    Abstract: Abstract 2067 Background: Pneumococcal invasive infections are over 13 times more common among patients with hematological malignancies than in the general population. Hematopoietic stem cell transplant (HSCT) recipients, particularly those with chronic graft-versus-host disease (cGvHD), are especially vulnerable. Two types of vaccines are available for prevention: pneumococcal polysaccharide vaccine (PPSV) and pneumococcal conjugate vaccine (PCV). PPSV covers more serotypes, but concerns have been raised over immune response, particularly in immunocompromised patients. The primary goals of this study were to evaluate clinical impact of pneumococcal bacteremia, serotypes involved and resistance profile in our hematological population. Methods: We performed a retrospective study based at Hôpital Maisonneuve-Rosemont and Hôpital Notre-Dame du CHUM, both in Montreal, Canada. Among all positive blood cultures for Streptococcus pneumoniae between January 1st, 2003 and December 31st, 2011, we selected patients with hematological malignancies. We reviewed patients' charts and electronic files. Serotypes and antibiotic susceptibility testing were done by the Laboratoire de Santé Publique du Québec. The study was approved by the hospitals' ethics committee. Results: During the eight-year period, 54 episodes of pneumococcal bacteremia (in 52 patients) occurred among patients with hematological malignancies. Median age was 63.5 years (range 21 to 91); 44% were female. One patient was splenectomized. The main hematological malignancies were multiple myeloma (42%) and non-Hodgkin lymphoma (29%). There were 16 HSCT recipients: 10 allogeneic (8 with cGVHD) and 6 autologous. Twenty-nine patients (55%) were on chemotherapy, including 10 HSCT recipients. Among the patients with cGVHD, 3 were on trimethoprim-sulfamethoxazole (TMP-SMX) prophylaxis and 1 on pentamidine. At the time of the bacteremia, 81% of patients had a diagnosis of pneumonia (mean Fine score 118. Eleven patients were treated in the emergency department or in the outpatient clinic. Among the 43 hospitalized patients (80%), median hospitalization duration was 10 days. Intensive care unit admission was required for 9 episodes (17%). Eight patients died less than 30 days after the diagnosis of bacteremia, leading to a mortality rate of 15% and 19% among HSCT recipients. Vaccine history was not available in most patients' charts. However, all HSCT recipients should have received PPSV, as recommended in our institution. Twenty-four different serotypes were found. Theoretical coverage of these serotypes by the available vaccines was 26% for PCV-7, 35% for PCV-10, 50% for PCV-13, and 61% by PPSV; 33% episodes were not covered by any vaccine. Among patients with HSCT, the percentage of episodes not covered by any vaccine increased to 44%. PPSV covered 56% of episodes. Resistance to TMP-SMX was found in 17% of isolates. However, since 2006, all were susceptible. A third of isolates were resistant to erythromycin; the proportion of resistant isolates was higher among HSCT patients (50%). Only one isolate was resistant to fluoroquinolones, but was sensitive to all other antibiotics tested. One strain was of intermediate sensitivity to ceftriaxone, and was resistant to erythromycin and TMP-SMX. This patient was on TMP-SMX prophylaxis, and did not require admission. All strains were sensitive to penicillin and vancomycin. Conclusion: Pneumococcal bacteremia carries significant morbidity and mortality in patients with hematologic malignancies. Our mortality rate is similar to that found in other studies. Vaccination remains the cornerstone of prevention, but is imperfect in terms of immune response and coverage. One third of episodes were not covered by any available vaccine, and this proportion increased among HSCT recipients. Antibacterial prophylaxis in patients with cGVHD is therefore important and penicillin remains a good choice. In terms of treatment options for respiratory infection, given the high erythromycin resistance rate (34% vs. 20–25% found in the general population), macrolides should not be used as single agents. Third generation cephalosporins and fluoroquinolone resistance is very low, making it an interesting choices for respiratory infections. Disclosures: Labbé: Pfizer: Research Funding.

