    Abstract: Introduction Asplenic patients are at high risk of potentially fatal invasive infections, such as sepsis, meningitis, and pneumonia. It has been shown that infection from influenza viruses can precede or increase the risk of bacterial infection and of serious complications of the underlying disease. International and national guidelines recommend annual influenza vaccination in asplenic subjects. Following the Covid-19 pandemic, the major government and medical-scientific institutions in the US and in Europe have been planning how to contain infection during the 2020-2021 influenza season. Extending influenza vaccination is the safest and most effective way to reduce the circulation of influenza virus and to promote the correct diagnosis and management of suspected cases of SARS-CoV-2. Influenza vaccination also reduces complications associated with the underlying disease and visits to Emergency Units. Our study aims to evaluate influenza vaccination in a large population of asplenic patients and explore the main causes for non-vaccination to identify critical areas for improvement in the vaccination programme in these at-risk patients for the 2020-2021 influenza season. Methods The Italian Network of Asplenia (INA) is made up of 88 doctors working in 50 clinical centers in 27 cities and 16 of the 20 regions of Italy. It aims to build a large, prospective cohort of asplenic patients throughout Italy through which to study the interaction between asplenia and its associated underlying conditions, collecting precise, accurate data also in cases of rarer diseases. The study also aims to improve the quality of healthcare for this at-risk population. The number of patients enrolled in the Network who had had at least one dose of influenza vaccine at the time of diagnosis of asplenia was retrieved from the INA database. All participating centers were asked to answer a questionnaire to report the main obstacles for influenza vaccination. Results At 1st August 2020, 1,670 patients had been enrolled in the INA (783 females; 887 males). All underlying causes of asplenia are shown in Table 1. Only 466 (28%) patients had had at least one influenza vaccination, while 1,204 (72%) had never been vaccinated since diagnosis of asplenia. Thirty-five (70%) of the 50 centers answered the questionnaire. Main causes of non-vaccination were physicians' ambivalence concerning vaccination and patients' inadequate awareness or logistical problems. Conclusions These data show very low seasonal influenza vaccination cover even though asplenic patients are considered at-risk of complications associated with infection from influenza viruses. Since the 2020-2021 influenza season could see influenza viruses in circulation with SARS-CoV-2, influenza vaccination must be expanded as widely as possible, in particular to subjects of all ages at high risk. These results reveal important areas of concern in the management of asplenic patients and the need to improve the quality of information to physicians and patients alike. The INA co-ordinating center will launch a campaign to provide information and organize ad hoc meetings to widen influenza vaccination coverage in asplenic patients and reduce the pressure on the national health service during the next influenza season.     Abstract: To determine interobserver variability in axillary nodal contouring in breast cancer (BC) radiotherapy (RT) by comparing the clinical target volume of participating single centres (SC-CTV) with a gold-standard CTV (GS-CTV). Methods: The GS-CTV of three patients (P1, P2, P3) with increasing complexity was created in DICOM format from the median contour of axillary CTVs drawn by BC experts, validated using the simultaneous truth and performance-level estimation and peer-reviewed. GS-CTVs were compared with the correspondent SC-CTVs drawn by radiation oncologists, using validated metrics and a total score (TS) integrating all of them. Results: Eighteen RT centres participated in the study. Comparative analyses revealed that, on average, the SC-CTVs were smaller than GS-CTV for P1 and P2 (by −29.25% and −27.83%, respectively) and larger for P3 (by +12.53%). The mean Jaccard index was greater for P1 and P2 compared to P3, but the overlap extent value was around 0.50 or less. Regarding nodal levels, L4 showed the highest concordance with the GS. In the intra-patient comparison, L2 and L3 achieved lower TS than L4. Nodal levels showed discrepancy with GS, which was not statistically significant for P1, and negligible for P2, while P3 had the worst agreement. DICE similarity coefficient did not exceed the minimum threshold for agreement of 0.70 in all the measurements. Conclusions: Substantial differences were observed between SC- and GS-CTV, especially for P3 with altered arm setup. L2 and L3 were the most critical levels. The study highlighted these key points to address. Advances in knowledge The present study compares, by means of validated geometric indexes, manual segmentations of axillary lymph nodes in breast cancer from different observers and different institutions made on radiotherapy planning CT images. Assessing such variability is of paramount importance, as geometric uncertainties might lead to incorrect dosimetry and compromise oncological outcome.

