    Abstract: – The purpose of this paper is to investigate the determinants of best management practices in an Australian state-run healthcare system, namely New South Wales (NSW), and studies the impact of a range of hospital factors in driving best management practices as a means of enhancing healthcare delivery. Design/methodology/approach – This study adapts a unique survey instrument globally tested to quantify the multi-dimensional nature of hospital management practices in 42 acute care public hospitals of NSW. The authors then analysed the role of hospital-specific characteristics in driving best management practices, namely hospital size (measured by the number of hospital beds, employees and doctors), level of skill and education, degree of hospital manager autonomy and organisational hierarchy. Findings – The findings of this study show the areas of strength and potential areas of improvement in NSW hospitals. The authors find a positive association between the adoption of better management practices and hospital size (measured by the number of hospital beds and employees), level of skills and education, degree of hospital manager autonomy and organisational hierarchy. However, hospital size as measured by the number of doctors did not have a statistically significant relationship. Practical implications – This paper is of interest to both hospital administrators, clinical doctors and healthcare policy-makers who want to improve and develop strategies for better management in the healthcare sector. Originality/value – This study provides an internationally comparable robust measure of management capability in public hospitals, and contributes to the evidence-base of management practices and performance in hospitals.

    Abstract: – The purpose of this paper is to investigate the quality of management practices of public hospitals in the Australian healthcare system, specifically those in the state-managed health systems of Queensland and New South Wales (NSW). Further, the authors assess the management practices of Queensland and NSW public hospitals jointly and globally benchmark against those in the health systems of seven other countries, namely, USA, UK, Sweden, France, Germany, Italy and Canada. Design/methodology/approach – In this study, the authors adapt the unique and globally deployed Bloom et al. (2009) survey instrument that uses a “double blind, double scored” methodology and an interview-based scoring grid to measure and internationally benchmark the management practices in Queensland and NSW public hospitals based on 21 management dimensions across four broad areas of management – operations, performance monitoring, targets and people management. Findings – The findings reveal the areas of strength and potential areas of improvement in the Queensland and NSW Health hospital management practices when compared with public hospitals in seven countries, namely, USA, UK, Sweden, France, Germany, Italy and Canada. Together, Queensland and NSW Health hospitals perform best in operations management followed by performance monitoring. While target management presents scope for improvement, people management is the sphere where these Australian hospitals lag the most. Practical implications – This paper is of interest to both hospital administrators and health care policy-makers aiming to lift management quality at the hospital level as well as at the institutional level, as a vehicle to consistently deliver sustainable high-quality health services. Originality/value – This study provides the first internationally comparable robust measure of management capability in Australian public hospitals, where hospitals are run independently by the state-run healthcare systems. Additionally, this research study contributes to the empirical evidence base on the quality of management practices in the Australian public healthcare systems of Queensland and NSW.

    Abstract: This work proposes a new OLED driver architecture with 10-bit segmented DAC and switched capacitor multiply-by-two circuit application. A 30-channel 10-bit switched capacitor driver chip prototype is implemented in 0.18-[Formula: see text]m CMOS technology. In this architecture, the achieved output range is 1.5–4.8[Formula: see text] V for an input range of 1.5–3.15[Formula: see text]V, which is suitable for OLED driver with different colors. This architecture is not only converting the digital input signal to analog output for the display panel but also giving amplified high output voltage range. In the segmented DAC, 6-bit coarse DAC and 4-bit fine DAC are used for the input voltage range 1.5–3.15[Formula: see text] V. In a conventional RDAC for the output voltage of 4.8[Formula: see text]V, it requires 2[Formula: see text] switches i.e., 14-bit RDAC for the same resolution. Hence, conventional RDAC driver is four times larger than the proposed innovative very compact and high speed 10-bit segmented DAC switched capacitor OLED driver. The new architecture drastically reduces the number of switches and complex metal routing which results in reduced power consumption and good settling time. In the proposed OLED driver, no extra buffer is required as switched capacitor op-amp is applied for the same purpose with a gain of more than one. This high-resolution design with small die area also improves the linearity and uniformity with low-power consumption. The post-simulated results show that the OLED driver exhibits the maximum DNL and INL of 0.03 LSB and [Formula: see text]0.06 LSB, respectively, with an LSB voltage of 3[Formula: see text] mV. The one-channel area is 0.586[Formula: see text]mm [Formula: see text] 0.017[Formula: see text]mm and the settling time is 4.25[Formula: see text] [Formula: see text]s for 30[Formula: see text] k[Formula: see text] and 30[Formula: see text] pF driving load.

