    Abstract: Purpose . To present the feasibility, safety, and efficacy of carotid endarterectomy in patients with type II internal carotid artery occlusions, including the long-term outcomes. Methods . From March 2008 to August 2015, 74 consecutive patients (48 men with a mean age of 65.1 ± 8.06 years) underwent carotid endarterectomy because of internal carotid artery (ICA) segmental occlusions. These were verified with preoperative carotid duplex scans (CDS) and CT angiography (CTA). Also, brain CT scanning was performed in all these patients. The indication for treatment was made jointly by a vascular surgeon, neurologist, and an interventional radiologist in a multidisciplinary team (MDT) context. After successful treatment, all the patients were followed-up at 1, 3, 6, and 12 months, then every 6 months thereafter. Results . The most common symptom at presentation was transient ischaemic attack (TIA) in 49 patients (66.2%), followed by stroke in the past six months in the 17 remaining patients (23%). Revascularisation of the ICA with endarterectomy techniques was performed successfully in all the patients with an average clamp time of 11.9 min. All the procedures were performed under general anaesthesia in combination with a superficial cervical block. The early complication rate was 8.1% and included two cardiac events (2.7%) (one rhythm disorder and one acute coronary syndrome), three TIAs (4.1%), and one intracerebral hemorrhage (1.3%). Only one patient with the intracerebral hemorrhage died 5 days after surgery giving a postoperative mortality of 1.3% for this series. During the follow-up period (mean 50.4 ± 31.3 months), the primary patency rates at 1, 3, 5, and 7 years were 98.4%, 94.9%, 92.9%, and 82.9%, respectively. Likewise, the survival rates were 98.7%, 96.8%, 89%, and 77.6%, respectively. Ultrasound Doppler controls during follow-up detected 8 ICA restenoses; however, only 3 of these patients required further endovascular treatment. Conclusions . Carotid endarterectomy of internal carotid artery (ICA) segmental occlusion is a safe and effective procedure associated with acceptable risk and good long-term results. Therefore, the current guidelines which do not recommend carotid endarterectomy in this patient group should be reassessed, with the requirement for ongoing large-scale randomized controlled trials to compare CEA with best medical therapy in this patient cohort.

    Abstract: Background/Aim. Prostate carcinoma (PCa) is second occurring carcinoma that affects the male population. Although PCa incidence rates are high, most cases have a favorable prognosis, with comfortable long-term life quality. The aim of the study was to compare long non-coding RNA (lncRNA) growth arrest-specific transcript 5 (GAS5) plasma levels between healthy individuals and patients with PCa, and also between PCa patients with different prognostic scores. Methods. The present study included a total of 40 patients with PCa and a control group of 20 healthy individuals. PCa patients were divided into two subgroups (20 patients each) based on the prognostic criteria of the American Joint Committee on Cancer. The patient data were collected and analyzed; lncRNA GAS5 levels were quantified using the re-al-time polymerase chain reaction method. Statistical analysis was conducted using the IBM SPSS Statistics 26.0 computer program (IBM, USA, 2019). Results. The relative quantification of lncRNA GAS5 expression levels showed down-regulation in PCa patients compared to healthy individuals; however, the difference was marginally statistically significant (p = 0.056). With further analysis of the given results, we concluded that the expression level of lncRNA GAS5 was not significantly different in the first patient subgroup and the healthy individuals (p = 0.268). Patients from the second subgroup had significantly lower plasma levels of lncRNA GAS5 than healthy individuals (p = 0.033). The difference in the level of lncRNA GAS5 expression between patients with favorable prognoses (Group 1) and the ones with worse prognostic scores (Group 2) did not indicate statistical significance (p = 0.275). In both Group 1 (p = 0.805) and Group 2 (p = 0.454), the plasma levels of lncRNA GAS5 were not significantly different in comparison to the age (? 65 vs. 〉 65 years). Conclusion. One of the main objectives of PCa research is identifying novel and more efficient biomarkers. Conducted research provides strong evidence about the significance of lncRNAs GAS5 in PCa, as well as the correlation between decreased expression of lncRNA GAS5 and poor prognosis in various tumors.