    Abstract: PURPOSE: High-dose therapy followed by autologous stem cell transplantation (AT) is the standard treatment for newly diagnosed multiple myeloma patients. Which is the best treatment option for patients relapsing after AT has not been defined. To address this issues the efficacy of Thalidomide and Dexamethasone (TD), AT and Conventional Chemotherapy (CC) was evaluated. MATERIALS AND METHODS: We retrospective analysed the outcome of 90 multiple myeloma patients, median age 61, range 34–77, who received a first salvage treatment between January 1999 and September 2003. All patients received AT at diagnosis. Relapse was defined as introduction of therapeutic modality that was different from maintenance treatment. After a median time from diagnosis of 32 months, 43 patients were treated with TD (Thalidomide 100 mg/day associated with DEX 40 mg on days 1-4 each month), after a median time from diagnosis of 29 months, 28 patients were treated at first relapse with AT (86% single MEL100, 11% double MEL100, 4% single MEL200), and after a median time from diagnosis of 32 months, 19 patients were treated with CC (32% Doxorubicin, Cyclophosphamide, Etoposide, Cisplatinum combination chemotherapy, 26% Doxorubicin containing regimens, 26% Cyclophosphamide containing regimens, 11% other therapies). Data were analysed when the median follow-up from the start of salvage TD was 30 months (range 4.5–45), from the start of salvage AT was 18 months (range 3.5–24) and from the start of salvage CC was 21 months (range 2–19.5). End points of the study were response, progression free survival (PFS) from first relapse and overall survival (OS) from first relapse and from diagnosis. RESULTS: Patients characteristics were similar among different groups. At relapse the response rate after TD was: 19% near complete remission (nCR) (absence of M-Protein detected by electrophoresis), 28% partial response (PR) (M-Protein reduction 50–99%), 35% stable disease (SD) (M-Protein reduction 0–49%) and 19% progressive disease (PD). After AT was: 11% nCR, 71% PR, 11% SD, 7% PD; and after CC was: 16% PR, 32% SD and 53% PD. Response rate was significantly lower for patients receiving salvage CC in comparison with AT and TD (p 〈 0.001). The median PFS from relapse was 20.3 months for TD, 9 months for AT and 4.5 months for CC (p 〈 0.001). The OS from relapse at 50 months was 58% for TD, 13% for AT and 21% for CC (p = 0.008). The median OS from diagnosis was 116.34 months for TD, 41.3 months for AT and 59.5 months for CC (p 〈 0.001). The multivariate analysis indicates that TD, β2 microglobulin and age were the only independent risk factors associated with improved outcome. CONCLUSION: In conclusion, TD improved PFS and OS in myeloma patients relapsing after autologous transplant.

    Abstract: In newly diagnosed multiple myeloma (MM) patients, the combination melphalan, prednisone and thalidomide (MPT) induces a fast tumor response with a high response rate, but evidence that this translate into improved outcome is limited. This multicenter trial compared the efficacy and the toxicity of oral MPT with oral melphalan and prednisone (MP) in previously untreated patients. From January 2002 to December 2004, we randomised 255 patients, who were older than 65 years of age (median age 72). Data analysis was performed on July 2005. The MPT regimen included oral melphalan (4 mg/m2 for 7 days) and prednisone (40 mg/m2 7 days) for six four week cycles plus thalidomide (100 mg per day continuously until any sign of relapse or progressive disease) The MP regimen was as MPT without thalidomide. Patients who were not assigned to receive thalidomide were permitted to cross over to receive thalidomide after relapse or disease progression. Patients treated with MPT experienced higher response rates and a longer time to progression (primary end points) than patients who did not receive thalidomide. The overall response rate was 76% for MPT and 48% for MP alone (P & lt;0.0001), and the near complete response rates were 28% and 7%, respectively (P & lt;0.0001). Median progression free survival in the MPT and in the MP groups was 33 months and 14 months, respectively (hazard ratio, 0.47; P & lt;0.001). MPT increase median progression free survival by almost 19 months. The 2-yr survival rate was 82% in MPT patients and 65% in MP patients (hazard ratio, 0.68; P=0.2). In MPT group, 33 patients did not complete the 6 courses because of progression disease (9), toxicity (16), death (2), and withdrawal of consent or lost to follow-up (6). In MP group, 32 patients did not complete the 6 courses because of progression disease (19), toxicity (3), death (3), and withdrawal of consent or lost to follow-up (7). By looking at those patients who completed the assigned 6 cycles in both arms, the 2-yr survival rate was 90% in MPT patients and 71% in MP patients, the difference was statistically significant (hazard ratio, 0.39; P & lt;0.01). Grade 3 or 4 adverse events were reported in 49% of patients treated with MPT and in 25% of those treated with MP: they included thromboembolism (12% versus 2% of patients), infections (10% versus 1%), peripheral neuropathy (10% versus 1%), and hematologic toxicity (22% versus 25%) respectively. In the first 64 patients who received MPT, grade 3–4 adverse events were reported in 58% of patients. In the last 65 MPT patients, the incidence of grade 3–4 adverse events was 40%. By comparing the first cohort with the second one, thromboembolism dropped from 22% to 3% (P & lt;0.01) and neurotoxicity from 13% to 8% (P=NS), respectively. The oral MPT was superior to the standard MP in patients with newly diagnosed myeloma. The adequate mangement of side effects reduced toxicity.

