    Abstract: Introduction. Vascular calcifications (VC) are common in patients with chronic kidney disease and present one of manifestations of mineral and bone disorders in these patients. Objective. The aim of this pilot study was to examine the prevalence and risk factors of VC in pre-dialysis patients with Balkan endemic nephropathy (BEN) and other kidney diseases. Methods. The study involved 32 pre-dialysis patients, 15 with BEN and 17 with other kidney diseases. All the patients underwent an interview, objective examination, routine laboratory analyses and measurement of serum concentration of intact parathyroid hormone (iPTH), 25-hydroxyvitamin D3 [25(OH)D3] and osteopontin. VCs in iliac, femoral, radial, and digital arteries were evaluated and Adragao VC score was calculated. The samples of radial artery were collected during the first creation of an arteriovenous fistula, and expression of osteocalcin, bone morphogenic protein-2 osteopontin, and matrix Gla-protein in arterial wall were examined. Results. Patients with BEN were significantly older (71.1 ? 6.1 vs. 54.7 ? 11.1 years), but they had significantly lower systolic and mean blood pressure (95.7 ? 13.2 mmHg vs. 104.3 ? 7.4 mmHg) and lower serum concentration of phosphorus (1.32 ? 0.36 mmol/l vs. 1.65 ? 0.35 mmol/l) and cholesterol (4.3 ? 1.1 mmol/l vs. 5.2 ? 0.8 mmol/l) than patients with other kidney diseases. Mean VC score was significantly lower in patients with BEN than in those with other kidney diseases (2.8 ? 1.7 vs. 4.6 ? 1.8; p = 0.009), but expression of four examined proteins in arterial wall differed insignificantly between the two groups. VC score correlated significantly with serum concentrations of cholesterol, triglycerides (positively), and iPTH (negatively). Conclusion. Pre-dialysis BEN patients had a significantly lower mean score of VC than patients with other kidney diseases.

    Abstract: The current study describes the experimental design guided development of PEGylated nanoemulsions as parenteral delivery systems for curcumin, a powerful antioxidant, as well as the evaluation of their physicochemical characteristics and antioxidant activity during the two years of storage. Experimental design setup helped development of nanoemulsion templates with critical quality attributes in line with parenteral application route. Curcumin-loaded nanoemulsions showed mean droplet size about 105 nm, polydispersity index 〈 0.15, zeta potential of −40 mV, and acceptable osmolality of about 550 mOsm/kg. After two years of storage at room temperature, all formulations remained stable. Moreover, antioxidant activity remained intact, as demonstrated by DPPH (IC50 values 0.078–0.075 mg/mL after two years) and FRAPS assays. In vitro release testing proved that PEGylated phospholipids slowed down the curcumin release from nanoemulsions. The nanoemulsion carrier has been proven safe by the MTT test conducted with MRC-5 cell line, and effective on LS cell line. Results from the pharmacokinetic pilot study implied the PEGylated nanoemulsions improved plasma residence of curcumin 20 min after intravenous administration, compared to the non-PEGylated nanoemulsion (two-fold higher) or curcumin solution (three-fold higher). Overall, conclusion suggests that developed PEGylated nanoemulsions present an acceptable delivery system for parenteral administration of curcumin, being effective in preserving its stability and antioxidant capacity at the level highly comparable to the initial findings.

    Abstract: Uvod. Adenomiomatoza zucne kese je benigno, degenerativno i proliferativno oboljenje koje se karakterise proliferacijom epitela i formiranjem mukoznih dzepova u zadebljalom hipertroficnom misicnom sloju. Etiologija bolesti je nepoznata i retko je povezana sa kalkulozom. Adenomiomatoza moze biti generalizovana, segmentalna ili lokalizovana. Prikaz slucaja. Pacijentkinja unazad tri godine ima tegobe u vidu recidivantnih bolova ispod desnog rebarnog luka, pracenih mukom, gadjenjem i povracanjem. UZ pregledom zucne kese potvrdjeno je da se radi o hronicnom holecistitisu sa difuznim zadebljanjem zida velicine do 20 mm, sa prisutnom mikrokalkulozom u lumenu i vecom kolicinom uslojene zuci. Operativni zahvat zapocet i zavrsen laparoskopski. Diskusija Smatra se da bi oboljenje moglo nastati kao posledica neurovegetativne disfunkcije koja dovodi do spazma u izlaznom delu zucne kese i pocetnom delu duktusa cistikusa, uz istovremene snazne kontrakcije zucne kese koje vremenom dovode do hipertrofije mukoze i misicnog sloja. Pojedini autori smatraju da ceste upale zucne kese i zucnih puteva, kao i neki kongenitalni faktori, kao sto su dug, uzak i izuvijan duktus cistikus mogu biti uzrok nastanka bolesti. Na osnovu klinicke slike ne moze se reci da li se radi o adenomiomatozi ili kalkulozi zbog slicnosti simptomatologije. Zlatni standard u dijagnostici navedenog oboljenja jeste ultrazvuk. Glavni vid lecenja simptomatske adenomiomatoze zucne kese predstavlja laparoskopska holecistektomija kad god je to moguce. Zakljucak. Adenomiomatoza je benigno oboljenje zucne kese slicne simptomatoligije kao i kalkuloza. Kod simptomatske adenomiomatoze glavni vid lecenja je laparoskopska holecistektomija.

