    Abstract: To evaluate the outcome and toxicities of stereotactic ablative radiotherapy (SABR) for skeletal metastasis in a tertiary cancer center. Methods This is a retrospective review of 22 patients treated with SABR for skeletal metastases for oligometastases (OM) or oligoprogression (OP) since October 2012. There are a total of 27 treatments with 20 spinal and seven non‐spinal metastases. Treatment outcome including local control (LC), progression‐free survival (PFS), overall survival (OS), pain control, treatment‐related toxicity and failure pattern are described. Patients are assessed by interval computed tomography (CT), positron emission tomography‐CT, magnetic resonance imaging or bone scintigraphy by physicians’ discretion. Toxicities are graded by common toxicities criteria version 4.03. Result The median age of the patients is 64 years. Primary sites include lung (50%), breast (32%), nasopharynx (9%), prostate (4.5%) and colon (4.5%). Twelve patients with OM and 10 with OP are included. Dose to most spinal and non‐spinal metastases is 35 and 50 Gy, respectively, in five fractions. With a median follow up of 15.6 months, there are three local failures (1‐year LC 91.2%). The median PFS and OS are 10.1 and 37.3 months, while PFS of OP and OM group is 6.6 and 10.6 months, respectively. Two‐third of symptomatic patients have at least 1‐year complete pain control. There are two vertebral fractures and one grade 3 esophagitis. Conclusion Our series shows excellent LC of SABR to skeletal metastases with limited toxicities in OM and OP diseases. However, its benefit of survival warrants further studies.

    Abstract: Ureterocutaneous fistula is a major complication following renal transplantation. Timely management can minimise patient and graft comorbidities. We reported three cases of post‐renal transplant ureterocutaneous fistula. Two of them were successfully repaired with Boari flap and one of them were repaired with ileal interposition. Literature review revealed that double‐J stent insertion during renal transplant can significantly reduce post‐transplant major ureteric complications (MUC). Evidence did not show any difference in MUC between early removal of stent (≤14 days) and late removal ( 〉 14 days). Yet incidence of UTI could be reduced in early stent removal.

    Abstract: FLNC ‐related myofibrillar myopathy could manifest as autosomal dominant late‐onset slowly progressive proximal muscle weakness; involvements of cardiac and/or respiratory functions are common. We describe 34 patients in nine families of FLNC ‐related myofibrillar myopathy in Hong Kong ethnic Chinese diagnosed over the last 12 years, in whom the same pathogenic variant c.8129G 〉 A (p.Trp2710*) was detected. Twenty‐six patients were symptomatic when diagnosed; four patients died of pneumonia and/or respiratory failure. Abnormal amorphous material or granulofilamentous masses were detected in half of the cases, with mitochondrial abnormalities noted in two‐thirds. We also show by haplotype analysis the founder effect associated with this Hong Kong variant, which might have occurred 42 to 71 generations ago or around Tang and Song dynasties, and underlain a higher incidence of myofibrillar myopathy among Hong Kong Chinese. The late‐onset nature and slowly progressive course of the highly penetrant condition could have significant impact on the family members, and an early diagnosis could benefit the whole family. Considering another neighboring founder variant in FLNC in German patients, we advocate development of specific therapies such as chaperone‐based or antisense oligonucleotide strategies for this particular type of myopathy.

    Abstract: This study aimed to determine the association of biological (BAB) and psychological illness attributional beliefs (PAB) with medication adherence behaviour in patients with psychosis in Hong Kong. Methods A cross‐sectional survey of 301 outpatients with psychosis in Hong Kong was conducted from August to October 2016. The survey included a set of questionnaires examining patient medication adherence behaviours (using five behavioural items from the Medication Adherence Rating Scale), illness attributional belief, insight into the illness, and attitudes towards antipsychotics and medical professionals. The associations between these variables were analysed using path analysis and adjusted for covariates of perceived social support, experience of side‐effects, and gender. Results The data fit a model in which medication adherence behaviour was associated with illness attributional belief, insight, and attitudes (chi‐square = 32.33, p  = .26; RMSEA = 0.02; SRMR = 0.04; and CFI = 0.97). BAB was positively and directly associated with medication adherence behaviour. PAB was positively and indirectly associated with medication adherence behaviour through insight into the illness and attitude towards medical professionals. PAB can strengthen the relationship between BAB and insight, and only the high PAB group exhibited a positive relationship between BAB and attitude towards medical professionals. Conclusions An integration of biological and psychological attributional beliefs in patients with psychosis is essential for better medication adherence behaviour. Future interventions should aim to modify patients' illness attributional beliefs by integrating both biological and psychological illness attribution to improve medication adherence.

    Abstract: To explore the beliefs and attitudes towards menstruation of Chinese undergraduates in H ong K ong and to compare those of (1) male and female undergraduates with those of (2) undergraduates studying health‐related vs. nonhealth‐related programmes. Background Menstruation is typically viewed as a forbidden topic or a troublesome experience. These negative beliefs and attitudes result from existing myths and taboos associated with cultural factors and health education levels. Design A cross‐sectional survey was conducted in all universities in H ong K ong. Undergraduates were invited through convenience sampling to complete a questionnaire assessing their attitudes and beliefs towards menstruation. Methods A questionnaire on ‘beliefs about and attitudes towards menstruation’ was adopted. Questionnaires were self‐administered by the respondents. Results A total of 450 questionnaires were distributed, and a response rate of 96·6% was obtained; 416 completed questionnaires were collected and analysed. Many Chinese undergraduates agreed that menstruation is annoying, causes disability, involves prescription and proscription and is not pleasant. When comparing the beliefs and attitudes towards menstruation of C hinese male undergraduates with those of female undergraduates, females tended to disagree that menstruation should be maintained secret, but tended to agree that it was annoying. When comparing the beliefs and attitudes towards menstruation of C hinese undergraduates studying health‐related programmes with those under nonhealth‐related programmes, the latter group exhibited a higher level of belief in prescription and proscription for menstruation than the former group. Conclusion Chinese undergraduates in H ong K ong were influenced by the traditional C hinese culture and social environment, resulting in negative attitudes towards menstruation. This study recommends that sex education, especially reproductive health education, be extended to tertiary education. Relevance to clinical practice This study provides relevant information on planning the content of sex education or reproductive health education for C hinese undergraduates.