    Abstract: Introduction. Lenalidomide (RevlimidR) is a novel, orally active immunomodulatory drug effective for the treatment of refractory myeloma. In this multicenter trial, we evaluate the potential additive and synergistic effect of the combination RevlimidR, melphalan and prednisone (R-MP). Materials and Methods. Patients (pts) with newly diagnosed symptomatic multiple myeloma older than 65 years were treated with 9 courses of RevlimidR (5–10 mg/day for 21days every 4–6 weeks) plus MP (melphalan 0.18–0.25 mg/kg and prednisone 2 mg/kg for 4 days every 4–6 weeks). The trial was designed to define the toxicity profile of R-MP and to analyze the efficacy of this combination. Four different dose levels were tested: 1. melphalan 0.18 mg/kg + RevlimidR 5 mg/day; 2. melphalan 0.25 mg/kg + RevlimidR 5 mg/day; 3. melphalan 0.18 mg/kg + RevlimidR 10 mg/day; 4. melphalan 0.25 mg/kg + RevlimidR 10 mg/day. Each cohort included 6 pts. Dose limiting toxicity (DLT) was defined as: any grade ≥ 3 non-hematologic toxicity; grade 4 neutropenia lasting & gt;7 days; any other grade 4 hematologic toxicity and any treatment delay due to toxicity that occurred during the first cycle. All pts received ciprofloxacin and aspirin as prophylaxis. Results. At present, 24 pts (median age 72, range 61–77) received at least one R-MP course and were evaluated. No DLTs were observed in the first 2 dose levels; 1 DLT was observed with melphalan 0.18 mg/Kg and RevlimidR mg/kg (grade 4 neutropenia lasting & gt; 7 days); 2 DLTs were reached with melphalan 0.25 and RevlimidR 10 mg (1 neutropenic fever, 1 grade 3 cutaneous toxicity). After 1 cycle of R-MP, no one was in complete remission (according to the EBMT/IBMTR criteria), 2 pts (9.5%) showed a myeloma protein reduction of 75–99%, 7 pts (28.6%) a response of 50–74%, and 15 (61.9%) a response of & lt;50% (1 of these pts showed a 30% reduction in the size of soft tissue plasmacytomas), no disease progressions were observed. After 3 cycles of R-MP, myeloma protein reduction of 75–99% was detected in 1 patients (11,1%), response of 50–74% in 8 patients (55.6%) and response & lt;50% in 5 patients (33.3%), no disease progressions were observed. Grade 3 or 4 adverse events were reported in 9 patients (35%). They included: 1 thrombo-embolism (4.2%); 5 grade 4 neutropenias (20.9%) ;4 grade 3 neutropenias (16.7%); 4 grade 3 thrombocytopenias (16.7%); 1 febrile neutropenia (4.2%); 2 grade 3 dermatological toxicities (8.3%); 1 grade 3 metabolic toxicity (4.2%) and 1grade 4 metabolic toxicity (4.2%). One pt discontinued RevlimidR because of grade 3 dermatological toxicity. Dose- reduction was required in 4 pts (1 grade 4 neutropenia & gt;7 days, 1 treatment delay due to toxicity, 2 grade 3 dermatological toxicities). Conclusions. R-MP was well tolerated with a manageable toxicity. Significant response rate was observed. It represents a feasible and promising approach for newly diagnosed pts who are not candidates for transplant. Fifteen additional pts were treated with the fix dose of melphalan 0.18 mg/kg + RevlimidR 10 mg/day, results are too premature to assess efficacy. An update of these data will be presented.