    Abstract: Introduction. Cytological smear tests of the cervix are routine diagnostic methods used for detection of precancerous lesions and tumors of the cervix; they are highly sensitive and specific in the detection of precancerous squamous intraepithelial lesions. Glandular lesions are much less frequently found in the cervical smear. The most common glandular lesions detected in the cervical smear are endocervical and endometrial adenocarcinomas. Cervical metastases are rare, although there are case reports in the literature. Case Report. In this case report, we present a 64-year-old woman with an abnormal cervical smear and postmenopausal metrorrhagia. Numerous accumulations, as well as individual atypical epithelial cells, were detected in the cervical smear and a cytological diagnosis of a highgrade squamous intraepithelial lesion was made based on cytomorphological features suspicious for invasion (Bethesda Classification, 2014) so a cervical biopsy and curettage of the endocervical canal were performed. A high-grade serous adenocarcinoma was diagnosed by histopathological examination of cervical biopsy and cervical canal curettage specimens, after which a transvaginal ultrasound examination was performed, which showed tumors on both ovaries and free fluid in the abdominal cavity. The patient underwent abdominal hysterectomy with bilateral salpingo-oophorectomy. Histological examination confirmed high-grade ovarian papillary serous adenocarcinoma with psammoma bodies. Conclusion. The cytological diagnostic features and criteria for serous adenocarcinoma in Papanicolaou smears are still vague and insufficiently defined in the literature, which is the reason for very common errors in the interpretation, so further research on the pathogenesis, diagnosis and therapy of this tumor is of great importance.

    Abstract: & lt;p & gt; & lt;strong & gt;Introduction. & lt;/strong & gt; Extramural venous invasion (EMVI) is a significant predictive factor of the prognosis for patients with colorectal carcinoma and it is directly connected with the relapse of a disease, especially with the appearance of distant metastases. & lt;strong & gt;Methods. & lt;/strong & gt; The research comprises 90 patients with colorectal cancer. Representative samples of tumor tissues obtained by surgical resection are fixed in 4% formalin and embedded into paraffin blocks. Semi-series incisions of 4 & amp;mu;m thickness were stained by HE method and Van Gieson & amp;rsquo;s method. & lt;strong & gt;Results. & lt;/strong & gt;Out of 90 analyzed patients, 21 (23,33%) were with EMVI, and 69 (76.67%) without EMVI on the histological preparations stained by HE method. EMVI was found in 7 more tumors on the preparations stained by Van Gieson & amp;rsquo;s method. Sensitivity of the method determining EMVI on the histological preparations stained by Van Gieson & amp;rsquo;s method was better for 25% (7/28) than on the preparations stained by HE method. EMVI was not found in patients at A and B1 stage of disease according to Astler- Coller classification, but out of 77 patients which were at B2, C1 and C2 stage EMVI was found in 28 (36,36%) patients, p & amp;le;0,01. EMVI was found in 18/77 (23,38%) patients who had a tumor of low histological gradus, p & amp;le;0,01 and in 10/13 (76,92%) patients who had a tumor of high histological gradus, p & amp;le;0,01. In patients with neural invasion, EMVI was found in 18/22 (81,82%), whereas in patients without neural invasion EMVI was found in 10/68 (14,71%), p & amp;le;0,01. In patients with weak intensity of peritumoral lymphocytic reaction EMVI was found in 20/41 (48,78%) whereas in patients with moderate and noticeable level it was found in 8/49 (16,33%), p & amp;le;0,01. There was a significant correlation between extramural venous invasion and other parameters of unfavorable prognosis. & lt;strong & gt;Conclusion. & lt;/strong & gt; EMVI is an important indicator in administration of postoperative adjuvant therapy in patients with colorectal carcinoma. A special histochemical method of elastic fiber stain should be applied in everyday practice when EMVI is not found on HE stained preparations in patients with CRC. & lt;/p & gt;

    Abstract: & lt;p & gt; & lt;strong & gt;Introduction. & lt;/strong & gt;The Bcl-2 gene codes an oncoprotein, inhibits a programmed cell death or apoptosis and it plays a very important role in colorectal cancerogenesis. The aim of our study is to determine Bcl-2 expression in colorectal carcinomas in relation with the stage of disease, histology of tumor type, localization and macroscopic growth pattern. & lt;strong & gt;Methods. & lt;/strong & gt; Immunohistochemichal detection of Bcl-2 protein expression was carried out on 90 resected colorectal carcinomas. The patients were divided into three groups according to the degree of Bcl-2 expression in a tumor: group 0 - there were no cells with positive immunohistochemical reaction; group I- up to 20 % of positive cells, and group II- above 20% of positive cells. The groups were compared in relation to the stage of disease, T stage of local spread of disease, histology of tumor type, localization and macroscopic growth pattern. & lt;strong & gt;Results. & lt;/strong & gt; 58% of patients at the second stage of disease had no expression of Bcl-2. Higher percentage (61%) of the patients with metastases ( stages III and IV) had high level of Bcl-2 expression. Tumors with polipoid growth pattern have higher level of Bcl-2 expression. & lt;strong & gt;Conclusion. & lt;/strong & gt;There is a statisticaly significant difference in Bcl-2 protein expression in patients surgically treated at the II and III stage of disease, stage T3a/b, T3c/d of local spread of disease and tumors whith polipoid growth pattern in relation to infiltrative growth pattern. & lt;/p & gt;
