    Abstract: An early detection of functional decline with age is important to start interventions at an early state and to prolong the functional fitness. In order to assure such an early detection, functional assessments must be conducted on a frequent and regular basis. Since the five time chair rise test (5CRT) is a well-established test in the geriatric field, this test should be supported by technology. We introduce an approach that automatically detects the execution of the chair rise test via an inertial sensor integrated into a belt. The system’s suitability was evaluated via 20 subjects aged 72–89 years (78.2 ± 4.6 years) and was measured by a stopwatch, the inertial measurement unit (IMU), a Kinect® camera and a force plate. A Multilayer Perceptrons-based classifier detects transitions in the IMU data with an F1-Score of around 94.8%. Valid executions of the 5CRT are detected based on the correct occurrence of sequential movements via a rule-based model. The results of the automatically calculated test durations are in good agreement with the stopwatch measurements (correlation coefficient r = 0.93 (p 〈 0.001)). The analysis of the duration of single test cycles indicates a beginning fatigue at the end of the test. The comparison of the movement pattern within one person shows similar movement patterns, which differ only slightly in form and duration, whereby different subjects indicate variations regarding their performance strategies.

    Abstract: Chronic kidney disease (CKD) is a silent and poorly known killer. The current concept of CKD is relatively young and uptake by the public, physicians and health authorities is not widespread. Physicians still confuse CKD with chronic kidney insufficiency or failure. For the wider public and health authorities, CKD evokes kidney replacement therapy (KRT). In Spain, the prevalence of KRT is 0.13%. Thus health authorities may consider CKD a non-issue: very few persons eventually need KRT and, for those in whom kidneys fail, the problem is ‘solved’ by dialysis or kidney transplantation. However, KRT is the tip of the iceberg in the burden of CKD. The main burden of CKD is accelerated ageing and premature death. The cut-off points for kidney function and kidney damage indexes that define CKD also mark an increased risk for all-cause premature death. CKD is the most prevalent risk factor for lethal coronavirus disease 2019 (COVID-19) and the factor that most increases the risk of death in COVID-19, after old age. Men and women undergoing KRT still have an annual mortality that is 10- to 100-fold higher than similar-age peers, and life expectancy is shortened by ~40 years for young persons on dialysis and by 15 years for young persons with a functioning kidney graft. CKD is expected to become the fifth greatest global cause of death by 2040 and the second greatest cause of death in Spain before the end of the century, a time when one in four Spaniards will have CKD. However, by 2022, CKD will become the only top-15 global predicted cause of death that is not supported by a dedicated well-funded Centres for Biomedical Research (CIBER) network structure in Spain. Realizing the underestimation of the CKD burden of disease by health authorities, the Decade of the Kidney initiative for 2020–2030 was launched by the American Association of Kidney Patients and the European Kidney Health Alliance. Leading Spanish kidney researchers grouped in the kidney collaborative research network Red de Investigación Renal have now applied for the Redes de Investigación Cooperativa Orientadas a Resultados en Salud (RICORS) call for collaborative research in Spain with the support of the Spanish Society of Nephrology, Federación Nacional de Asociaciones para la Lucha Contra las Enfermedades del Riñón and ONT: RICORS2040 aims to prevent the dire predictions for the global 2040 burden of CKD from becoming true.

    Abstract: Hintergrund Obwohl für ältere Menschen digitale Ansätze in der Prävention ein hohes Potenzial haben, und vermehrt zum Einsatz kommen, bestehen bis heute noch Ungleichheiten im Zugang und bei der Nutzung. Eine Ursache könnte in der unzureichenden Berücksichtigung der zukünftigen Nutzer*innen in der Technikentwicklung liegen, die häufig sehr spät und mit unpassenden Methoden einbezogen werden. Ziel dieser Arbeit war die Analyse der Teilnahmemotivation und Vorstellungen über eine zukünftige Beteiligung im Forschungs- und Entwicklungsprozess von Gesundheitstechnologien für ältere Menschen. Methodik Es wurden quantitative und qualitative Daten aus einer Bedarfs- und zwei Evaluationsstudien ausgewertet. Die quantitativen Daten wurden deskriptiv und die qualitativen Daten inhaltsanalytisch mit deduktiver-induktiver Kategorienbildung analysiert. Ergebnisse Das Alter der 103 Teilnehmenden (50 weiblich) lag im Median bei 75 Jahren (64–90), die überwiegend im Umgang mit Technologien interessiert und in der Studienteilnahme erfahren waren. Es wurden neun Kategorien für die Teilnahmemotivation abgeleitet. Die häufigste Motivation war die Förderung und Unterstützung der eigenen Gesundheit. Sowohl am Anfang des Forschungsprozesses als auch an dessen Ende konnten sich die Befragten eine Beteiligung vorstellen. In der Technikentwicklung wurden unterschiedliche Vorstellungen, aber ein allgemeines Interesse geäußert. Es wurden vor allem Methoden favorisiert, die einen Austausch in der Gruppe ermöglichen. Schlussfolgerungen Es konnten Unterschiede in der Teilnahmemotivation und bei den Vorstellungen über eine Beteiligung herausgearbeitet werden. Die Ergebnisse geben wichtige Hinweise aus der Sicht älterer Menschen und ergänzen den bestehenden Forschungsstand.

