    Abstract: Almost all orthodontic wires suffer from corrosion as they are intra-orally engaged. This chemical structure alteration appears on the surface of these wires, surface topography can be easily visualised with scanning electron microscope method. The aim of our study was to assess the intraoral corrosion of the retrieved orthodontic Ni-Ti archwires. Archwire retrieval procedure yielded approximately 30 retrieved wires, placed intra-orally for 1-5 months. SEM analysis was performed and surface changes were interpreted. Our SEM results showed, that surface corrosion and pitting can be seen on the surface of retrieved Ni-Ti wires, the depth of corrosion depends on the time wires have been engaged in the oral cavity. With regards of metal liberation consequently surface corrosion, practitioners should be avare of these chemical changes which can affect the resistence of the orthodontic appliance and patient health.

    Abstract: Dental sealants are excellent means to prevent pits and fissure caries. Nowadays, the application of sealants is extended to therapeutic use in arresting non-cavitated carious lesions. This relatively new concept still lacks evidence to support its routine use. The aim of this study was to evaluate the effectiveness of a resin-based sealant applied on first permanent molars with carious lesions (ICDAS 1–3), in comparison with its effectiveness when applied on sound surfaces (ICDAS 0). Included in the study were 114 children aged between six and eight years old, with a high caries risk (according to the CAMBRA system), with all four permanent molars erupted and with deep pits and fissures. A total number of 407 molars were sealed and assessed. A total of 49 were excluded (they had caries, which according to the ICDAS II classification were classified with code 4–6 or had older sealants or fillings). Out of these 407 molars, 213 were sound (code 0) and 194 had caries lesions as follows: 56 teeth classified as code 1, 79 teeth classified as code 2, and 59 teeth classified as code 3 according to the ICDAS II classification. The retention of the sealant and carious lesions were assessed clinically at 6, 12, 18, and 24 months. Regarding sealant retention, a statistically significant difference (p 〈 0.05) among the two types of sealed teeth, sound (ICDAS 0) and decayed (ICDAS 3), showed at 18- and 24-month follow-up intervals. Regarding caries lesions, a statistically significant difference (p 〈 0.05) showed between sound (ICDAS 0) and decayed (ICDAS 3) molars at 24-month follow-up. Our study results supported the resin-based sealant effectiveness in arresting incipient carious lesions, which according to the ICDAS II classification have received codes 1 and 2 but did not support sealant effectiveness in arresting caries lesions classified according to the same classification with code 3.

    Abstract: Introduction. White spot lesions are early signs of demineralization, which may or may not lead to the development of caries. An inactive white spot lesion might act as an arrested dental caries and affect the esthetic appearance by displaying a milky white color. These lesions need a non-invasive or a minimally invasive treatment. One of the materials taken into consideration in treating these lesions is ICON, a biomimetic material that infiltrates the white spots and can reestablish the esthetic function. The aim of the current study was to evaluate the esthetic efficacy of a minimally invasive technique and a relatively new product on the dental market, ICON, in the treatment of different non-cavitated carious lesions. Case presentation. A 14-year-old patient presented to the dental clinic with asymptomatic, non-cavitating lesions on the cervical aspect of all permanent teeth. The lesions were diagnosed as incipient caries on the buccal, occlusal, and palatal surfaces of the teeth, and the proposed treatment to arrest their progression and improve the esthetic appearance was the use of the ICON infiltration technique. Conclusion. The ICON resin infiltration treatment increased esthetics, visibly diminishing the appearance of non-cavitating carious lesions.

    Abstract: Lasers can interact with dental tissues inducing changes in tooth characteristics, regarding the surface morphology and elemental composition. The alterations were observed in Calcium, Phosphorus, Sodium, Oxygen and Carbon elements, which influence the resistance and chemical properties of dental hard tissues. The thermal energy used by laser reduces water content of the tissues and as a consequence, the level of oxygen composition decreases after irradiation. Numerous studies had demonstrated its ability to ablate hard-tissues, without any detrimental thermal effects such as cracking or melting for the adjacent tissues. The aim of this article is to present the in vitro effects of Er:YAG laser in comparison with sonic, ultrasonic and manual instruments on the root cement during scaling and root planning, evaluated by scanning electron microscope. The study was conducted on extracted teeth, divided in four groups according to the method used for scaling and root planning. The specimens were analyzed by scanning electron microscopy and the morphological alterations of the cement were evaluated based on a scoring system. Data were statistically analyzed using Mann-Whitney test and the level of significance was set at p[0.05.We noted unfavorable results on the root cement after using Er:YAG laser as craters and cracks induced by heat. There was a greater amount of roughness on the root surface after Er:YAG was used for scaling and root planning compared to manual, sonic and ultrasonic methods. Despite favorable results obtained after the use of Er:YAG laser during periodontal treatment, further clinical studies are necessary in order to determine in which moment of the therapy these methods are most suitable.

