    Abstract: Background According to World Health Organization, an estimated 700000 to 1 million new cases and 26000 to 65000 deaths occur annually from Kala-azar. In the countries of South-East Asia region, Kala-azar occurs mainly in three countries- India, Bangladesh and Nepal. The disease occurs in agricultural villages where houses are frequently constructed with mud walls. Even people residing in endemic areas have poor practices for its prevention. Methods A descriptive cross-sectional study was conducted among 150 community people of Katahari Gaupalika, Morang. Simple Random sampling technique was used for the selection of three wards among seven wards, probability proportionate was used for the selection of the population from the ward. The desired sample was selected purposively. Data were collected from face-to-face interview techniques using a structured questionnaire. Statistical analysis was done by using SPSS 16 version. Results It is found that nearly two-thirds of the respondents (63.3%) had moderate awareness, more than one-fifth (22%) of the respondents had adequate awareness while (14.7%) of the respondents had inadequate awareness of Kala-azar. Similarly, more than half (55.3%) had moderate preventive practice, (25.3%) had inadequate preventive practice and nearly one-fifth (19.3%) had adequate preventive practice. The mean score for awareness and practice was 16.633 and 5.54 respectively. Conclusion The study was conducted to assess the level of awareness of Kala-azar. This concluded moderate awareness of respondents on Kala-azar which suggested that there is still a need to improve the awareness of the respondents particularly on its causative agent, preventive measures and complication. However, to improve their awareness to an adequate level, awareness-raising programs focusing on preventive practice should be conducted

    Abstract: Diabetes insipidus is a condition characterised by a large volume of diluted urine production andincreased thirst. In this case report, a 49-year-old gentleman presented with 3 months of polyuria andpolydipsia. He had a repeated history of hypokalemia. On the evaluation of polyuria and polydipsia,he was diagnosed with partial nephrogenic diabetes insipidus based on his inability to concentrateurine after a water deprivation test and his less than 50% response to exogenous desmopressin.On the evaluation of recurrent hypokalemia, the investigation reports met biochemical criteria forthe diagnosis of Gitelman syndrome. He was encouraged to increase his fluid intake as required,and potassium chloride supplementation relieved his symptoms. This case report demonstrates thereversibility of nephrogenic diabetes insipidus with a correction of hyperkalemia.

    Abstract: Cavernoma, or cerebral cavernous angioma, are hamartomatous lesions formed by sinusoidal vascular spaces without cerebral parenchyma in between. Cavernoma is a rare disorder that is diagnosed infrequently and incidentally, so it is called incidentaloma. However, cavernoma can present with seizures, headaches, and other focal neurological deficits, with seizures being the most frequent presentation. Cavernoma is angiographically concealed, and it's diagnosis is challenging. So, the cavernoma is diagnosed based on an MRI. We present the rare case of a patient who presented with a complex focal seizure and migraine‐like headache caused by a small frontal lobe cavernoma.

    Abstract: Obstructive infantile hydrocephalus may arise due to anatomic or functional obstruction of cerebrospinal fluid flow. Obstruction of the aqueduct of sylvius (aqueductal stenosis) causes dilation of the lateral and third ventricles, while the size of the fourth ventricle remains relatively normal. Obstructive infantile hydrocephalus with meningoventriculitis is a rare phenomenon, and literature with only 2 other children with similar findings have been reported. We hereby report a case of a 16-week-old infant who developed Escherichia coli meningoventriculitis, later complicated by the development of hydrocephalus, challenging the management. The diagnosis was based on the magnetic resonance imaging of the brain, which showed hydrocephalus, and the cerebrospinal fluid culture showing Escherichia coli meningoventriculitis. The case was managed with serial ventricular drainage along with antibiotics followed by staged ventriculoperitoneal shunting. Serial measurement of head circumference is essential to prompt diagnostic suspicion in the case of paediatric meningitis.

    Abstract: Background. The incidence of chronic liver disease is increasing in the Nepalese population. Liver transplantation (LT) is the best option for patients with end-stage liver disease (ESLD). Nepal’s first liver transplant was performed in 2016 in an international collaborative effort at Shahid Dharmabhakta National Transplant Centre (SDNTC), Bhaktapur, Nepal. We aim to report details of the first five patients who had undergone liver transplantation in SDNTC before the beginning of the COVID-19 outbreak in the history of transplantation in Nepal. Method. A descriptive analysis of the clinical data of five adult recipients of liver transplantation at SDNTC was done. We described the patient’s demographics, length of stay, and survival of all the first five patients who had undergone four living donor liver transplantations and one brain-dead donor liver transplantation in SDNTC before the beginning of the COVID-19 outbreak. Results. Recipients were between 36 and 63 years old. The recipients of the four live donor liver transplants (LDLT) and one brain-dead donor liver transplant (DDLT) had alcoholic liver disease and cryptogenic liver disease, leading to end-stage liver disease. The model for end-stage liver disease (MELD) scores ranged from 23 to 34. Out of five, four recipients and four donors are doing well and relishing the prospect of a normal life, while the recipient of a brain-dead donor liver transplant passed away due to postoperative primary graft failure. Conclusion. Despite the small number of liver transplants that have been done, the success of these has created confidence in a sustainable liver transplantation program in Nepal.

