    Notes: Abstract In a multicenter trial we retrospectively evaluated the clinical and sonographic data of 49 neonatal ovarian cysts, 44 of which were detected prenatally and 5 on the first day after delivery. Of the 44 prenatally detected cysts 39 were purely cystic, 5 echogenic or had a mixed pattern. In 20 patients the cystic appearance changed during delivery from purely cystic to a mixed pattern being independent on the size of the cyst. 26 of the 44 cysts were treated surgically. Salpingotorsion was found in 8 and was independent on the size of the cyst. In 15 a salpingo-oophorectomy or oophorectomy was performed, in 11 the ovary was saved. 23 patients were followed sonographically: 15 cysts showed complete resolution within 14 months without correlation to the sonographic pattern. The volume of these cysts varied between 5 and 71 ml. Neonatal ovarian cysts disappear spontaneously frequently and rarely cause severe symptoms. The authors recommend follow-up by ultrasound as the primary modality. Surgical intervention is recommended only if the cyst is space-occupying and percutaneous puncture can not be performed or in the case of emergency.

    Notes: Abstract As part of a multicenter surfactant rescue study, the chest X-rays of 239 preterm and term infants were analyzed. To study the influence of surfactant administration on radiographic appearance, 130 patients with a clinical and radiological diagnosis of typical respiratory distress syndrome were selected, in whom adequate chest x-rays before and within 48 h after treatment were available. Median gestational age was 30 weeks (range 25–38 weeks), median birth weight was 1335 g (range 625–3450). The time of surfactant application ranged between 90 min and 24 h after birth (median 6 h). The most common finding after surfactant administration was uniform (n=47) or disproportionate (n=46) improvement of pulmonary aeration, which showed a significant correlation to posttreatment reduction of oxygen requirement (p〈0.001). Asymmetric clearance was more often localized on the right side and usually disappeared within two to five days. Only in 13 patients no change of ventilation was found. Development of interstitial emphysema (n=24, including three patients with pneumothorax) after surfactant treatment was an unfavourable prognostic sign. 54% of these patients (13 of 24) died within the first month of life, compared to 8% (7 of 93) in the group of patients with initial improvement of ventilation.

    Notes: Abstract Over the period of one year, the hips of 92 preterm neonates were examined by ultrasound. Using the Graf classification only 7% showed an angle alpha between 50 and 60 degrees, which is characteristic of type IIa hips. In all other cases the angle alpha was above 60 degrees (type I). Sonographically there were no pathological cases (type IIg or worse). A reason for the relatively low number of type IIa hips could be that the short osseous acetabular rim and the broad cartilagenous Y-joint in this age group result in a “false” increase of the angle alpha.

    Notes: Abstract. Tissue transition projection (TTP) represents a three-dimensional reconstruction technique for volumetric image data sets. To demonstrate the principle characteristics of TTP, a simple phantom consisting of two pipes with a simulated, wall-adherent polyp was scanned with spiral CT, and images were reconstructed by means of volume rendering for both opaque surface reconstructions and TTP. Tissue transition projection was used in 7 patients for reconstruction of the small intestine or the colon. Unlike three-dimensional reconstructions with opaque surfaces, TTP enhances surface transitions while suppressing homogeneous areas, allowing delineation of the bowel wall similar to conventional double-contrast studies.

    Notes: Abstract In order to evaluate the influence of radiation exposure on image quality in digital storage phosphor radiography, 200 digital storage phosphor chest radiographs, obtained on a neonatal Intensive Care Unit, and the same number of conventional screen-film radiographs (speed 250) were assessed for the visibility of anatomical structures and catheters. The same exposure parameters were used in both groups. Normal variations of radiation exposure under free exposure conditions were estimated in all digital images using a formula calculating radiation dose in the screen-plane from image sensitivity, latitude and average grey value of the right lung. There was already a significant (P 〈 0.001) decline in image quality in the digital images with a 30–50% reduction in radiation exposure, which was most severe for structures such as trachea, retrocardiac space, lung texture and low-contrast catheters. Compared with optimally exposed conventional images, only those digital images with a slightly higher than normal dose had an equivalent image quality.

