    Kurzfassung: Chicken pox is a self-limiting, infectious disease caused by Varicella-zoster virus (VZV). In adults though rare, severe form of disease may cause complications like hepatitis, pneumonia, encephalitis.     Kurzfassung: Background: Antibiotics are available as non-prescription drugs in pharmacies and irrational use is not uncommon. Diagnosis and treatment of most of the bacterial diseases are empirical. Microbial sensitivity patterns of common infections like respiratory tract infection, urinary tract infection, enteric fever, wound infection are not routinely available for decision making in drug selection. Lack of hospital restrictions on antibiotic use and inappropriate usage for prophylaxis are the main reasons for inappropriate therapy. Objective: To determine the pattern of antibiotic use in hospitalized patients of different departments of Dhaka Medical College Hospital. Materials & Methods: In this observational study, hospital records of total 400 patients were surveyed on 2 separate days of December 2013. Patients of both sexes from different departments receiving antibiotics were included in the study. Data was collected in a predesigned data collection sheet. Results: Of the total 400 patients 53.5% were male & 46.5% were female. 21% patients were of paediatric age group (upto 12 years) and 79% were adults. In maximum number of patients empirical antibiotic therapy was started. Culture and sensitivity tests were done before or during the course of treatment in only 48 patients (12%). Most commonly used antibiotic was ceftriaxone (49.75%) followed by metronidazole (17.5%) and ciprofloxacin (12.75%), while 7 patients (1.75%) received anti tubercular regimen. Combined antibiotics were used in 162 patients (40.5%). Conclusion: Most of the patients received empirical antibiotic therapy. Culture and sensitivity tests should be more practiced and emphasized before starting treatment. Energetic measures to stop inappropriate use of antibiotics should be taken to slow down the emergence and spread of antimicrobial resistance.DOI: http://dx.doi.org/10.3329/jom.v16i1.22399 J MEDICINE 2015; 16 : 35-38

    Kurzfassung: Background: Non-adherence to anti-hypertensive medications can have a negative impact on long term cardiovascular outcome. Various studies have been conducted on this issue but factors are not yet explored properly, particularly in Bangladesh. This study was conducted to find out the prevalence and factors associated with pattern of compliance to anti-hypertensive medications in a tertiary level hospital.Methods: This descriptive study was done on 146 indoor hypertensive patients, included by purposive sampling in july2015 who were taking anti-hypertensive for last 6 months. Data were collected through a questionnaire after obtaining informed consent.Results: This study revealed that 55.47% patients were compliant and 44.53% were noncompliant. Age 〉 60 yrs. are more non-compliant (56.6%). Female (65.11%), house wife and urban population are more compliant. Population, with longer duration of hypertension are more non-compliant. Among the co-morbidities diabetic patients are more non-compliant. Population taking two anti-hypertensive are more compliant (76.47%).Conclusion: In our study most of the populations are compliant but yet significant number of populations are non-compliant. Collective participation of physician, patient and health care delivery system can improve the situation more. As consequence of non-compliance to antihypertensive is grave, community based studies should be conducted to explore the extent of non-compliance.J Dhaka Medical College, Vol. 24, No.1, April, 2015, Page 62-66