    Abstract: This paper presents a prototype of an auto-ranging phase-locked loop (PLL) with low jitter noise over a wide operating frequency range using the multiband programmable voltage-controlled oscillator (VCO) gain stage with automatic band selection. We successfully reduce the VCO gain (Kvco) and retain the desired frequency band. The proposed PLL comprises a prescaler, phase frequency detector (PFD), charge pump (CP), programmable VCO and automatic band selection circuit. The PLL prototype with all subblocks was implemented using the TSMC 0.18 μm 1P6M process. Contrary to conventional PLL architectures, the proposed architecture incorporates a real-time check and automatic band selection circuit in the secondary loop. A high-performance dual-loop PLL wide tuning range was realized using an ASIC digital control circuit. A suitable way to maintain the Kvco low is to use multiple discrete frequency bands to accommodate the required frequency range. To maintain a low Kvco and fast locking, the automatic frequency band selection circuit also has two indigenous, most probable voltage levels. The proposed architecture provides the flexibility of not only band hopping but also band twisting to obtain an optimized Kvco for the desired output range, with the minimum jitter and spurs. The proposed programmable VCO was designed using a voltage-to-current-converter circuit and current DAC followed by a four-stage differential ring oscillator with a cross-coupled pair. The VCO frequency output range is 150–400 MHz, while the input frequency is 25 MHz. A sequential phase detection loop with a negligible dead zone was designed to adjust fine phase errors between the reference and feedback clocks. All circuit blocks of the proposed PLL were simulated using the EDA tool HSPICE and layout generation by Laker. The simulation and measured results of the proposed PLL show high linearity, with a dead zone of less than 10 pV. The differential VCO was used to improve the linearity and phase noise of the PLL. The chip measured results show rms jitter of 19.10 ps. The PLL prototype also has an additional safety feature of a power down mode. The automatic band selection PLL has good immunity for possible frequency drifting due to temperature, process and supply voltage variations. The proposed PLL is designed for −40 to +85 °C, a wide temperature range.

    Abstract: A chopper-embedded bandgap reference (BGR) scheme is presented using 0.18 μm CMOS technology for low-frequency noise suppression in the clock generator application. As biasing circuitry produces significant flicker noise, along with thermal noise from passive components, the proposed low-noise chopper-stabilized BGR circuit was designed and implemented for a wide range of temperature from −40 to 125 °C, including a startup and self-biasing circuit to reduce critical low-frequency noise from the bias circuitry and op amp input offset voltage. The BGR circuit generated a reference voltage of 1.25 V for a supply voltage range of 2.5–3.3 V. The gain of the implemented BGR operational transconductance amplifier is 84.1 dB. A non-overlapping clock circuit was implemented to reduce the clock skew effect, which is also one of the noise contributors. The noise analysis of a chopped bandgap voltage reference was evaluated through cadence periodic steady-state (PSS) analysis and periodic noise (PNoise) analysis. The low-frequency flicker noise was reduced from 1.5 to 0.4 μV/sqrt(Hz) at 1 KHz, with the proposed chopping scheme in the bandgap. Comparisons of the noise performance of the chopper-embedded BGR, with and without a low-pass filter, were also performed, and the results show a further reduction in the overall noise. A reduction in the flicker noise, from 181.3 to 10.26 mV/sqrt(Hz) at 100 KHz, was observed with the filter. All circuit blocks of the proposed BGR scheme were designed and simulated using the EDA tool HSPICE, and layout generation was carried out by Laker. The BGR architecture layout dimensions are 285.25 μm × 125.38 μm.

