    Abstract: This clinical trial aimed to compare the ultrasound-guided in-plane infraclavicular cannulation of the axillary vein (AXV) and the ultrasound-guided out-of-plane cannulation of the internal jugular vein (IJV). DESIGN: A prospective, single-blinded, open label, parallel-group, randomized trial. SETTING: Two university-affiliated ICUs in Poland (Opole and Lublin). PATIENTS: Mechanically ventilated intensive care patients with clinical indications for central venous line placement. INTERVENTIONS: Patients were randomly assigned into two groups: the IJV group ( n = 304) and AXV group ( n = 306). The primary outcome was to compare the IJV group and AXV group through the venipuncture and catheterization success rates. Secondary outcomes were catheter tip malposition and early mechanical complication rates. All catheterizations were performed by advanced residents and consultants in anesthesiology and intensive care. MEASUREMENTS AND MAIN RESULTS: The IJV puncture rate was 100%, and the AXV was 99.7% (chi-square, p = 0.19). The catheterization success rate in the IJV group was 98.7% and 96.7% in the AXV group (chi-square, p = 0.11). The catheter tip malposition rate was 9.9% in the IJV group and 10.1% in the AXV group (chi-square, p = 0.67). The early mechanical complication rate in the IJV group was 3% (common carotid artery puncture—4 cases, perivascular hematoma—2 cases, vertebral artery puncture—1 case, pneumothorax—1 case) and 2.6% in the AXV group (axillary artery puncture—4 cases, perivascular hematoma—4 cases) (chi-square, p = 0.79). CONCLUSIONS: No difference was found between the real-time ultrasound-guided out-of-plane cannulation of the IJV and the infraclavicular real-time ultrasound-guided in-plane cannulation of the AXV. Both techniques are equally efficient and safe in mechanically ventilated critically ill patients.

    Abstract: 〈 b 〉 Background 〈 /b 〉 〈 br / 〉 In most countries, premature deliveries constitute 5% to 18% of births. Some preterm children, especially those born before 32 weeks of pregnancy, experience serious medical complications, which can affect their subsequent development and functioning. Even those who have an IQ within the normal range can be at risk of worse functioning. This study aimed to investigate the differences in development of hot and cool aspects of executive functions in children born prematurely in comparison to those born on time. It is also focused on evaluating relationships between executive functions in premature children and their socio-emotional competences. 〈 br / 〉 〈 br / 〉 〈 b 〉 Participants and procedure 〈 /b 〉 〈 br / 〉 All children participating in the study were preschoolers. The sample consisted of 20 children born before 32 weeks of gestation and 28 term controls (children born on time). Hot and cool aspects of executive functions were examined in both groups using tasks extracted from the 〈 i 〉 Preschool Self-Regulation Assessment 〈 /i 〉 (PSRA). Parents of children born prematurely also completed the 〈 i 〉 Strengths and Difficulties Questionnaire 〈 /i 〉 (SDQ), which is a brief behavioral screening questionnaire that consists of five scales: emotional symptoms, conduct problems, hyperactivity/inattention, peer relationship problems and prosocial behavior. 〈 br / 〉 〈 br / 〉 〈 b 〉 Results 〈 /b 〉 〈 br / 〉 Premature children scored lower for both hot and cool executive functions in comparison to the children born at term in two of the five tasks. In addition, an association between worse executive functioning and more severe problems was found in the preterm group. This link applies to both general and specific problems, such as hyperactivity/inattention and behavioral problems. 〈 br / 〉 〈 br / 〉 〈 b 〉 Conclusions 〈 /b 〉 〈 br / 〉 Prematurely born children may have larger deficits both in hot and cool aspects of executive functions compared to their peers born at term. Deficits in hot aspect may be reflected in hyperactivity/inattention symptoms and conduct problems, whereas difficulties in cool aspect may be more related to the general picture of problems in prematurely born children.