    Abstract: Results to date indicate that high-dose therapy (HDT) with autologous stem cell support improves survival of patients with symptomatic multiple myeloma (MM). We performed a multicenter, sequential, randomized trial designed to assess the optimal timing of HDT and autotransplantation. Among 202 enrolled patients who were up to 56 years old, 185 were randomly assigned to receive HDT and peripheral blood stem cell (PBSC) autotransplantation (early HDT group, n = 91) or a conventional-dose chemotherapy (CCT) regimen (late HDT group, n = 94). In the late HDT group, HDT and transplantation were performed as rescue treament, in case of primary resistance to CCT or at relapse in responders. PBSC were collected before randomization, after mobilization by chemotherapy, and, in the two groups, HDT was preceded by three or four treatments with vincristine, doxorubicin, and methylprednisolone. Data were analyzed on an intent-to-treat basis using a sequential design. Within a median follow-up of 58 months, estimated median overall survival (OS) was 64.6 months in the early HDT group and 64 months in the late group. Survival curves were not different (P = .92, log-rank test). Median event-free survival (EFS) was 39 months in the early HDT group whereas median time between randomization and CCT failure was 13 months in the late group. Average time without symptoms, treatment, and treatment toxicity (TWiSTT) were 27.8 months (95% confidence interval [CI]; range, 23.8 to 31.8) and 22.3 months (range, 16.0 to 28.6) in the two groups, respectively. HDT with PBSC transplantation obtained a median OS exceeding 5 years in young patients with symptomatic MM, whether performed early, as first-line therapy, or late, as rescue treatment. Early HDT may be preferred because it is associated with a shorter period of chemotherapy. © 1998 by The American Society of Hematology.

    Abstract: The expression of microtubule-associated protein 2 (MAP2), developmentally regulated by alternative splicing, coincides with neurite outgrowth. MAP2 proteins contain a microtubule-binding domain (C-terminal) that promotes microtubule assembly and a poorly characterized domain, the projection domain(N-terminal), extending at the surface of microtubules. MAP2b differs from MAP2c by an additional sequence of 1372 amino acids in the projection domain. In this study, we examined the role of the projection domain in the protrusion of microtubules from the cell surface and the subsequent process formation in Sf9 cells. In this system, MAP2b has a lower capacity to induce process formation than MAP2c. To investigate the role of the projection domain in this event, we expressed truncated forms of MAP2b and MAP2c that have partial or complete deletion of their projection domain in Sf9 cells. Our results indicate that process formation is induced by the microtubule-binding domain of these MAP2 proteins and is regulated by their projection domain. Furthermore, the microtubule-binding activity of MAP2b and MAP2c truncated forms as well as the structural properties of the microtubule bundles induced by them do not seem to be the only determinants that control the protrusion of microtubules from the cell surface in Sf9 cells. Rather, our data suggest that microtubule protrusion and process formation are regulated by intramolecular interactions between the projection domain and its microtubule-binding domain in MAP2b.

    Abstract: The objective of this study was to determine if sex influences the pharmacokinetics and hemodynamics of the CYP2D6 substrate metoprolol and its interaction with diphenhydramine (CYP2D6 inhibitor) in healthy young participants with high (extensive metabolizer [EM]) or low (poor metabolizer [PM] ) CYP2D6 activities. A prespecified comparative analysis of data from 2 sequential clinical trials that included 16 EM and 4 PM women and 10 EM and 6 PM men was performed. The participants in the 2 trials were administered a single oral dose of 100 mg metoprolol in the presence of steady‐state diphenhydramine or placebo. Serial plasma and urine samples were obtained for 48 hours, and hemodynamic data was obtained for 12 hours after metoprolol. In the placebo arm, EM and PM women had 62% and 59% higher S‐metoprolol AUC 0–∞ and 26% and 71% lower CL/F, respectively, compared to men with the same phenotype (all Ps 〈 .05 women compared to men). These differences dissipated on body weight (WT) correction. Women (especially PMs) experienced greater negative chronotropic effects of metoprolol than men (P 〈 .0001 women compared to men). Diphenhydramine coadministration increased S‐metoprolol AUC by 84% in EM women and 45% in EM men (P 〈 .009 women compared to men). In the diphenhydramine arm, sex differences in pharmacokinetics persisted even after WT correction, resulting in greater negative chronotropic effects in women (all Ps 〈 .05 women compared to men). Metoprolol dose should be adjusted for body weight, particularly in women. Coadministration of a CYP2D6 inhibitor such as diphenhydramine, by a patient at similar doses in the 2 sexes, could result in a greater inhibition of clearance of CYP2D6 substrates with a resulting higher risk of pronounced pharmacological and adverse effects in women compared to men.

