Abstract: Objective : Study the advantages and pitfalls of both 0.15 mg/kg and 0.2 mg/kg of Cisatracurium for Intubation. Background: The patients coming to a Tertiary Care Hospital during period 2018-2019 for surgery under General Anaesthesia, who met the Inclusion Criteria were studied in a hundred patients. After giving all components of balanced general anaesthesia, patients randomised into 2 groups either 0.15 mg/kg or 0.2mg/kg of Cisatracurium as a muscle relaxant and Intubation was done at 3 minutes. Measurements and Results: Heart rate, Blood pressure, Systolic, Diastolic, Mean, SpO2, ETCO2, TOF monitoring at the adductor pollicis, and BIS were monitored for 7 minutes and Intubating conditions in both the groups were evaluated, and scored according to the Steyn’s Modification of Helbo-Hansen Scoring system. In haemodynamic changes in Group A [0.15 mg/kg] Heart rate and Blood pressure showed an increasing trend before coming down to baseline by 7 minutes. In Group B [0.2mg/kg] the trend was decreasing from baseline after a minor increase in heart rate at 2 minutes and Mean Blood pressure was lower at all intervals when compared to baseline. Regarding Intubation In Group A maximum number of participants were having good score (56%) followed by poor (30%) and excellent (14%) whereas in Group B excellent score was maximum (62%) followed by good (32%) and poor (6%). With respect to Train of four stimulation Group B showed decreased response to neuromuscular stimulation from 3 minutes onwards compared to Group A. Bispectral index monitoring, in both groups, showed a decreasing trend at every time interval. Conclusion: Cisatracurium is a potent non-depolarising neuromuscular blocking agent and tracheal intubation can be accomplished with good to excellent intubating conditions according to Steyn’s Modification of Helbo-Hansen Scoring System at 3 minutes following 0.15 mg/kg and 0.2 mg/kg of Cisatracurium. From the study of 100 patients in 2 groups, we found that Cisatracurium in a dose of 3ED95 (0.15mg/kg) has haemodynamic stability in view of Heart rate and Blood Pressure. Cisatracurium at a dose of 0.2mg/kg was more haemodynamically stable than Cisatracurium at 0.15 mg/kg during intubation.

Abstract: Introduction : Acute Respiratory Syndrome related Coronavirus 2 (SARS-CoV2) is a novel Coronavirus causing the current pandemic. Among all the efforts to curtail the pandemic, vaccination is an important pillar towards halting transmission and reducing morbidity and mortality due to this disease. SARS-CoV2 has affected healthcare professionals (HCPs) globally . HCPs were a priority group that was vaccinated in the first phase in India. No vaccine is 100% effective. The effectiveness of the SARS-CoV2 vaccine in this high risk group needs to be studied.

Abstract: CEOs in corporations may not always employ logical reasoning when making decisions and may be swayed by personal characteristics such as overconfidence and narcissism. The decisions made by CEOs ultimately affect the company's performance. This study examines how CEO overconfidence and narcissism affect firm performance in the presence of capital structure variables. The study surveyed 39 infrastructure companies listed on the Indonesia Stock Exchange between 2017 and 2021, totaling 175 observations. The data were analyzed using Stata 16 to test the direct effect, and indirect effects were tested using the Sobel test. Results indicate that neither CEO overconfidence nor narcissism has a significant impact on a firm's performance, as measured by ROA or ROE. Furthermore, CEO narcissism has no impact on firm performance mediated by capital structure, while capital structure can mediate the effect of CEO overconfidence on ROA but not on ROE

Abstract: The manufacturing sector listed on the Indonesia Stock Exchange (IDX) has experienced a decline in performance that could be attributed to its ownership structure. Besides, Indonesian manufacturing companies rank highly among foreign debt borrowers, with a total debt of US$ 392.6 billion, as reported by Bank Indonesia's Foreign Debt Statistics (SULNI) emphasizing the importance of analyzing their capital structure to understand their financial health, as it is influenced by their debt and capital holdings. This study aims to thoroughly examine the pivotal roles of ownership structure and capital structure in shaping the future trajectory of companies in Indonesia's manufacturing sector, with a specific focus on how their financial decision-making affects their performance. This study analyzed data from 97 listed companies on the IDX over five years (2017-2021) using a statistical method called panel regression. The findings indicate that the way a company is owned does not significantly influence its performance. However, the study does uncover that capital structure, measured by DAR, negatively impacts ROA without affecting ROE. On the other hand, the capital structure measured by DER negatively impacts ROE without affecting ROA. Firm size plays a significant and positive role in influencing company performance as a control variable.