    Abstract: Acute bacterial cystitis is the most common form of community-acquired urinary tract infections in women. Uri-nary tract infections are the second most common reason for antibiotic prescription in the USA. There is a need for alternative treatments that would help to avoid the use of systematic antimicrobials thus reducing the incidence of microbial resistance and side effects of the drugs. The aim of this study is to evaluate the role of antibiotic ointment treatment in recurrent UTIs in women. We followed up 34 non-pregnant women with a previous history of recurrent UTIs who were instructed to apply antibiotic ointment Bacitracin/Neomycin, locally, on the external urethral meathus area, once a day, for 30 days. 88% of patients responded to the treatment, with no further recurrences in 67.6% of cases and there was a reduced ( 〈 3 cases per year) recurrence in 20.5% of patients. Bacitracin/Neomycin ointment could be a safe, cheap and successful tool for recurrent UTIs in women. To our best knowledge, this is the first published pilot-study of topical antibiotic ointment usage in management and prevention of recurrent urinary tract infections. We need a double blinded, randomized study with a larger number of patients in order to collect more evidence to support this conclusion.

    Abstract: The long non-coding RNA (lncRNA) GAS5 can be a marker for early diagnosis and postoperative follow-up in the patients with prostate cancer, whereby lower levels of GAS5 correlate with tumorigenesis and unfavourable clinical course. Expression of the GAS5 can be affected by rs145204276 polymorphism, a 5 base pairs insertion-deletion polymorphism shown as ?AGGCA/-?. The aim of this study was to analyse the association between rs145204276 and prostate cancer susceptibility and prognosis. This study was included 121 healthy subjects and 70 patients with prostate cancer. Diagnosis of prostate cancer was established by histopathology after the surgery. Genotyping was performed by allelic discrimination method using the TaqMan? assay. In the healthy subjects, the obtained frequencies of GAS5 rs145204276 genotypes were 80.2% of ins/ins, 16.5% of ins/del and 3.3% of del/del. The allele frequencies were 88.5% of ins and 11.5% of del, respectively. In the patient group, the frequencies of ins/ins, ins/del and del/del genotypes were 70%, 20% and 10%, respectively; the frequency of ins allele was 80% and the frequency of del allele was 20%. Observed frequencies of GAS5 rs145204276 genotypes were not significantly different between healthy subjects and patients with prostate cancer, and also between prognostic groups of prostate cancer. This study demonstrate no significant association between GAS5 rs145204276 and sussceptibility/prognosis of prostate cancer.

    Abstract: Background/Aim. Renal transplantation is the best and preferred way of treating patients with end-stage renal disease, as it offers improved survival and better quality of life compared to dialysis. The aim of this study was to present single-center (Military Medical Academy in Belgrade, Serbia) results of the kidney allograft and patient survival from 1996 to 2017. Methods. A retrospective 22-year co-hort study was conducted. Variables of interest were graft and patient survival in kidney transplanted patients. Age, gender, serum creatinine levels, and induction therapy after transplantation were analyzed in this group of patients as well. Results. Among 386 transplanted patients, 316 had a living donor and 70 patients had a deceased donor. Pre-emptive renal transplantation was done in 29 (7.5%) patients and AB0-incompatible kidney transplantation in 21 (5.4%) patients. One-year, 5-year, 10-year, and 20-year overall patient survival after kidney transplantation in the observed group was 97.7%, 95.3%, 93.8%, and 91.7%, respectively. One-year, 5-year, 10-year, and 20-year graft survival in our patients were 93.8%, 85.5%, 78.5%, and 73.3%, respectively. Conclusion. The outcome of graft and patient survival in the Belgrade Military Medical Academy kidney transplantation program is good and in line with the leading world medical centers.

