    Abstract: Rhodnius Stål, 1859 is the second largest genus of Triatominae after Triatoma Laporte, 1832, and includes several important Chagas vectors. Genitalia in Reduviidae are frequently used for species identification, but the current use of terminology for it is inconsistent in Triatominae. Here, Rhodnius micki sp. nov. , is described from Bolivia and considered as belonging to the pictipes group based on its morphological characters and distribution. Detailed documentation of the genitalia of Rhodnius micki sp. nov. is provided with emphasis on its everted phallus, especially the endosomal sclerites, which are potentially useful as species-level diagnostic features in Rhodnius . To further verify the validity of this species, the head shapes and wing venation patterns of five species in Rhodnius are compared with morphometric analysis. After reviewing taxonomic and comparative morphology papers of assassin bugs, a vocabulary with a terminology of morphological characters, especially of external male genitalic characters, is assembled with the preferred terms and the synonyms listed. Establishing a consistent terminological framework will greatly facilitate future research on the homology of these structures across Triatominae and will ultimately contribute to our understanding of the evolution of these groups.

    Abstract: In the multicenter, open-label, phase III FOWARC trial, modified infusional fluorouracil, leucovorin, and oxaliplatin (mFOLFOX6) plus radiotherapy resulted in a higher pathologic complete response rate than fluorouracil plus radiotherapy in Chinese patients with locally advanced rectal cancer. Here, we report the final results. METHODS Adults ages 18 to 75 years with stage II/III rectal cancer were randomly assigned (1:1:1) to five cycles of infusional fluorouracil (leucovorin 400 mg/m 2 , fluorouracil 400 mg/m 2 , and fluorouracil 2.4 g/m 2 over 48 hours) plus radiotherapy (46.0 to 50.4 Gy delivered in 23 to 25 fractions during cycles 2 to 4) followed by surgery and seven cycles of infusional fluorouracil, the same treatment plus intravenous oxaliplatin 85 mg/m 2 on day 1 of each cycle (mFOLFOX6), or four to six cycles of mFOLFOX6 followed by surgery and six to eight cycles of mFOLFOX6. The primary end point was 3-year disease-free survival (DFS). RESULTS In total, 495 patients were randomly assigned to treatment. After a median follow-up of 45.2 months, DFS events were reported in 46, 39, and 46 patients in the fluorouracil plus radiotherapy, mFOLFOX6 plus radiotherapy, and mFOLFOX6 arms. In each arm, the probability of 3-year DFS was 72.9%, 77.2%, and 73.5% ( P = .709 by the log-rank test), the 3-year probability of local recurrence after R0/1 resection was 8.0%, 7.0%, and 8.3% ( P = .873 by the log-rank test), and the 3-year overall survival rate was 91.3%, 89.1%, and 90.7% ( P = .971 by log-rank test), respectively. CONCLUSION mFOLFOX6, with or without radiation, did not significantly improve 3-year DFS versus fluorouracil with radiation in patients with locally advanced rectal cancer. No significant difference in outcomes was found between mFOLFOX6 without radiotherapy and fluorouracil with radiotherapy, which requires additional investigation of the role of radiotherapy in these regimens.

