compared seminal calbindin 2 (CALB 2) levels and semen parameters in men with and without varicocele. CALB 2 is also known as calretinin and 29 kDa calbindin. The study was a case-control study conducted from April (2021) to March (2022) in the andrology department at Beni-Suef University hospital. The study included four matched groups: group (I) were controls (fertile normozoospermic men without varicocele) ( n =24). Group (II) were fertile normozoospermic men with varicocele ( n =24). Group (III) were infertile oligoasthenoteratozoospermia (OAT) men without varicocele ( n =24). Group (IV) were infertile OAT men with varicocele ( n =24). The lowest levels of seminal CALB 2 were found in patients with severe oligozoospermia which showed a statistically significant difference when compared to seminal CALB 2 in patients with normal, mildly low, or moderately low sperm counts. There were significant negative correlations between sperm concentration, sperm motility and percentage of normal sperm forms and seminal CALB 2. Seminal plasma CALB 2 may play a role in the negative impact of varicocele on the semen parameters especially sperm concentration, sperm motility and percentage of sperm normal forms. Future studies are needed to verify these findings.

Abstract: Background: Remarkably, the current study is one of the first to deploy galectin-1 (Gal-1) in determining the degree of impairment of spermatogenesis among cases with non-obstructive azoospermia (NOA) as well as utilizing it as a biomarker to predict the rate of sperm retrieval in these patients. The purpose of the study was to evaluate the seminal plasma and serum levels of Gal-1 in NOA patients as well as their correlations with Johnsen’s tubular biopsy scoring (JTBS). Methods: The current case control study included totally 48 patients with NOA whose ages ranged from 24 to 46 years old and 50 age matched healthy controls. Gal-1 levels were measured in both seminal plasma and serum of all subjects by the enzyme-linked immunosorbent assay (ELISA). Results: A significant negative correlation between seminal plasma levels of Gal-1 and JTBS was detected (r= -0.281, p=0.048) in the NOA cases. Interestingly, the receiver operating characteristic (ROC) curve had demonstrated that the cutoff value of seminal plasma levels of Gal-1 in determining azoospermia was 〉 0.735 ng/ml and the area under the curve (AUC) was 0.858. The sensitivity, specificity, positive predictive, and negative predictive values for seminal plasma levels of Gal-1 were 76, 92, 90.5, and 79.3, respectively. In addition, sensitivity, specificity, positive predictive, and negative predictive values for serum levels of Gal-1 were 38, 66, 52.8, and 51.6, respectively. Conclusion: Seminal plasma levels of Gal-1 are higher in NOA men versus healthy controls. Interestingly, negative correlation of seminal plasma levels of Gal-1 with JTBS was determined. Thus, it can be used as a good predictor for NOA cases.

Abstract: Penile length and girth have long been sources of anxiety for men. Objectives The authors sought to measure satisfaction with intercourse and erectile function 3 and 6 months after girth augmentation in patients with semi-rigid penile implants who received autologous fat injections. Methods Thirty married participants in Egypt were recruited and divided into 2 groups between January 2016 and August 2017. Fifteen patients who underwent semi-rigid penile implant insertion and 15 controls all received autologous fat injections and were followed-up for 3 and 6 months. Results Median penile girth increased significantly in both groups, although fat loss was noted after 6 months. There was a positive correlation between the amount of fat injected and the change in penile girth measured 6 months after the procedure in the controls. Conclusions Autologous fat transfer is a simple and safe procedure that can be used to augment penile girth in patients whose girth is less than 11.5 cm after penile prosthesis insertion.