    Abstract:  With a view to overcome the shortcomings of traditional Electronic Health Record (EHR) system so as to assure the interoperability by providing open access to sensitive health data, while still preserving personal data privacy, anonymity and avoiding data misuse, Decentralized Electronic Health Record System was developed. The aforementioned issue concerning traditional EHR system can be addressed by implication of emerging technology of the era namely Block chain, together with Inter Planetary File System (IPFS) which enables data sharing in decentralized and transactional fashion, thereby maintaining delicate balance between privacy and accessibility of electronic health records. A block chain based EHR system has been built for secure, efficient and interoperable access to medical records by both patients and doctors while preserving privacy of the sensitive patient’s information. Patients can easily and comprehensively access to their medical records across providers and treatment sites using unique properties of block chain and decentralized storage. A separate portal for both the patients and doctors has been built enabling the smart contracts to handle further interaction between doctors and patients. So, in this system, it is demonstrated how principles of decentralization and block chain architectures could contribute to EHR system using Ethereum smart contracts and IPFS to orchestrate a suitable system governing the medical record access while providing patients with comprehensive record review along with consideration for audit ability and data sharing.

    Abstract: Identifying pulmonary pathology while evaluating electrolyte disorders is crucial for optimal patient management. Physicians working in endemic regions of tuberculosis should consider this pathology as a differential for electrolyte imbalances. Abstract Hyponatremia, a common electrolyte imbalance, can arise from various underlying etiologies such as diuretics, diarrhea, vomiting, congestive heart failure, and liver and renal disease. We present a case report of a 74‐year‐old man highlighting the association between pulmonary tuberculosis (TB) and the development of hyponatremia. GeneXpert assay of the patient's sputum sample led to the identification of underlying active pulmonary TB as the cause of hyponatremia. The patient was started on anti‐TB therapy, and concurrent fluid restriction and sodium supplementation were initiated to correct the electrolyte imbalance. Over the next 3 days, the patient demonstrated clinical improvement with the resolution of hyponatremia. This case also highlights the importance of considering TB as a potential etiology in patients presenting with hyponatremia, especially in endemic areas. Further research is warranted to explore the mechanistic pathways linking pulmonary TB and hyponatremia, aiding in the development of targeted therapeutic interventions.

    Abstract: Vaccination against Coronavirus Disease 2019 (COVID-19) is essential for controlling the ongoing cases of this disease. Citrobacter infections of the bones and joints are extremely uncommon. Thromboembolism and deep vein thrombosis are very rare complications. Case Presentation: We present a rare case of osteomyelitis, septic arthritis, deep venous thrombosis and pulmonary embolism in a 15-year-old previously healthy boy occurring shortly after receiving the second dose of the Moderna COVID-19 vaccine. He experienced pain, swelling in the right leg, shortness of breath, and fever, followed by chest pain and leg edema. Treatment included anticoagulation, ketorolac for pain management, antipyretics and intravenous antibiotics (Tazobactam/Piperacillin, Linezolid, Clindamycin) for osteomyelitis. Discussion: The risk of Covid-19 vaccine-related thrombotic events is minimal. Thrombotic events reported among mRNA is very rare. C.freundii bone and joint infections are very rare, accounting for small percentage of cases.Some documented cases include cefotaxime- resistant strains causing necrotizing fascitis and osteomyelitis,including post arthroplasty infections. Due to the diverse range of susceptibility patterns and the widespread occurrence of drug resistance, personalized treatment based on culture and sensitivity testing is recommended. However, in rare cases, severe complications like deep vein thrombosis and joint infections associated with Citrobacter infection may occur and should be reported to the Vaccine Adverse Events Reporting System (VAERS). Conclusion: Administering COVID-19 vaccine to enhance natural antibodies is crucial, despite the low risk of infection, thromboembolism and deep vein thrombosis. Healthcare providers should stay vigilant about adverse effects post-vaccination and promptly report those cases