    Abstract: Total mesorectal excision with fluorouracil-based preoperative chemoradiotherapy and postoperative chemotherapy is a standard treatment of locally advanced rectal cancer. This study investigated the addition of oxaliplatin with and without preoperative radiotherapy. Methods In this multicenter, open-label, phase III trial, we randomly assigned (1:1:1) Chinese adults (age 18 to 75 years) with locally advanced stage II/III rectal cancer to three treatments: five 2-week cycles of infusional fluorouracil (leucovorin 400 mg/m 2 , fluorouracil 400 mg/m 2 , and fluorouracil 2.4 g/m 2 over 48 h) plus radiotherapy (46.0 to 50.4 Gy delivered in 23 to 25 fractions during cycles 2 through 4) followed by surgery and seven cycles of infusional fluorouracil, the same treatment plus intravenous oxaliplatin 85 mg/m 2 on day 1 of each cycle (modified FOLFOX6 [mFOLFOX6]), or four to six cycles of mFOLFOX6 followed by surgery and six to eight cycles of mFOLFOX6. Random assignment was performed by using computer-generated block randomization codes. The primary end point was 3-year disease-free survival. Secondary end points of histopathologic response and toxicity are reported. Results A total of 495 patients were enrolled from June 2010 to February 2015; 475 were evaluable (fluorouracil-radiotherapy, n = 155; mFOLFOX6-radiotherapy, n = 157; mFOLFOX6, n = 163). In the fluorouracil-radiotherapy, mFOLFOX6-radiotherapy, and mFOLFOX6 groups, the rate of pathologic complete response (pCR) was 14.0%, 27.5%, and 6.6%, and downstaging (ypStage 0 to 1) was achieved by 37.1%, 56.4%, and 35.5% of patients, respectively. Higher toxicity and more postoperative complications were observed in patients who received radiotherapy. Conclusion mFOLFOX6-based preoperative chemoradiotherapy results in a higher pCR rate than fluorouracil-based treatment. Perioperative mFOLFOX6 alone had inferior results and a lower pCR rate than chemoradiotherapy but led to a similar downstaging rate as fluorouracil-radiotherapy, with less toxicity and fewer postoperative complications.

    Abstract: JOURNAL/nrgr/04.03/01300535-202405000-00044/inline-graphic1/v/2023-09-28T063346Z/r/image-tiff Traumatic spinal cord injury is potentially catastrophic and can lead to permanent disability or even death. China has the largest population of patients with traumatic spinal cord injury. Previous studies of traumatic spinal cord injury in China have mostly been regional in scope; national-level studies have been rare. To the best of our knowledge, no national-level study of treatment status and economic burden has been performed. This retrospective study aimed to examine the epidemiological and clinical features, treatment status, and economic burden of traumatic spinal cord injury in China at the national level. We included 13,465 traumatic spinal cord injury patients who were injured between January 2013 and December 2018 and treated in 30 hospitals in 11 provinces/municipalities representing all geographical divisions of China. Patient epidemiological and clinical features, treatment status, and total and daily costs were recorded. Trends in the percentage of traumatic spinal cord injuries among all hospitalized patients and among patients hospitalized in the orthopedic department and cost of care were assessed by annual percentage change using the Joinpoint Regression Program. The percentage of traumatic spinal cord injuries among all hospitalized patients and among patients hospitalized in the orthopedic department did not significantly change overall (annual percentage change, –0.5% and 2.1%, respectively). A total of 10,053 (74.7%) patients underwent surgery. Only 2.8% of patients who underwent surgery did so within 24 hours of injury. A total of 2005 (14.9%) patients were treated with high-dose (≥ 500 mg) methylprednisolone sodium succinate/methylprednisolone (MPSS/MP); 615 (4.6%) received it within 8 hours. The total cost for acute traumatic spinal cord injury decreased over the study period (–4.7%), while daily cost did not significantly change (1.0% increase). Our findings indicate that public health initiatives should aim at improving hospitals’ ability to complete early surgery within 24 hours, which is associated with improved sensorimotor recovery, increasing the awareness rate of clinical guidelines related to high-dose MPSS/MP to reduce the use of the treatment with insufficient evidence.

    Abstract: People with type 1 diabetes mellitus (T1DM) are significantly more likely to have osteoporosis (OP). Astragali Radix is a Chinese herbal medicine containing various active ingredients, and several clinical trials have been reported to use it to treat OP and T1DM, respectively. Objective: To evaluate the targets and potential mechanisms of Astragali Radix administration on OP and T1DM. Methods: The targets of Astragali Radix were identified using the Traditional Chinese Medicine Systems Pharmacology (TCMSP) database. The OP and T1DM datasets were downloaded from the Gene Expression Omnibus (GEO) database. The weighted gene correlation network analysis (WGCNA) method was used to identify the co-expression genes associated with OP and T1DM. In addition, the common gene targets of OP and T1DM were screened using two public databases. Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analyses were performed using the R tool. After the validation of key genes, molecular docking was performed to visualize small molecule-protein interactions. Results: The compound target network mainly contained 17 compounds and 147 corresponding targets. There were 561 GO items and 154 signaling pathways in KEGG, mainly including the AGE-RAGE signaling pathway in diabetic complications and osteoclast differentiation. The results of molecular docking showed that flavonoids were the top compound of Astragali Radix, which had a high affinity with CDK2, VEGFA, and MYC. Conclusion: Flavonoids in Astragali Radix may regulate multiple signaling pathways through MYC, CDK2, and VEGFA, which may play a therapeutic role in OP and T1DM.