    Abstract: Orthodontic treatment with fixed devices should only be indicated in case of a stable, non-active periodontal disease status. Throughout orthodontic treatment, a careful assessment of the periodontal status is advised. Due to its anti-inflammatory and antimicrobial effects, laser therapy is frequently used as an adjunct to classic periodontal therapy. The aim of this study was to evaluate the advantages and limitations of the use of laser therapy on periodontal status during orthodontic treatment. Throughout the 9 months during which this placebo-controlled, single-blind clinical trial was conducted, 32 patients were included in the study, divided into two groups: microscope “+” (patients who observed the bacteria within the dental plaque-sample examination on the screen of a dark-field microscope in real time) and microscope “−” (patients who did not see the oral pathogens using a dark-field microscope). For all patients, using the split-mouth study design, laser therapy was applied to one hemiarch (HL), whereas the other hemiarch received treatment without active light (HC). After one month, by analyzing the main indicators of periodontal health status, we found that the plaque index (PI) and bleeding on probing (BOP) values were significantly decreased after receiving treatment (for PI: HL-p = 0.0005, HC-p = 0.0297; for BOP: HL-p = 0.0121, HC-p = 0.0236), whereas the probing-depth (PD) values remained almost the same as before treatment (HL-p = 1.5143; HC-p = 1.4762). Conclusions: The use of the dark-field microscope proved to be beneficial in sensitizing patients to the presence of bacteria in the oral cavity and motivated them to strictly follow the rules of oral hygiene. Laser treatment can be a valuable aid in periodontal therapy, but only in adjunction with mechanical therapy.

    Abstract: About 50% of tooth loss is caused by either endodontic infection, periodontal disease or the combination of the two in the form on endo-perio lesions (EPL). Combined EPL develop due to the intimate anatomic and functional relation between endodontic and periodontal tissues. Both the pulp and periodontium share the same embryologic and anatomic origin. The various pathways connecting the two, added up to the extremely alike microorganisms in both illnesses’ etiology, create a complex condition in which interdisciplinary approach is required. Despite numerous decades of literature describing these lesions, they remain a continuous challenge for practitioners in both diagnosing and managing. The purpose of this article is to present a comprehensive review of various aspects of the combined EPL and to emphasize the importance of making a correct diagnosis and adopting the appropriate treatment method in the management of these challenging situations.

    Abstract: Oral health is a critical indicator of children’s quality of life, which at this early age, depends exclusively on the attention, involvement, and guidance of parents or caregivers. Assessing carious damage and measuring the prevalence of early severe tooth decay in temporary teeth in children is obtained by calculating the dmft index, giving the sum of an individual’s decayed, missing, and filled teeth. The aim of our study was to conduct a clinical examination of the oral status of institutionalized children from Romania. We selected and included in the study 144 children, both boys and girls in equal number, with ages of five or six years old, from which 110 were eligible for the study and met the inclusion criteria. Of all children, 20.90% were five years old girls, 27.27% five years old boys, 26.36% six years old girls, and 25.46% six years old boys. Of all, 10,45% had more than three incisors presenting decays, equally affected by gender. Of the boys’ group, 20% of age six had more de two canines affected, and 26.7% of five years old girls had more than three affected canines. Of the five years boys’ group, 24.3% had more than three affected temporary molars, 44.9% of six years boys had more than five. Of the five years old girls, 33.3% had more than four temporary molars affected and, 56.6% of the six years old girls’ group had more than five molars presenting carious lesions. The dental status and dmft values were similar for five years old and six years old children and similar boys and girls. Due to the vulnerability of young children that consists not only in their inability to identify, express, and address their own needs but also the lack of parental support, lack of an optimal diet for age, and proper hygiene, they reach adolescence with an impaired dental status, inappropriate for their age.

    Abstract: Acute lymphoblastic leukemia (ALL) is a malignant condition defined by the proliferation of immature hematopoietic bone marrow cells, which replace the normal bone marrow elements. Furthermore, immature cells will accumulate in various areas of the body. Chemotherapeutic drugs are cytotoxic and have unwanted effects on normal cells. This toxicity is acute, or it may be chronic. Approximately 60% of survivors of ALL will suffer from one or more health problems or adverse side effects of this treatment. Oral complications are a frequent consequence of oncology therapy in pediatric patients. Xerostomia, carious lesions, gingivitis, tumors of the oral cavity, bone deformities of the jaw, temporomandibular joint disorders, trismus, and infections due to abundant microbial flora are common in these children. The risk of dental anomalies (dental or root agenesis, root malformations, enamel defects, dental hypoplasia) is higher in those who started treatment before permanent teeth erupt. All children are at risk of developing oral lesions due to poor hygiene, but children who have survived cancer are at even higher risk. The survival rate of children diagnosed with ALL has significantly increased thanks to remarkable advances in chemotherapy. Therefore, it is important that these patients are constantly monitored by the dentist to treat complications arising from the treatments in a timely manner. In children diagnosed with ALL, oral health is of great importance as the oral environment changes as a side effect of hematological disorders and their treatments. Treatment of carious lesions, periodontal problems and lesions of the oral mucosa must be done before starting chemotherapy by cleaning the oral cavity to eliminate any possible risk of infection. The participation of the dentist is important in improving the preventive protocols and oral examination of the pathological processes that occur in children with ALL. Interdisciplinary collaboration between physicians is essential during the treatment of pediatric cancer patients.

    Abstract: Fracture of endodontic files can be considered a real concern during root canal treatment. This procedural accident is caused both by overusing instruments due to economic reasons and using an incorrect technique in root canals with difficult anatomy. A broken endodontic file does not always regard the treatment as a failure. Frequently the broken fragment can be bypassed, and the root canal treatment completed. In other cases, when the file is separated in a tooth with associated apical disease, its retrieval using ultrasonic tips is advised. Separation of a file usually occurs in molars, mainly in the mesiobuccal canal due to major curvature, poor access or small diameter. Our research was conducted to determine the in vitro efficiency of the ultrasonic removal method and the bypass technique by using 40 extracted molars with moderately curved roots. #25 K-files were deliberately broken in the middle third of mesiobuccal root canals after making a small excavation in the apical third of their active part. The results showed a significantly higher success rate of the ethylenediaminetetraacetic acid (EDTA) aided bypassing technique compared to the ultrasonic removal method.
