    Abstract: The efficacy of flexible cryoprobe in providing high‐quality tissue specimens through bronchoscopy for making a diagnosis remains debatable. In this study, we have compared the diagnostic yield of cryoprobe with conventional sampling by forceps. Methods Forty‐one patients scheduled to undergo transbronchial lung biopsy ( TBLB ) in a pulmonary hospital in T ehran, I ran. Each patient underwent conventional TBLB and flexible cryoprobe TBLB ( FCLB ) sequentially. Specimen adequacy was defined by the presence of at least 50 alveolar spaces or a positive diagnostic yield. Adequacy of specimens, number and percentage of alveolar spaces without artifact, type of artifact, presence of bronchiolar structures and the diagnosis made based on the results of the two methods separately were compared. Results The mean values of tissue section area obtained by forceps and cryoprobe were 6 mm 2 [standard deviation ( SD ) ± 6.7] and 22 mm 2 ( SD  ± 19.1), respectively ( P   〈  0.001). Specimens were adequate in 26 cases of conventional TBLB and 40 cases of FCLB ( P   〈  0.001). Of adequate specimens, 14 samples obtained by TBLB and 28 samples obtained via FCLB were diagnostic. A significant difference was also detected between diagnostic and non‐diagnostic specimens ( P  = 0.04). Frequency of specimens with 〉 75% artifact‐free lung parenchyma was significantly higher in FCLB method. Conclusion FCLB method provides larger tissue samples with better quality compared with TBLB . Higher‐quality specimens are associated with less artifact and higher diagnostic yield. Multisite randomized trials are required to improve our knowledge about the benefits and indications of TBLB with cryoprobe.

    Abstract: The aim of this study was to develop Dental Public Health (DPH) competencies and to assess the self‐perceived achievements of undergraduate students in regard to these competencies. Materials and Methods In the first phase, by using the mixed method of the RAND‐UCLA model, a list of the proposed competencies was developed and validated based on three‐round expert consensus using both rating and group discussing method. In the second phase, 120 senior (final‐year) dental students were asked to determine their achievements based on the finalised list of competency statements on a 0–10 numerical scale. Descriptive statistical analysis was then performed using SPSS (version 22) to determine the mean score of the items and domains. Some background factors were also tested for any relationship. Result After three rounds of implementing the RAND‐UCLA method, 31 statements in 10 domains were developed and validated. The mean total score of the self‐reported competency was 176.87 ± 52.4 amongst the recruited dental students with a range of 37 to 304. The highest mean ± sd score (7.7 ± 1.8) was found for the preventive oral health services field, whilst the lowest one (4.5 ± 2.3) was reported for understanding the components and functions of the health system. The average self‐reported scores of professional ethics and professionalism, evidence‐based practice and oral health determinants were also acceptable amongst them. Conclusion The finalised competencies gained the consensus level of agreement and appropriateness by representatives of all DPH experts in the country. However, according to the dental students’ self‐reports, they had moderate competencies in most cases. Therefore, content and applied methods of training and evaluation may need to be revised to support students’ development. Also, the complementary evaluation method at real work setting is highly suggested.

    Abstract: In this study, a case of Griscelli Syndrome (GS) in a 7 years old girl was reported. The patient initially presented with fever and pancytopenia in laboratory results; after ruling out the malignancies, she went under treatment with the diagnosis of infectious disease and was discharged after two weeks. Nevertheless, ten days after discharge, she developed new symptoms. Due to patient symptoms and general appearance, microscopic analysis of her hair shaft was done, and the abnormal distribution of pigments in the shaft was observed, indicating GS.

    Abstract: The most potent killing machinery in our immune system is the cytotoxic T lymphocyte ( CTL ). Since the possibility for self‐destruction by these cells is high, many regulatory activities exist to prevent autoimmune destruction by these cells. A tumour (cancer) grows from the cells of the body and is tolerated by the body's immune system. Yet, it has been possible to generate tumour‐associated antigen ( TAA ) ‐specific CTL that are also self‐antigen specific in vivo , to achieve a degree of therapeutic efficacy. Tumour‐associated antigen‐specific T ‐cell tolerance through pathways of self‐tolerance generation represents a significant challenge to successful immunotherapy. CD 4 +   CD 25 +   F ox P 3 + T cells, referred to as T regulatory ( T reg) cells, are selected in the thymus as controllers of the anti‐self repertoire. These cells are referred to as natural T regulatory (n T reg) cells. According to the new consensus ( Nature Immunology 2013; 14:307–308) these cells are to be termed as (t T reg). There is another class of CD 4 + Treg cells also involved in regulatory function in the periphery, also phenotypically CD 4 +   CD 25 ± , classified as induced T reg (i T reg) cells. These cells are to be termed as peripherally induced T reg (p T reg) cells. In vitro ‐induced Treg cells with suppressor function should be termed as i T reg. These different Treg cells differ in their requirements for activation and in their mode of action. The current challenges are to determine the degree of specificity of these Treg cells in recognizing the same TAA as the CTL population and to circumvent their regulatory constraints so as to achieve robust CTL responses against cancer.

