    Abstract: Background: So far, planning of clinical trials in multiple myeloma (MM) has been based on traditional outcome measures such as progression free (PFS) and overall survival (OS). However, treatment with drugs, which prolong survival, usually impacts quality of life (QoL). A good example is lenalidomide (LEN) maintenance therapy in transplant eligible newly diagnosed MM. While LEN is the only approved drug for maintenance therapy, clearly and consistently prolonging PFS in a number of trials, treatment comes with an increased risk for infections, secondary malignancies, fatigue and diarrhea. One consequence could be to define QoL as an additional study endpoint but the MM patient's perspective on this topic has not been systematically investigated yet. Methods: To address this, we performed an online survey with MM patients with previous lenalidomide (LEN) maintenance. The survey contained a total of 205 questions, including the validated QoL C30- and My20-questionnary and questions pertaining the patient's preference for either prolonged survival or increased QoL. To assess differences between patient subgroups we used statistical analyses such as a random decision forest. Results: 210 patients with MM completed the online survey within 48 days. 92 (47%) of patients chose a shortened time of PFS in favor of an increased QoL (QoL-group). An almost equal number of patients 81 (42 %) chose the exact opposite option (PFS-group). 11% of patients were unable to make this kind of decision. Thus, a sole measurement of the endpoint PFS would disregard the preference of ~1/2 of the patients. We used an unbiased artificial intelligence supported statistical approach to explore additional features of the QoL- and the PFS-group. Interestingly, the top variable associated with the QoL-group was pain related to bone disease at the time of diagnosis (P = 0.01). There was a trend for more advanced treatment lines in patients in the QoL-group compared to the PFS-group. Patients who preferred PFS were found to be more likely to hand over responsibility to their physicians. Conclusion: We demonstrate that MM patients can be divided into two nearly equally sized groups, where one group favors PFS and the other one prefers QoL, with bone pain at diagnosis and the number of previous treatment lines impacting the decision. Since our results suggest that survival as the only endpoint for maintenance therapy studies in MM should be reconsidered, we propose PFS and QoL measures as co-primary endpoints to account for the heterogeneity in patients´ preferences and to collect the information necessary for shared decision-making in future patients.     Abstract: Physical and mental well-being during the COVID-19 pandemic is typically assessed via surveys, which might make it difficult to conduct longitudinal studies and might lead to data suffering from recall bias. Ecological momentary assessment (EMA) driven smartphone apps can help alleviate such issues, allowing for in situ recordings. Implementing such an app is not trivial, necessitates strict regulatory and legal requirements, and requires short development cycles to appropriately react to abrupt changes in the pandemic. Based on an existing app framework, we developed Corona Health, an app that serves as a platform for deploying questionnaire-based studies in combination with recordings of mobile sensors. In this paper, we present the technical details of Corona Health and provide first insights into the collected data. Through collaborative efforts from experts from public health, medicine, psychology, and computer science, we released Corona Health publicly on Google Play and the Apple App Store (in July 2020) in eight languages and attracted 7290 installations so far. Currently, five studies related to physical and mental well-being are deployed and 17,241 questionnaires have been filled out. Corona Health proves to be a viable tool for conducting research related to the COVID-19 pandemic and can serve as a blueprint for future EMA-based studies. The data we collected will substantially improve our knowledge on mental and physical health states, traits and trajectories as well as its risk and protective factors over the course of the COVID-19 pandemic and its diverse prevention measures.

    Abstract: We report on the synthesis and characterization of a series of (mostly) air‐stable diorganyl bis(pyridylimino) isoindolide (BPI) aluminum complexes and their chemistry upon visible‐light excitation. The redox non‐innocent BPI pincer ligand allows for efficient charge transfer homolytic processes of the title compounds. This makes them a universal platform for the generation of carbon‐centered radicals. The photo‐induced homolytic cleavage of the Al−C bonds was investigated by means of stationary and transient UV/Vis spectroscopy, spin trapping experiments, as well as EPR and NMR spectroscopy. The experimental findings were supported by quantum chemical calculations. Reactivity studies enabled the utilization of the aluminum complexes as reactants in tin‐free Giese‐type reactions and carbonyl alkylations under ambient conditions, which both indicated radical‐polar crossover behavior. A deeper understanding of the physical fundamentals and photochemical process was provided, furnishing in turn a new strategy to control the reactivity of bench‐stable aluminum organometallics.