    Abstract: Perinatal mood disorders are a common phenomenon among young mothers: postpartum depression affects about 13-20% of women, prenatal depression occurs even in 19% of pregnant women. However, in Poland, there are no systemic solutions regarding screening, diagnosis, and treatment of patients suffering from postnatal depression. From 2019, new standards of perinatal care are introduced to Poland, imposing the obligation to monitor the mental state during pregnancy and in the postpartum period. Furthermore, the Ministry of Health is launching a nationwide program for the prevention of postnatal depression which will include, among others, screening tests and free diagnostic consultations. After the introduction of the screening system, the question arises: what treatment should be offered to women with the diagnosis of postnatal depression? Understanding postnatal depression as a phenomenon of complex etiology, including biological and social factors, to answer this question, we will devote special attention to the psychological dimension. Based on the theoretical background, research results, and our own experience, we formulate practical guidelines for the implementation of short-term, early psychological interventions. We also present examples of the application of our guidelines in practice.

    Abstract: Countries recognize the risk of mental health difficulties during the perinatal period and the potential benefits of screening and early detection of depressive symptomatology. This study aimed to analyse mothers’ views on screening for postpartum depression (PPD) in Poland, where a new standard of perinatal care imposed (from January 2019) the obligation to monitor women’ postpartum mental state. Material and methods 150 women participated in the study. In the first stage, PPD symptoms were assessed with the Edinburgh Postnatal Depression Scale (EPDS) among postpartum women during midwives’ home visits. The second stage consisted of a telephone survey with the EPDS and questions exploring mothers’ perception of midwife competencies in screening for PPD. Results Most women identified as relatively high midwives’ competencies in communicating information about PPD, interpretation of the EPDS score and their ability to create comfortable conditions of the assessment and further discussion about postpartum mental health changes. Women with an elevated level of PPD symptoms assessed as significantly lower midwives’ competence in this last aspect and those who had a caesarean section tend to assess as lower the usefulness of provided information on care of a newborn. Conclusions Midwives should be aware and prepared for a possible critical attitude of patients, which may be a sign of a depression. Otherwise, medical staff may not be willing to interact with a mother and offer her help and support. The ability to create a friendly condition in spite of adversity can contribute to the desire of women to undergo screening, discussion about the result and further treatment.

    Abstract: Exposure to stressful situations, such as emergencies, infectious diseases, and natural disasters, may lead to a heightened risk of perinatal mental health problems. Declared on March 11th, 2020, the global COVID-19 pandemic triggered an additional burden on women in the perinatal period. Safety recommendations, such as social distancing and isolation, were opposite to the usual advice given to new mothers. Besides fear, changes in financial stability and daily life reorganization contributed to increased depressive symptoms. As the periods of epidemic waves and lockdowns were associated with a more significant burden for young families, we aimed to assess the intensification of depressive and anxiety symptoms during the pandemic concerning the time intervals of the three lockdowns introduced in Poland. Methods: 1588 postpartum women took part in the online self-assessment with the Edinburgh Postnatal Depression Scale (EPDS) and General Anxiety Disorder 2 (GAD-2) questionnaire between January 1, 2020, and March 31, 2021. This self-screening is a part of a prevention program The Next Stop: Mum, implemented in the North of Poland. Results The highest severity of PPD symptoms and anxiety were observed during the second lockdown in Poland: the mean score in the EPDS and anxiety assessment was significantly higher than the mean scores from previous pandemic periods. Since the second lockdown, the average EPDS and GAD-2 scores remained similarly high. Moreover, with the duration of the COVID-19 pandemic, the percentage of women with elevated symptoms of postpartum depression and anxiety began to increase. However, the Polish National Health Fund data indicate that only 0,7% of women giving birth in the northern macro-region of Poland received diagnosis and help from public funds. In The Next Stop: Mum project, 250 women benefited from psychological consultations. Conclusion Increased severity of depression and anxiety symptoms during the pandemic indicates the need for additional psychological support for postpartum women. However, very few women are diagnosed in health facilities in the first year postpartum and thus are rarely referred for further treatment. The study shows that the availability of services and the focus on social and individual barriers may be critical factors in implementing perinatal mental health programs and practices. This may be especially needed in a country where the screening obligation is new. In case of a further pandemic, policymakers and health care professionals should be aware that the duration of the restrictions and the repetition of lockdowns are associated with the aggravation of symptoms. The online screening without the possibility to discuss the results is only partially effective in increasing referrals for possibly affected women.
