Abstract: Background: Shiveringis a common complication following anesthesia. Many drugs have been documented as a prophylaxis of postanesthesia shivering. The aim of this study was to find out a better drug as an antishivering agent. Materials and methods: This comparative study was carried out in the Department of Anesthesiology in Chittagong Medical College Hospital over a period of 6 months starting from November 2015 to April 2016. A total 90 obstetric patients with American Society of Anesthesiology (ASA) physical status I & II underwent cesarean section received placebo (Group-C, n-30), Inj-Ondansetron 8mg (Group-O, n-30) and Inj-Meperidine 0.4mg/kg (Group-M, n-30). These drugs were administered during intraoperative period just after cord clamp. Incidence of shivering, hemodynamic parameters were observed immediate after spinal anesthesia & then 5 min interval for 30 min throughout the intraoperative period. Results: Shivering was observed in 16 patients (53.33%) of group-C, 4 patients (13.33%) of group O and 8 patients (26.66%) of group M. The number of patient with a shivering grade 3 & 4 were very highly significant (p 〈 0.001) in group C compared with other groups at 15 min & 20 min after block & significant (p 〈 0.05) at 25 min & 30 min after block. Conclusion: Ondansetron was found to be more effective and haemodynamically stable drug than Meperidine in prophylaxis of post spinal shivering. JCMCTA 2021 ; 32 (1) : 53-58

Abstract: Background: Propofol gains popularity due to its smooth induction and rapid recovery. But one of its side effects is pain during injection. So, the aim of this study was to compare the efficacy of tramadol and lidocaine pretreatment for alleviation of pain on propofol injection in our setting. Materials and methods: 80 patients of either sex (ASA grade I and II, age 20-50 years) who underwent elective Surgery under general anaesthesia were randomized into two equal groups (n=40). Group L (Control group) received pretreatment injection 2% lidocaine 40mg (2ml) Group T received pretreatment injection tramadol 50 mg (2ml). After that, 1/4th of calculated induction dose of propofol (2mg/kg) was administered. The level of pain was evaluated by four point verbal response scale. Results: The overall incidence of pain on propofol injection was lower in lidocaine (20%) than in tramadol group (22.5%). The incidence of score ‘0’ (No pain) was higher in lidocaine (80%) than in tramadol group (77.5%) but it is not statistically significant (p 〉 0.05). Conclusion: pretreatment with 2% lignocaine and tramadol effectively reduced the incidence and severity of propofol injection pain.   JCMCTA 2021 ; 32 (2) : 90-94

Abstract: Background: Obstructed labour is one of the most common preventable cause of maternal and prenatal morbidity and mortality in developing countries. Objective: The purpose of the present study was to determine the risk factors as well as to asses the outcome of obstructed labour. Method: This cross sectional study was conducted in the Department of Gynecology & Obstetrics at Shaheed Ziaur Rahman Medical College Hospital, Bogra during the period from January 2007 to December 2007. One hundred and five cases with features of obstructed labour were selected as per inclusion and exclusion criteria in a consecutive method. A detailed history included sociodemographic feature, obstetric history, features of obstruction, intrapartum events were recorded to detect risk factors. Condition of patients, mode of delivery, preoperative and post operative complications, maternal and fetal outcomes were recorded. Results: A total number of 3171 deliveries were conducted during this period and 132 cases of obstructed labour were found constituting an incidence of 4.2%. The highest frequency was found among the unbooked, primigravid patients that were illiterate or only having primary education level. The commonest cause was cephalo-pelvic disproportion (47.5%) followed by fetal malpostion (25.7%) and malpresentation (24.8%). The majority of the patients were between 25-29 years, caeserean section was the most common mode of delivery (78.09%). Maternal morbidity due to different complication accounted for 76.19% of the case while the fetal morbidity was 51.31% of the cases. The maternal mortality was 1% and prenatal mortality was 24.76%. Conclusion: In this study the incidence of obstructed labour was very high. The commonest cause was cephalo-pelvic disproportion followed by fetal malpostion and malpresentation. Abstract: This cross-sectional, analytical type of study was performed in Department of Anatomy, Dhaka Medical College, Bangladesh, from July 2016 to June 2017 on 52 right hander male and 52 right hander female, 40 left hander male and 34 left hander female Bangladeshi medical students. Convenient purposive sampling technique was adopted. History of any injury of upper limb was excluded to construct standard measurement. Handedness of each medical student was determined by the Edinburgh handedness inventory. Hand breadth and hand length was measured with the help of vernier caliper. Paired and unpaired student‟s „t‟ test and Pearson‟s correlation coefficient test were done for statistical analyses. Handedness score showed nonsignificant positive correlation with right hand shape index (r = +0.087, P 〉 0.05) and left-hand shape index (r = +0.008, P 〉 0.05) in right hander males. Handedness score showed non-significant negative correlation with right hand shape index (r = -0.017, P 〉 0.05) and left-hand shape index (r = -0.003, P 〉 0.05) in right hander females. Handedness score showed non-significant negative correlation with right hand shape index (r= - 0.210, P 〉 0.05) and left-hand shape index (r = -0.247, P 〉 0.05) in left hander males. Handedness score showed non-significant positive correlation with right hand shape index (r= +0.051, P 〉 0.05) and left-hand shape index (r = +0.213, P 〉 0.05) in left hander females. To summarize, handedness score showed nonsignificant positive correlation with right hand shape index and left-hand shape index in right hander males and left hander females. However, handedness score showed non-significant negative correlation with right hand shape index and left-hand shape index in right hander females and left hander males. Abstract: A cross-sectional, descriptive study was done to determine the breath of the thyroid lamina and the differences in its values in respect of age and sex in Bangladeshi cadavers. A total 60 postmortem human larynges were collected between October 2008 and March 2009. Among those, 45 (male 23 and female 22) were collected from unknown dead bodies (aged between 9 and 60 years) under autopsy in the mortuary of Department of Forensic Medicine of Mymensingh Medical College, while the other 15 (male 6 and female 9) were collected from stillborn babies of viable age (28 to 40 weeks of gestation) in the Department of Obstetrics & Gynaecology of Mymensingh Medical College Hospital, Mymensingh, Bangladesh. The breath of thyroid lamina was measured from laryngeal prominence to the posterior margins of lamina at its transverse plane using slide calipers and differences were observed in between age groups and sexes. The mean±SD breadth of thyroid lamina was found 12.40±1.30 mm in group A (from 28 to 40 weeks of gestation) ranging between 9 and 14 mm, while 26.19±3.69 mm in group B (from 9 to 16 years) ranging between 21 and 35 mm, and 30.76±4.17 mm in group C (from 17 to 60 years) with a range of 24 to 38 mm. The mean breadth of thyroid lamina was highest in age group C (30.76 mm) and was lowest in age group A (12.40 mm). The mean difference of breadth of thyroid lamina between group A & B, A & C and B & C were statistically significant (P=0.000). The breadth of thyroid lamina was found higher in male (32.70±3.96 mm) than that of female (28.00±2.69 mm) in Group C and the difference was statistically significant (p=0.001). Our study revealed that the breadth of thyroid lamina is greater in adult male than female and overall, the value increases with age. CBMJ 2022 July: vol. 11 no. 02 P: 131-135
