    Abstract: Zusammenfassung. Fragestellung: Kinder haben ein Recht auf Mitbestimmung. Welche Phasen und Bereiche von Mitbestimmung innerhalb der Kinder- und Jugendpsychiatrie besonders relevant sind, wurde nur selten empirisch untersucht. Somit ist es Ziel der vorliegenden Studie, den Wunsch und das Erleben von Beteiligung für verschiedene Dimensionen der stationären Behandlung in der Kinder- und Jugendpsychiatrie differenziert zu betrachten. Methode: Es wurde eine prospektive multizentrische Fragebogenerhebung durchgeführt. Das Erhebungsinstrument umfasste 100 Fragen, zusammengefasst in 16 Skalen, zum Wunsch und Erleben von Beteiligung innerhalb verschiedener Phasen (Information, Deliberation, Entscheidung), Bereiche (Behandlung, Klinikalltag, Persönliche Freiheiten) und Partizipation im weiteren Sinne (Respekt, Vertrauen). Die Daten wurden deskriptiv und inferenzstatistisch ausgewertet. Ergebnisse: 81 Kinder und Jugendliche aus fünf psychiatrischen Kliniken nahmen an der Studie teil. Insgesamt wünschen sie sich mehr Beteiligung als sie erleben. Je höher die Stufe der Beteiligung, desto größer die Differenz zwischen Wunsch und Erleben. Für Entscheidungen zur Kommunikation mit Familie und Freunden ist der Wunsch nach Beteiligung besonders hoch. Die deutlichste Differenz zwischen Wunsch und Erleben zeigte sich im respektvollen und vertrauensvollen Umgang mit den Kindern und Jugendlichen. Für weibliche Patienten ist diese Differenz noch größer als für männliche. Schlussfolgerung: Partizipation ist mehr als Aufklärung bei Behandlungsentscheidungen. Besonders bei höheren Stufen von Beteiligung und bei Entscheidungen zur Kommunikation mit Familie und Freunden besteht noch Potenzial für den Einbezug der Kinder und Jugendlichen. Ein respekt- und vertrauensvoller Umgang mit den Patientinnen und Patienten, unabhängig von Alter, Geschlecht und Erkrankung, sind dabei die Grundlage.

    Abstract: Zusammenfassung. Unbegleitete minderjährige Flüchtlinge sind auf ihrer Flucht meist komplexen traumatischen Ereignissen ausgesetzt, besonders massiv trifft dies auf minderjährige Mädchen zu. An einem Wochenende wurde in der Kinder- und Jugendpsychiatrie ein 16-jähriges Mädchen zur Krisenintervention mit akuter Suizidalität aufgenommen. Das Mädchen war zuvor ohne Begleitung aus Eritrea geflüchtet und lebt in einer Jugendhilfeeinrichtung. Am Vortag war eine Schwangerschaft (23. SSW p. c.) festgestellt worden, die aus mehreren Vergewaltigungen während ihrer Flucht vom Sudan nach Nigeria hervorging. Alle Anzeichen der Schwangerschaft waren von dem Mädchen verleugnet worden. Als diese medizinisch festgestellt wurde, wollte die Patientin sich durch einen Sprung aus dem Fenster das Leben nehmen. Die Mitarbeiter der Jugendhilfeeinrichtung fragten nach einer ‚psychiatrischen Indikation‘ für einen Schwangerschaftsabbruch. Implikationen für eine medizinische Indikation aus psychiatrischen Gründen bei einem Schwangerschaftsstatus der 23. Woche werden sowohl unter ethischen Aspekten als auch unter der Betrachtung der posttraumatischen Symptomatik in dem vorliegenden Case Report diskutiert.