    Abstract: Wir berichten über die Synthese und Charakterisierung einer Reihe von weitgehend luftstabilen Diorganyl‐bis(pyridylimino)isoindolid(BPI)−Aluminiumkomplexen und deren Reaktivität durch elektronische Anregung mittels sichtbarem Licht. Der redox‐aktive, “nicht‐unschuldige” BPI‐Pinzettenligand ermöglicht eine effiziente Charge‐Transfer‐Homolyse der Al−C‐Bindung. Dadurch dienen die vorgestellten Verbindungen als Ausgangsmaterialien für die Generierung von kohlenstoffzentrierten Radikalen. Die photo‐induzierte homolytische Spaltung der Al−C‐Bindung wurde mittels stationärer und transienter UV/Vis‐Spektroskopie, Abfangexperimenten der entsprechenden Radikale, sowie EPR‐ und NMR‐Spektroskopie analysiert und zudem mithilfe von quantenchemischen Rechnungen untersucht. Studien zur Reaktivität der entsprechenden Aluminiumkomplexe ermöglichten deren Nutzung als Reagenzien in Giese‐artigen Reaktionen und der Alkylierung von organischen Carbonylverbindungen, wobei Hinweise auf einen Wechsel von radikalischen zu polaren Mechanismen (radical‐polar‐crossover) gefunden wurden. Durch diese Arbeit wurde ein vertieftes Verständnis der physikalischen Hintergründe und der ablaufenden photochemischen Prozesse generiert, wodurch eine neue Strategie der Reaktionskontrolle von luftstabilen Organometallverbindungen des Aluminiums etabliert wurde.

    Abstract: MOST modern criticism of King Lear directs itself resolutely against the notion, said to be held by some critics, that the play forms part of the Christian tradition, that it ultimately affirms the victory of good over evil, specifically, the victory of love over hate, and that it therefore makes virtue prevail in the end. 1 On the contrary, Dr. Johnson felt that “Shakespeare has suffered the virtue of Cordelia to perish in a just cause, contrary to the natural ideas of justice” (“Notes” to King Lear) , and the majority of modern critics holds, as Swinburne did, 2 that the play is deeply pessimistic, that in writing King Lear Shakespeare deliberately examined such Christian concepts as divine providence, retributive justice, or the existence of some universal moral order, and came to the conclusion that at best man's world was a “great stage of fools” ( iv . vi .182), whose false sense of security should make the philosopher chuckle; or that at worst it was a “wheel of fire” ( iv . vii .47) upon which not only Lear but every man is bound, an image that should make the poet cry in agony. 3

    Abstract: Tinnitus is an auditory phantom perception in the absence of an external sound stimulation. People with tinnitus often report severe constraints in their daily life. Interestingly, indications exist on gender differences between women and men both in the symptom profile as well as in the response to specific tinnitus treatments. In this paper, data of the TrackYourTinnitus platform (TYT) were analyzed to investigate whether the gender of users can be predicted. In general, the TYT mobile Health crowdsensing platform was developed to demystify the daily and momentary variations of tinnitus symptoms over time. The goal of the presented investigation is a better understanding of gender-related differences in the symptom profiles of users from TYT. Based on two questionnaires of TYT, four machine learning based classifiers were trained and analyzed. With respect to the provided daily answers, the gender of TYT users can be predicted with an accuracy of 81.7%. In this context, worries, difficulties in concentration, and irritability towards the family are the three most important characteristics for predicting the gender. Note that in contrast to existing studies on TYT, daily answers to the worst symptom question were firstly investigated in more detail. It was found that results of this question significantly contribute to the prediction of the gender of TYT users. Overall, our findings indicate gender-related differences in tinnitus and tinnitus-related symptoms. Based on evidence that gender impacts the development of tinnitus, the gathered insights can be considered relevant and justify further investigations in this direction.

    Abstract: Machine learning (ML) models are evaluated in a test set to estimate model performance after deployment. The design of the test set is therefore of importance because if the data distribution after deployment differs too much, the model performance decreases. At the same time, the data often contains undetected groups. For example, multiple assessments from one user may constitute a group, which is usually the case in mHealth scenarios. Methods In this work, we evaluate a model’s performance using several cross-validation train-test-split approaches, in some cases deliberately ignoring the groups. By sorting the groups (in our case: Users) by time, we additionally simulate a concept drift scenario for better external validity. For this evaluation, we use 7 longitudinal mHealth datasets, all containing Ecological Momentary Assessments (EMA). Further, we compared the model performance with baseline heuristics, questioning the essential utility of a complex ML model. Results Hidden groups in the dataset leads to overestimation of ML performance after deployment. For prediction, a user’s last completed questionnaire is a reasonable heuristic for the next response, and potentially outperforms a complex ML model. Because we included 7 studies, low variance appears to be a more fundamental phenomenon of mHealth datasets. Conclusions The way mHealth-based data are generated by EMA leads to questions of user and assessment level and appropriate validation of ML models. Our analysis shows that further research needs to follow to obtain robust ML models. In addition, simple heuristics can be considered as an alternative for ML. Domain experts should be consulted to find potentially hidden groups in the data.