    Abstract: Comprehensive measurements are needed in older populations to detect physical changes, initiate prompt interventions, and prevent functional decline. While established instruments such as the Timed Up and Go (TUG) and 5 Times Chair Rise Test (5CRT) require trained clinicians to assess corresponding functional parameters, the unsupervised screening system (USS), developed in a two-stage participatory design process, has since been introduced to community-dwelling older adults. In a previous article, we investigated the USS’s measurement of the TUG and 5CRT in comparison to conventional stop-watch methods and found a high sensitivity with significant correlations and coefficients ranging from 0.73 to 0.89. This article reports insights into the design process and evaluates the usability of the USS interface. Our analysis showed high acceptance with qualitative and quantitative methods. From participant discussions, suggestions for improvement and functions for further development could be derived and discussed. The evaluated prototype offers a high potential for early detection of functional limitations in elderly people and should be tested with other target groups in other locations.

    Abstract: Caregivers experience high musculoskeletal loads during their daily work, which leads to back complaints and a high rate of absenteeism at work. This reinforces the already existing discrepancy between the supply and demand of caregivers. Ergonomically correct working can significantly reduce musculoskeletal load. Therefore, we developed a system that recognizes potentially harmful body postures. In a study with 13 caregiver students, we analyzed the body postures, as well as muscle activities, and loads during the transfer of a patient from bed to wheelchair. The body postures were measured by a full-body motion capture system and a Multi-Kinect System. Muscle activities and loads were recorded via surface electromyography and a force plate. The posture analysis system is based on the motion capture data and considers the recommendations for ergonomic working in the care sector. The system generates a result report visualizing the skeleton model as well as color-coded information about inclination and torsion angles. The motion capture data were also related to EMG and force data and analyzed according to biomechanical assumptions.

    Abstract: Comprehensive and repetitive assessments are needed to detect physical changes in an older population to prevent functional decline at the earliest possible stage and to initiate preventive interventions. Established instruments like the Timed “Up & Go” (TUG) Test and the Sit-to-Stand Test (SST) require a trained person (e.g., physiotherapist) to assess physical performance. More often, these tests are only applied to a selected group of persons already functionally impaired and not to those who are at potential risk of functional decline. The article introduces the Unsupervised Screening System (USS) for unsupervised self-assessments by older adults and evaluates its validity for the TUG and SST. The USS included ambient and wearable movement sensors to measure the user’s test performance. Sensor datasets of the USS’s light barriers and Inertial Measurement Units (IMU) were analyzed for 91 users aged 73 to 89 years compared to conventional stopwatch measurement. A significant correlation coefficient of 0.89 for the TUG test and of 0.73 for the SST were confirmed among USS’s light barriers. Correspondingly, for the inertial data-based measures, a high and significant correlation of 0.78 for the TUG test and of 0.87 for SST were also found. The USS was a validated and reliable tool to assess TUG and SST.

    Abstract: When older adults fall below the thresholds of functional geriatric assessment (FGA), they may already be at risk of mobility impairment. A reduction in (jumping) power could be an indication of functional decline, one of the main risk factors for falls. Objective This paper explores whether six-month delta (∆) values of muscle power can predict 24-month follow-up FGA in older adults. Methods This observational study of independent, healthy, high-performing community-dwelling adults aged 70 + years involved FGA (mobility, balance, and endurance tests) at baseline ( t 0 ), after 6 months ( t 1 ), and after 24 months ( t 2 ); maximum jumping power (max JP) was determined at t 0 and t 1 . A predictive linear model was developed in which the percentage change of Δmax JP 0,1 was transferred to all FGA ( t 0 ) values. The results were compared with measured FGA values at t 2 via sensitivity and specificity in terms of the clinically meaningful change (CMC) or the minimal detectable change (MDC). Results In 176 individuals (60% female, mean age 75.3 years) the mean percentage (SD) between predicted and measured FGA ranged between 0.4 (51.3) and 18.11 (51.9). Sensitivity to identify the CMC or MDC of predicted FGA tests at t 2 ranged between 17.6% (Timed up and go) and 75.0% (5-times-chair-rise) in a test-to-test comparison and increased to 97.6% considering clinically conspicuousness on global FGA. Conclusion The potential of jumping power to predict single tests of FGA was low regarding sensitivity and specificity of CMC (or MDC). 6 months Δmax JP seem to be suitable for predicting physical function, if the measured and predicted tests were not compared at the test level, but globally, in the target group in the long term.

    Abstract: The kinetochore proteins assemble onto centromeric chromatin and regulate DNA segregation during cell division. The inner kinetochore proteins bind centromeres while most outer kinetochore proteins assemble at centromeres during mitosis, connecting the complex to microtubules. The centromere–kinetochore complex contains specific nucleosomes and nucleosomal particles. CENP-A replaces canonical H3 in centromeric nucleosomes, defining centromeric chromatin. Next to CENP-A, the CCAN multi-protein complex settles which contains CENP-T/W/S/X. These four proteins are described to form a nucleosomal particle at centromeres. We had found the CENP-T C-terminus and the CENP-S termini next to histone H3.1 but not to CENP-A, suggesting that the Constitutive Centromere-Associated Network (CCAN) bridges a CENP-A- and a H3-containing nucleosome. Here, we show by in vivo FRET that this proximity between CENP-T and H3 is specific for H3.1 but neither for the H3.1 mutants H3.1C96A and H3.1C110A nor for H3.2 or H3.3. We also found CENP-M next to H3.1 but not to these H3.1 mutants. Consistently, we detected CENP-M next to CENP-S. These data elucidate the local molecular neighborhood of CCAN proteins next to a H3.1-containing centromeric nucleosome. They also indicate an exclusive position of H3.1 clearly distinct from H3.2, thus documenting a local, and potentially also functional, difference between H3.1 and H3.2.