    Abstract: In vitro, bilirubin is a strong antioxidant, but in vivo its capacity to act as a scavenger of toxic oxygen radicals remains poorly documented. The aim of this study was to evaluate if bilirubin had antioxidant properties in jaundiced infants. The antioxidant capacity of neonatal plasma was measured in Trolox equivalents (TEAC, mmol/l) and correlated in vitro with plasma bilirubin concentrations (r 〈 sup 〉 2 〈 /sup 〉 = 0.99). Plasma TEAC was compared before and after exchange transfusions for neonatal hyperbilirubinemia (250–435 μmol/l). The antioxidant properties of the paired blood samples before and after exchange transfusions (TEAC: 1.67 ± 0.12 vs. 1.37 ± 0.09 mmol/l, n = 11) varied in proportion to the serum bilirubin levels. The changes in other antioxidants were not large enough to account for the magnitude of change in antioxidant capacity. Therefore, in vivo, the plasma antioxidant capacity of jaundiced newborn infants is related to the level of bilirubin.

    Abstract: Résumé: Notre objectif dans cet article est d'expliquer quels sont les défis que rencontrent deux communautés francophones en Ontario, l'une de la Péninsule de Niagara, l'autre de la région Simcoe, face aux changements engendrés par ce qu'il est convenu d'appeler la mondialisation, et comment les membres de ces deux communautés utilisent leurs ressources linguistiques pour s'intégrer socialement et professionnellement dans une économie en voie de transformation. Nous avons effectué quelques 40 entrevues semi-dirigées par région auprès d'un éventail de population pouvant démontrer des pratiques sociales et langagières variées en matière de bilinguisme et d'alphabétisation, de formation et d'accès à l'emploi. Cette comparaison de deux régions nous montre que la mondialisation de l'économie favorise le développement des services à la clientèle, ce qui suppose une importance accrue pour la capacité de communiquer, principalement à l'oral, bien que l'écrit soit en progression. Si les employeurs de ces deux régions peuvent compter sur une population bilingue pour combler leurs besoins actuels, selon eux la réserve de main-d'æuvre disponible se trouverait en voie de tarissement. Chez les francophones, la réserve de main-d'æuvre qui se trouve sans emploi, fait face à divers défis, tels que se procurer l'équivalent d'un diplôme d'études secondaires, faire preuve de mobilité, soit en se relocalisant pour un emploi, soit en effectuant un trajet quotidien sur de grandes distances, ce qui suppose renoncer jusqu'à un certain point au réseau de solidarité communautaire, et ce qui peut avoir une influence sur la reproduction sociale de la communauté.

    Abstract: A novel approach is proposed to evaluate the impact of forestry on ecosystem quality in life cycle assessment (LCA) combining a naturalness assessment model with a species richness relationship. The approach is applied to a case study evaluating different forest management strategies involving concomitantly silvicultural scenarios (plantation only, careful logging only or the current mix of both) combined with an increasing share of protected area for wood production in a Québec black spruce forest. The naturalness index is useful to compare forest management scenarios and can help evaluate conservation needs considering the type of management foreseen for wood production. The results indicate that it is preferable to intensify forest management over a small proportion of the forest territory while ensuring strict protection over the remaining portion, compared to extensive forest management over most of the forested area. To explore naturalness introduction in LCA, a provisory curve relating the naturalness index (NI) with the potential disappeared fraction of species (PDF) was developed using species richness data from the literature. LCA impact scores in PDF for producing 1 m3 of wood might lead to consistent results with the naturalness index but the uncertainty is high while the window leading to consistent results is narrow.
