    Abstract: Age ≥ 65 years is regarded as a relative contraindication for bariatric surgery. Advanced age is also a recognised risk factor for adverse outcomes with Coronavirus Disease-2019 (COVID-19) which continues to wreak havoc on global populations. This study aimed to assess the safety of bariatric surgery (BS) in this particular age group during the COVID-19 pandemic in comparison with the younger cohort. Methods We conducted a prospective international study of patients who underwent BS between 1/05/2020 and 31/10/2020. Patients were divided into two groups — patients ≥ 65-years-old (Group I) and patients 〈 65-years-old (Group II). The two groups were compared for 30-day morbidity and mortality. Results There were 149 patients in Group 1 and 6923 patients in Group II. The mean age, preoperative weight, and BMI were 67.6 ± 2.5 years, 119.5 ± 24.5 kg, and 43 ± 7 in Group I and 39.8 ± 11.3 years, 117.7±20.4 kg, and 43.7 ± 7 in Group II, respectively. Approximately, 95% of patients in Group 1 had at least one co-morbidity compared to 68% of patients in Group 2 ( p = 〈 0.001). The 30-day morbidity was significantly higher in Group I (11.4%) compared to Group II (6.6%) ( p = 0.022). However, the 30-day mortality and COVID-19 infection rates were not significantly different between the two groups. Conclusions Bariatric surgery during the COVID-19 pandemic is associated with a higher complication rate in those ≥ 65 years of age compared to those 〈 65 years old. However, the mortality and postoperative COVID-19 infection rates are not significantly different between the two groups. Graphical abstract

    Abstract: Amaç: Tıp fakültesi öğrencilerinin COVID-19 pandemisi sırasında algıladıkları stres ve cep telefonlarından mahrum kalma korkusunun (nomofobi) araştırılması amaçlanmıştır. Yöntem: Algılanan Stres Ölçeği (ASÖ), Nomofobi Ölçeği (NÖ) ve sosyodemografik verilerden oluşan çevrimiçi anket 341 öğrencinin katılımıyla tamamlandı. Bulgular: COVID-19 pandemisi sırasında akıllı telefon kullanımının her iki cinsiyette istatistiksel olarak benzer şekilde arttığı bildirildi. NÖ’ye göre, kadınların %67'si hafif, %32,1'i orta, %0,9'u aşırı derecede nomofobikken; erkeklerin %55'i hafif, %39,3'ü orta ve %5,7'si aşırı derecede nomofobikti. Erkeklerin ortalama ASÖ puanlarının kadınlardan anlamlı derecede yüksek olduğu bulundu. 20 yaş ve altındaki öğrencilerin ASÖ ve NÖ puan ortalamalarının 20 yaşın üzerindeki öğrencilerin puanlarına benzer olduğu görüldü. ASÖ puanları ile NÖ’nün tüm boyutları arasında pozitif bir korelasyon vardır. Sonuç: Tıp fakültesi öğrencileri hafif/orta düzeyde nomofobik bulundu. COVID-19 pandemisi sırasında erkek tıp öğrencilerinin kız öğrencilere göre daha fazla etkilendikleri, kendilerini stres altında hissettikleri ve nomofobi eğilimlerinin daha yüksek olduğu saptandı.

