    Abstract: Integrated Nutrient Management (INM) is necessary to enhance sustainable yield in an eco-friendly way. A field experiment was conducted in the research field of Midwest Academy and Research Institute College of Live Sciences, Tulsipur, Dang from November 2018 to January 2019 to investigate the effect of integrated nutrient management on growth and yield of radish. Mino Early variety was used in the experiment. The experiment was laid out on Randomized Complete Block Design with four replications and 5 treatments. Nitrogen (N) was supplied through different sources. The treatment combinations were: control (T1), 100% recommended N through chemical fertilizer (T2), 50% recommended N through chemical fertilizer + 50% N through farmyard manure (FYM) (T3), 50% recommended N through chemical fertilizer + 50% N through poultry manure (T4) and 50% recommended N through chemical fertilizer + 50% vermicompost (T5). Significant effect was noted on leaf numbers, root length, root diameter and yield per ha but no significant effect was noted on the germination percentage and plant height. The highest germination percentage  (77.00 %), plant height (13.27 cm), root length (16.94 cm), root diameter (3.01 cm), and yield per ha (16.55 t/ha) was recorded at T4 (50% recommended N through chemical fertilizer + 50% N through poultry manure). T5 (50% recommended N through chemical fertilizer + 50% vermicompost) recorded the highest leaf numbers (10.40). In our experiment, T4 (50% recommended N through chemical fertilizer + 50% N through poultry manure) was found to be superior, so in inner terai places like Tulsipur, Dang it is suggested to apply 50% recommended N through chemical fertilizer + 50% N through poultry manure to obtain a high yield of radish.

    Abstract: Obstructed Hemivagina and Ipsilateral Renal Anomaly syndrome is a rare obstructive uterovaginal anomaly involving both mesonephric and paramesonephric ducts. It usually presents after menarche with non-specific symptoms like pelvic pain, dysmenorrhea, or paravaginal mass and examination findings of paravaginal or pelvic mass. Because of non-specific symptoms and signs, the diagnosis is usually overlooked, which leads to complications like endometriosis, tubo-ovarian abscess compromising patient’s fertility, and quality of life. Therefore, in presence of these nonspecific clinical features along with imaging findings of uterine didelphys and unilateral renal agenesis, this syndrome should be considered in the diagnosis. We present a case of a 17-year-old lady with Obstructed Hemivagina and Ipsilateral Renal Anomaly syndrome, diagnosed by finding of paravaginal mass on examination and uterine didelphys with ipsilateral renal agenesis in USG and managed successfully by resection of vaginal septum.

    Abstract: This study was conducted at Ilam Municipality-2, Nepal to determine the response of organic manures on post harvest and nutrient restorative effect of cauliflower. Five manures, viz., bansoon, mustard oil cake, poultry manure, farmyard manure, and vermi-compost were evaluated. The postharvest losses, vitamin C content and soil nutrient restorative behavior were significantly highest with vermi-compost as compared to other manures. The maximum vitamin C content of 10.92 mg/100 gm was found with vermi-compost whereas the lowest of 9.66 mg/100 gm was found at farmyard manure. Moreover, the physiological losses were found to be least with vermi compost and the most with bansoon manure. Moreover, the restorative properties i.e. pH, N,P,K and organic %age were found to be significantly highest with vermi-compost as compared to other organic manures. This study concludes that vermi compost increases vitamin C content, postharvest longevity and improvement of physical and chemical properties of the soil.

    Abstract: This is a case of Kounis syndrome with chest tightness, lightheadedness, unconsciousness, and features of shock following multiple wasp stings. Despite cardiopulmonary resuscitation and management of allergic and cardiac symptoms, the patient died. Kounis syndrome should be suspected when a patient presents with allergic symptoms and acute coronary syndrome.