    Kurzfassung: Castlemans disease is a rare lymphoproliferative disorder. Castlemans disease is a disease which involves massive non-malignant proliferation of lymphoid tissues that usually presents as mediastinal masses as well as other groups of lymphnodes. That is why, sometimes, confusion may arise whether the lymphadenopathy is benign or malignant. However, some authors describes, histologically and prognostically Castlemans disease is distinct from malignant lymph-node hyperplasia.     Kurzfassung: Introduction: Since the first detection of corona virus disease (COVID-19) cases in Dhaka, Bangladesh on 8 March, 2020, numbers are rising alarmingly. Clinical data on COVID-19 in Bangladesh is lacking. We report early findings on demographic profile, clinical presentations and short-term clinical outcomes of confirmed COVID-19 patients admitted in a large teaching hospital in Dhaka, Bangladesh with preliminary analyses of their association with mortality. Materials and Methods: In this retrospective cross-sectional study, we included reverse transcription polymerase chain reaction (RT-PCR) confirmed COVID-19 patients aged ≥ 15 years, who were admitted in Dhaka Medical College Hospital (DMCH) between May 2 and 15, 2020, the first two weeks when DMCH started admitting COVID-19 patients. Data were collected between May 25 and 29, 2020 from patients or their attendants through telephone interview by a structured questionnaire, after having appropriate consent, irrespective of outcome. One hundred and eight consecutive patients met inclusion criteria through convenient sampling from ward registrar, 102 patients could be reached over phone and data from two patients were discarded in the data cleaning process. The statistical analysis was done by the Statistical Package for the Social Sciences (SPSS) version 22.0. Results: Among the total participants (n=100), mean age was 41.7±16.3 years, 63% were male and 60% patients had positive contact history. Appearance of symptom to hospital admission time was a median of 6 days (range 1 to 21 days) and mean hospital stay was 7.77 ± 5.62 days. Predominant presenting symptoms were fever (69%), cough (54%), breathlessness (41%), fatigue (40%), anorexia (26%) and diarrhea (19%). Hypertension (21%), diabetes mellitus (16%), heart diseases including ischemic heart disease (IHD) (8%) and renal diseases including chronic kidney disease (CKD) (8%) were frequent comorbidities. Ten out of hundred patients died. Older age (p= 0.001), male sex (p= 0.007), smoking (p= 0.001), breathlessness (p=0.001) and presence of comorbidities (p= 〈 0.05) were significantly associated with mortality. Conclusion: Frequent positive contact history and significant association of breathlessness, smoking and comorbidities with mortality in our study reinforces that abiding by the prevention and containment process, smoking cessation, ensuring proper oxygen therapy and addressing comorbidities adequately are very important measures to mitigate COVID-19 in Bangladesh like the rest of the world.     Kurzfassung: Chattogram city is grappling with problems caused by inadequate solid waste management (SWM). Because the successful policy intervention regarding proper SWM depends on the intrinsic motivation of the city dwellers along with government intervention, the objective of this paper was to conduct an attitudinal study of the residents of Chattogram regarding their roles and responsibilities towards the SWM, and thus, the sustainable development of the city. It is a cross-sectional study based on a sample size of 150. Data was collected using a structured, pretested questionnaire. The result showed that the residents were unable to prioritize the city’s environmental problem over other problems due to the lack of knowledge and awareness about the environmental problems and its impacts on the Chattogram. However, in respect of SWM in the city, their eagerness for participation is appreciable, even though they are more dependent on government policy initiatives. An attitudinal gap was also observed here among the respondents pertaining to the vacuum of knowledge and awareness. Therefore, strong policy decisions and the awareness campaign can make people aware of their roles and responsibilities for a sustainable city and intrinsically motivate them to participate in SWM.

    Kurzfassung: immunoproliferative small intestinal disease is considered as a rare condition and has endemicity inMediterranean countries and often associated with campylobacter jejuni infection. This conditionvaries in severity from benign to frankly malignant. Prolonged remission can be obtained with longterm antibiotic therapy but chemotherapy is required for those who have aggressive disease. Bangladeshidata on IPSID is sparse. Here our patient 21 years old male presented with chronic diarrhoea, occasionalvomiting, severe weakness and significant unintentional weight loss. He had cachectic body built,bipedal edema, easy bruising, glossitis with angular stomatitis. His upper GI endoscopy showed multiplenodular lesion in the duodenum. Biopsy was taken from nodular lesion and sent for histopathology,geneXpert and culture. Histopathology revealed IPSID. He was treated with tetracycline and improvedclinically.     Kurzfassung: Laurence-Moon-Bardet-Biedl syndrome is a rare autosomal recessive disorder characterized bystructural and functional abnormalities of different organ and tissues. Renal involvement is the single most feature of prognostic importance. We report a case with end stage kidney disease. The diagnosis was missed until patient presented with renal dysfunction.     Kurzfassung: Background: Cardiovascular autonomic dysfunction is a serious complication of diabetes mellitus (DM) and is independently associated with microalbuminuria in patients with type-2 DM. This study was conducted to determine the prevalence of cardiovascular autonomic dysfunction in type-2 diabetes patients with microalbuminuria.Materials & Methods: This descriptive type of observational study was done in the department of Medicine, Dhaka Medical College Hospital from August 2015 to January 2016.Results: A total of 50 patients was included in the study with male predominance (M:F ratio 2. 22:1). Peak age was between 40-60 years and majority of patients (32%) was between 41-50 years and mean duration of DM was more than 12 years. Out of 50 patients, 19 (38%) patients had mild microalbuminuria; 24 (48%) patients had moderate microalbuminuria and 7 (14%) patients had severe microalbuminuria. Common symptoms were lethargy, anorexia, nausea, vomiting, dizziness and bladder symptoms. Cardiac autonomic neuropathy (CAN) was found in 80% cases. Parasympathetic dysfunction was observed in 40 (80%) cases and sympathetic dysfunction was observed in 43 (86%) cases.Conclusion: As significant number of DM patients (80%) develop CAN, so cardiovascular autonomic function tests should be monitored to address major potential cardiovascular complications even in asymptomatic patients, especially among those with microalbuminuria.J MEDICINE JUL 2018; 19 (2) : 104-109