Abstract: Banyak perusahaan berlomba-lomba menunjukkan kinerja terbaiknya melalui keuntungan. Many companies are competing to show their best performance through profits. However, now the company's focus is not just looking for profits but also doing CSR to impact people and making green investments to impact the planet. Some previous studies show that green investment can affect financial performance. However, the results are inversely proportional between the companies participating in the corporate performance rating assessment program in environmental management (PROPER) and the average financial performance. In addition, there is still controversy in some previous studies regarding the effect of environmental policy on financial performance. Therefore, this study aims to determine the effect of green investment on financial performance moderated by environmental policy. The data used is data on companies that consistently obtained PROPER ratings during 2017-2021, so the panel regression analysis method is used in this study. The results of the analysis show that green investment and environmental policy cannot affect financial performance, but green investment can affect financial performance when moderated by environmental policy.

Abstract: Abstract: Mortality rate of newborns with congenital abnormalities worldwide is still high in the first four weeks after birth every year. This study aimed to obtain the profile of patients with congenital anomalies related with mortality after surgery at the Pediatric Surgery Department, Prof. Dr. R. D. Kandou Hospital, Manado, Indonesia. This was a descriptive and retrospective study. There were 98 samples during 2019 to 2021. Dependent variable was postoperative mortality outcome and independent variable risk factors were sex, gestational age, birth weight, and length of stay. Data were analyzed using univariate method. The results showed that postoperative death was 27.6%, and sex ratio was equally distributed (male 51%; female 49%). Gestational age was pre-dominanly aterm 77.5% vs preterm 22.5%. Birth weight of ≥2500 g was 77,5% and low birth weight ( 〈 2500 g) was 22.5%. Hospitalitation length of stay probably less than two weeks was 82.7%. Pediatric surgical congenital anomalies were as follows: Hirchsprung Disease (15.1%), anorectal malformation (41.7%), lateral inguinal hernia (11.3%), diaphragm hernia (3.1%), omphalocele (2.1%), gastroschisis (2.1%), undescended testis/UDT (4.1%), invagination (4.1%), duodenal atresia (3.1%), ileojejunal atresia (4.1%), esophageal atresia (5.1%), biliary atresia (1%), and mechanical bowel obstruction (3,1%). In conclusion, in the last two years, pediatric surgery services in surgical congenital anomalies had overall mortality rate of 27,6% without sex predominantly. Most patients were aterm babies with normal birth weight and relatively short length of stay. Keywords:  congenital anomalies; postoperative outcome

Abstract: Nasopharyngeal carcinoma (NPC) is an important cancer in Hong Kong. We aim to utilise liquid biopsies for serial monitoring of disseminated NPC in patients to compare with PET-CT imaging in detection of minimal residual disease. Method Prospective serial monitoring of liquid biopsies was performed for 21 metastatic patients. Circulating tumour cell (CTC) enrichment and characterisation was performed using a sized-based microfluidics CTC chip, enumerating by immunofluorescence staining, and using target-capture sequencing to determine blood mutation load. PET-CT scans were used to monitor NPC patients throughout their treatment according to EORTC guidelines. Results The longitudinal molecular analysis of CTCs by enumeration or NGS mutational profiling findings provide supplementary information to the plasma EBV assay for disease progression for good responders. Strikingly, post-treatment CTC findings detected positive findings in 75% (6/8) of metastatic NPC patients showing complete response by imaging, thereby demonstrating more sensitive CTC detection of minimal residual disease. Positive baseline, post-treatment CTC, and longitudinal change of CTCs significantly associated with poorer progression-free survival by the Kaplan–Meier analysis. Conclusions We show the potential usefulness of application of serial analysis in metastatic NPC of liquid biopsy CTCs, as a novel more sensitive biomarker for minimal residual disease, when compared with imaging.
