    Abstract: The emergence of SARS-CoV-2 and its worldwide spread has become a challenge for healthcare professionals related to rapid diagnosis of infection caused by the new coronaviruses, provision of specialized medical care, rehabilitation and secondary prevention. Currently, there is limited information on primary, secondary prevention and medical rehabilitation of this disease. The most common clinical manifestation of the new variant of coronavirus infection is bilateral pneumonia, with 34% of patients developing acute respiratory distress syndrome (ARDS). Given the unusual pandemic situation and the peculiar COVID-19 pathogenesis, the stereotyped application of generally accepted methods may be unsafe or ineffective. The Union of Rehabilitologists of Russia has prepared Temporary guidelines that are based on the advice on medical rehabilitation of patients with COVID-19 at various stages of special medical care published by WHO specialists, analysis of reports from clinics currently involved in rehabilitation of COVID-19 patients, as well as on the results of clinical studies conducted earlier on the rehabilitation of patients with Post-intensive care syndrome and adult respiratory distress syndrome (ARDS) of non-coronavirus etiology, regulatory documents of the Russian Ministry of Health and Federal Service on Surveillance for Consumer rights protection and human well-being. A syndrome-based approach to the use of means and methods of physical and rehabilitation medicine is also considered. The guidelines are intended for heads of medical organizations and their structural divisions, general practitioners, physicians, infectious disease doctors, pediatricians, emergency physicians of intensive care units in infectious hospitals, physiotherapy and sports medicine doctors, physiotherapists, medical psychologists, physical therapy instructors, nurses and other specialists of multidisciplinary rehabilitation teams working in the field of medical care for patients with COVID-19 at various stages.

    Abstract: The early intervention system is currently considered as an independent activity which has a clear conceptual model. The generalmechanisms of the early intervention system facilitate the child’s social adaptation and its integration into the educational environment. This creates the conditions for the child development and the formation of the child’s skills necessary in daily life situations. At the same time, it remains unclear how the routing of a child in need of rehabilitation is carried out, taking into account the need toreceive early intervention services for the target group of patients. Aim. To determine the place of early intervention in the structure of the rehabilitation service of the Republic of Tatarstan. Results and Discussion. The early intervention system contains functional, personality-oriented and environmental approaches,which are implemented using the domains of activity and participation of the International Classification of Functioning, Disabilitiesand Health. The use of this classification implies the need for joint work of the number of specialists who make up a multidisciplinaryteam, forms a unified methodology for constructing rehabilitation plans and early assistance programs, and offers a conceptual basisfor analyzing the effectiveness of their implementation. In addition, the application of the International Classification of Functioning,Disabilities and Health makes it possible to determine the role of early intervention when planning a child’s rehabilitation route. The articlepresents a model of routing a child in need of rehabilitation measures, implemented in the Republic of Tatarstan, which providesfor early identification of children with indications and their referral accordingly to these indications to early intervention, medical orsocial rehabilitation. Conclusion. Identifying children of the target group in need of the early intervention and their timely referral to organizations that areproviders of relevant services is one of the stages in solving urgent problems of rehabilitation. Currently, the departments of early interventionsystem are opened on the basis of medical, social and educational organizations in the Republic of Tatarstan, and includedin the rehabilitation structure playing an important integrating role in the formation of interagency cooperation.

    Abstract: Introduction. In medical rehabilitation of patients who have suffered a new coronavirus infection COVID-19, it is recommended to use reflexotherapy (acupuncture). Before applying reflexotherapy, it is recommended to conduct an acupuncture diagnosis which allows to choose a method of exposure, create an individual acupuncture prescription and carry out subsequent monitoring of its effectiveness. Aim. Prepare a literature review using Russian and foreign sources and present data from modern studies on the use of reflexology in the medical rehabilitation of patients who have undergone a new coronavirus infection COVID-19. Conclusion. Currently, various methods of reflexotherapy are used to impact on acupuncture points. The use of reflexotherapy increases the body’s defenses, has a significant positive effect on bronchial obstruction, regional lung ventilation, diaphragm functional ability, pulmonary hemodynamics and helps to restore the sense of smell and taste. Reflexotherapy methods reduce the manifestations of myofascial pain syndrome affecting the respiratory muscles, restore muscle strength, reduce shortness of breath and anxiety. The experience of doctors from different countries will increase the effectiveness of comprehensive rehabilitation and the quality of life of patients with various complications after a new coronavirus infection COVID-19.