    Abstract: Isochromans are valuable building blocks for the synthesis of natural products and bioactive compounds. Herein, we report a diastereoselective halo‐cycloacetalization of olefinic aldehydes and alcohols using N ‐haloamides as the halogenating agents. The reaction conditions are mild and can be operated in the absence of external catalysts. Olefinic aldehydes bearing different substituents and various alcohols are compatible with the reaction protocol, giving isochroman derivatives efficiently.

    Abstract: This study aimed to evaluate the effect of a virtual simulation education programme with debriefing in undergraduate nursing students. Perceived clinical competence and learning needs of students in a simulation environment were also measured. Background Evidence showed virtual simulation education programmes provided better knowledge acquisition. However, these studies to date did not incorporate virtual simulation in the combination of a debriefing model in nursing students. Design A one‐group pre‐test and post‐test design. Methods 188 final year undergraduate nursing students participated in the study. Linear mixed model analysis was conducted to evaluate the effect of the programme. Results Students have perceived a significant improvement in clinical competence and nursing process. Self‐efficacy has also boosted. Communication and critical thinking were applied better in the traditional clinical environment. Conclusion Perceived clinical competence of Chinese nursing students has significant improvements by using virtual simulation combining a debriefing model during the COVID‐19 period. Virtual simulation met students' learning needs. Future studies should include a control group for comparison and long‐term measurement. Relevance to clinical practice The study provided an innovative clinical learning pedagogy to serve as a potential alternative with traditional clinical practicum during the COVID‐19 period as this is substantially limited.

    Abstract: The frontal lobe hypothesis of age‐related cognitive decline suggests that the deterioration of the prefrontal cortical regions that occurs with aging leads to executive function deficits. Photobiomodulation (PBM) is a newly developed, noninvasive technique for enhancing brain function, which has shown promising effects on cognitive function in both animals and humans. This randomized, sham‐controlled study sought to examine the effects of PBM on the frontal brain function of older adults. Methods/designs Thirty older adults without a neuropsychiatric history performed cognitive tests of frontal function (ie, the Eriksen flanker and category fluency tests) before and after a single 7.5‐minute session of real or sham PBM. The PBM device consisted of three separate light‐emitting diode cluster heads (633 and 870 nm), which were applied to both sides of the forehead and posterior midline, and delivered a total energy of 1349 J. Results Significant group (experimental, control) × time (pre‐PBM, post‐PBM) interactions were found for the flanker and category fluency test scores. Specifically, only the older adults who received real PBM exhibited significant improvements in their action selection, inhibition ability, and mental flexibility after vs before PBM. Conclusions Our findings support that PBM may enhance the frontal brain functions of older adults in a safe and cost‐effective manner.

    Abstract: This work extends previous neuroimaging work in affective disorders by using functional near‐infrared spectroscopy (fNIRS) to show that decreased prefrontal cortex functioning relates to recent negative mood in a nonclinical population. It thus promotes the application of fNIRS for the neuroscientific study of individual differences in negative affective symptoms. Our findings of monotonic relationships between negative mood and decreased prefrontal cortex functioning are consistent with the dimensional constructs of depression and stress symptoms.

    Abstract: There is substantial evidence of word retrieval impairment as indicated by poor performance on the category fluency test in autism spectrum disorder (ASD). However, little is known about the neural mechanisms underlying this impairment. Functional neuroimaging studies have shown that the lateral frontal cortex plays a key role in flexible word retrieval. Thus, we examined whether individuals with ASD exhibited altered frontal processing during the category fluency test using functional near‐infrared spectroscopy (fNIRS). Twenty‐two adolescents with high‐functioning ASD (20 males) and 22 typically developing (TD) adolescents (16 males) aged 11–18 years were recruited. All underwent a category fluency paradigm, which required production of animal or means of transportation words for 1 min each although their frontal hemodynamic changes were recorded with fNIRS. We found that adolescents with ASD produced fewer animal but not transportation words (group‐by‐category interaction: P  = 0.003), suggesting differential word retrieval impairment. In addition, unlike TD adolescents who exhibited activation primarily in lateral frontal regions during word production, adolescents with ASD had comparable activation across lateral and medial frontal regions. More importantly, this lack of lateral–medial distinction of activation, which was associated with poor word retrieval, differed significantly between groups only in the animal category (group‐by‐category interaction: P  = 0.018). Thus, our findings implicate frontal lobe dysfunction in the impairment of differential word retrieval in adolescents with ASD. The relatively greater involvement of the medial frontopolar cortex might reflect the use of nonspecialized brain regions to compensate for the category‐dependent difficulties with word retrieval in ASD. Lay Summary Using an optical imaging tool, we found that adolescents with autism had difficulties with producing semantically related words and exhibited frontal lobe dysfunction. Nonetheless, poor word production and altered brain processing was only seen when these adolescents were asked to produce words from a category of living things but not nonliving things (i.e., animals but not means of transportation). Category‐dependent word retrieval problems and frontal lobe dysfunction might be two features of this disorder.
