    Abstract: Objective — This study aimed to evaluate the effect of Ophiocephalus striatus extract on body composition, levels of TNF-α, TGF-β1, IL-17, suPAR and neutrophils, diffusing capacity of the lungs for carbon monoxide (DLCO) and health-related quality of life (HRQoL) in stable COPD patients with muscle wasting. Methods — Clinical pre- and post- quasi-experimental study of 32 stable COPD patients with muscle wasting from the Pulmonary Outpatient Clinic of Saiful Anwar General Hospital Malang, determined in accordance with the Global Initiative for Chronic Obstructive Lung Disease (GOLD) 2016 and bioelectrical impedance analysis (BIA) measurements (FFM 〈 14.6 kg/m; BMI 〈 18.5 kg/m2). Measurements of the following health-related parameters for the QoL due to COPD were performed before and after the nutritional intervention of 3,000 mg of O. striatus extract daily for 12 weeks: TNF-α, TGF-β1, IL-17 and suPAR levels (by ELISA), neutrophils (by blood test), DLCO (by body plethysmography), and CAT score. Results — A non-significant reduction of TNF-α (p=0.302), IL-17 (p=0.275), neutrophil (p=0.619), and suPAR (p=0.674) levels, along with an increase in DLCO (p=0.369), occurred after 12 weeks of O. striatus extract administration. However, the level of TGF-β1 declined significantly (p=0.022), followed by an increase in QoL as assessed by the CAT score (p=0.000). There was no significant correlation between inflammatory cytokines and DLCO, nor with the CAT score. Conclusion — The study results demonstrated a potential role of oral nutritional supplementation in the management of COPD patients with muscle wasting.

    Abstract: Background: The levels of asthma control is an indicator of asthma management outcomes. A complex immunological mechanism influences it. We aimed to investigate the effect of Poly-Phytopharm on the asthma control tests (ACT) score, blood eosinophil levels, and miR-126 relative expressions in asthma patients. Methods: Quasi experimental method is used in 15 stable asthma patients who were not fully controlled at the pulmonary outpatient clinic of Dr. Saiful Anwar General Hospital Malang. Assessment of ACT score, blood eosinophil levels, and miR-126 relative expressions are carried out before and after supplementation of Poly-Phytopharm that consist of Nigella sativa 100 mg, Kleinhovia hospita 100 mg, Curcuma xanthorrhiza 75 mg, and Ophiocephalus striatus 100 mg extract, three times a day, two capsules respectively for 12 weeks. The ACT was scored by the investigator by direct questioning of patients, the blood eosinophil level was measured with blood analysis, and the relative expressions of miR-126 was detected with qPCR. Results: There were significant increase of ACT score (18.07±2.57 to 22.06±1.83, P=0.001), decrease in blood eosinophil levels (653.15±276.15 pg/mL to 460.66±202.04 pg/mL, P=0.038), and enhancement of serum miR-126 relative expressions (1.83±1.89 to 5.89±1.34, P=0.038). Conclusion: The administration of Poly-Phytopharm increases ACT score, decreases blood eosinophil levels and increases serum miR-126 relative expressions in not fully controlled stable asthma patients.     Abstract: Latar Belakang: Sejak Desember 2019 ketika pertama kali kasus COVID-19 diidentifikasi di Wuhan, China, SARS-CovV-2 menyebar ke seluruh dunia dan memberikan angka penyebaran infeksi dan mortalitas yang terus meningkat. Merokok memberikan dampak negatif terhadap kesehatan paru. Merokok sampai saat ini diasumsikan merupakan faktor risiko berbagai penyakit paru dan pernapasan seperti PPOK, kanker paru, termasuk pengaruh terhadap sistem pertahanan jalan napas terhadap berbagai mikroorganisme patogen. Prevalensi merokok di Indonesia sangat tinggi, dimana masih sedikit sekali studi yang dilakukan di Indonesia khususnya menilai risiko riwayat merokok terhadap derajat keparahan dan mortalitas pada pasien COVID-19 yang dilakukan rawat inap. Tujuan: Menganalisis riwayat merokok pada pasien COVID-19 yang dirawat di RS rujukan utama dan hubungannya dengan derajat keparahan penyakit dan mortalitas. Metode: Kami melakukan analisis observasional dengan pendekatan cross sectional, single-center, pada 77 pasien terkonfirmasi COVID-19. Data diambil antara bulan April-Juli 2020 pasien yang dirawat di Ruang COVID RS Dr. Saiful Anwar Malang, Indonesia. Uji statistik dipergunakan untuk menilai hubungan riwayat merokok dengan derajat keparahan dan mortalitas. Hasil: Subjek terdiri 77 pasien terkonfirmasi COVID-19 terbagi 2 kelompok yaitu 31 (40,25%) pasien ringan-sedang dan 46 (59,74%) pasien kelompok berat. Subjek dikelompokkan menjadi luaran hidup 52 (68,42%) dan meninggal 24 (31,57%) untuk analisis mortalitas. Analisis statistik menunjukkan riwayat merokok berkaitan dengan derajat keparahan penyakit (p=0,008; OR: 4,75, 95%CI 1,426-15,817) dan mortalitas (p=0,045, OR: 2,821, 95%CI 1,007-7,900) pasien COVID-19 yang dilakukan rawat inap di rumah sakit. Kesimpulan: Merokok berkaitan dengan risiko COVID berkembang ke derajat berat serta mortalitas pada pasien COVID-19 yang dilakukan rawat inap. Kata kunci: COVID-19, Keparahan, Mortalitas, Merokok   Background: Since December 2019, when the COVID-19 case was first identified in Wuhan, China, SARS-CoV-2 has spread worldwide and increased the incidence and mortality. To date, smoking is assumed to be a risk factor for various respiratory diseases such as COPD, lung cancer, also influences the airway defense system against various pathogenic microorganisms. The prevalence of smoking in Indonesia is very high where very few studies have been conducted in Indonesia especially assessing the risk of smoking and disease severity and mortality in hospitalized COVID-19 patients. Objective : To determine the risk of smoking history among hospitalized COVID-19 patients in tertiary care referral hospital and its association with disease severity and mortality. Methods: A cross-sectional single-center study was performed of 77 admitted laboratory-confirmed patients in a tertiary hospital in Malang City, East Java, Indonesia from April-July 2020. Statistical analysis was performed to evaluate the association between smoking and disease severity and mortality in COVID-19 patients. Results: A total of 77 patients with laboratory-confirmed COVID-19 were included in our analysis. Subjects divided in the mild-moderate and severe group 31 patients (40,25%) and 46 patients (59,74%) respectively. For mortality analysis, subjects divided into survivor 52 patients (68,42%) and non-survivor 24 patients (31,57%). Statistical analysis determine smoking associated with disease severity (p=0,008; OR : 4,75, 95%CI 1,426-15,817) and mortality (p=0.045, OR : 2,821, 95%CI 1,007-7,900) in hospitalized COVID-19 patients. Conclusion: In our cross-sectional study demonstrated history of smoking associated with disease severity and mortality in hospitalized COVID-19 patients.     Abstract: Background: Malignant mixed germ cell tumors account for 13-25% of all non seminoma germ cell tumors, almost all cases were found in males (85%), increased in the third decade, and had an average survival rate of 40-45%. With modern management the average 5-year survival rate can be over 80%.Case Report: We reported a case report in Saiful Anwar Malang Hospital, Indonesia, a young man, 26 years old with an malignant epithelial tumor derived from mediastinal or lung. The CT scan of the chest showed an lobulated solid mass of iso-hipodens on the anterior superior mediastinum, mass encompassing the aortic arch, left pulmonalis artery, subclavian artery. The result from transthoracic FNAB with USG guidance showed an malignant epithelial tumor derived from mediastinal or lung, impression of an adeno squamous carcinoma. The AFP, LDH, and NSE serum level was elevated. Patient underwent a surgical sternotomy and tumor debulking. The biopsy showed an malignant mixed germ cell tumor (yolksac and embrional carcinoma). These were confirming the diagnosis of malignant mixed germ cell mediastinal tumor (yolksac and embrinonal carcinoma). Patient received a combination bleomycin, etoposid, cisplatin 3 series chemoteraphy  and evaluated.Conclusion: Some procedures like biopsy, tumor marker, could differentiate the subtype of mediastinal germ cell tumor. The multimodality treatment by combining surgical therapy (surgical sternotomy and tumor debulking) with chemotherapy could increase the survival rate of patients.