    Abstract: Herein, we have prepared tin oxide (SnO2) nanoparticles (NPs), through a co-precipitation method, using SnCl2·2H2O dissolved in distilled water (DW) as a precursor. Then, the prepared NPs were heat treated in a muffle furnace, as a function of temperature, under an open atmosphere. The prepared SnO2 NPs were then re-dispersed in DW, followed by spray casting on a glass substrate, for preparing SnO2 thin films. The average thickness of the fabricated SnO2 thin films was 2.76 µm. We demonstrated a very clear variation in the structural, compositional, and morphological features of the different films (in particular, variation of the density of oxygen vacancies), which altered their electrical and optical properties. Raising the calcination temperature of the SnO2 thin films, from 250 °C to 650 °C, led to a monotonic reduction in the crystallite size, from 10.4 nm to 6.7 nm, and a decrease in the O/Sn ratio, from 5.60 to 4.79. A 14.5% decrease in the O/Sn ratio resulted in a decrease in the crystallite size by 3.7 nm (i.e., a 35.3% decrease in the NP size), and a decrease in the band gap of 0.11 eV. The lowering of the band gap, along with an increase in the oxygen vacancies in the films, accords well with previous studies. Besides, as the calcination temperature was raised, the refractive index and absorption coefficient values were also found to notably increase. Very interestingly, by simply altering the calcination temperature, we were able to produce SnO2 thin films with optical band gaps nearly equal to the fundamental band gap (2.96 eV), even though many earlier experimental studies had reported considerably greater values (3.36–4.24 eV). SnO2 thin films with lower oxygen vacancies exhibited relatively higher band gaps, which is likely to be favorable for the desired electron transport layer in perovskite solar cells.

    Abstract: Background: Achalasia Cardia is a rare esophageal motility disorder. Among various treatment options, Pneumatic Dilatation (PD) is the most widely used and cost effective modality till date. This is the first observational study aiming to evaluate the short term response and complications of PD for Achalasia Cardia in Nepal. Methods: This prospective observational study was conducted between 28th Jan 2020 to 27th Jan 2021. It included 39 patients with Achalasia Cardia diagnosed by clinical presentation, esophagoscopy, barium esophagogram and high resolution manometry. Two patients of Type III achalasia were excluded from study. Thirty seven patients underwent pneumatic dilatation with 30 mm Rigiflex balloon (Boston Scientific, USA) for a duration of 1 minute. Response was assessed by Eckardts score at 3 and 6 months. Result: Among 39 cases (mean age= 39.03±15.017 years, 59% men), commonest was Type II Achalasia (71.8%) followed by Type I (23.1%) and Type III (5.1%). Dysphagia was present in all patients (100%), followed by weight loss (84.6%), regurgitation (79.5%) and chest pain (35.9%). Mean basal Eckardts score and Lower Esophageal Sphincter pressure of the study population was 7.81±1.24 and 24.40±6.83 respectively. Response to pneumatic dilatation was 89.2%. Eckardts score changed significantly from7.81±1.24 to 1.03±1.82 at 6 months (p 〈 0.001). None of the patients had major complications. Younger age (23±6.377 years) had poor response to treatment, while predilatation Lower Esophageal Sphincter pressure, gender and type of achalasia did not affect the treatment outcome. Conclusion: PD is safe and effective treatment modality for Achalasia. Younger patients have poor response to treatment with Pneumatic Dilatation.

    Abstract: Background and Aims: Nonalcoholic Fatty Liver Disease (NAFLD) progresses to liver fibrosis and ultimately cirrhosis and its complications. Grading of liver fibrosis 2D SWE (Shear Wave Elastography) is a noninvasive study of liver fibrosis and stiffness. We compared different fibrosis scores (NAFLD fibrosis score, FIB 4 score, APRI score) and ultra-sonogram identified fatty liver with 2d shear wave elastography score. Methods: A hospital based prospective observational study was conducted from May 2019 for ten months period in the Gastroenterology and Liver Unit, NAMS, Nepal. Seventy patients with known fatty liver and its severity were identified by Ultrasonogram criteria were enrolled, who met the exclusion criteria. All patients underwent 2D shear wave elastography. Patients were evaluated for effects of obesity, diabetes mellitus, thyroid related disease and dyslipidemias on NAFLD. Tests were done to calculate different fibrosis scores (NAFLD Fibrosis, FIB4, APRI, AST/ALT ratio) and compared with 2d shear wave score. Results: Baseline Characteristics among 70 patients 39(55.7%) were male and 31(44.3%) were female with mean age of the patients 44.3 years. Most of the patients were overweight with mean BMI of 28.2, kg/m2among them 51(72.9%) patients were more than 25.kg/m2Most of the patients 41.4% had diabetes mellitus, 77.1% had hypertension, 30% had hypothyroidism and were 25.9% had metabolic syndrome. Majority of patients on ultrasound gradings were mild 64.28%, 25.72% were moderate and only 10 % were in severe groups. In comparison ultra-sonogram with 2 d shear wave elastography, mild grade mean fibrosis 7.07 kpa, for moderate grade mean fibrosis 8.22 kpa and for severe grade mean fibrosis was 18.16 kpa. Conclusion: In patients with NAFLD, measurement of liver stiffness by 2d shear wave elastography has positively correlated with FIB-4 score and non-inferior to NAFLD fibrosis score. The mean value of elastography increases significantly with the severity of steatosis on ultrasonography.

