    Abstract: Latar Belakang: Pandemi Coronavirus Disease 2019 (Covid-19) memberikan dampak luas dikarenakan risiko penularan virus SARS-COV-2 melalui kontak, droplet, dan kemungkinan airborne. Suatu tantangan bagi rumah sakit untuk menjamin mutu dan keselamatan. Dalam kondisi seperti ini, maka budaya keselamatan yang baik menjadi faktor kritikal untuk menjamin keselamatan baik pasien, pengunjung, maupun petugas rumah sakit. Komitmen Pimpinan dan dukungan seluruh staf khususnya staf klinis sangat diperlukan untuk terciptanya budaya keselamatan rumah sakit. Salah satunya pemenuhan standar layanan akreditasi rumah sakit untuk meningkatkan budaya keselamatan. Berdasarkan kondisi ini diperlukan suatu penelitian untuk mengukur sejauh mana standar akreditasi diimplementasikan untuk mendukung budaya keselamatan yang meliputi keselamatan pasien, keselamatan dan kesehatan pekerja rumah sakit. Tujuan: Penelitian ini bertujuan untuk mengukur budaya keselamatan staf klinis dengan prediktor yang dominan dalam mempengaruhi keselamatan pasien serta keselamatan dan kesehatan pekerja di rumah sakit terakreditasi Komisi Akreditasi Rumah Sakit (KARS) yang menjadi rujukan Covid-19. Metode: Penelitian ini merupakan cross-sectional study pada empat rumah sakit rujukan Covid-19 di DKI Jakarta diantaranya RSPAD Gatot Subroto, RSUP Persahabatan, RS Pusat Pertamina, dan RS Pertamina Jaya. Tiga variabel yang mempengaruhi budaya keselamatan yaitu iklim keselamatan, situasional, dan perilaku keselamatan. Responden adalah staf klinis berdasarkan jenis instalasi dan staf klinis di setiap rumah sakit yang dipilih secara acak. Kuesioner mengacu pada instrumen Standar Nasional Akreditasi Rumah Sakit (SNARS) edisi 1.1 dalam bentuk google form. Analisis data dilakukan secara univariat dan bivariat menggunakan uji chi-square sedangkan multivariat menggunakan regresi logistik. Hasil: Data penelitian ini diperoleh dari 560 responden yang terdiri dari instalasi dan staf klinis. Dari empat rumah sakit dihasilkan budaya keselamatan rumah sakit menurut 51,8% responden masuk dalam kategori baik dimana tiga dari sepuluh indikator yaitu kerjasama, lingkungan kerja, dan kepatuhan merupakan kategori baik dan dominan. Variabel situasional memiliki pengaruh terbesar terhadap budaya keselamatan (OR 4,46; 95% CI 2,67-7,42). Kepatuhan memiliki pengaruh terbesar terhadap keselamatan pasien (OR 5,59; 95% CI 3,27-9,56) dan manajemen risiko memiliki pengaruh terbesar terhadap keselamatan dan kesehatan pekerja (OR 5,59; 95% CI 3,29-9,49).  Kesimpulan: Variabel situasional memiliki pengaruh terbesar terhadap budaya keselamatan, baik keselamatan pasien maupun keselamatan dan kesehatan pekerja. Indikator kepatuhan dan manajemen risiko merupakan indikator yang memiliki pengaruh terbesar terhadap budaya keselamatan.

    Abstract: Infectious diseases are one of the biggest threats to humans. Currently, the world is in the outbreak condition causes of the COVID-19 virus which is started from Wuhan, China in December 2019. This disease was spread out rapidly throughout the World and was announced as a pandemic by the World Health Organization (WHO) on March 11, 2020(1). The infected number of SARS-CoV-2 was over 84 million people and caused over 1 million death cases in the worldwide. Indonesia had more than 800.000 infectious cases and 23.000 of death cases with the highest cases in Jakarta (2). This virus can be transmitted by two ways, such as direct contact (cough, sneeze, and droplet inhalation) and contact transmission (contact with oral, nasal, and eye mucous membranes) of person with COVID-19 (3). The current COVID-19 pandemic makes various challenges in prevention and control of infections in hospitals. Health care workers (HCWs) have been providing care to suspected, probable or confirmed COVID-19 patients that make them in high-risk condition. Several study indicated that many HCWs have been infected with SARS-CoV-2 in many hospitals worldwide (4)(5)(6).

    Abstract: At the end of 2019, the novel coronavirus (CoV) of severe acute respiratory syndrome (SARS), named SARS- CoV-2 was approved as a microbial agent that causes viral pneumonia in patients who are linked epidemiologically to the seafood market in Wuhan (Wuhan) Hubei province, China [1]. The World Health Organization (WHO) stated the coronavirus or COVID-19 as a pandemic because it has spread globally in the world since March 11, 2020 [2] . There have been more than 8 million cases reported with more than 450 thousand deaths around the world until June 19, 2020 [3]. Indonesia reported the first cases of COVID-19 in early March 2020 and currently 43,803 cases with 2,373 deaths [4] . Indonesia has the highest cases of COVID-19 in Southeast Asia [5]. WHO stated that according to the current evidence, coronavirus is transmitted among people through respiratory droplets and contact routes [6-8] . Droplet transmission is different from airborne disease. Droplet transmission occurs when a person closely contacted (within 1 m) with someone who has respiratory symptoms (e.g. coughing or sneezing,) and was therefore at risk of having his/her mucosae (mouth and nose) or conjunctiva (eyes) exposed to potentially infective respiratory droplets. Droplet transmission may also occur through fomites in the immediate environment around the infected person [9]. Increasing cases occur continuously become alert for our institution as health care providers.