    Abstract: Background: Thymoma is the most common thymus tumor; it covers 30% of the anterior mediastinum mass in adult. Almost half of thymoma does not present with any symptom and is found by accident. In 40% of patients have symptom of myasthenia gravis; other symptoms include chest pain, symptoms of bleeding and compression to other mediastinum’s structures. Ninety percent of thymoma is located in anterior mediastinum.Case: A 42-year-old woman came to Saiful Anwar Hospital with a complaint of difficulty in swallowing since three months before admission. We performed physical examination and chest X-ray AP’ and the result was within normal limit. In USG examination, there was no abnormality. From the laboratory examination, we found leucocytosis and from the CT-scan imaging, we found pocketed pleural effusion sinistra at the postero-inferior part.Based on these findings, we diagnosed (pre-operation diagnosis) this patient as anterosuperior mediastinum tumor: suspect thymoma. The pulmonologist consulted to Surgical Department for surgery. The surgeon performed sternotomy-tumor excision. The result from the vries coup and histopathogical examination was thymoma. FOB after the surgery showed an edema at the right vocal cord (post-extubation). Post-operative chest X-ray showed no mass in antero-superior mediastinum.

    Abstract: INTRODUCTION Lactate dehydrogenase (LDH), C-Reactive Protein (CRP), D-dimer and feritin are acute phase proteins which levels may rise in the presence of an inflammatory process of COVID-19. N-Acetylcysteine (NAC) is an antioxidant which has also been widely considered as adjunctive therapy in COVID-19. We aim to analyze the effect of NAC administration in improving oxidative stress and inflammation in COVID-19 through assessment of LDH, CRP, D-dimer, and feritin levels. METHOD A quasi-experimental study with pre-post design. LDH, CRP, D-dimer, and feritin levels were measured in admission and day 8 of administration of 5000 mg/72 hours of NAC. RESULT There is a significant decrease in CRP, LDH, D-dimer and Ferritin levels between admission and day 8 of NAC therapy. CRP levels were reduced from 185.31 ± 181.30 to 97.60 ± 161.86 (p 〈 0.001), LDH levels were reduced from 4.65 ± 4.13 to 1.87 ± 0.96 (p 〈 0.001), D-dimer levels were reduced from 185.31 ± 181.30 to 97.60 ± 161.86 (p 〈 0.001) and Ferritin levels were reduced from 4.65 ± 4.13 to 1.87 ± 0.96 (p 〈 0.001). DISCUSSION There is a significant decrease of LDH, CRP, D-dimer, and feritin levels from day-1 (admission) to day-8 of NAC administration in accordance with existing theories. This implies the ability of NAC to reduce oxidative stress and inflammation in COVID-19 patients. CONCLUSION There was a decrease in CRP, LDH, D-dimer and Ferritin after administration of NAC in COVID-19 patients and a positive correlation between severity and mortality towards acute phase protein in COVID-19 patients.