    Abstract: Introduction. Although intervention in patients with symptomatic carotid disease is generally accepted as beneficial, the management of asymptomatic disease is still controversial. We wanted to introduce and discuss treatment options in a patient with asymptomatic carotid stenosis and high embolic potential lesions of common and internal carotid artery detected by multidetector computed tomography (MDCT). Case Outline. A 78-year-old female patient was admitted to our institution for diagnostics and surgical treatment of asymptomatic high-grade carotid stenosis. Upon admission, color duplex ultrasonography of the carotid arteries revealed the left common carotid artery (CCA) stenosis of 50% and the ipsilateral internal carotid artery (ICA) stenosis of 60%, while the right CCA was narrowed by 60% and the ipsilateral ICA by 80%. Because of the left subclavian artery (LSA) occlusion, also described by ultrasonography, MDCT angiography was performed to assess arterial morphology for possible angioplasty. In addition to LSA occlusion, MDCT angiography surprisingly revealed significant left CCA (〉80%) and ICA (〉70%) narrowing by ulcerated plaques with high embolic potential. Surgical treatment of the left CCA and ICA was indicated and Dacron? tubular graft interposition was performed. The postoperative course was uneventful and the patient was discharged from the Institute on the third postoperative day. After the six-month follow-up the patient was doing well with well-preserved graft patency. Conclusion. Although color duplex ultrasonography is reliable and safe imaging modality in carotid stenosis diagnosis, MDCT angiography plays a significant role in patients with asymptomatic carotid stenosis since plaques with high embolic potential could be detected, which, if left untreated, could have severe neurological ischemic consequences.

    Abstract: Uvod/Cilj. U zemljama koje nemaju nacionalnu organizaciju za prikupljanje i distribuciju organa od umrlih osoba, nedostatak organa jos uvek je neresen problem. Program transplantacije bubrega od zivog nesrodnog davaoca - supruznika zapocet je da bismo povecali broj transplantacija. Cilj ispitivanja bio je da se uporedi prezivljavanje bolesnika, grafta i funkcija grafta kod primalaca kojima su davaoci nesrodnici - supruznici sa onima kojima su davaoci bili zivi srodnici. Metode. Retrospe ktivnom analizom identifikovali smo 14 bolesnika koji su dobili bubreg od supruznika u periodu od 1996. do 2009. godine (grupa I). U kontrolnoj grupi bilo je 14 bolesnika koji su bubreg dobili od zivog srodnog davaoca. Odabrani su iz postojece baze uparivanjem sa bolesnicima grupe I prema polu, zivotnoj dobi, osnovnom bubreznom oboljenju, imunoloskom i virusoloskom pretransplantacionom statusu, prvom modalitetu lecenja terminalne bubrezne slabosti i ABO kompatibilnosti. U periodu pracenja, 41 ? 38 meseci, registrovani su imunosupresivna terapija, hirurske komplikacije, epizode akutnog odbacivanja, infekcija citomegalovirusom i funkcija grafta preko nivoa serumskog kreatinina na pocetku i kraju pracenja. Svi bolesnici su pretransplantaciono imali negativanu unakrsnu reakciju. Pretransplantacioni titar ABO izoaglutinina kod krvno-grupno nepodudarnih bolesnika iznosio je nula. Rezultati. Bolesnici sa supruznickim graftom imali su znacajno losiju HLA podudarnost (p 〈 0,01). Nisu uocene razlike medju grupama u pracenim parametrima. Dva bolesnika iz prve grupe vratila su se na hemodijalizu nakon 82, odnosno 22 meseca zbog teskih komorbiditeta. Zakljucak. Uprkos losijoj HLA podudarnosti, supruznicki graftovi imali su podjednako dobro prezivljavanje i funkciju kao i bubrezi dobijeni od srodnog davaoca.