    Abstract: Allotetraploid is a new species produced by distant hybridization between red crucian carp ( Carassius auratus red var., abbreviated as RCC) and common carp ( Cyprinus carpio L., abbreviated as CC). There is a significant difference in growth rate between allotetraploid and its parents. However, the underlying molecular mechanism is largely unknown. In this study, to find direct evidence associated with metabolism and growth rate in protein level, we performed quantitative proteomics analysis on liver tissues between allotetraploid and its parents. A total of 2502 unique proteins were identified and quantified by SWATH‐MS in our proteomics profiling. Subsequently, comprehensive bioinformatics analyses including gene ontology enrichment analysis, pathway and network analysis, and protein–protein interaction analysis (PPI) were conducted based on differentially expressed proteins (DEPs) between allotetraploid and its parents. The results revealed several significant DEPs involved in metabolism pathways in liver. More specifically, the integrative analysis highlighted that the DEPs ACSBG1 , OAT , and LDHBA play vital roles in metabolism pathways including “pentose phosphate pathway,” “TCA cycle,” and “glycolysis and gluconeogenesis.” These could directly affect the growth rate in fresh water fishes by regulating the metabolism, utilization, and exchange of substance and energy. Since the liver is the central place for metabolism activity in animals, we firstly established the comprehensive and quantitative proteomics knowledge base for liver tissue from freshwater fishes, our study may serve as an irreplaceable reference for further studies regarding fishes’ culture and growth.

    Abstract: In this study, a quasi-stumps group structure was proposed and placed upstream of the bridge piers to mitigate the scour of the waterflow on the riverbed. Both experiment and numerical simulations using FLOW 3D were employed to study the protective effect of this structure. The numerical results were in good agreement with the experimental findings. It was found that the quasi-stumps group can effectively reduce the flow velocities around the bridge piers, thereby promoting the deposition of suspended sediment. As a result, there was no erosion around the piers, and instead, siltation was formed, which contributed to the stability of the piers. The deposition height around the piers increased as the L (the horizontal distance between the quasi-stumps group and the piers) decreased and both the P (the height of the quasi-stumps group) and S (the ratio of the area of a single leaf on the quasi-stumps group to the cross-sectional area of a single pier) increased. As the L, P, and S increased, the quantity of suspended sediment deposition over the entire riverbed increased. The optimal combination of the quasi-stumps group’s protective effect was determined to be L = D (pier diameter), P = H (water depth), and S = 0.148.

    Abstract: Fuzzy systems (FSs) are popular and interpretable machine learning methods, represented by the adaptive neuro-fuzzy inference system (ANFIS). However, they have difficulty dealing with high-dimensional data due to the curse of dimensionality. To effectively handle high-dimensional data and ensure optimal performance, this paper presents a deep neural fuzzy system (DNFS) based on the subtractive clustering-based ANFIS (SC-ANFIS). Inspired by deep learning, the SC-ANFIS is proposed and adopted as a submodule to construct the DNFS in a bottom-up way. Through the ensemble learning and hierarchical learning of submodules, DNFS can not only achieve faster convergence, but also complete the computation in a reasonable time with high accuracy and interpretability. By adjusting the deep structure and the parameters of the DNFS, the performance can be improved further. This paper also performed a profound study of the structure and the combination of the submodule inputs for the DNFS. Experimental results on five regression datasets with various dimensionality demonstrated that the proposed DNFS can not only solve the curse of dimensionality, but also achieve higher accuracy, less complexity, and better interpretability than previous FSs. The superiority of the DNFS is also validated over other recent algorithms especially when the dimensionality of the data is higher. Furthermore, the DNFS built with five inputs for each submodule and two inputs shared between adjacent submodules had the best performance. The performance of the DNFS can be improved by distributing the features with high correlation with the output to each submodule. Given the results of the current study, it is expected that the DNFS will be used to solve general high-dimensional regression problems efficiently with high accuracy and better interpretability.