    Abstract: A 20-channel liquid crystal display (LCD) driver architecture is implemented with [Formula: see text]m CMOS technology. This work presents a novel design of 10-bit compact and high-resolution two-stage DAC to improve the linearity and uniformity of each channel performance. A complete column driver, including a compact DAC, low power buffer, global R-string and multiplexing circuit design, is implemented, and the layout of this 20-channel, 10-bit LCD driver is generated using [Formula: see text] m CMOS technology. All the circuit blocks of the proposed LCD column driver were simulated using the EDA tool HSPICE and layout generation by Laker. This work also realizes a high-performance class AB operational amplifier with a gain of 140[Formula: see text]dB for the proposed LCD driver. The 10-bit compact LCD driver has a 1.4 mV LSB and an output voltage of 1.7 V is achieved for the input range of 0.25–1.7[Formula: see text] V. The compact DAC voltage selector with decoder in this design uses fewer switches in comparison to conventional tree-type RDAC, occupying a smaller chip area with fast response. The proposed design is sufficiently robust for high-color depth and resolution LCD driver applications. The experimental results exhibit maximum differential nonlinearity (DNL) and integral nonlinearity (INL) of 0.065 LSB and −0.12 LSB, respectively. The one channel area is [Formula: see text]m and the settling time is [Formula: see text] s for the [Formula: see text] and 20[Formula: see text] pF driving load.

    Abstract: Despite the high global prevalence of chronic hepatitis B (CHB) infection, datasets covering the whole hepatitis B viral genome from large patient cohorts are lacking, greatly limiting our understanding of the viral genetic factors involved in this deadly disease. We performed deep sequencing of viral samples from patients chronically infected with HBV to investigate the association between viral genome variation and patients’ clinical characteristics. We discovered novel viral variants strongly associated with viral load and HBeAg status. Patients with viral variants C1817T and A1838G had viral loads nearly three orders of magnitude lower than patients without those variants. These patients consequently experienced earlier viral suppression while on treatment. Furthermore, we identified novel variants that either independently or in combination with precore mutation G1896A were associated with the transition from HBeAg positive to the negative phase of infection. These observations are consistent with the hypothesis that mutation of the HBeAg open reading frame is an important factor driving CHB patient’s HBeAg status. This analysis provides a detailed picture of HBV genetic variation in the largest patient cohort to date and highlights the diversity of plausible molecular mechanisms through which viral variation affects clinical phenotype.

    Abstract: Objective: To detect retinal involvement in coronavirus disease 2019 (COVID-19) patients in acute and convalescent phase by their fundus screening. Methods: In a prospective, cross-sectional, observational study (July–November 2020), 235 patients (142 acute and 93 convalescent phase) underwent fundus screening in a tertiary care center in North India. For convalescent phase, “hospitalized” patients (73) were screened at least 2 weeks after hospital discharge, and “home-isolated” patients (20) were screened 17 days after symptom onset/COVID-19 testing. Results: None in acute phase showed any retinal lesion that could be attributed exclusively to COVID-19. Five patients (5.38%) in convalescent phase had cotton wool spots (CWSs) with/without retinal hemorrhage, with no other retinal finding, and no visual symptoms, seen at a median of 30 days from COVID-19 diagnosis. Conclusions: CWSs (and retinal hemorrhages) were an incidental finding in COVID-19, detected only in the convalescent phase. These patients were much older (median age = 69 years) than the average age of our sample and had systemic comorbidities (diabetes mellitus, hypertension, etc.). We propose the term “COVID-19 retinopathy” to denote the presence of CWSs at the posterior pole, occasionally associated with intraretinal hemorrhages, in the absence of ocular inflammation in patients with a history of COVID-19 disease.