    Abstract: BACKGROUND: Currently, the incidence of developmental disorders and the level of disability in premature infants, especially those born with a low and extremely low body weight, remain high, that determines the relevance of habilitation methods. AIM: to develop and evaluate the effectiveness of the early habilitation complex in premature infants with a perinatal brain damage when transferring to the stage III of rehabilitation. MATERIALS AND METHODS: The study included 86 patients born with a very low and extremely low body weight, who had a hypoxic-ischemic or hemorrhagic brain damage and were observed at the Children's Republican Clinical Hospital of the Ministry of Health of the Republic of Tatarstan during the first three years of life. A comparative analysis of the development and functional outcomes of the disease was carried out for children of two groups. The first group, when transferred to the stage III of rehabilitation, was assigned a complex of early sensorimotor habilitation, developed on the basis of N.A. Bernsteins concept to be carried out at home for three months after an appropriate training. The second group was not prescribed such a complex. RESULTS: The effect of the early sensorimotor rehabilitation complex on the functional (motor, speech and cognitive) outcomes in prematurely born patients was evaluated and described. In the first group with an assigned complex of sensorimotor exercises, despite the absence of a significant effect by the age of 6 months, it was found by the age of one and a half years, that the average indices of the development of large (p=0.022) and fine motor skills (p=0.023), expressive speech (p=0.006) and cognitive functions (p=0.040) were higher, while, by the age of three years, the differences in speech (p=0.001) and cognitive (p=0.001) outcomes retained their statistical significance compared to the development indicators in children who did not receive the same early habilitation complex. CONCLUSION: It was found that premature infants who received the early habilitation complex had better speech and cognitive outcomes by the age of three, compared to patients who did not receive the complex. The significance of the positive influence on the ontogenesis of motor skills persists during the second year of life.

    Abstract: To date, the introduction of the International Classification of Functioning, Disability and Health (ICF) into the practical work of rehabilitation institutions and units of different levels and different profiles is a key moment in the organization of the rehabilitation process in accordance with a new legislative framework and a new model of the medical rehabilitation system being created in Russia. Namely, the ICF is the basis for the rehabilitation diagnosis formulation as well as rehabilitation goals and the development of the rehabilitation strategy for each individual patient, and the practical ICF use presents the greatest difficulties for members of the multidisciplinary rehabilitation team (MDRT), since it requires to restructure their thinking stereotypes and behavior « from dysfunction – to limitation of activity», «from correction of disorders to restoration of activity». In this regard, the ICF usage issues in the process of rehabilitation of patients who underwent arthroplasty of large joints, presented in this article, are very relevant. Aim. To describe the most frequent ICF domains used in the formulation of a rehabilitation diagnosis in patients who have undergone the lower limb joints replacement. Conclusion. Identification of key problems based on ICF in patients undergoing total knee and hip arthroplasty allows a more focused and specialized approach to the rehabilitation process for this category of patients, correct formulation of rehabilitation goals and drawing up a plan of rehabilitation measures. In patients who have yet to undergo joint replacement, the use of classification allows to clarify the indications for surgical intervention.

    Abstract: BACKGROUND: Medical rehabilitation of spinal cord injured patients is an important medical and social problem associated with the high level of disability in this patient category. AIM: This study aims to improve the efficiency of medical rehabilitation of spinal cord injured patients based on a set of measures of nutritional support and raising the exercise tolerance. MATERIALS AND METHODS: A total of 78 patients in the intermediate recovery period of spinal cord injury were monitored, and their nutritional status was evaluated in the dynamics. In the first (main) group, a complex correction of the nutritional status with additional use of high protein hypercaloric mixtures for enteral nutrition was performed before and after hand cycling. In the second (control) group, the rehabilitation techniques were performed on the generally accepted basis. RESULTS: As a result of the complex correction, the proportion of patients with mild and moderate malnutrition was significantly lower in the first (main) group than that in the second (control) group (p=0.05). Peak oxygen consumption increased in the first group of patients with mild malnutrition on 34.1% (p=0.019) and with moderate malnutrition on 23.6% (p=0.02) compared to that in the control group. All spinal cord injured patients of the first (main) group were found to have improved their motor and social activity according to the FIM scale compared to that in the control group (p 0.001). CONCLUSIONS: Combining adequate nutritional support and proper handcycling significantly contributes to reducing malnutrition in patients with spinal cord injury (p=0.05). Tolerance to physical exercises during the complex correction of malnutrition significantly increased (p 0.01) as well as the motor and social activities and, therefore, the quality of life (p 0.0001).

    Abstract: Patients with a new coronavirus infection (COVID-19) need rehabilitation. Rehabilitation approaches of patients with another pathology also require to be reviewed under the conditions of the SARS-CoV-2 epidemic. It is necessary to develop clinical recommendations for the rehabilitation care delivery in patients with COVID-19 and other diseases during a pandemic. A group of experts analyzed the data of current reviews on rehabilitation with COVID-19, as well as previous studies on rehabilitation in patients with Post-intensive care syndrome (PICS) and acute respiratory distress syndrome of non-coronavirus etiology, and developed thereupon the main positions of clinical recommendations. We discuss the main rehabilitation problems in patients with COVID-19, in particular structures and functions impairments, leading to limitations and restrictions of self-care, mobility, domestic life, communication, interpersonal interactions and relationships, professional activities and those determining care requirement. The article also provides general recommendations on medical rehabilitation management in a pandemic at all three stages, including patients routing, ensuring the infectious safety of medical personnel and patients. The necessary individual rehabilitation program components for patients with COVID-19 at all stages of rehabilitation have been identified, including patients examination, nutritional deficiency correction, restoration of respiratory function, exercise tolerance, muscle strength, self-monitoring and training in new movement conditions, psychoemotional state disorders and cognitive functions, as well as independence in everyday life recovery. Medical rehabilitation during the COVID-19 epidemic should include all rehabilitation care components and helps to optimize vital functions, to prevent complications and to improve the patients life quality.