    Abstract: Defibrotide (DF) is a novel orally bioavailable polydisperse oligonucleotide with anti-thrombotic and anti-adhesive effects, which has been shown to be active in various micrangiopathies, including the treatment and prophylaxis of veno-occlusive disease. While DF has minimal inhibitory effect on multiple myeloma (MM) in cell isolates, it showed single agent activity on human MM xenografts in SCID/NOD mice and markedly increased responsiveness of MM to cytotoxic agents, including melphalan, cyclophosphamide and dexamethasone in the same models. DF might thus enhance the response rate of Melphalan, Prednisone and Thalidomide, while protecting against the prothrombotic state seen with this combination in the treatment of MM. In this multicenter, open label, non-randomised phase I/II trial, dosing safety and efficacy of melphalan, prednisone, thalidomide, and DF (MPTD) were determined in pts with relapsed/refractory MM. Between March 2006 and July 2006 we enrolled 14 MM pts in first or second relapse. Primary refractory or pts receiving therapeutic anticoagulation were excluded. Oral melphalan was administered at 0.25 mg/Kg on D1–4, oral prednisone at 1.5 mg/kg on D 1–4, thalidomide was delivered at 50 mg on D1–35 on cycle 1 and at 100 mg from cycle 2 to cycle 6. DF dose levels were: Level + 1 DF = 17 mg/Kg i.v. or 2.4 g p.o. D1–4, followed by 1.6 g p.o. through D 35 Level + 2 DF = 34 mg/Kg i.v. or 4.8 g p.o. D 1–4, followed by 3.2 g p.o. through D 35 Level + 3 DF = 51mg/Kg i.v. or 7.2 g p.o. on D 1–4, followed by 4.8 g p.o. through D 35. Each course was repeated every 35d for a total of 6 courses and no DVT prophylaxis was used. Eleven pts received at least one MPTD-course and were evaluated. 9 pts were in 1st relapse (5 after autologous transplant, 4 conventional chemotherapy), 2 pts in 2nd relapse (autologous transplant, conventional chemotherapy, thalidomide based regimen). In the first 8 pts who received DF at Level +1 one DLT was observed: G3 ileus, which resolved. G3 or 4 hematological toxicities were reported in 7 pts (63%), which were manageable. Two pts (18%) experienced G 3 non-hematologic toxicity: 1 pt ataxia and motor neuropathy and 1 pt with G 3 ileus described above. G 1–2 non-hematological toxicities included fatigue (5 pts), neuropathy (4), infections (3), constipation (2), urinary frequency (1), hyperglycemia (1), rash (2), CVS (hypotension and bradycardia) (2) and lower limb edema (2). There were no DVTs reported, only 1 episode of G 1 bleeding and no G4 non-hematological toxicity. M-protein reduction was evaluated according to the EBMT/IBMTR criteria, after 1 and 3 cycles of MPTD see table. After 3 cycles the overall response rate was 80%. Initial results suggest that MPTD is an effective oral salvage therapy with manageable toxicity and no DVT seen to date in pts with relapsed, refractory MM. Accrual continues to define maximal tolerated dose. An update of these results will be presented.     Abstract: Survival for patients with multiple myeloma varies from less than 6 months to longer than 10 years. Several clinical and laboratory parameters have been described as independent predictors of survival and could help to select patients who may most benefit from different therapies. Thalidomide may have the ability to change the course of the disease or be active in patients who failed previous lines of therapies. In the present study, we evaluated baseline demographic and clinical parameters to determine their value in predicting outcome of relapsed or refractory patients treated with Thalidomide and Dexamethasone (TD). Of 138 enrolled patients (median age 61 years), 59% were treated at diagnosis with autologous transplantation and 41% with conventional chemotherapy. Patients agreed to use contraception, and women of childbearing age had a pregnancy test before enrolment. Exclusion criteria were a second malignancy, psychiatric disease and any grade 2 peripheral neuropathy. Abnormal cardiac function, chronic respiratory disease, abnormal liver or renal functions were not considered criteria for exclusion. The study was approved by the local Institutional Review Board. Written informed consent was obtained on study entry in accordance with the Declaration of Helsinki. Thalidomide was administered at 100 mg/day continuously and Dexamethasone at 40 mg/day on days 1–4 each month. Several parameters such as serum β2-microglobulin, serum C-reactive protein, IgA isotype, haemoglobin, stage, bone marrow plasmacytosis, age, serum creatinine, gender, stem cell transplantation at diagnosis, and time to first progression were analyzed in association with response rate, event-free survival (EFS) and overall survival (OS). No factor was associated with response rate by both univariate and multivariate analyses. The EFS from start of TD therapy and OS from diagnosis were 13.1 months and 74.9 months, respectively. By univariate analysis, clinical factors predictive of shorter EFS from start of TD were age younger than 65 years (p=0.04), autologous transplantation at diagnosis (p=0.02) and stage III (p=0.02). By multivariate analysis, factors associated with shorter EFS, after TD treatment, were age 65 years or younger (p=0.03), autologous stem cell transplantation at diagnosis (p=0.03) and time to first progression ≤ 12 months (p 〈 0.05). Age ≥ 65 years (p=0.04), haemoglobin ≤ 10 g/dL (p=0.002), creatinine ≥ 2 mg/dL (p=0.05), stage III (p=0.05) and time to first progression ≤ 12 (p 〈 0.0001) were associated with shorter OS. Factors that remained significantly associated with shorter OS in multivariate models were haemoglobin ≤ 10 g/dL (p=0.009) and time to first progression ≤ 12 months (p 〈 0.0001). Serum elevated β2-microglobulin and C-reactive protein, generally considered poor prognostic factors, were not predictive of poor EFS or OS. Time to first progression 〉 12 months was the best indicator of survival. Interestingly, younger age and transplant at diagnosis negatively affect EFS but not OS from the start of TD.