    Abstract: Bartonella spp. comprise a genus of Gram-negative alphaproteobacteria that are slow growing, fastidious, and facultative intracellular pathogens with zoonotic potential. Immunofluorescent antibody assays (IFAs), Western blot (WB), and enzyme-linked immunosorbent assays (ELISAs), the frequently used modalities for the serological diagnosis of canine and human Bartonelloses, generate numerous false negative results. Therefore, the development of a reliable serodiagnostic assay for Bartonelloses is of clinical and epidemiological importance. Pap31, a heme binding surface protein of B. henselae, is associated with bacterial adhesion and related to bacterial colonization. To our knowledge, B. henselae Pap31 and its fragments (N-terminal (NTD), middle (MD), and C-terminal (CTD) domains) have not been investigated for the serodiagnosis of canine and human Bartonelloses. In this study, we evaluate the diagnostic utility of B. henselae recombinant whole Pap31 (rPap31) and Pap31 fragments by ELISA using sera from 70 dogs (36 Bartonella spp. IFA-positive (naturally infected), and 34 Bartonella spp. IFA- and PCR-negative (control dogs)) and 36 humans (18 Bartonella spp. IFA-positive (naturally infected) and 18 controls)). In the dogs, the area under the curve (AUC) score of recombinant whole Pap31 was 0.714 with a sensitivity of 42% and specificity of 94% at an OD cutoff value of 0.8955. Among the evaluated recombinant Pap31 proteins for the diagnosis of canine Bartonelloses, rPap31-NTD yielded the highest AUC score of 0.792 (95% CI 0.688–0.895) with a sensitivity of 44% and specificity of 100% at a cutoff value of 1.198. In concordance with this finding, rPap31-NTD also had the highest AUC score of 0.747 (95% CI 0.581–0.913) among the Pap31 recombinant proteins for the diagnosis of human Bartonelloses, with 39% sensitivity and 94% specificity at a cutoff value of 1.366. Recombinant whole Pap31 (rPap31) resulted in 72% sensitivity and 61% specificity at a cutoff value of 0.215 for human Bartonelloses. Due to either low sensitivity or questionable specificity, our findings indicate that recombinant Pap31 and the selected fragments may not be appropriate diagnostic targets in detecting anti-Bartonella antibodies in Bartonella-infected dogs and humans. The findings from this study can be used to further assess the antigenicity and immunogenicity of B. henselae Pap31 as a diagnostic target.

    Abstract: The Perovskite absorber layer, the electron transport layer (ETL), the hole transport layer (HTL), and the transparent conducting oxide layer (TCO) are the major components that make up a Perovskite solar cell. Between ETL and HTL, the absorber layer is sandwiched, on which electron-hole pairs are created after absorption of solar radiation. Despite substantial progress toward efficiency, long-term stability still remains a serious concern. Present work focuses toward contributing on the later issue by adopting Titanium dioxide (TiO2) as ETL and reduced graphene oxide (rGO) as HTL. Specifically, in the present work, we report our efforts on the preparation of compact titanium dioxide (C-TiO2) and mesoporous titanium dioxide (M-TiO2) layers as an ETL and a reduced graphene oxide thin film as a HTL. The C-TiO2 film was spin casted on FTO glass followed by casting of M-TiO2 film using Doctor Blading technique. Similarly, the rGO film was produced by spray casting over the glass substrate. The asprepared ETL and HTL layers were characterized by measuring their optical properties (transmittance and reflectance of thin films). Then, the bandgap, Eg was extracted from reflectance and transmittance curves for ETL and HTL respectively. In the case of rGO, we found the value of Eg to be 2.1 eV, which varies between 2.7eV and 0.02eV depending upon its reduction level based on the previously reported values. Similarly, the bandgap of the C-TiO2 was 4.51 eV which was reduced to 4.12 eV after the addition of M-TiO2 , which are 0.9 to 1.1 eV higher than previously reported values. However, bandgap shows decreasing trend after employing M- TiO2 over C-TiO2. In a Perovskite solar cell, both ETL and HTL will be investigated.