It is available on CD-ROM, shows good image quality, and is easy to use in medium-price-range personal computer systems. Because of the amount and quality of image material, the Schering Atlas's price is appropriate for institutions, but not for private users.
    Notes: Zusammenfassung Der Schering-Atlas stellt u. E. ein praxisnahes, zum Training von Studenten und Ärzten ausgezeichnet geeignetes Lernprogramm zur Schnittbildanatomie in CT und MR dar. Der Schering-Atlas beinhaltet sämtliche für den Lernenden wesentlichen anatomischen Strukturen. Verzichtet wurde im Schering-Atlas auf die Darstellung sehr kleiner und selten identifizierbarer anatomischer Strukturen, was ihn als Nachschlagewerk für den schnittbildradiologisch versierten Benutzer doch einschränkt. In der Anwendung lassen sich bezüglich Bedienbarkeit und Arbeitsgeschwindigkeit an Personalcomputern mittlerer Preisklasse keine Mängel feststellen. Der Preis des Softwarepaketes erscheint uns in Anbetracht der hohen Qualität und Vielzahl des angebotenen Bildmaterials für klinische Institutionen zwar tragbar, für viele private Anwender aber sicherlich zu hoch. Immerhin hat sich ein System wie der Schering-Atlas, auch in Anbetracht der Möglichkeit des interaktiven Trainings, letztlich zu messen an dem, was an konventioneller Literatur zu diesem Preis zur Verfügung steht.

Einige wirtschaftlichen, wissenschaftlichen und sozialen Aspekte dieses weltweiten Umwandlungsprozesses werden unter dem Betrachtungswinkel telemedizinischer Anwendungsgebiete und Teleresourcing aufgezeigt und entsprechende Informationsquellen im Internet angegeben. Die Möglichkeiten und Perspektiven der Telemedizin werden kontrovers diskutiert.
    Notes: Summary The discussion about the implementation of telemedicine and teleresourcing and its consequences has been under way for some time now. The rate at which telemedical applications are being developed, leaves little time for consideration of the economic, scientific and social aspects. There is a need for integration of all the existing fragments into one coherent telemedical concept. Different aspects of telemedical concepts are discussed.

Der Artikel präsentiert eine Auswahl von kommerziell erhältlichen Teleradiologiesystemen und vergleicht diese im Hinblick auf die oben genannten Eigenschaften.
    Notes: Summary Teleradiologysystems can differ considerably in their features. The most important differences lie in the mode of image data acquisition, data transfer, data safety aspects and the possibilities of interaction between seperate teleradiology units. A selection of commercially available teleradiologysystems is presented and compared.

    Description / Table of Contents: Summary Computer-aided diagnosis schemes in mammography aim at improving the mammographic detection of breast cancer. Through special computer algorithms, it is possible to automatically detect suspicious areas in mammographic images. Typical examples are schemes for automatic detection of microcalcifications and masses in mammography. The computer results may be presented to the reporting radiologist, with the computer acting as a diagnostic aid. Various computer-aided diagnosis schemes in mammography are introduced and possible clinical applications are discussed.
    Notes: Zusammenfassung Verfahren zur computerassistierten Diagnose in der Mammographie dienen der verbesserten mammographischen Detektion von Mammakarzinomen. Mit Hilfe spezieller Computerverfahren ist es möglich, verdächtige Bezirke in Mammographieaufnahmen automatisch zu erfassen. Typische Beispiele hierfür sind Verfahren zur automatischen Detektion von Mikroverkalkungen und Verdichtungsherden in der Mammographie. Die Ergebnisse können dem befundenden Radiologen zur Verfügung gestellt werden, wobei dem Computer die Rolle einer Diagnosehilfe zukommt. In der vorliegenden Arbeit werden verschiedene Computerverfahren zur Diagnoseunterstützung in der Mammographie vorgestellt und deren Einsatzmöglichkeiten in der klinischen Praxis diskutiert.

    Description / Table of Contents: Zusammenfassung Mammakarzinome sind in etwa 5 % auf eine genetische Disposition zurückzuführen. Am häufigsten finden sich Mutationen im Bereich der Gene BRCA1 und BRCA2. Frauen mit einer genetischen Disposition erkranken in etwa 70–90 % im Laufe ihres Lebens an einem Mammakarzinom. Das Erkrankungsalter bei diesen Frauen liegt in der Regel deutlich niedriger als bei den spontanen Formen des Mammakarzinoms, so daß vorhandene Früherkennungskonzepte auf der Basis eines Mammographiescreenings nicht ohne weiteres auf dieses Hochrisikokollektiv übertragbar sind. Im folgenden wird ein integriertes Konzept zur Früherkennung bei Frauen mit genetischer Prädisposition für ein Mammakarzinom auf der Basis von Brustselbstuntersuchung, klinischer Untersuchung, Sonographie, Mammographie und Magnetresonanztomographie vorgestellt.
    Notes: Summary Breast cancer is in 5 % of cases due to a genetic disposition. BRCA1 and BRCA2 are by far the most common breast cancer susceptibility genes. For a woman with a genetic predisposition, the individual risk of developing breast cancer sometime in her life is between 70 and 90 %. Compared to the spontaneous forms of breast cancer, woman with a genetic predisposition often develop breast cancer at a much younger age. This is why conventional screening programs on the basis of mammography alone cannot be applied without modification to this high-risk group. In this article, an integrated screening concept for women with genetic predisposition for breast cancer using breast self-examination, clinical examination, ultrasound, mammography and magnetic resonance imaging is introduced.