    Abstract: There is a paucity of data comparing 30-day morbidity and mortality of sleeve gastrectomy (SG), Roux-en-Y gastric bypass (RYGB), and one anastomosis gastric bypass (OAGB). This study aimed to compare the 30-day safety of SG, RYGB, and OAGB in propensity score-matched cohorts. Materials and methods This analysis utilised data collected from the GENEVA study which was a multicentre observational cohort study of bariatric and metabolic surgery (BMS) in 185 centres across 42 countries between 01/05/2022 and 31/10/2020 during the Coronavirus Disease-2019 (COVID-19) pandemic. 30-day complications were categorised according to the Clavien–Dindo classification. Patients receiving SG, RYGB, or OAGB were propensity-matched according to baseline characteristics and 30-day complications were compared between groups. Results In total, 6770 patients (SG 3983; OAGB 702; RYGB 2085) were included in this analysis. Prior to matching, RYGB was associated with highest 30-day complication rate (SG 5.8%; OAGB 7.5%; RYGB 8.0% ( p  = 0.006)). On multivariate regression modelling, Insulin-dependent type 2 diabetes mellitus and hypercholesterolaemia were associated with increased 30-day complications. Being a non-smoker was associated with reduced complication rates. When compared to SG as a reference category, RYGB, but not OAGB, was associated with an increased rate of 30-day complications. A total of 702 pairs of SG and OAGB were propensity score-matched. The complication rate in the SG group was 7.3% ( n  = 51) as compared to 7.5% ( n  = 53) in the OAGB group ( p  = 0.68). Similarly, 2085 pairs of SG and RYGB were propensity score-matched. The complication rate in the SG group was 6.1% ( n  = 127) as compared to 7.9% ( n  = 166) in the RYGB group ( p  = 0.09). And, 702 pairs of OAGB and RYGB were matched. The complication rate in both groups was the same at 7.5 % ( n  = 53; p  = 0.07). Conclusions This global study found no significant difference in the 30-day morbidity and mortality of SG, RYGB, and OAGB in propensity score-matched cohorts.

    Abstract: Background: Hypotension is common after anesthesia induction and may have adverse outcomes. The aim of this study was to investigate whether arterial elastance (Ea) is a predictor of post-induction hypotension. Methods: Between January and June 2022, the hemodynamic parameters of 85 patients who underwent major surgery under general anesthesia were prospectively evaluated. The noncalibrated pulse contour device MostCare (Vytech, Vygon, Padua, Italy) was used to measure hemodynamic parameters before and after anesthesia induction. The duration of the measurements was determined from one minute before induction to 10 min after induction. Hypotension was defined as a greater than 30% decrease in mean arterial pressure from the pre-induction value and/or systolic arterial pressure of less than 90 mmHg. The patients were divided into post-induction hypotension (−) and (+) groups. For the likelihood of post-induction hypotension, a multivariate regression model was used by adding significantly different pre-induction parameters to the post-induction hypotension group. Results: The incidence of post-induction hypotension was 37.6%. The cut-off value of the pre-induction Ea for the prediction of post-induction hypotension was ≥1.08 mmHg m−2mL−1 (0.71 [0.59–0.82]). In the multivariate regression model, the likelihood of postinduction hypotension was 3.5-fold (1.4–9.1), increased by only an Ea ≥ 1.08 mmHg m−2mL−1. Conclusion: Pre-induction Ea sho wed excellent predictability of hypotension during anesthetic induction and identified patients at risk of general anesthesia induction-related hypotension.