    Abstract: The inclusion of antioxidants (AOXs) has been proposed in various protocols to conserve the normal physiology of spermatozoa during (cryo) preservation. The main aim of this review was to understand the necessity of AOXs inclusion in semen extenders of caprine and ovine semen samples and to decipher physico‐chemical space of AOXs used in semen extenders till now. A total 27 full‐text relevant articles were finally discussed here. This systematic review showed that the inclusion of AOXs may improve the success of semen cryopreservation although at least three studies could not support this finding. AOXs have been not added after assaying total antioxidant capacity of the sample of interest, and this is principal measurement bias of all papers. Furthermore, no rational dose–response curve and precise posology have been considered in comparable studies. Furthermore, new methodologies are requested to detect the oxidative status of semen specimens before AXOs fortification and new methodologies like imaging are also needed to detect various injuries of sperms during semen (cryo)preservation. Defining computational chemical and physical spaces of AOXs which used in semen (cryo)preservation would be an interdisciplinary effort to hasten deciphering epoch‐making compounds. In conclusion, more in‐depth analytical, toxicological and pharmacological methodologies should be pursued in supplementation or addition of AOXs during caprine and ovine semen (cryo)preservation after determining the oxidative status of semen samples.

    Abstract: Chronic migraine is a debilitating disorder that has a significant impact on patients and society. Nearly all migraineurs frequently reported light sensitivity during a headache attack. Pituitary adenylate cyclase‐activating polypeptide ( PACAP ) plays an important role in the activation of trigeminal system and migraine pain. To identify the effect of chronic ghrelin treatment on endogenous PACAP and associated symptoms of migraine, an experimental chronic migraine model was induced by intermittent intraperitoneal (i.p) injection of nitroglycerin ( NTG ). Photophobia and anxiety‐like behaviors were determined in the modified elevated plus maze on days 2, 4, 6, 8, and 10 and in the light/dark box on days 3, 5, 7, 9, and 11. Blood levels of PACAP and cortisol were assessed by enzyme‐linked immunosorbent ( ELISA ) kits. Chronic injection of NTG evoked photophobia and anxiety‐like behaviors and treatment with ghrelin (150 μg/kg) for 11 days effectively attenuated photophobia and anxiety‐like behaviors in the both paradigms. We further found that NTG increased the blood levels of PACAP and cortisol, which was significantly reduced by ghrelin treatment. Additionally, staining with Hematoxylin and Eosin (H & E) revealed that ghrelin reduced NTG ‐induced increase in the number of satellite glial cells in the trigeminal ganglion. Furthermore, for the first time we showed that repeated administrations of NTG increased white blood cell ( WBC ) counts and mean platelet volume ( MPV ), and decreased platelet counts. These results indicated that ghrelin decreased migraine associated symptoms possibly through attenuating endogenous PACAP and cortisol levels. Therefore, ghrelin may hold therapeutic potentialities in managing the chronic migraine.

    Abstract: The objectives of this study were to analyze the clinical features and laboratory profiles and risk factors associated with critical illness of children with severe acute respiratory syndrome coronavirus 2 (SARS‐CoV‐2). Methods One hundred and sixty‐six coronavirus disease 2019 (COVID‐19) Iranian pediatric patients were recruited through a collaborative research network between March and May 2020. Demographics, clinical, laboratory, and radiological results were obtained from patient files. Results Of 166 patients, 102 (61%) and 64 (39%) were males and females, respectively. Ninety‐six (57.8%) and 70 (42.2%), had moderate and severe conditions, respectively. Thirty (18%) of patients died. The common symptoms were fever (73%), cough (54%), and shortness of breath, headache decrease in neutrophil and platelet counts; increase values in lactate dehydrogenase, decrease in the blood pH and HCO 3 were significantly associated with the disease severity. 54% and 56% of patients showed abnormal radiographic appearance in Chest X‐ray and in chest computed tomography scan, respectively. Sixty‐one (36.7%) of patients were referred to intensive care unit (ICU). The coexistence of comorbidity was the main factor associated with ICU admission, shock, arrhythmia, acute kidney injury, acute respiratory distress syndrome, acute cardiac injury, and death. Conclusions We describe a higher than previously recognized rate of COVID‐19 mortality in Iranian pediatric patients. Epidemiological factors, such as the relatively high case fatality rate in the country and the presence of underlying diseases were the main factors for the high death rate.

    Abstract: Damage or loss of the bone tissue leads to mobility decline contributing to one of the major issues of human well‐being. Tissue engineering is used to recover fractures and damaged parts of the bone tissue. Various biomaterials and scaffolds have been evaluated for regeneration of hard tissues, however, polymers are the most frequently required biomaterials for the improvement of synthetic bone scaffolds due to their suitable mechanical properties and similar degradation rates to the proteins in hard and soft tissues. Synthetic polymeric materials in bone replacement have several advantages because their physical characteristics can be designed according to their application and their composition can be changed easily. On the other hand, the high adaptability, tenability, and biocompatibility of synthetic materials have received considerable acceptance in the field of tissue engineering. This article presents an update on materials for the fabrication of scaffolds in bone tissue engineering; in addition, we provide information of different kinds of scaffolds and their application usage for hard tissue regeneration.