    Abstract: Introduction: Cancer patients have an increased risk of morbidity and mortality due COVID-19 partly due to their immunocompromised status. We aimed to investigate the associations of clinicopathological factors and survival outcomes in cancer patients with COVID-19. Methods: This was a retrospective cohort study comprised of cancer patients treated in Dharmais National Cancer Center, Indonesia. Main inclusion criteria were pathologically confirmed malignancy with positive results of RTPCR COVID-19 tests. Results: A total of 16,511 visitors had visited and registered for RTPCR test in Dharmais National Cancer Center from May 2020 to January 2021. Logistic regression showed that male gender (p-value = 0.019; OR = 1.732), haematological type of malignancy (p-value 〈 0.001; OR = 3.073), patients not underwent cancer therapy (p-value = 0.008; OR = 0.485), low RTPCR Ct values (p-value = 〈 0.001; OR = 3.340), poor performance status (p-value = 〈 0.001; OR = 8,194), and disease severity (p-value = 〈 0.001; OR = 5.448) were associated with mortality. Conclusion: Overall mortality rate in Dharmais cancer patients (25%) was higher than other cancer patients treated in other hospitals in Asia. Moreover, the mortality rate was similar across all age groups. Poor survival in young age might be explained by the fact that median age of cancer patients was 46 years old. In addition to male gender, cancer patients with low Ct values and having delayed cancer treatment were vulnerable groups of having poor outcomes when diagnosed with COVID-19. Long-term follow-up is required to examine the survival rate in cancer patients with COVID-19.

    Abstract: Aim: To describe the clinical data and disease severity of thoracic malignancy patients with COVID-19 and its relation to the mitigation process at the Dharmais National Cancer Center, Indonesia. Methods: Total 5256 cancer patients registered from May 2020 to March 2021. There were 681 cancer patients diagnosed with COVID-19. Forty-five thoracic malignancy patients were enrolled. Data from medical records were obtained at the Dharmais Cancer Hospital, then analyzed using SPSS version 25. Comparative result was considered significant, as p-value 〈 0.05. Results: There were 12.9% of total patients registered infected by COVID-19, which 6% with thoracic malignancy dominated by Non-small cell lung carcinoma (57.8%). Patients who have asymptomatic (31.1%), mild (13.3%), and moderate COVID-19 disease (8.9%) were alive. Patient with severe disease (46.7%) tend to deteriorate. Neutrophilia (mean 78.0%), lymphopenia (mean 13.0%), high neutrophil to lymphocyte ratio (mean 13.1), hyperuricemia (mean 31.6 mg/dL), high fibrinogen (mean 521.7 mg/dL), and high d-dimer (mean 3821.6 ng/mL) were significantly associated with disease severity (p-value 〈 0.05). Conclusions: Only small number of cancer patients affected by COVID-19 and mostly do not progress to severe disease, showing the strict mitigation strategy was successful. Severe disease patients have a poor prognosis, with neutrophilia, lymphopenia, high neutrophil to lymphocyte ratio, hyperuricemia, high fibrinogen, and high d-dimer may be valuable for predicting poor prognosis.

    Abstract: Background: Sepsis is the leading cause of morbidity and mortality in children worldwide, with around 75,000 inpatients each year and nearly 50% dying in pediatric hospitals. Acute lymphoblastic leukemia (ALL) in childhood is a malignancy originating from lymphoid progenitor cells, usually at the age of 2–6 years. Children with ALL contribute 30% to childhood cancer cases under 15 years old. Sepsis in pediatric patients increases mortality significantly. A previous study showed that the prevalence of sepsis in pediatrics is still high. Thus, this study aims to report ALL patients with sepsis in our institution.Methods: This study was a descriptive cross-sectional study at the National Cancer Center (NCC) - Dharmais Cancer Hospital. We recruited acute lymphoblastic leukemia (ALL) patients aged 2-18 years with suspected or documented sepsis based on Systemic Inflammatory Response Syndrome (SIRS) by The International Consensus Conference on Pediatric Sepsis. The data were collected by medical records from January 2014 to December 2018.Results: A total of 94 pediatric patients in the study included 57 males and 37 females with an average age of 5 years. The age range was 1–18 years with a median age of 5 years. The prevalence of sepsis in pediatric with ALL was 11 patients (11.7%) and 45.5% of deaths. The clinical conditions were as follows: abnormal temperature (8 [72.7%]), abnormal blood pressure, systolic (7 [63.6%] ) and diastolic (7 [63.6%]), abnormal pulse rate (9 [81.8%] ), abnormal respiratory rate (8 [72.7%]), and normal saturation (6 [54.5%] ). We also did a laboratory check followed by all sepsis patients who had abnormal leukocytes (11 [100%]), and abnormal lymphocytes count (8 [72.7%] ).Conclusions: The mortality rate and prevalence of sepsis in children with ALL in our institution are still high. Further prospective studies are required to explore the risk factors and predictors of sepsis based on its severity and adherence of health workers to implement guidelines on patients with sepsis in the hospital.