Üniversitesi Çocuk Hematolojisi Anabilim Dalı’na başvuran ve çalışmaya katılmayı kabul eden çocukluk çağı kanser tanısı remisyonda olan 8-18 yaş arası çocuk ve ergenler ile daha önce çocukluk çağı kanser tanısı almamış sağlıklı kontrol grubu çocuk ve ergenler ile çalışma tamamlanmıştır. Çocuk ve ergenlere Travma Sonrası Büyüme Envanteri (PTGI), Revize Edilmiş Çocuk Olayın Etkisi Ölçeği -13 (CRIES-13), Çocuklarda Anksiyete ve Depresyon Ölçeği-Yenilenmiş-Çocuk Formu (ÇADÖ-Y-ÇF) ölçekleri uygulanmıştır. Ebeveynlere de Sosyodemografik form ve Çocuklarda Anksiyete ve Depresyon Ölçeği-Yenilenmiş-Ebeveyn Formu (ÇADÖ-Y-EF) uygulanmıştır. Bulgular: Çalışmaya 8-18 yaş arası, 27 çocukluk çağı kanser tanısı remisyonda olan hasta ve 25 sağlıklı kontrol katılmıştır. İki grup arasında anne baba yaşı, aile yapısı, anne ve babanın öğrenim durumu ve ortalama aylık gelir dağılımında istatistiksel olarak anlamlı düzeyde bir farklılık saptanmamıştır (p 〉 0.05). ÇADÖ-Y-EF Panik Bozukluk, Obsesif Kompulsif Bozukluk, Depresyon alt ölçek skorları kontrol grubunda remisyon grubuna göre istatistiksel olarak anlamlı düzeyde yüksek saptanmıştır (p=0.048; p=0.045; p=0.047). İki grubun CRIES-13 puanları karşılaştırıldığında istatistiksel olarak anlamlı farklılık bulunmamıştır (p=0.659). İki grubun PTGI ölçek ve alt ölçekleri karşılaştırıldığında; total PTGI puanında istatistiksel olarak anlamlı farklılık saptanmazken (p=0.066), PTGI ölçeğinin Yaşam Felsefesinde Değişim ve Başkalarıyla İlişkilerde Değişim alt ölçek ortalaması hasta grubunda kontrol grubuna göre istatistiksel olarak anlamlı düzeyde yüksek saptanmıştır (p=0.038; p=0.05). CRIES-13 ve PTGI ölçek puanları arasındaki ilişkiye bakıldığında, remisyon grubunda istatistiksel olarak anlamlı ilişki bulunmamıştır. Sonuç: Kanseri yenenler bu olumsuz deneyimden büyüyerek, güçlenerek ve olumlu kazanımlarla sağ çıkarak; kendilerinden, dünyadan ve geleceklerinden beklentilerini ayarlayabilmektedir. Günümüzdeki kanser prevalansı ve tedavilerle artan sağkalım oranları göz önüne alındığında bu konuda yapılacak çok merkezli ve daha büyük örneklemli yeni çalışmalara ihtiyaç vardır.

    Abstract: Biogenesis of spliceosomal small nuclear ribonucleoproteins (snRNPs) and their recycling after splicing require numerous assembly/recycling factors whose modes of action are often poorly understood. The intrinsically disordered TSSC4 protein has been identified as a nuclear-localized U5 snRNP and U4/U6-U5 tri-snRNP assembly/recycling factor, but how TSSC4’s intrinsic disorder supports TSSC4 functions remains unknown. Using diverse interaction assays and cryogenic electron microscopy-based structural analysis, we show that TSSC4 employs four conserved, non-contiguous regions to bind the PRPF8 Jab1/MPN domain and the SNRNP200 helicase at functionally important sites. It thereby inhibits SNRNP200 helicase activity, spatially aligns the proteins, coordinates formation of a U5 sub-module and transiently blocks premature interaction of SNRNP200 with at least three other spliceosomal factors. Guided by the structure, we designed a TSSC4 variant that lacks stable binding to the PRPF8 Jab1/MPN domain or SNRNP200 in vitro. Comparative immunoprecipitation/mass spectrometry from HEK293 nuclear extract revealed distinct interaction profiles of wild type TSSC4 and the variant deficient in PRPF8/SNRNP200 binding with snRNP proteins, other spliceosomal proteins as well as snRNP assembly/recycling factors and chaperones. Our findings elucidate molecular strategies employed by an intrinsically disordered protein to promote snRNP assembly, and suggest multiple TSSC4-dependent stages during snRNP assembly/recycling.

    Abstract: DNA and Histone 3 Lysine 27 methylation typically function as repressive modifications and operate within distinct genomic compartments. In mammals, the majority of the genome is kept in a DNA methylated state, whereas the Polycomb repressive complexes regulate the unmethylated CpG-rich promoters of developmental genes. In contrast to this general framework, the extra-embryonic lineages display non-canonical, globally intermediate DNA methylation levels, including disruption of local Polycomb domains. Here, to better understand this unusual landscape’s molecular properties, we genetically and chemically perturbed major epigenetic pathways in mouse trophoblast stem cells. We find that the extra-embryonic epigenome reflects ongoing and dynamic de novo methyltransferase recruitment, which is continuously antagonized by Polycomb to maintain intermediate, locally disordered methylation. Despite its disorganized molecular appearance, our data point to a highly controlled equilibrium between counteracting repressors within extra-embryonic cells, one that can seemingly persist indefinitely without bistable features typically seen for embryonic forms of epigenetic regulation.