    Abstract: Background: Identity diffusion plays a central role in the onset of borderline personality disorders. Dialectical Behavioral Therapy for Adolescents (DBT-A) is a treatment program for adolescents with emotional instability and dysregulation. The interest of this study is to examine the potential effects of a standardized and certified DBT-A therapy program on the identity development of adolescents in an inpatient setting. Methods: A total of 138 adolescents aged 13 to 18 years with symptoms of severe emotional instability were assessed before and after a certified and standardized 12-week in-patient DBT-A program targeting emotional regulation with the following standardized instruments: the Assessment of Identity Development in Adolescence (AIDA), Scale of the Experience of Emotions (SEE), and Symptom Checklist (SCL-90-R). Results: The results indicate a significant change in identity development, emotion regulation, and general symptoms of psychopathology after treatment with DBT-A. Conclusions: In this large sample of adolescents, DBT-A significantly improved identity development and reduced identity diffusion, however, without a treatment-as-usual control group as a limitation. Nevertheless, our results may become clinically relevant for the prevention of chronic impairment in emotionally unstable adolescents.

    Feasibility and Effectiveness of a New Short-Term Psychotherapy Concept for Adolescents With Emotional Dysregulation

    Abstract: Background: The ‘Stress-Trauma-Symptoms-Regulation-Treatment’ (START) is an innovative manualized short-term treatment program for stabilization and stress resilience in emotionally dysregulated adolescents, based on an approach of stress and management and emotional regulation. The current pilot trial aims to assess the feasibility and effectiveness of the START intervention program for improvement of emotion regulation. Methods: Sixty-six adolescents between the age of 13–18 years admitted to a psychiatric unit for acute emotional or behavioral dysregulation took part in the START program for 5 weeks in an open group setting with two sessions per week (70 min/session). Before treatment, we assessed a history of adverse experience with the Child and Adolescent Trauma Screen (CATS) and the Child Posttraumatic Cognitions Inventory (CPTCI). Before and after treatment, the participants completed the FEEL-KJ, a self-report screening instrument of emotional regulation and coping strategies. Results: A large proportion of this sample had experienced traumatic events based on the CATS (75%) and the CPTCI (82%). The mean FEEL-KJ score significantly decreased after the intervention ( d = −0.248, p = 0.037), while no difference was observed with regard to mean level of adaptive emotion regulation strategies ( d = 0.202, p = 0.207). A positive effect of the intervention was observed on three components of the adaptive FEEL-KJ scale: accepting (d = 0.289, p = 0.08), forgetting ( d = 0.271, p = 0.049) and dealing with anger ( d = 0.309, p = 0.034). Conclusion: START demonstrates preliminary evidence for improvement in emotional dysregulation after a 5-weeks course of treatment. Therefore, this short-term intervention can possibly be regarded as a tool to improve emotional stability in children with a high load of trauma-related psychopathology. The results are promising and warrant future studies, specifically randomized controlled trials on the effectiveness of START for strengthening resilience at-risk-populations.

    Adolescents in acute mental health crisis—Pilot-evaluation of a low-threshold program for emotional stabilization

    Abstract: Referrals for child and adolescent acute psychiatric treatment have spiked in the last two years. To provide these adolescents with a fast-acting intervention, a novel treatment approach for acute emotional dysregulation was evaluated in this study. Methods 156 adolescents between the age of 13 and 18 years who were admitted to a psychiatric unit for acute emotional or behavioral dysregulation participated in a 5-week-group program (Stress-Arousal- Regulation-Treatment, START) which consisted of two sessions per week (60 min/session). Pre- and post intervention psychometric measures were derived for each participant applying the Self-Control Scale (SCS-13), the FEEL-KJ capturing adaptive and maladaptive emotion regulation strategies, the Perceived Stress Scale (PSS-10), as well as the Strengths and Difficulties Questionnaire (SDQ). Results The mean score of maladaptive emotion regulation strategies of the FEEL-KJ significantly decreased post treatment ( d =  −0.24 , p =  .001), while there was an increase in adaptive emotion regulation strategies ( d  = 0.25, p =  .001 ). The post- intervention-assessment revealed significantly lower scores in the PSS-10 ( d =  −0.34 , p  & lt; 0 .001), as well as SDQ mental health problems ( d  = −0.17 , p =  .018) and disorders ( d =  −0.15 , p  = .015). The Self-Control Score increased significantly after the intervention (d =  0.21, p =  .001 ) . Conclusions In this large sample, the low-threshold intervention START significantly improved emotion regulation and self-control and reduced perceived stress as well as several scales of the strengths and difficulties questionnaire, pointing to a good feasibility and indicating efficiency to provide support to adolescents with acute mental health problems when applying this short-term treatment.
